1.Traumatic hemipelvectomy: a case report and literature review

Objective To discuss the early clinical management of the pa tients with traumatic hemipelvectomy. Methods A case was reported and literature reviewed. Results The patient was a 4-year-old boy who incurred a left groin and proximal thigh crush with nearly complete transection of this extremity and arrived at our hospital 7 hours after injury. After the vital signs were stabili zed with reasonable wound care and 2 hours of fluid resuscitation, the patient r eceived a thorough debridement and the hip disarticulation and fixation of pubic symphysis. The postoperative course was relatively smooth. While the successful treatments of this patient corresponded to the experiences in the literature, s pecial considerations regarding this severe injury were discussed. Conclusion Im mediate prehospital hemostasis by local pressure, rapid transport, aggressive in itial resuscitation, early recognition of the severity and prompt decision to co mplete the amputation, and attention to details in the rehabilitation phase are necessary for successful management of these patients.
2.Development of orthopaedic trauma in China

Modern Chinese Orthopaedic traumatology has stemmed from the orthopaed ics of Traditional Chinese Medicine(TMC)and western orthopaedics.On the one hand,with a long history and its unique system of theory and treatment,TMC orthopaedics h as made a great con-tribution to the development of modern Chinese orthopaedic traumatology.On the other hand,the introduction and development of western medicine have dramatically pro moted the development of modern Chinese orthopaedic traumatology.Modern Chinese surgeons of orthopaedic trauma have accomplished numerous successful f irst cases in the world since 1950s an d have thus expanded their professional influence.At pr esent,the Chinese orthopaedic trau ma community has grown much stronger and established its ow n academic associations and journals.More and more de-partments of orthopaedic traumatology have been successively establis hed as an independent en-tity in large and middle-sized hospi tals in China.So far,the Chinese orthopaedic trauma com-munity,on the whole,has reached the international level,but there are q uite a lot to be improved in the aspect of treatment.To end the backwardness,in the future Chinese orthopaedic trauma community should make great efforts in education of orthopedists on comp rehensive knowledge,establishment of formal profession al training system,national standardization of orthopaedic procedures in clinical diagnosis an d treatment,active membership of re lative international aca-demic associations and increasing p rofessional exchanges with international counterparts,as well as in research and development of operative techniques and fixators.All these will be the main strategies and developmental trends in the future for Chinese orthopaed ic traumatology. [
3.Biopsy of breast microcalcification using high-frequency ultrasound-guided Mammotome Breast Biopsy System

Objective To evaluate the clinical application of high-frequency ultrasound-guided Mammotome Breast Biopsy System in the biopsy of breast microcalcification.Methods The sterotactic biopsy of microcalcification in breast was performed under the guidance of high-frequency ultrasonography in 24 patients.Results The location of the lesion was accurately determined in all the 24 patients.Pathological findings showed malignant breast tumors in 3 patients(12.5%) and benign breast diseases in 21 patients(87.5%).The malignant tumors included ductal carcinoma in situ in 2 patients and invasive ductal carcinoma in 1 patient.The benign diseases included cystic lobular hyperplasia in 9 patients,ductal hyperplasia and dilation in 6 patients,galactophore cirrhosis in 5 patients,and intraductal papilloma in 1 patient. Conclusions Use of the Mammotome Breast Biopsy System under the guidance of high-frequency ultrasonography is a safe and effective option for the diagnosis of impalpable breast lesions.
4.Damage control orthopaedics: state of the art management of polytrauma

While the basic concept of saving life and decreasing disability h as not changed in the management of polytrauma, the timing and strategies of tre atment have been gradually modified. Damage control is one of the new strategies , and its application in orthopedic traumatology is known as damage control orth opedics. Damage control orthopedics involves formalized three stages in the mana gement of fractures of long bones and pelvis. Stage 1 consists of temporary fixa tion of the unstable fractures and hemostasis. Stage 2 comprises resuscitation, warming, oxygen delivery and administration of coagulation factors to the patien t in the intensive care unit. In Stage 3 definitive fixation is done for the fra cture. Indications of damage control orthopedics: for patients with polytrauma w hose conditions are unstable or in extremis the damage control strategy is recom mended. For the borderline patients early total care may be applied, but the sur gery should be performed with great caution. It has to be converted to the damag e control strategy if conditions of the patient deteriorate during the operation . Since damage control orthopedics is an evolving practice, further work is need ed to enhance its effectiveness and to reduce the incidence of ARDS and MOF.
5.Detection of telomerase activity by fine needle aspiration biopsy in the diagnosis of thyroid cancer

Objective To investigate the value of detection of telomerase activity by fine needle aspiration biopsy (FNAB) of thyroid mass in the preoperative diagnosis of thyroid cancer. Methods Comparation of the results of detection of telomerase activity and biopsy of the of samples of thyroid masses aspirated by fine needle guided by B-us were performed .The results were compared to the postoperative pathological results. Results Of the 32 cases of thyroid carcinomas, the correct diagnostic rate of FNAB was 43.8%, while the positive rate of telomerase activity was 75.0%, the difference between the two examinations was significant ( P
6.Evaluation of the curative effect in breast-conserving therapy

Objective To study the curative effect of breast-conserving therapy in patients with breast cancer.Methods 100 cases with breast cancer were divided into breast-conserving therapy 50 cases and radical therapy 50 Cases.The primary manner was wide excision of tumor and dissection of axillary lymph node.Adjuvant radiotherapy,chemotherapy and incretion treatment were given postoperatively.Results In 100 cases of breast cancer,the surviral of 83 cases(83.0%),17 patients(17.0%)died,30 cases(30.0%)of metastasis,local recurrence in 13 cases (13.0%);ER,PR-positive are 62 cases(62.0%),both wholly negative in 18 cases(18.0%),ER-positive only 12cases(12.0%),only 8 cases(8.0%)of PR positive;comparing two groups,survival and recurrent rate had no significant difference(X2=3.503,X2=4.284,P>0.05),metastasis rate was significant different(X2=6.789,P<0.05);satisfaction degree was 99.0% more than 12 months,and got ideal appearance was over 90%.Conclusion Breastconserving therapy can get long-term effect like Auchineol's.Breast-conserving therapy can gain good cosmetic resuits.
7.Analysis of risk factors of pulmonary embolism in diabetic patients

Objective To study the related risk factors of pulmonary embolism(PE) in diabetic patients.Methods 58 diabetic cases were underwent lower limbs 99mTc-MAA veins imaging(and/or ultrasonography) and pulmonary perfusion imaging.The related laboratory data [fasting blood glucose(FBG),blood cholesterol,blood long chain triglycerides(LCT)]and clinial informations [age,disease courses,chest symptoms(chest pain and short of breathe),lower limbs symptoms(swelling,crooted veins and diabetic foot) and acute complication(diabetic ketoacidosis and hyperosmolar non ketotic diabetic coma)]were collected simultaneously.SPSS was used for χ2-test and Logistic regression analysis.Results 28 patients(48.3%) were showed to be with lower limbs deep vein thrombosis (DVT) and by 99Tcm-MAA imaging,10 cases(17.2%) with PE.The PE ratio(32.1%) of the patients with DVT was more higher than no DVT(3.3%)(χ2 = 6.53,P<0.05).Single factor analysis showed the PE ratios had significant difference to factors of chest symptoms,lower limbs symptoms and acute complication(all P<0.05),respectively.There were no significant difference in other factors of age,disease courses,FBG,blood cholesterol,LCT,blood pressure,weight index,DM type(all P>0.05),respectively.Multiplicity analysis indicated:the related risk factors of PE included chest symptoms(Score = 13.316,P<0.01) and lower limbs symptoms(Score = 7.780,P<0.01).There were no significant differences in other factors(all P>0.05),respectively.Conclusion The serious DM with chest symptoms,lower limbs symptoms and/or DVT could be controlled as early as possible by all kinds of treatment.It would decrease the PE complication.
8.Severity of limb ischemic injury and treatment of popliteal artery injury

Objective To explore the role of severity of limb ischemic injury in the treatment of popliteal artery injury.Methods A retrospective analysis was conducted of 259 patients with popliteal artery injury who had been treated from January 2002 to December 2014.They were 187 males and 72 females with a median age of 37.6 years (range,from 14 to 68 years).The time from injury to revascularization was from 4 hours to 29 days.According to the Rutherford classification system,the limb ischemic injury was categorized into 4 grades:grade Ⅰ:presence of sense and motion;grade Ⅱ:presence of sense but loss of active motion;grade m:loss of sense or motion;grade Ⅳ:stiffness.For all the patients with injury of grades Ⅰ and Ⅱ,limb salvage was adopted and opening decompression to osteofascial compartment was performed depending on the intraoperative and postoperative limb swelling.For the patients with grade Ⅲ injury,opening decompression to osteofascial compartment was performed to explore the activity of muscles in those with a strong will to preserve their limbs.When the muscle activity was poor but bright red bleeding was found at the broken muscle ends and there were no fish-like changes in muscles,limb salvage was suggested;otherwise amputation was chosen.For all the patients with grade Ⅳ injury,amputation was suggested.Results Of the 146 cases of limb salvages,142 were successful and 4 failed due to uncontrollable infection,giving a total success rate of 54.83% (142/259).Complete rupture of the popliteal artery occurred in 23 cases,incomplete rupture or tear in 17,contusion and thrombosis in 219.No patient died due to popliteal artery injury or its complications.The patients with injury of grades Ⅰ,Ⅱ,Ⅲ and Ⅳ were respectively 51,88,67 and 53;their success rates of limb salvage were respectively 100.00% (51/51),96.59% (85/88),8.96% (6/ 67) and 0.For the patients whose time from injury to revascularization was < 6 h,6 h to 12 h,13 h to 24 h,25 h to 1 w,and > 1 w,the success rates of limb salvage were respectively 80.77% (42/52),54.58% (73/134),20.51% (8/39),33.33% (6/18),and 81.25% (13/16).The 142 patients whose limbs had been salvaged obtained an average follow-up of 31.4 months (range,from 6 months to 8 years).Of them,those with grade Ⅰ injury obtained normal plantar sensation and active ankle flexion and extension.Of the 85 patients with grade Ⅱ injury,all recovered normal or nearly normal plantar sensation,29 achieved partial recovery of active ankle and digital flexion and extension,but the other 56 obtained no recovery of active ankle motion.Of the 6 patients with grade l injury,none obtained recovery of motion function,4 achieved partial recovery of plantar sensation but the other 2 had no recovery.Conclusion The classification of popliteal artery injury into 4 grades according to its severity of ischemic injury can provide helpful guidance to the treatment and prognosis assessment of the popliteal artery injury.
9.Clinical significance of serial monitoring of co-stimulating signals in double hand allograft

BACKGROUND: Researches on the hand allograft have been transited from laboratory aspects to clinical application.OBJECTIVE: To evaluate the clinical significance of serial monitoring of co-stimulating signals in double hand transplantation.DESIGN: Case controlled observation.SETTING: Department of Traumatic Orthopaedics, Nanfang Hospital,Southern Medical University.PARTICIPANTS: From September 2001 to December 2001, a patient who was proposed to have double hand transplantation in the Department of Traumatic Orthopaedics, Nanfang Hospital Affiliated to Southern Medical University was taken as the subject of the experiment, meanwhile 15healthy adult man were enrolled as controls.METHODS: Before operation, blood sample was collected from the patient once before given immunodepressant and twice after given immunodepressant; blood sample was also collected before arteriovenous connection during operation, as well as once a day during the first week after operation, once every other day on the second week, twice a week on the third and fourth week, followed by once a week for total two month. The first week after operation was taken as inducing period with the remainder as maintaining period. 2 mL peripheral venous blood was collected from healthy adult and anti-coagulated by EDTA of mass scores of 20 g/L.Flowcytometry was used to carry out a serial monitoring of T cell surface co-stimulating moleculars (CD28, CD54, CD11a).MAIN OUTCOME MEASURES: The change of co-stimulatingmolecules on the surface of T lymphocyte before and after transplantation.RESULTS:The changes of the three co-stimulating molecules were consistent with each other after double hand transplantation (CD28,CD54,CD11a),displaying decrement during inducing period comparing to pre-operation [(9.84±5.28)%, (55.50±3.62)%; (71.03±5.33)%, (95.10±1.26)%;(9.40±9.17)%, (29.70±3.23)%] and keeping at lower level at maintaining period [ (22.54±6.56) %, (91.28±8.12 ) %, ( 11.22±4.08 ) %].CONCLUSION:The serial monitoring of the co-stimulating signals in the peripheral blood of patients with linb allograft is beneficial to the observation of the posopertaive immunologic reaction, providing important guidance for the application of immunosuppression medicine, as well as the prediction and diagnosis of postoperative immune rejection. Meanwhile, costimulating signal induced immunological tolerance is proved promising for the application of hand transplantation.
10.Flow-through anterolateral thigh flap for reconstruction of Gustilo type Ⅲ C open fractures in upper and lower extremities

Objective To analyze the outcome of the flow-through anterolateral thigh flap for reconstruction in the Gustilo type Ⅲ C traumatized extremities.Methods From June 2008 to June 2011,sixteen flow-through anterolateral thigh flaps were used for limb salvage.All in this series suffered from Gustilo type Ⅲ C open fractures in upper (4 cases) or lower extremities (12 cases).All patients had the presence of wide segmental soft tissue defects and segmental artery defects with compromised circulation.Four patients received primary operations and 12 patients received debridement combined with vacuum sealing drainage and secondary operations.Results All patients were followed up from 12 months to 40 months (average of 22 months).The mean age was 37.5 years old (from 18 to 62 years).The mean timing of free flap transfer was 6.7 days after injury (from 5 hours to 16 days).The mean artery defect was 12 cm in length (from 6 to 16 cm).All the flaps survived completely with a soft tissue texture.After operation,infection occurred in one patient,venous thrombosis occurred 1 day post-operatively in one patient,local flap necrosis occurred in two patients.No donor site morbidity was noted.In the last follow-up,according to Johner-Wruhs and Berton scores,the overall excellent/good rate for lower and upper extremities were 83.3% and 75.0% respectively.Conclusion Flow-through anterolateral thigh flaps provide for reconstruction of both the vessels and soft tissue simultaneously.Because of avoiding sacrificing one of the major vessels,it particularly indicates for only one major artery survival in the extremity.The clinical results show that the above reconstruction technique is useful for upper and lower extremities salvage.
