1.Analysis of adverse drug reaction induced by Chinese patent medicine in Shouguang city from 2012 to 2014

Objective To recognize the characteristics and causes of the adverse drug reaction (ADR)of traditional Chinese patent medicine,and promote the clinical safe drug use.Methods By retrospective study,ADR reports about Chinese patent medicine collected by Shouguang adverse drug reaction monitoring center from January 1,2012 to December 31,2014 were analyzed statistically.Results A total of 479 reports in accordance with the inclusion criteria were collected,which comprised 479 patients.Of them,224 were male and 255 were female,often occurs in patients over 50 years old[53.0%(254 /479)].Chinese patent medicine involved in these reports were mainly blood -regulating formula,heat agent and formula for resuscitation,respectively 278 cases(58.0%),64 cases (13.4%),51 cases (10.6%).Intravenous administration was an important way to trigger ADR [78.5%(376 /479)],78.5% injection had adverse effects and suitability of medicine were 55.9%.Main clinical manifesta-tions were lesion of skin and its appendages[35.1%(183 /522)]and digestive system damage[18.4%(96 /522)]. Conclusion The application of traditional Chinese medicine syndrome differentiation as the primary principle,atten-tion to individual differences of patients,improve the level of rational use drugs,strengthen the proprietary Chinese medicine preparation process and improve the drug instructions can reduce the adverse reaction occurred to some extent of Chinese patent medicine.
2.An efficiency calibration method without a radioactive source using BOMAB phantom and Monte Carlo simulation for Inspector 2000 gamma spectroscopy system

Objective To establish an efficiency calibration method without a radioactive source for Inspector 2000 gamma spectroscopy system.Methods At the fwst step,geometry parameters of the detector were properly specified by comparisons of FEPE(Full Energy Peak Efficiency)between Monte Carlo(MC)calculations and the average measured values of HPGe and NaI detectors using point sources(137 Cs).The differences between calculations and measurements were generally within ±10% for gamma rays.Then,Monte Carlo simulation was used to evaluate the counting efficiency of Nal(TI)and HPGe detectors for BOMAB phantoms.Corresponding efficiency curves and functions were fitted.Results The counting efficiency received from the fitted efficiency functions had a good agreement with those from MC simulation.The bias for Nal detector ranged from-19% to 18%,while the bias for HPGe detector ranged from-11% to 17%.The above errors were totally acceptable in the on-site deployment during nuclear and radiological emergency events.Conclusions Monte Carlo method is simple,time and laborsaving in determing the counting efficiency of gamma spectroscopy system.
3.Treatment of encephalopathy occurred after orthotopic liver transplantation

Objective To investigate the causes and treatment of encephalopathy occurred after orthotopic liver transplantation(OLT). Methods The clinical data of consecutive 51 patients who undergoing OLT were analyzed retrospectively. Results Encephalopathy occurred in 6 cases within one month after operation.The incidence of encephalopathy after OLT was 11.8%(6/51). All the 6 cases were cured by application of sedative agent and dopamine recipient blocking agent(aloperidin). Conclusions The etiology of encephalopathy following OLT is thought to be multifactors including metabolism desturbance before and after OLT,blood dynamics alternation during opration,application of immuosuppression drug etc. Application of sedative agent and dopamine recipient blocking agent may result in ideal effects in treating the encephalopathy.
4.Establishment of three-dimension visualized digital models of foramen ovale piercing path

BACKGROUND:In recent years, increasing research emphasizes the puncture position of the foramen ovale in skul , but most of the positin methods require a higher personal experience of surgeons and lack of individualized quantitative parameters. OBJECTIVE:To establish a visualized digital model of the foramen ovale in skul , explore the reasonable puncture path and puncture depth of percutaneous treatment of foramen ovale puncture for trigeminal neuralgia, and develop the individualized treatment of trigeminal neuralgia. METHODS:Head CT images from healthy adult male volunteers were obtained and were input into three-dimensional reconstruction software MIMICS 10.01, the three-dimensional visualized models of the skul and skin were established. Using the models, the puncture path of the foramen ovale was designed and the preliminary model of the puncture locator was plotted. RESULTS AND CONCLUSION:The three-dimensional visualized digital model of the foramen ovale puncture path was established with CT scan images by using MIMICS software, which provides reliable anatomical data for clinical teaching and lays the groundwork for the simulation of puncture surgery. On the three-dimensional models, the“needle points”,“target points”, and“midpoint”were determined, and the triangle consisted of the three points was regarded as“positioning plane”. Using these parameters, the positioning instrument is characterized by simple structure, convenient operation, high positioning precision and short period of exposure to radiation, it simulates the foramen ovale puncture needle depth and needle direction in a precise and individualized manner.
5.Finite element method for analyzing the stress distribution of acetabulum under different loads

BACKGROUND:The hip is a complicated structure and irregular in shape. It is hard to measure stress distribution and transmission. OBJECTIVE:To establish a three-dimensional finite element model of the hip joint and upper femur, and analyze the stress distribution and transmission characteristics of the acetabulum region under different loads, and explore mechanics mechanism of hip fracture based on CT data. METHODS:The three-dimensional finite element hip and femur model were reconstructed in Mimics 14.0 based on the CT data of a healthy adult man. After dividing mesh, assigning material and transforming into finite element model, the stress distributions of anterior wal , the top, and the posterior wal of the acetabulum, the stress of acetabulum areas and displacement of acetabular unit were calculated with finite element software Ansys 13.0 software under 300, 600, 900 and 1 200 N. RESULTS AND CONCLUSION:(1) A three-dimensional finite element model of the hip and the femur was successful y established, consisting of 284 183 nodes and 160 665 units. (2) The characteristics of the stress distribution of acetabulum region:the maximal stress was concentrated on the posterosuperior part of acetabular crest, fol owed by the posterior wal and the anterior wal in order in upright position under different loads. The stress transmitted by four ways:from acetabular crest to ilium, along linea terminalis of pelvis to sacroiliac joint, in the acetabular sockets, and along the pubic ramus. The stress and the propagation distance were increasing as the loads increased. Acetabular element stress variable was increased. (3) Above results indicated that three-dimensional finite element model of the human hip joint established by Mimics 14.0 based on CT data matches the anatomical structure in a great degree, could be used in the biomechanics analysis under different loads, and has a guiding significance for design of artificial hip prosthesis.
6.Three-dimensional models of organs based on image data

BACKGROUND:Recently, studies on three-dimensional reconstruction and biomechanics became more and more. Three-dimensional models of organs were established by modeling software based on image data using computer. Mechanical analysis was conducted using finite element analysis software. After literature retrieval, we found that the principle of three-dimensional reconstruction of human organs is not clear, and the process description is relatively simple. Some is not accorded with the fact. Above studies cannot guide related research. OBJECTIVE:To explore the principle, process, results and further application of three-dimensional reconstruction models of organs, and to provide evidence for future studies. METHODS:We retrieved China National Knowledge Infrastructure for representative literatures about three-dimensional reconstruction of human organs using the computer, and analyzed the principle, process, results and further application of three-dimensional reconstruction models of organs. RESULTS AND CONCLUSION:In combination with established three-dimensional bone models, we explained the principle, process, and application of three-dimensional reconstruction in detail, and laid the theoretical foundation for subsequent biomechanical research. With continuous development of tissue engineering technology, scholars have begun to study the pathogenesis of bone injury from various angles and different aspects so as to better prevent and treat this disease. The related research is stil in its primary stage, and stil needs further investigations. 
7.Protective effect of flurbiprofen on hepatic ischemia-reperfusion injury in rats and action mechanism

Objective To investigate the protective effect of a COX inhibitor,flurbiprofen (Flurb) on hepatic ischemia/reperfusion (IR) injury in rats and the action mechanism.Method C57BL/6 mice were randomized into sham,IR and Flurb (4 different doses) groups.The model of segmental (70%) warm hepatic ischemia was established in IR and Flurb groups.Flurbiprofen of different doses (5,7.5,10 and 15 mg/kg) was injected via the tail vein 20 min before ischemia.At different time points after reperfusion,liver cell necrosis and apoptosis were evaluated by HE and TUNEL staining.The COX and inflammatory cytokine gene expression was detected by using realtime PCR.Liver mitochondria were separated and mitochondrial permeability transition (MPT) pore sensitivity was examined by using swelling assay and fluorescence spectrophotometry assay.Result In flurbiprofen groups of different doses,the serum AST and ALT levels were significantly decreased at 6 h after reperfusion as compared with IR group.Moreover,10 mg/kg Flurb pretreatment significantly inhibited the mitochondrial permeability transition (MPT) pore opening,and thus alleviated liver cell damage and prevented mitochondria-related cell death and apoptosis by inhibiting COX-2 and inflammatory factor genes expression such as IL-1β,IL-6 and TNF-α.Conclusion Flurbiprofen protects mice from hepatic I/R injury possibly by inhibiting mitochondrial permeability transition and IL-1β,IL-6 and TNF-α expression,which may provide experimental evidence for clinical use of flurbiprofen to protect liver function in surgical settings other than its conventional use for pain relief.
8.Prevention and treatment of acute renal failure (ARF) following orthotopic liver transplantation

Objective To investigate the prevention and treatment of acute renal failure (ARF) following orthotopic liver transplantation(OLT).MethodsClinical data of 51 patients who underwent liver transplantion consecutively between October 2001 and April 2003 were analyzed retrospectively. Results Preoperatively 9 out of 51 patients had suffered renal dysfunction (Cr≥132 ?mol/L or BUN≥18 ?mol/L), ARF developed in 7 cases within one week postoperatively. Kidney function recovered to normal in 6 cases in 3 to 6 days after administration of vasoactive drugs (dopamine) and daclizumab (Zenapax). One patient failing to response needed hemodialysis for 11 days before his kidney finally functioned. Conclusions The etiology of ARF following OLT is multifactorial, with preoperative, intraoperative and postoperative factors being involved. Individually applied immunosuppressant, improving renal perfusion, and hemodialysis were important for the recovery of kidney function.
9.Anti-rejection effects of CTLA-4 Ig fusion protein on cardiac allografts in mice and its mechanism in vivo

Objective: To determine the anti rejection effects of CTLA 4 Ig fusion protein on cardiac allografts in mice and to discuss its mechanism in vivo . Methods: BALB/c recipients were performed cervical heterotopic heart transplantation to receive C57BL/6 donor hearts with a cuff technique. BALB/c recipients were intraperitoneally injected with CTLA 4 Ig [100 ?g/d?15 times], control immunoglobins and PBS to observe the survival time of allografts with ECG. The hyporesponsiveness of splenic T cell, the polarization of the T subsets were analyzed after the recipients treated with CTLA 4 Ig. Results: After treated with CTLA 4 Ig, the survival of cardiac grafts was significantly prolonged compared with the control groups, and more than 40% cardiac grafts survived over 2 months. The splenic T cells isolated from recipients did not respond to restimulation of donor splenocytes in MLR, but did exhibit the capacity to proliferate in response to C3H splenocytes(third party).The levels of IL 2 and IFN ? decreased and the level of IL 10 increased in CTLA 4 Ig treated mice. Conclusion: Administration of CTLA 4 Ig can induce donor specific tolerance, which induce T subsets to polarize toward Th2 subset and hyporesponsiveness to alloantigen, and prolong the survival time of the cardiac grafts effectively. 10.Research advances in electroencephalographic monitoring of minimal hepatic encephalopathy

Hepatic encephalopathy (HE) is a serious neuropsychological complication in advanced liver disease, and is a major cause of death in patients with liver disease. The paper briefly introduces the advances in application of HE grading, examination methods, and electroencephalography (EEG) in the diagnosis of HE, and points out that EEG has been developed greatly in the field of HE, with huge potentials for the diagnosis, evaluation, prognosis, and guidance for treatment of HE. However, the clinical value of EEG monitoring in HE has not been widely acknowledged in the medical world, and further investigation is still needed in the future.
