Journal of Medical Research 2006;0(09):-
Objective To establish the prevalence of renal involvement in a large group of patients with type 2 diabetes and evaluate several risk factors of early stages and advanced stages of albuminuria. Methods Five hundred and sixty-six patients(265 males, mean age:57.8?13.3 years, diabetes duration 8.35?6.4 years;301 females, mean age:60.0?12.4 years, diabetes duration 9.9?7.3 years) from the endocrinology unit of Beijing Tongren hospital were included in this study. Urinary albumin excretion rate(UAER),age ,diabetes duration, arterial blood pressure, body mass index(BMI),waist to hip ratio(WHR),fasting blood glucose(FBG),HbA1c,lipid profile were evaluated. Results The prevalence of microalbuminuria, macroalbuminuria was 24.6% and 16.8% respectively. On logistic regression analysis, using the presence or absence of microalbuminuria as dependent variable, arterial blood pressure and, fasting blood glucose were included in the model in both males and females. When analyzing patients with microalbuminuria vs those with macroalbuminuria, in females, arterial blood pressure and triglycerides were included in the model. In males, only blood pressure was included in the mole. Conclusions High fasting blood glucose and arterial blood pressure were independent risk factors of microalbuminuria in type 2 diabetic patients .High arterial blood pressure and disorder in lipid profile were independently associated with macroalbuminuria.

BACKGROUND: Decoy receptor 1 (DcR1), which acts as decoy receptors, is resistant to apoptosis induction by related apoptosis inducing ligand (TRAIL). OBJECTIVE: To investigate the expression of the DcR1 (TRAIL-R3) protein in human herniated and normal lumbar intervertebral discs (IVD). METHODS: The expression and distribution of DcR1 proteins were assessed by using immunohistochemistry in 20 herniated lumbar IVD and 8 normal lumbar IVD tissues samples following discoidectomy from January to September 2010. RESULTS AND CONCLUSION: Percentage of nucleus pulposus and anulus fibrous cells with DCR1 expression was obviously more in the herniated group than in normal IVD group. The current results indicate that the expression of DCR1 rises after herniation.

The discovery of microRNA (miRNA) has opened up a new train of thought for the diagnosis and treatment of esophagus cancer.let-7 is one of the most widely research of miRNA.In a variety of tumors, the expression of let-7 is down-regulated.let-7 can play the role of tumor suppressor gene by targeting to high mobility group A2 (HMGA2) and inhibit the cell proliferation.Recent studies have shown that the lower the degree of differentiation of cells,let-7 expression level is lower. It is expected as a symbol of poorly differentiated tumors. In addition,let-7 and esophagus cancer's radiation and chemotherapy sensitivity are closely related.

Objective To study the relationship between the polymorphism of N 5,10 methy lenetetrahydrofolate reductase (MTHFR) gene and the susceptibility to diabetic microangiopathy (DMAP) in type 2 diabetes mellitus (DM) of Han nationality in north China. Methods By PCR RFLP, MTHFR C677T mutation was detected in 229 type 2 diabetic patients, including 102 non complication diabetics (NCD group), 67 diabetic nephropathy (DN group) and 60 diabetic retinopathy (DR group), and 62 healthy persons as control (CON group). Results The frequency of homozygous BB genotype and allele B in DN and DR were significantly higher than that in NCD and CON, but NCD and COD showed similar frequency of BB genotype and allele B. Conclusion In type 2 DM of Han nationality in north China, there is a strong correlation between the polymorphism of MTHFR gene and DMAP, and the allele B may be a susceptibile gene of DMAP.

Objective To investigate the etiology of papular urticaria: is it caused only by arthropod-like insects-bite allergy, or by multiple factors such as food allergy, disturbance of gastrointestinal function and infection? Methods We searched, by computer and manually, the foreign and domestic literature related to the etiology of papular urticaria published since 1950s, and according to the methods of evidence-based medicine, systematically evaluated the evidence supporting either the insect-bite theory or the multiple factor theory. Results Twenty-nine articles ( 22 English and 7 Chinese ) supported the theory of hypersensitivity to bites from certain insects such as mosquitoes, gnats, fleas, mites, bedbugs etc. Two articles in Chinese mentioned the possibilities other than insect-bite, but the reliability was unconvincing, because the authors did not present the source of data or statistical methods used in the articles. The evidence from epidemiology, histopathology, laboratory and clinical researches all supported the insects-bite theory. No proven evidence was found supporting other aetiological hypotheses. Conclusion Our results suggest that papular urticaria is caused only by the allergy to stings or bites of arthropods, and other hypotheses still lack convincing evidence.

Objective To explore the effects of embolizing intracranial ruptured aneurysm in early stage using Guglielmi Detachable Coil (GDC). Methods 10 intracranial ruptured aneurysms including 8 cases of the first ruptured and 2 cases of the second ruptured were studied. The locations of aneurysm were as following: 1 case in anterior cerebral artery, 1 case in middle cerebral artery, 5 cases in anterior communicating artery, 2 cases in posterior communicating artery, and 1 case of multianeurysm in anterior and posterior communicating artery. Hunt-Hess grade was as following: 2 cases of gradeⅠ,4 cases of gradeⅡ, 2 cases of gradeⅢ, 1 case of grade Ⅳ, and 1 case of grade Ⅴ. Fisher CT scale was as following: 2 cases of gradeⅠ, 6 cases of gradeⅡ,and 2 cases of grade Ⅲ. Embolization of intracranial ruptured aneurysm was performed in early stage using GDC, 7 cases in 3 days after ruptured, and 3 cases in 6 days after ruptured. Results Among 11 aneurysms in 10 cases, 8 cases were 100% embolization, 2 cases≥95%, and 1 case≥90%. Followed-up from 1 month to 2 years after operation 1 case was dead, 7 cases recovered well, 1 case was in vegetable state, and 1 case occurred subarachnoid hemorrhage again. Conclusions GDC early embolization is preferable technique for treating intracranial ruptured aneurysm.

Objective To explore the microneurosurgical treatment of recurrent intra-spinal canal tumors. Methods The clinical data on the microsurgical parameters, operative procedures, operative efficacy and post-operative complications of 16 cases with recurrent intra-spinal canal tumors from June 1993 to June 2002 were reviewed retrospectively. Results Under the operative microscope, the tumors were totally removed in 10 cases, subtotally in 4 cases, and partially in 2 cases. Fllowed up from 5 months to 8 years, 11 cases recovered well, 4 cases suffered from moderate disability, and 1 case died. Preoperative CT or MRI scan, knowledge of the anatomy and structures, and skillful operative techinique all were important to microsurgical treatment of recurrent intra-spinal canal tumors. Conclusion Once diagnosis of recurrent intra-spinal canal tumors, the operative treatment is effective. Radiation therapy for the malignant tumors or incompletely removed recurrent tumors is necessary.

Objective To investigate clinical presentation, diagnosis, gestational outcome and treatment of Eisenmenger’s syndrome during pregnancy. Methods Two patients with Eisenmenger’s syndrome during pregnancy were reported retrospectively. Results The first case was admitted in labor at 34 weeks and 4 days in pregnancy,She had dysphoria, tachypnea and dyspnea right after vaginal delivery and died immediately The diagnosis of pulmonary embolism was highly suspected clinically. The second case was admitted because of fetal growth restriction Cesarean section was performed after cyanosis worsened at 35 weeks and 4 days She was discharged from the hospital two weeks after delivery. Conclusion Eisenmenger’s syndrome during pregnancy was associated with extremely poor maternal and fetal outcome Pregnancy is contraindicated in such patients Contraception is necessary after marriage Pregnancy should be terminated as early as possible in such women. In cases who intends to continue the pregnancy, closely observation must be provided to ensure the safety for both mother and the fetus.
