2.Constructing cultivation mode for health service management professionals based on self-organization system theory

3.Treatment of lung cancer with malignant pleural effusion using intrathoracic injection of BCG-PSN combined with mitomycin and cisplatin

Objective To evaluate the efficacy and safety of the intrathoracic injection of polysachride nucleic acid fraction of Bacillus Calmette Guerin(BCG PSN) combined with mitomycin and cisplatin for the treatment of lung cancer with malignant pleural effusion. Methods A total of 53 lung cancer patients with malignant pleural effusion were randomly divided into two groups: BCG PSN combined with chemotherapy group (treatment group, 27 patients) and simple chemotherapy group (control group, 26 patients). Drugs were injected into thoracic cavity, which was repeated every 7 day interval. Effective rate, Karnofsky score and toxicities were evaluated. Results After treatment, the effective rate was 88.8% in treatment group and 53 9% in the control group. Significant difference was found in the two groups( P
4.Nursing of three patients with continuous ambulatory peritoneal dialysis complicated by abdominal fistula

Objective To investigate nursing of patients with continuous ambulatory peritoneal dialysis (CAPD) complicated by abdominal fistula. Methods Three patients with CAPD complicated by abdomial fistula who were treated in the nephrology department from January 2013 to September 2014 were nursed, including disease observation, chest drainage, measurable CAPD and health education. Result Two patients eventually turned to hemodialysis and 1 cases took pleural elemene injection 20 mL +lidocaine 5 mL, suspended peritoneal dialysis for 4 weeks to continue to receive the peritoneal dialysis treatment. Conclusions Abdominal fistula can be detected early based on clinical manifestations. Doctors and nurses in coordination, early treatment can reduce the pain of patients and the occurrence of complications.
5.Adenoviral-mediated transference of the rat angiotensin Ⅱ type 2 receptor gene inhibits the proliferation of vascular smooth muscle cells in vitro

Objective To study the effect on the proliferation of vascular smooth muscle cells(VSMC) after transferring angiotensin Ⅱ(AngⅡ) type 2 receptor(AT2R) gene.Methods The recombinant adenoviral vector AdcCMV-AT2R, containing rat AT2 receptor gene which was constracted by homologous recombination, was used to transfer AT2 receptor gene to rat VSMC in vitro. The expression of AT2R mRNA was detected by RT-PCR. The rate of expression and the change of cell cycle in VSMC were analysed by flow cytometry. Cell devision index, incorporation of bromodeoxyuridine(BrdU)and 3-(4,5-dimethyl-thiazol-2-yl)2,5-diphenytetrazolium bromide(MTT) were used to determine the proliferation of VSMC, respectively.Results RT-PCR showed that the expression of AT2R mRNA increased obviously in transferred VSMC, and the peak value of expression rate was about 89.51% at 48 hours. When the expression of AT2R was at peak value, the ratio of S and G2-M periods was reduced from 31.7% to 13.9%(P<0.05) and the index of cell division from 37.4% to 9.6%(P<0.01). The OD values of MTT and BrdU incorporation were reduced by 61.4% and 51.6% respectively(P<0.01).Conclusion Our study indicates that AdCMV-AT2R can generate high level expression of AT2 receptor in cultured rat VSMC and its expression can significantly inhibit the proliferation of rats VSMC in vitro. It may be a new selection for the gene therapy of restenosis after angioplasty.
6.Effects of atorvastatin on nitric oxide, endothelin-1 and myocardial function in a rabbit model of acute myocardial infarction and reperfusion

AIM:To evaluate the effects of atorvastatin on nitric oxide(NO),endothelin-1(ET-1)and myocardial no-reflow in a rabbit model of acute myocardial infarction and reperfusion(AMI/R). METHODS:Twenty-four rabbits were randomized into 3 groups:8 in AMI/R group,8 in atorvastatin-treated group(5 mg·kg~(-1)·d~(-1))and 8 in sham-operated group. Animals in the former two groups were subjected to 60 min of coronary occlusion followed by 120 min of reperfusion. Data on haemodynamics were collected. NO in blood sample,and in normal,and in infarcted reflow and no-reflow myocardium were evaluated respectively by nitrate reductase method. The levels of ET-1 in blood sample,and in normal,infarcted reflow and no-reflow myocardium were determined by radioimmunoassay. RESULTS:(1)Compared to the baselines,the heart rate(HR),systolic blood pressure(SBP),diastolic blood pressure(DBP),left ventricular systolic pressure(LVSP),maximal rate of increase and decline in left ventricular pressure(±dp/dt_(max))and cardiac output(CO)in AMI/R and atorvastatin-treated groups were significantly declined,whereas left ventricular end-diastolic pressure(LVEDP)was increased after 60 min of coronary occlusion and 120 min of reperfusion(P<0.05 or P<0.01). However,in atorvastatin-treated group,LVSP,LVEDP,±dp/dt_(max) and CO at the time point of 120 min of reperfusion recovered more significantly than those at the time point of 60 min of coronary occlusion(P<0.01),which was more significant than those in AMI/R group(P<0.05 or P<0.01). Compared to AMI/R group,the SBP and DBP were significantly heigher in atorvastatin-treated group(P<0.01).(2)In atorvastatin-treated group,the levels of ET-1 in blood sample were significantly lower than those in AMI/R group(P<0.01),and the levels of NO were significantly higher(P<0.01). Moreover,the levels of NO or ET-1 in infarcted reflow myocardium were significantly lower than that in AMI/R group(P<0.05 or P<0.01).(3)Atorvastatin could ameliorate myocardial function. CONCLUSION:Atorvastatin is effective in increasing NO and reducing ET-1 in blood plasma and local myocardium,and in protection of endothelial cells. Atorvastatin also has a beneficial effect on improving left ventricular function during acute myocardial infarction and reperfusion in rabbits.

The implementation of the Malpractice handling Regulations, which has set new and higher demands on medical institutions and medical workers, will have a certain impact on hospital administration and patient care. Medical institutions and medical workers must be highly aware of this. The entire staff of each hospital must study the Regulations carefully and try to obtain an objective understanding of it. Management of medical institutions must be strengthened, weak links identified, measures formulated and regulations perfected. Medical workers ought to strive for better professional skills, and supervision and management of medical records ought to be intensified.
9.The fingerprints of Fructus Gardeniae by HPCE

AIM: A capillary electrophoresis fingerprints(CEFP) method of Fructus Gardeniae was established to evaluate its quality. METHODS: The background electrolyte(BGE) was 25 mmol/L sodium borate solution containing 10% acetonitrile.The detection wavelength was 228 nm and 24 kV was applied.Fructus Gardeniae was extracted by water and injectded for 15 s(9 cm).Some parameters were used to evaluate the similarities.(RESULTS): 24 co-possessing peaks were selected as the fingerprint peaks of Fructus Gardeniae taking chlorogenic acid peak as the reference peak.The similarities between each of the ten places and the standard CEFP of Fructus Gardeniae were evaluated both qualitatively and quantitatively.The CEFP was also evaluated by the information index(I) and the relative information index(I_r). CONCLUSIONS: The CEFP has acceptable precision,reproducibility and can be used to control the quality of Fructus Gardeniae.
10.Determination of cucurbitacin B in Pedicellus Melo by capillary zone electrophoresis

AIM:To develop a new method to determine cucurbitacin B in Pedicellus Melo by capillary zone(electrophoresis(CZE).) METHODS:All the samples were analysed on a fused silica capillary(75 cm?75 ?m I.D.) by using 50 mmol/L sodium borate solution(containing 5% acetonitrile) as the background electrolyte with the running voltage at 12.0 kV and detection wavelength of 265 nm.And clorprenaline hydrochloride was applied as the internal standard. RESULTS:The results showed a good linear correlation between relative peak area of cucurbitacin B(y) and its concentration(x) over the range of 0.15~1.2 mg/mL.Regression equation was y=(0.003 4x)+0.058,r=0.999 7,and the average recovery for baicalin was 100.2% with the RSD at(2.11%). CONCLUSION:The method is rapid,simple,reproducible and produces low pollution.It serves as a novel way to determine cucurbitacin B.
