1.Pentraxin 3 and cerebral ischemia

Pentraxin (PTX) 3 is a typical long PTX. As an inflammatory marker, the plasma PTX3 level is elevated in atherosclerosis, cardiovascular disease, infection, inflammation, autoimmune disease and tissue injury. Recently studies have shown that the plasma PTX3 level rapidly increased during the acute phase of ischemic cerebrovascular disease. Detection of plasma PTX3 level may be contribute to the diagnosis, treatment and prognostic judgment of acute cerebral infarction.

3.Comparison of escitalopram with citalopram in treatment of depression

Objective To evaluate efficacy and safety of escitalopram in treating depression .Methods In this randomized , single-blind, citalopram parallel controlled, fixed dosage study, the study group was treated with escitalopram tablet 10mg/d and the control group with citalopram tablet 20mg/d.Observation duration was 6 weeks.Hamilton Rating Scale for Depression ( 17-item HAMD),Clinical Global Impression (CGI) and Hamilton Rating Scale for Anxiety (HAMA) were used to measure the efficacy.Items including Treatment Emergent Symptoms Scale (TESS), lab examination and vital signs for assessment of safety .Results Eighty-eight patients were randomly divided into study group (43 patients) and control group (45 patients).After 6 weeks treatment, a signif-icant greater reduction in HAMD 17 ,CGI-SI and HAMA total score was found for both study group and control group showing significant difference with their baseline ( P <0.01 );but there were no significant difference between two groups ( P >0.05 ) .However , after 1 week treatment , the HAMD17 and CGI-SI score reductions between two groups were significant difference ( P <0.01 ) .The score of HAMA was no significant difference between two groups ( P >0.05 ) .In study group , the positive response rates was 81.4%,and in controls was 80.8%, there was no significant difference between two groups ( P >0.05 ) .There was no significant difference about the incidence of adverse reaction between two groups ( P >0.05 ) .Conclusions Escitalopram is a safe and effective antidepressant as citalopram , however , it has a more rapid reaction than the latter .

5.Value of echocardiography in detection of coronary sinus in fetus

Objective To detect the normal values of the fetal coronary sinus(CS) in length(L) .diameter (D) and diameter-to-length ratio(D/L) using fetal echocardiography and assess the clinical significance of dilated CS. Methods Fetal echocardiography were performed in 218 normal fetuses from the 20th to the 40th week of gestation. The L,D and D/L of CS were measured and the standard values of the normal fetal CS were established. A second group of 8 fetuses with dilated CS were assessed compared to the values of normal fetuses. Results The L and D of normal fetal CS correlated well with the gestational weeks (Linear regression equation is L= - 0. 94 + 0. 4 weeks, r = 0. 661, P<0. 0001 ;D= - 0. 07 + 0. 08 weeks, r = 0. 433, P <0. 0001). And the D/L did not vary significantly throughout pregnancy(D/L = 0. 21 0.04, r = 0. 115, P = 0.08). Therefore, the D value of normal fetal CS is less than 3.9 mm and D/L is less than 0.3 throughout pregnancy (20-40 gestational weeks). Conclusions The values of D and D/L of fetal CS in prenatal echocardiography might be useful and important parameters for fetal cardiac or extracardiac anomaly.
6.Doppler ultrasonography in detection of middle cerebral artery peak systolic velocity for the diagnosis of fetal anemia

Objective To detect the normal value of the fetal middle cerebral artery peak systolic velocity (MCA-PSV) with Doppler ultrasonography, and to investigate the association between MCA-PSV and fetal anemia. Methods Doppler ultrasonography was performed in 535 normal pregnant women from the 14th to the 40th week of gestation. MCA-PSV was measured, and the standard value of the normal fetal MCA-PSV was established. Fetuses with abnormal high MCA-PSV were assessed with a standard of MCA-PSV>mean vale of gestational weeks+2 standard deviation or 1.5 multiples of mean value of gestational weeks. Results The value of normal fetal MCA-PSV correlated well with the gestational weeks (linear regression equation was: MCA-PSV=-9.955+1.561×weeks; r=0.795, P<0.0001). Using 1.5 multiples of the mean of different gestational weeks to diagnose the moderate and severe fetal anemia, 7 fetuses were screened out of 209 of obstetric ultrasonography; 4 of them had MCA-PSV>1.5 multiples of mean value of gestational weeks and were proved with induced labor. Conclusion The value of 1.5 multiples of mean value of gestational weeks of fetal MCA-PSV in prenatal ultrasonography might be an important clue for the diagnosis of fetal anemia.
7.Comparison of cordocentesis and intrahepatic umbilical vein sampling with ultrasonographic guidance in the ovine fetus

Objective To compare success and complication rate of cordocentesis (CC) with intrahepatic umbilical vein sampling (IUVS) under the guidance of ultrasonography in ovine fetus. Methods Twenty near-term singleton pregnant ewes were randomly divided into CC group and IUVS group (each n=10). All of the ewes underwent CC or IUVS in ovine fetus guided with ultrasonography. Success and complication rate of CC and IUVS were calculated. Results Success rates of IUVS on the first, less than 3 times and 5 times attempt was 32.00%, 68.00% and 84.00%, respectively, higher than that of CC (15.00%, 40.00% and 75.00%, respectively, P<0.05). Sever complication rates of IUVS in comparison with CC was not statistically different (P>0.05). Conclusion Fetal IUVS may be a reasonable option to obtain fetal intravascular access and facilitate therapeutic intervention.

9.Clinical Observation of Calcitriol Soft Capsules Combined with Telmisartan Tablets in the Treatment of Early Diabetic Nephropathy

OBJECTIVE:To observe the clinical efficacy and safety of Calcitriol soft capsules combined with Telmisartan tab-lets in the treatment of early diabetic nephropathy(DN),and the effect on the levels of inflammatory factors. METHODS:Totally 110 patients with early DN were randomly divided into observation group and control group. The control group was orally given Telmisartan tablets with the initial dose of 40 mg,qd,and the maximum dose was 80 mg,qd;the observation group was orally giv-en Calcitriol soft capsules 0.25μg based on the treatment of control group,qd. The course was 1 month. The clinical data was com-pared,including the clinical efficacy and 24 h urinary protein,serum creatinine(Scr),urinary albumin excretion rate(UAER),se-rum C reactive protein (CRP),tumor necrosis factor-α(TNF-α) and interleukin-6 (IL-6) before and after treatment. The adverse reactions were observed. RESULTS:After treatment,the total effective rate in observation group was significantly higher than con-trol group,with significant difference (P<0.05);the 24 h urinary protein,Scr,UAER,and levels of CRP,TNF-α and IL-6 in observation group were significantly lower than control group and before treatment,with significant differences(P<0.01 or P<0.05). There were no obvious adverse reactions in 2 groups. CONCLUSIONS:Calcitriol soft capsules combined with Telmisartan tablets has better efficacy than only Telmisartan tablets in the treatment of DN,and can more effectively improve the levels of CR, TNF-αand IL-6,which is helpful to delay progression of patients.
10.Establishment of an electronic medical records system and integration of clinical data

This paper introduces the implementation of an electronic medical records system and the integration of clinical data in our hospital. The electronic medical records system is grounded on XML based electronic inpatient records and focused on information of patient treatment, which has integrated information of prescriptions , outpatient visits, PACS, medical examination technologies, laboratories, nursing records, reports of infectious diseases and hospital infections, licenses for antibiotics and pharmaceutical repositories, and the clinical data with electronic medical records as the carrier. This system has provided physicians with effective tools for diverse paperless medical records and favorable conditions for diagnosis and treatment, it has improved the work efficiency of physicians, optimized hospital process, minimized errors and improved quality of medical treatment. With the development of real-time quality control of medical records, we have improved efficiency and quality of hospital management.
