Objective To evaluate the influence of renal insufficiency (RI) on long-term major adverse cardiac events (MACE) of patients with acute myocardial infarction (AMI) plus metabolic syndrome (MetS) and received percutaneous coronary intervention (PCI). Methods This was a retrospective study. From February, 2011 to Octorber, 2013 , we consecutivly enrolled 223 AMI patients with MetS in the First Affiliated Hospital of China Medical University. There were 88 patients with RI in group A, and 135 patients as the control group (group B). Patients were followed up for major adverse cardiac events (MACE) for 1 year. Results Compared with group B, the incidence of 1-year MACE of patients in group A was increased (36.4% vs. 18.5%, P= 0.003). Result of Cox proportional hazard regression analysis showed that RI was a predictive factor for 1-year MACE (HR = 3.56,95%CI 1.004 ~ 4.170, P = 0.002). Conclusion The incidence of 1-year MACE for AMI patients with RI and MetS post-PCI was high. RI was a risk factor for poor prognosis of AMI patients with MetS.
3.Integrating Clinical Knowledge into Pharmacology Teaching: Investigation and Estimate

Objective:The purpose of this study was to improve teaching quality of pharmacology by integrating different form of clinical knowledge.Methods:We tried to add the clinical documents such as some related videos and pictures,many common drugs' trade-names and analysis of typical cases into teaching of pharmacology in undergraduate course in our college.Results:Both the theory examination scores and ability of aggregate analysis of cases were higher in the experiment groups than in the controlled ones.And the students considered that the teaching mode with clinical knowledge could stir up their interest in learning pharmacology and increase teaching effect.Conclusion:The teaching mode of incorporating clinical knowledge into pharmacology would be helpful to improve teaching quality of pharmacology.
4.Application of rapid prototyping (3D printing) fracture model in the teaching of skeletal frac-ture

Objective To evaluate the effect of rapid-prototyping (RP) fracture model in the teaching of extremity fracture. Methods 60 clinical medical undergraduates of the Fourth Military Medical University, who were receiving “surgery” teaching in 2012, were randomly divided into A and B groups equally by lottery. Undergraduates of Group A were taught by traditional methods while undergraduates of Group B were explained by using RP fracture model about the basic anatomy, fracture mechanism, injury mechanism of important vessels and nerves besides traditional methods. Degree of satisfaction of the undergraduates and examination were applied immediately after class. Original data were imputed into SPSS software (version 17.0) for comparison between the two groups using t-test. Results The difference of the two groups in the degree of satisfaction of the undergradu-ates, the average score of fracture mechanism, injury mechanism, clinical manifestation, key point of diagnosis, treatment principles and the total score all has statistical significance (P=0.000). Conclu-sions Students' self-initiative, learning interest and studying efficiency can be inspired by RP fracture models and their professional examination performance has also improved significantly.
5.Analysis of relative risk factors for diabetic nephropathy

Objective: To study relative risk factors for diabetic nephropathy (DN). Methods: A total of 238 patients diagnosed as type 2 diabetes mellitus (DM) were enrolled in the study. According to urine microalbuminuria to urine creatinine ratio (UACR), patients were divided into pure DM group (group DM1, n=90), early diabetic nephropathy group (group DM2 , n=73) and clinical diabetic nephropathy group (group DM3 ,n=75). Clinic data of all patients were collected; Fasting blood glucose (FBG), 2h postprandial blood glucose (2hPB), blood lipids, uric acid (UA), fibrinogen (Fg) and glycosylated haemoglobin (HbA1c) were measured in all patients, and their correlations with DN were analyzed. Results: Compared with group DM1, the course of disease in DM [(7.25±6.29) years vs. (10.25±7.67) years vs. (13.53±7.82) years], levels of FBG [(8.46±2.52) mmol/L vs. (9.52±3.38) mmol/L vs. (10.82±3.30) mmol/L], 2hPB [(18.40±5.64) mmol/L vs. (20.27±5.94) mmol/L vs. (22.59±6.14) mmol/L], HbA1c [(7.96±1.65) % vs. (8.60±1.76) % vs. (9.55±2.09) %], triglyceride [TG, (1.72±0.86) mmol/L vs. (2.34±1.87) mmol/L vs. (3.16±1.85) mmol/L], Fg [(3.49±0.93) g/L vs. (3.88±1.21) g/L vs. (4.99±2.10) g/L] and UA [(295.42±52.34) μmol/L vs. (324.18±96.29) μmol/L vs. (351.23±56.88) μmol/L] significantly increased in group DM2 and group DM3 in order (P<0.01~0.001). Logistic gradual regression analysis indicated that course of DM, HbA1c, TG, Fg and UA were risk factors for DN (OR=1.008~1.910, P<0.01~0.001). Conclusion: The course of DM, blood glucose, blood lipid, uric acid and fibrinogen are risk factors of diabetic nephropathy; increased UACR reflects progress of patient’ condition in DM patients, its detection is used for diabetic prognosis and treatment.
6.Management and outcomes of patients with ST-elevation myocardial infarction in Liaoning province

Objective To analyze the management and outcomes of patients with ST-segment elevation myocardial infarction (STEMI) in Liaoning province.Methods The data were collected from a prospective and multicenter registry study including 8 tertiary hospitals and 12 secondary hospitals in Liaoning province.Total 1429 patients with acute STEMI admitted to hospitals from June 2009 to June 2010 were included in the study.A unified follow-up questionnaire was applied on patient discharged.Results The average age of patients was (63 ± 13)years.37.4% of patients recognized the disease as heart disease and 39.7% were transported by emergency ambulance with a median symptom-to-door time of 150 min.52.9% patients underwent emergency reperfusion therapy,including fibrinolytic therapy (24.4%) and primary percutaneous coronary intervention (PCI,28.1%).The in-hospital treatment included aspirin (99.6%),clopidogrel (81.9%),statins (90.1%),low molecular weight heparin (89.5%),β-blocker (66.0%),angiotensin converting enzyme inhibitor (ACEI)/ angiotensin receptor blocker (ARB)(66.6%).The in-hospital mortality was 10.7% ; the mortality in females was higher than that in males (18.3% vs.7.9%,P < 0.01) and the mortality in older patients (≥ 65 years) was higher than that in younger patients (<65 years)(17.0% vs.5.2%,P <0.01).The follow-up treatment included:aspirin (81.1%),clopidogrel (45.0%),statins (61.0%),β-blocker (48.3%),ACEI/ARB (42.4%).The follow-up mortality was 5.0% after hospital discharge.Conclusions Longer pre-hospital delay is commonly seen in STEMI patients.There is still certain gap of emergency reperfusion therapy and the evidence-based medication with related clinical guidelines of STEMI management in Liaoning.
7.Clinical application of stem cells for bone injury

Development and application of stem cells and their related technologies have paved new ways for prevention and treatment of orthopedic diseases. Application of stem cells for clinical treatment of bone defects is extremely promising, fracture, nonunion and osteonecrosis of the femoral head (ONFH) in particular. However, promotion of clinical stem cell therapy still faces difficulties and lacks corresponding standards. This article reviews clinical researches on stem cell therapy for bone injury and summarizes the injury types, cell types, cell numbers and their clinical efficacy, hoping to provide references for clinical treatment of bone injury with stem cells.
8.Effects of sensory and motor nerve homogenates at different concentrations on proliferation and osteogenesis differentiation of bone marrow mesenchymal stem cells

Objective To investigate the effect of sensory and motor nerve homogenates at different concentrations on the proliferation and osteogenesis differentiation of bone marrow mesenchymal stem cells (BMSCs) in rats.Methods The saphenous nerve and the muscle branch of the sciatic nerve in rats were extracted surgically as sensory and motor nerve tissues,respectively.The primary nerve homogenates (10 mg/mL) were prepared as per 10 mg tissue with 1 mL osteoblast inducing conditional media,and 10 times diluted after filtration purification to prepare sensory and motor nerve homogenates at concentration gradients of 1.0,0.1,0.01,0.001 and 0.0001 mg/mL.Cultivation GFP ± rat pups BMSCs in vitro were trained to P3 generation.The experiment was carried out in 3 groups.The sensory and motor nerve homogenates of 500 μL at the above 6 concentration gradients were added during cultivation respectively in the sensory nerve group (n =18) and the motor nerve group(n =18) while 500 μL of osteoblast inducing conditional media was added in the control group(n =3).Cell proliferation quantity detection and alkaline phosphatase (ALP) staining were used to assess the proliferation and differentiation of BMSCs after 14 days.Results According to the results of CCK-8,the cellular absorbance values at concentrations of 1.0 and 0.1 mg/mL homogenate in the sensory nerve group (1.957 ±0.065 and 1.751±0.073) were significantly greater than in the control group (1.145±0.087) while the cellular absorbance value at concentration of 10.0 mg/mL homogenate in the motor nerve group (0.304 ± 0.619) was significandy smaller than in the control group (1.145 ± 0.087) (P < 0.05).According to the ALP staining,the amounts of cellular calcium nodules in the sensory and motor nerve groups (2.667 ± 0.816 and 3.000 ± 0.632,respectively) were significantly smaller than in the control group (11.833 ± 1.471) (P < 0.05).Conclusion Sensory nerve homogenate is different from motor nerve homogenate in that it may promote proliferation of BMSCs and inhibit osteogenesis differentiation of BMSCs in a certain rage of concentrations.
9.Prevascularization of tissue-engineered bone grafts promotes repair of femoral bone defects in rats

Objective To investigate the effect of prevascularized tissue-engineered bone graft on regeneration of femoral bone defects in rats.Methods Models of femoral bone defect were created at the bilateral hind limbs of 20 healthy female 10 week-old rats which were divided into 2 even groups randomly (n =10).In group A,conventional tissue-engineered bone grafts were transplanted into the femoral bone defects;in group B,tissue-engineered bone grafts and vascular bundles were implanted into the femoral defects.At 1,4 and 8 weeks after operation,3 rats were sacrificed each time in each group to harvest samples.The remaining one in each group served as a spare animal.Regeneration of bone defects and degradation of scaffolds were assessed by radiologic modality and hematein eosin staining.Results At week 1,the new bone ratio (BV/TV) was 5.47% ± 1.90% in group A and 8.49% ± 1.26% in group B,showing no significant difference (P > 0.05);at weeks 4 & 8,the BV/TV were 17.54% ±2.04% and 39.73% ± 4.01% in group A,significantly lower than those in group B (25.32% ± 2.15% and 53.22% ± 2.94%) (P < 0.05).At weeks 1 & 4,the scaffold degradation ratios (RSV/SV) were 97.33% ± 2.52% and 80.60% ±4.00%,showing no significant differences from those in group B (95.67% ±3.51% and 75.22% ±6.20%) (P > 0.05).At week 8,the scaffold degradation ratio in group A (65.46% ±4.51%) was significantly higher than that in group B (50.19% ±4.91%) (P < 0.05).At week 8,hematein eosin staining showed better integration of scaffolds with the femur,faster degradation of the interior scaffolds and greater osteogenetic activity in group B.Conclusion Prevascularization of tissue-engineered bone graft may increase new bone volume and scaffold degradation rate,promoting repair of femoral bone defects in rats.
