1.Effect of CD40 ligandization on breast cancer cell line and endothelial vein cell lline in vitro

Purpose:To investigate the effect of CD40 ligandization on breast cancer cell line and endothelial vein cell line.Methods:The expression of CD40 and its ligand on breast cancer cell line(M231),EVC cell line,fresh clinic breast cancer cell were determined by indirect immunofluorescence assay with flow cytometry(FCM) analysis.Then M231 cells cultured with CD40 agonsitic monoclone antibody,adriamycin alone or in combinations for 72 hours and proliferation of M231cells was determined by MTT assay.FCM was employed to study the cells' death or apopotosis with Annexin V PI assay.Results:M231 and ECV cell line and fresh clinic breast cancer cell are highly expressed CD40 but no CD40L.CD40 ligandization can not only inhibit the proliferation of M231 cell line but also inhibit ECV cell line by promoting the death or apoptosis of these cells.Combined CD40 agonsitic monoclone antibody with adriamycin may obviously inhibit proliferation of M231 and ECV cell line.Conclusions:CD40 ligandization may have double therapeutic effect to breast cancer: inhibit the proliferation of breast cancer cell and inhibit tumor angiogenesis by inhibiting vascular endothelial cell.
2.The research on the effects of three drainage methods afteroperation of breast cancer

Objective In order to study the effects of three drainage methods after operation of breast cancer. Methods We demonstrated the drainage reliability of the negative pressure bottles by measuring their negative pressures.We reviewed the incidence of flap dropsy of 103 patients with breast cancer operated from 1988 to 2001.Results Negative pressure of the bottle made by ourselves is 83.33 kPa at the room temperature and 74.12 kPa after 200ml fluid being drained. The incidence of skin flap dropsy of the negative pressure bottle is significantly lower than those in another two groups.Conclusion The negative pressure bottle is reliable, convenient and applicable in all levels of hospital.
3.Economic Benefit of Interferon in Treatment of Chronic Hepatitis B Virus Infection

OBJECTIVE:To evaluate the economic burden of chronic hepatitis B(CHB)and its complications,and to e-valuate the benefits of treatment with interferon-?.METHODS:The components of the economic burden of disease included direct medical costs and non-medical costs as well as indirect economic loss per patient-year in patients with chronic hepatitis B virus infections,including chronic hepatitis B,compensated and decompensated cirrhosis,and hepatocellular carcinoma.Net cost savings due to treatment of CHB with interferon-?were estimated using clinical data from2therapeutic schemes used in hospital.RESULTS:For the patients at different stages of hepatopathy,the direct medical cost and economic burden per year were different:9000yuan,11362yuan(CHB);13865yuan,19412yuan(compensated cirehosis);25678yuan,36979yuan(decompensated cirrhosis);26501yuan,40264yuan(liver cancer),Compared with conrentional therapy against CHB,the direct medical cost and total cost were reduced following4months treatment of interferon-?after5-year follow-up.The reduced amounts were831yuan,2038yuan per person,respectively.The surrival rate rised by2.8%.CONCLUSION:CHB infections create a heavy economic burden on the society,as well as patients and their families.We have seen the potential cost-savings due to treatment with interferon-?.
4.Effect of health management in risk behavior control of chronic diseases

Three hundred and ninety-five administrative staffs were randomly selected and provided health management to evaluate its effects in risk factors control of chronic diseases. At 1 year, the rate of people health knowledge and attitude were increased from 14.2% to 39.2%, 7.1% to 25.1%, P≤0.01 ; risk behavior exposure, including incorrect dietary intake, sedentary habit, alcohol consumption or smoking, reduced to some extent (P<0.05).
5.Survey and Analysis of Label Items in 32 Biological Products Injection for Treatment Instructions

OBJECTIVE:To provide reference for the further standardization of biological products injection for treatment in-structions. METHODS:32 biological products injection for treatment instructions collected from Ma’anshan Center for Disease Control and Prevention and Ma’anshan Central Hospital during Jan. to Jun. in 2015. The information of marked item was summa-rized and analyzed. RESULTS & CONCLUSIONS:Among 32 biological products injection for treatment instructions, the mark rates of drug names,main ingredients,character,indications,specification,usage and dosage,package,term of validity,opera-tive norm,license number,manufacturing enterprise and other items all reached 100%. Although the mark rates of ADR,contrain-dication,precaution,drug use of pregnant women and nursing mothers,drug interactions,pharmacokinetics,drug use of the elder-ly,storage and other items were relatively high,but some items lacked the specific description of the content. The mark rates of drug use of children,drug overdose,toxicology,warnings,clinical trial were 56.25%,62.50%,59.38%,37.50% and 18.75%. Some biological products injection for treatment instructions are not revised timely and not complete in content and non-standard in writing,which can not meet the needs that clinical pharmacists and patients get enough drug safety information from instructions. Manufacturing enterprise is suggested to label the content of package inserts completely,verify and supplement related content, standardize and improve the instructions.
6.Diagnosis and treatment of gastroparesis after abdominal surgery

Objective To investigate the possible contributing risk factors, diagnosis and treatment of gastroparesis after abdominal surgery. Methods Potential risk factors, clinical manifestations and therapeutic approaches of 22 cases of gastroparesis after abdominal operation were analyzed retrospectively. Results In this series, gastroparesis more frequently occured in those patients having a long-term preoperative gastric outlet obstruction, malignant tumor, O type blood group pancreaticoduodenectomy or simple gastrojejunostomy without gastric resection. Endoscopy, upper gastrointestinal radiography and radioisotope scintigraphy were effective methods in the diagnosis of gastroparesis, and normal gastrointestinal movement was achieved mostly in 5 weeks postoperatively by conservative treatments.Conclusions Postoperative gastroparesis can recover spontaneously after a longer period of supportive treatment combined with prokinetic drugs, and reoperation should be avoided.
7.To develop a ring ligator and to discuss the application of it in velopharyngeal ring ligation procedure

Objective To study how to improve velopharyngeal ring ligafion procedure by using ring ligator in velopharyngeal ring ligatian procedure and settle the problem that there is no standard to measure the interspace of the cavity of pharynx.Methods Select some deft palate patients who were between 3 to 17 ages.Then made five groups according to age (3 to 5,6 to 8,9 to 11,12 to 14,15 to 17).Then made five different types of ring ligators accrding to the normal size of the cavity of pharynx of different ages.Results The ring ligators were easily used and all the patients were repaired satisfied.Conclusion The application of ring ligator in velopharyngeal ring ligation procedure is simple and feasible.
8.Primary effects of comprehensive lifestyle modification on status of pre-chronic diseases

Objective To evaluate the effectiveness of comprehensive lifestyle modification in the primary health care system on status of pre-chronic diseases in a conventional healthcare program.Methods Two hundred and twenty-nine adult volunteers in 5 conventional healthcare centers were divided into 4 groups:group of prehypertension(n=67),group of prediabetes(n=38),group of abnormity serum cholesterol(n=71)and group of osteopenia(n=53).A muhicomponent behavioral intervention including education,physical activity,dietary practice,cease smoke,calcium supplement and exposure to sunlight was conducted for 12 months.The main outcome of each group was blood pressure,blood glucose,serum cholesterol and bone mineral density.Results Over 12 months,compared with the status before intervention,participants in each group were statistically significantly improved,the mean net reduction in systolic BP/diastolic BP was 9.4 mm Hg/6.6 mm Hg(1 mm Hg=0.133 kPa,t=5.93/8.29.P<0.05)in group of prehypertension;the reductions of blood glucose,serum cholesterol were 0.9 mmol/L(t=3.69,P<0.05)and 0.5 mmol/L(t=4.09,P<0.05)in group of prediabetes and group of abnormity serum cholesterol,and the increase of bone mineral density on lumbar spine was 0.015 g/cm2(t=2.03,P<0.05)in group of osteopenia,Conclusions For chronic disease prevention,ater one year the comprehensive lifestyle intervention produced beneficial changes in diet,physical activity,and biochemical parameters.This type of intervention is a feasible option to prevent chronic disease and should be implemented in the primary health care system.

OBJECTIVE:
To study the effectiveness of preserving laryngeal function for senile hypopharyngeal cancer patientsolder than 65.
METHOD:
The clinical data of 58 surgery cases of senile hypopharyngeal cancer patients more than 65 years old were colleted and analyzed. Thirty-one cases preserved the laryngeal function, while the rest did not. Perfect preoperative preparation was done before surgery. Surgical resection specimens were sent to frozen-section examination. When the negative incisal margin was confirmed, the defect was repaired by the appilication of local stitching, ribbon muscle flap, major myocutaneous flaps, split thickness skin and replacement of esophagus by stomach. Radical radiotherapy was used after surgery. Survival rate was calculated by the Kaplan-Meier method. Chi-square test was used to compare complications of the two groups.
RESULT:
The 3 years and 5 years survival rate for all cases were 48.3% (28/58) and 27.6% (16/58), respectively. For patients with laryngeal function preservation, the 3 years and 5 years survival rate were 51.6% (16/31), 29.0% (9/31), respectively. For cases without laryngeal function preservation, the 3 years survival rate and 5 years survival rate were 44.4% (12/ 27), 25.9% (7/27), respectively. The result showed no obvious difference in survival rate between two groups (P > 0.05). Surgery complication rate were 45.2% (14/31) and 40.7% (11/27), without obvious differences between the two groups (P > 0.05).
CONCLUSION
It is feasible for senile hypopharyngeal cancer patients to choose suitable operation based on their physical conditions and the tumor extension. The key issues include well perioperative treatment management, correct indications grasp, and intraoperative repair skills improvement.
