1.Clinical Study on the Treatment of Diabetes with Combination of Chinese and Western Medicine

Objective To probe the effect of combinative therapy of Chinese and western medicine on the long-term prognosis of diabetic patients.Methods The methodology of comparative analysis is used.Patients were randomly divided into two groups.Both groups were given the treatment of diet control and exercise.The group with the remedy of western medicine(Group A) was treated with the project to reduce the blood sugar by taking western medicine according to the blood glucose,weight,former dose-taking,together with basic therapy.The group with the combinative therapy of Chinese and western medicine(Group B) was given the same treatment as Group A in addition with syndrome differentiation treatment for Yin deficiency and extreme heat,Qi and Yin deficiency,Yin and Yang deficiency,Qi stagnation and blood stasis.After the course of the treatment,the change of blood glucose,blood lipid and TCM syndrome were collected for statistics,the complication was observed by two years follow-visit.Results The two groups showed no noticeable difference in blood glucose and blood lipid after the treatment.But Group B showed relative superior improvement in the reduction of low-glucose and other complication two years later.Conclusion The combinative therapy of Chinese and western medicine is effective in improving clinical symptom of diabetes patients,and it may prevent and reduce the incidence of diabetic complications.
2.Cardiac Protection during Open Heart Surgery: A View from Coronary Endothelial Function

Cardioplegic (and organ preservation) solutions were initially designed to protect the myocardium (cardiac myocytes) during cardiac surgery (and heart transplantation). Due to the differences between the cardiac myocytes and vascular (endothelial and smooth muscle) cells in structure and function, the solutions may have adverse effect on coronary vascular cells. However, such effect is often complicated by many other factors such as ischemiareperfusion injury, temperature, and perfusion pressure or duration. In evaluation of the effect of a solution on the coronary endothelial function, a number of points should be taken into consideration. First, the overall effect on endothelium should be identified. Second, the effect of the solution on the individual endothelium-derived relaxing factors (nitric oxide, prostacylin, and endothelium-derived hyperpolarizing factor) must be distinguished. Third, the effect of each major component of the solution should be investigated. Fourth, the effect of a variety of new additives in the solution may be studied. In the last decades, we have focused our research on the endothelial function during open heart surgery and have for the first time found that high potassium concentration impairs the EDHF-mediated function. This review attempts to discuss the above issues based on available literature in order to provide information for further development of cardioplegic or organ preservation solutions.

4.Physique properties of 520 aged people in Jiangsu Province of China

BACKGROUND:It is important for the aged population to improve the locomotor activity, maintain a high-level quality of life, and delay the loss of self-care ability.
OBJECTIVE:To analyze the physique properties and influencing factors of the aged population in some areas of Jiangsu province, and to investigate the rule of physiological aging process.
METHODS:In nine test sites of three cities (Nanjing, Yanhceng and Changzhou) of Jiangsu Province, 520 effective samples were col ected and composed of 42.9%males and 57.1%females. We tested the reliability and validity of inspected questionnaire on the aged physique and sports, and then adopted the questionnaire to get general demographic information about their gender, age, education, used occupation, income and physical exercise. At the same time, physical tests were performed (including nine projects such as body shape, physical function, body quality).
RESULTS AND CONCLUSION: ① More than half of the surveyed aged have reached overweight level according to their body mass index, while only a few have normal body mass index, especial y the females. ② Most of the surveyed aged have abnormal blood pressure, and a large number of the aged are in moderate to severe hypertension. ③ There is a gradual y reduction in upper and lower limbs muscle strength, balance ability and walking ability along the aging, a substantial drop is found after 75 years old. There is no obvious difference between male group and female group expect grip strength. ④ Physical exercise can delay the degenerative change in muscle strength and balance ability of the aged. Education has great influence on the physical quality and used occupation influences blood pressure. Having a retired wage also has significant effects on physical quality index and blood pressure, but no influence is found in body shape.
5.Relationship between dental caries and salivary proteome by electrospray ionization ion-trap tandem mass spectrometry in children aged 6 to 8 years.

OBJECTIVETo explore the relationship between salivary proteome and dental caries and to promote the biomarker studies of dental caries susceptibility by comparing the salivary proteome of caries-active children and caries-free children with electrospray ionization ion-trap tandem mass spectrometry (ESI-MS/MS).
METHODSTen caries-active children and ten caries-free children were sampled. The salivary proteome of the two groups was studied, and the differential protein between the two groups was analyzed by ESI-MS/MS after sodium dodecyl sulfate polyacrylamide gel electrophoresis, filter-aided sample preparation, and liquid chromatography.
RESULTSThe concentration of salivary protein was higher in the caries-active group than in the caries-free group. The polypeptide counts of thecaries-active and caries-free groups were 602 and 481, which belonged to 286 and 227 proteins, respectively. The differential polypeptide count of the two groups was 361, and the differential protein count was 118. The detected proteins included matrix metalloproteinase-9 (MMP9), mucin-7 (MUC7), lactotransferrin (LTF), carbonic anhydrase 6 (CA6), azurocidin (AZU), and cold agglutinin.
CONCLUSIONThe total salivary protein was higher in the caries-active group than in the caries-free group. The preliminary detection of differential proteins (MMP9, MUC7, LTF, CA6, AZU, and cold agglutinin) may lay some foundation for biomarker research of dental caries susceptibility.

6.EFFECTS OF HIGH FAT DIET ON GENE EXPRESSION PROFILE OF INTESTINE IN C57BL/6 MICE

Objective To explore the influence of high fat diet on the intestinal gene expression profile in C57BL/6 mice. Method C57BL/6 mice were randomly assigned to two groups (n=8). The control group consumed an ordinary diet. The experimental group was fed with a high fat diet. All mice were sacrificed at the end of 6 w and the intestinal gene expressions were detected by oligonucleotide microarray analysis with Affymetrix GeneChip Mouse U430A consisting of 13 097 genes. Results Among the 13 097 genes obtained from gene expression profile analysis, there were 88 and 179 genes up -and down-regulated respectively, in mice fed with high fat diet compared with the control. The differentially expressed genes were mainly related to free radical oxidative stress, DNA repair, induction of apoptosis, transport, signal transduction and inflammation immune response. Conclusion High fat diet may widely modulate the expression of many genes in the intestine in mice.
7.Treatment of distal tibial fractures with ultra-distal tibial intramedullary nails combined with blocking screws

Objective To determine the surgical procedures and effects of ultra-distal tibial intramedullary nails combined with blocking screws in treatment of distal tibial fractures.Methods From April 2008 to September 2012,21 cases of distal tibial fractures were treated with ultra-distal tibial intramedullary nails combined blocking screws.All fractures were statically locked and closed using undreamed technique.For relatively simple fracture,blocking screws were considered when the reduction and stability was not satisfied after the insertion of intramedullary nails; for severe comminuted fractures,blocking screws were inserted directly under C-arm fluoroscopy.Partial weight-bearing was permitted 3 weeks after surgery.Quality of reduction,fracture union,and function assessment were measured at follow-up.Results No skin necrosis and soft tissue and bone infections occurred after a mean follow-up of 17.5 months (range,12-22 months).All fractures were healed with an average healing time of 12 months (range,8-26 months).X-ray findings revealed the fracture of < 5° angulation on coronal and sagittal planes.There was no deformation or breakage of blocking screws and intramedullary nails.According to the criteria of Tormetta,the results were excellent in 19 cases and good in 2.Conclusion Blocking screws assists reduction and improve the fixation stability by narrowing the canal in treatment of distal tibial fractures and expands the application of intramedullary nails.
8.The value of flow cytometry for the differential diagnosis between refractory cytopenia with multiple dysplasia and aplastic anemia

Objective To evaluate the value of flow cytometry ( FCM) for the differential diagnosis between myelodysplasia (MDS) subtype refractory cytopenia with multiple dysplasia (RCMD) and aplastic anemia (AA). Methods The flow cytometric data of bone marrow samples from 168 cases of RCMD and 77 cases of AA were analyzed retrospectively in blind, and its results were compared with gold standard to evaluate its diagnosis values. Results The specificity of abnormal of single immunophenotype in the surface of granulocytes and myeloblasts was high (range 75.3% -100% ) , but the sensitivity was very low (range 5.4%-50% ). In parallel tests, the sensitivity and specificity of the combination of CD+34 cells≥1% , myeloblasts ≥3% , abnormal expression of CD117 in granulocytes and loss of CD13 in myeloblasts or increased intensity of CD33 in granulocytes were higher than other combinations. The sensitivity and specificity of above combination were more than 62% and 92% , respectively. In the scoring method,different score was given to 8 markers according to different diagnostic value, which were CD+34 cells ≥1 % ,myeloblasts≥3% , abnormal expression of CD117 in granulocytes, loss of CD13 in myeloblasts, increased intensity of CD33 in granulocytes, loss of CD13 in granulocytes, loss of CD10 in granulocytes, and decreased SSC in granulocytes. The sensitivity and specificity were both high if we defined that the total score ≥1.5 was RCMD and the score < 1. 5 was AA. Conclusions The value of abnormal of single immunophenotype for differential diagnosis between RCMD and AA is low. Parallel tests can increase the diagnostic sensitivity obviously and not decrease the specificity. CD+34 cells≥1% , myeloblasts≥3% and abnormal expression of CO117 in granulocytes were the most important markers. The scoring method is precise to distinguish RCMD from AA.
9.The clinical application of vertebral artery CT angiography in fracture of transverse foramen of cervical vertebra

Objective:To evaluate the value of CT angiography (CTA) for vertebral artery (VA) injury in the transverse foramen region of cervical vertebra.Methods:The clinical and CTA imaging data of 56 patients with cervical spine trauma in People's Hospital of Yuxi City from January 2016 to December 2019 were retrospectively analyzed. To observe the VA were or not injuried and their degrees, and the relationship between fracture in transverse foramen region and injured of the VA by applying post processing techniques such as volume reconstruction (VR), image fusion, curved planar reconstruction (CPR), multi-planar reconstruction (MPR), vascular analysis and so on.Results:There were 35 cases in the positive group of transverse perforation stenosis among the 56 patients, and 21 cases in the negative group. There were 19 cases injuried the VA in the positive group of stenosis, but just 5 cases′VA were injuried in the negative group of stenosis, with statistically significant differences ( P<0.05). The risk of VA injuried caused by different degrees of transverse perforation stenosis in the positive stenosis group was statistically significant ( P<0.05). The VA damaged distribution were: 3 cases in V1 segment, 16 cases in V2 segment, 5 cases in V3 segment. The degree of luminal stenosis were: 9 cases of mild stenosi, 7 cases of moderate stenosis, 5 cases of severe stenosis, and 3 cases of luminal occlusion. Conclusions:The CTA of VA can clearly show its anatomical route, degree of injury, range and its adjacent relationship with the fracture end of the transverse foramina of cervical vertebra, which can provide evidences of objective and reliable imaging for spinal surgeons to assess their conditions and select the treatment plans.
10.Use of a saline-coupled bipolar sealer (Aquamantys(R)) in liver resection for primary hepatic malignancy: a retrospective cohort study

Objective To evaluate the efficacy and outcomes with the use of a saline-coupled bipolar sealer during liver resection.Methods 101 patients underwent liver resection for primary hepatic carcinoma at the People's Hospital Affiliated to Zhengzhou University from August 2015 to December 2016.The patients were divided into two groups according to whether the Aquamantys(R) system was used or not.In group A (n =62) the clamp crushing technique was used for liver parenchymal transection.In group B (n =39) the Aquamantys(R) system was used.The intraoperative and postoperative complication rates were compared.Results The operation time in group B was significantly longer than group A (216.4 min vs.253.5 min,P < 0.05).The intraoperaitve blood loss in group B was significantly less than group A (381.1 ml vs.257.2 ml,P < 0.05),and less blood transfusion was required (211.3 ml vs.90.9 ml,P < 0.05).The volume of abdominal drainage fluid in group B in the first and the 5th day was significantly less than group A (242.6 ml vs.199.2 ml,P<0.05;84.3 ml vs.70.4 ml,P<0.05,respectively).The drainage tube in group B was taken off earlier than in group A (8.1 d vs.7.0 d,P < 0.05).The average hospitalization stay after surgery in group B was also significantly shorter (13.4 d vs.11.6 d,P < 0.05).There was no significant difference in the overall postoperative complication rate (P > 0.05) between the 2 groups,and no death was observed.Conclusion The use of a saline-coupled bipolar sealer (Aquamantys(R)) in liver resection for hepatocellular carcinoma was associated with less intraoperative blood loss and was better for the patients' postoperative recovery.
