1.Relationship between acute myocardial infarction and original sites of premature ventricular contrac-tion

Objective:To explore the characteristics of original site of premature ventricular contraction (PVC) in pa‐tiets with acute myocardial infarction (AMI) .Methods:Among ECG‐detected single‐source PVC patients ,a total of 150 idiopathic PVC patients and 150 AMI patients were randomly selected as control group and AMI group respec‐tively .Original sites of PVC was compared between AMI group and control group and between left and right ventri‐cle of AMI group ,and the relationship between original sites of PVC and diseased coronary in AMI patients was an‐alyzed .Results:Compared with control group ,there was significant rise in percentage of PVC originated from left ventricular apex (6.0% vs .46.0% ) ,and significant reductions in percentages of PVC originated from right ventric‐ular outflow tract (54.7% vs .27.3% ) and right ventricular anterior wall (10.0% vs .3.3% ) in AMI group , P<0.05 or <0.01. In AMI group ,compared with PVC originated from right ventricular outflow tract ,there was sig‐nificant rise in incidence rate of PVC originated from left ventricular apex (27.3% vs .46.0% ) , P=0.001 ;com‐pared with right ventricle , there was significant reduction in incidence rate of outflow tract PVC (66.1% vs . 13.6% ) ,and significant rise in incidence rate of apex PVC (24.4% vs .78.4% ) in left ventricle , P<0.01 both . Relevance test analysis of two‐way disordered classified data indicated that diseased coronary was not related to orig‐inal sites of PVC in AMI patients (χ2 =0.519 ,P=0.134) .Conclusion:Original sites of premature ventricular con‐traction has its own characteristics in patients with acute myocardial infarction ,which may possess certain guidance significance for its treatment .
2.Analysis of hospitalization expenses of malignancy in Shanxi Cancer Hospital from 2003 to 2010

Objective To understand the malignancy hospitalization expense of malignancy in Shanxi Cancer Hospital and provide a reference for the effective control of hospitalization expense.Methods 89 716 malignancy hospitalization cases in Shanxi Cancer Hospital from 2003 to 2010 were reviewed.Hospital costs of top six malignant tumors were analyzed.Results The hospitalization expense of 6 kinds of malignancy increased in varying degrees.In all cancer patients,the top six number of cases were lung cancer,cervical cancer,stomach cancer,breast cancer,esophageal cancer and colon cancer,there were a total of 89 716 passengers.The growth rate of cervix malignancy' s expense was the fastest.Conclusion Malignancy hospitalization is expensive,comprehensive measures need to be used to control the expense and make full use of health resources.

4.Risk factors of portal venous thrombosis after laparoscopic splenectomy for hypersplenism in cirrho-tic portal hypertension

Objective To investigate the risk factors of portal venous thrombosis ( PVST) following laparoscopic splenectomy ( LS) in cirrhotic patients with hypersplenism .Methods The clinical data of 62 patients with secondary hypersplenism due to cirrhotic portal hypertension admitted to the Clinical Medical College of Yangzhou University were retrospectively analyzed .These patients underwent LS from January 2013 to April 2014.Based on whether the patients had PVST on postoperative day (POD) 7 or not, the pa-tients were categorized into the non-PVST and PVST groups .Fifteen perioperative variables were analyzed in these two groups .Results On univariate analysis , age and portal vein diameter in the PVST group were significantly higher and velocity of portal blood flow in the PVST group were significantly lower than those in the non-PVST group, (all P<0.05).Bivariate correlation analysis performed for the relationship between these variables and occurrence of PVST on POD 7 showed age >50 years, portal vein diameter >13 mm and velocity of portal blood flow >18 cm/s were significantly correlated .Multivariate logistic regression showed that age >50 years and portal vein diameter >13 mm were significantly independent risk factors of PVST, and velocity of portal blood flow >18 cm/s was an independent protective factor .Conclusion Age>50 years and portal vein diameter >13 mm were independent risk factors , while velocity of portal blood flow >18 cm/s was an independent protective factor of postoperative PVST .
5.Study on characteristics of physical self of head and neck cancer patients with depression

ObjectiveTo study the relationship between depression and physical self and related factors of head and neck cancer patients (HNCPs).MethodsOne hundred and twenty-eight HNCPs were assessed with self-rating depression scale,physical self scale,negative physical self scale and symptom checklist-90 (SCL-90).SPSS 16.0 was used to analyze the data.ResultsThe evaluation of appearance,sport characters and figure characters were significantly different between sexual groups,the average physical self scale level of the male patients was higher than that of the females (3.33 ± 0.56 vs.2.87 ± 0.32,P < 0.05).The appearance,sport characters,figure characters and sexual characters of physical self-concept were significantly different between age groups,the body satisfaction scale from higher to lower were under 45 years old patients,beyond 60 years old,between 46 and 59 years old.ConclusionThe evaluation of appearance is reversely correlated with the score of SCL-90,the evaluation of short,appearance and fat in negative physical self are positively correlated with the score of SCL-90.
6.Diagnosis of intracranial dermoid cyst with spiral CT

Objective To explore diagnosis value of spiral CT on intracranial dermoid cyst based on retrospective analysis.Methods 10 cases of intracranial dermoid cyst were enrolled in and confirmed by surgery and pathology.The retrospective analysis was based on the CT manifestations,causes,pathological and clinical features.Results Among the 10 cases,5 lesions were located in the posterior cranial fossa,3 in besides saddle,1 in up saddle and 1 in the temporal fossa.The morphology of nidus was round or round like with clear boundary.There was no edema around the nidus.Fat was found in 7 cases appeared hypodensity with insufficiency uniformity on CT,and the density was lower than that of cerebrospinal fluid with CT value ranged from-6 HU to 80 HU.There was a few calcification on the edge in 1 case.Hyperdensity irregular and lumpy hair-liked shadow was found in 2 hypodensity cases.1 lesion closed to the cranial plate was compressed thin and buckled.Conclusions Intracranial dermoid cyst has typical CT manifestations.It will be an accurate diagnosis based on clinical analysis and CT manifestations.
7.Postoperative complications of laparoscopy-assisted D2 radical total gastrectomy for gastric cancer

Objective To evaluate postoperative complications of laparoscopy-assisted D2 radical total gastrectomy for gastric cancer as compared with open procedures. Methods In this study,358 patients of gastric cancer undergoing laparoscopy-assisted D2 total gastrectomy or open D2 total gastrectomy between January 2011 and December 2012 were retrospectively reviewed and analyzed.Patients were non-randomly divided into laparoscopic group ( LAP,n =165 ) and open surgery group ( OPEN,n =193 ).Operative time,intraoperative blood loss,postoperative hospital stay and complications were compared between the two groups. Results Operative time [ 225 ( 195 - 340 ) min vs.230 ( 195 - 300 ) min,P >0.05 ] and number of lymph nodes dissected [ (26 ± 4) vs.(27 ± 4 ),P > 0.05] between the LAP group and the OPEN group were not significantly different.Compared with OPEN group,blood loss in the LAP group was less [ 160 ( 80 - 600 ) ml vs.270 ( 150 - 600) ml,P < 0.01 ] and postoperative hospital stay was shorter [ ( 11.4 ± 2.6) d vs.( 13.7 ± 2.4) d,P < 0.01 ].Postoperative complications developed in 18 patients in the LAP group and in 30 patients in the OPEN group,the difference was not statistically significant between the two groups. Conclusions In the hands of experienced surgeons laparoscopy-assisted D2 radical total gastrectomy for gastric cancer is safe,effective and miniinvasive.
8.Study on the factors influencing the quality of life among patients with breast cancer

Objective To describe the quality of life of breast cancer patients and explore the risk factors for their quality of life (QOL). Methods QOL of 260 patients with breast cancer were studied using FACT-B, SCL-90, Social Support Rating Scale and multiple linear regression models to study the main factors influencing the QOL of patients with breast cancer on functional scales (physical, social, emotional, role). Results Economic conditions (β =2.024, P =0.003), emotion regulation ability (β =4.993, P =0.000) and self-evaluation of marital quality (β =-3.954, P =0.001) were associated with total QOL scores, emotion regulation ability (β =0.987, P =0.008), the number of emergency incidents (β=-0.866, P =0.011) were associated with physical domain scores, economic conditions (β =1.016, P =0.000), marital status (β =-1.018, P =0.041), emotion regulation ability (β =0.971, P =0.008), self-evaluation of marital quality (β =-1.382, P =0.001) were associated with social and family domain scores, emotion regulation ability (β =1.094, P =0.000), self-evaluation of marital quality (β =-0.892, P =0.006) were associated with emotional domain scores, regional factors(β =1.653, P =0.008), emotion regulation ability (β =0.962, P=0.01), self-evaluation of the QOL (β =-1.705, P =0.000) were associated with function domain scores. Conclusion Guiding the patients to have a correct understanding of the breast cancer, strengthening their confidence of life, increasing physical activity, having a good family environment and necessary social welfare so that improving the QOL of the patients.
9.Spleen-preserving distal pancreatectomy: 18 cases report

Objective To explore the feasibility and effect of spleen-preserving distal pancreatectomy in patients with distal pancreatic injures and its outcome.Methods Retrospectively analysed the follow up results of 18 patients undergoing spleen-preserving distal pancreatectomy in Clinical Medical College of Yangzhou University from March 2008 to November 2012.Results The operations were successful in all of these 18 patients,B-mode ultrasonography and CT scan follow-up revealed that there were no significant changes in the size and structure of the spleens.The operation time was 152 to 188 minutes (mean,172 minutes),and the intraoperative blood loss was 155 to 356 mL (mean,191 mL).The length of postoperative hospital stay was 13 to 19 days (mean,15 days).No bleeding after operation,no pancreatic leakage,and no intraabdominal infection occurred.Conclusions Distal pancreatectomy with spleen and supply vessel preserving is effective and feasible methods for the patients with distal
10.The inhibitory effect of DPC4 gene expression on growth of pancreatic cancer cells in vitro

Objective To observe the inhibitory effect of DPC4 gene on growth of pancreatic carcinoma in vitro. Methods The human DPC4 cDNA was subcloned to the retroviral vector pLXSN and then packaged with GP+E86 and PA317 packaging cells respectively. AntiG418 clones were acquired and named as PA317/ pLXSN DPC4+ cells. The DPC4 gene was restored to the human pancreatic cancer cell line BxPC-3 by the infection of the pLXSN/DPC4 and then had a stable expression after antiG418 selection, which was demonstrated by RT-PCR and Western blot. The inhibitory action of DPC4 gene expression on growth of these daughter cells was observed.Results DPC4/Smad4 gene integration in GP+E86、PA317/ pLXSN DPC4+ cells was detected by polymerase chain reaction. The BxPC-3 cells, which were null for DPC4, had stable expression of DPC4 after the infection of the pLXSN/DPC4. The expression of DPC4 gene was able to inhibit the growth of these cancer cells in vitro and downregulate the VEGF mRNA level.Conclusions This study suggested that there can be marked inhibition of growth and angiogenetic ability of pancreatic cancer cells after infection by retrovirus vector containing DPC4.
