1.Clinical study on treatment of senile severe pneumonia by Qingre-Tongfu enema combined with non-invasive ventilation with syndrome of excess of Yangming fushi

Objective:To investigate the effect of Qingre-Tongfu enema combined with noninvasive positive pressure ventilation on the ventilation function of elderly patients with severe pneumonia. Methods:A total of 60 elderly patients with severe pneumonia with syndrome of excess of Yangming fushi from September 2017 to August 2019 in the respiratory department of the First People’s Hospital of Dongcheng District, Beijing were randomly divided into two groups, 30 cases in each group. The control group was treated with western medicine to control infection and noninvasive positive pressure ventilation. The treatment group was treated with Qingre-Tongfu enema on the basis of the control group. The Traditional Chinese Medicine Syndrome score (TCMSSS), Acute Physiology and Chronic Health Evaluation Ⅱ(APACHE-Ⅱ) and Clinical Pulmonary Infection Score (CPIS) were compared before and after treatment, The Pneumonia Severity Index (PSI) was compared. Serum procalcitonin (PCT) was detected by upconversion luminescence immunoassay, CRP was detected by double antibody sandwich ELISA, WBC was detected by automatic hematology analyzer; partial pressure of carbon dioxide (PaCO 2), oxygen saturation (SaO 2), partial pressure of oxygen (PaO 2) were detected by blood gas analyzer, oxygenation index (OI) and respiratory rate (RR) were calculated. The complications were investigated and the clinical efficacy was evaluated. Results:The total effective rate was 96.7% (29/30) in the treatment group and 73.3% (22/30) in the control group, with significant difference between the two groups ( χ2=4.706, P=0.030). After treatment, the levels of serum PCT, CRP and WBC in the treatment group were significantly lower than those in the control group ( t=15.359, 25.784 and 13.460, respectively, P<0.01); after treatment, the levels of SaO 2 [(93.18 ± 3.79)% vs. (88.78 ± 5.56)%, t=3.584], PaO 2 [(86.81 ± 4.01) mmHg vs. (80.01 ± 4.76) mmHg, t=5.975], OI [(285.53 ± 15.05) mmHg vs. (227.65 ± 12.37) mmHg, t=16.272] in the treatment group significantly were higher than those in the control group ( P<0.01); PaCO 2 [(43.28 ± 6.84) mmHg vs. (48.83 ± 7.66) mmHg, t=-2.956], RR [(22.00 ± 3.79) times/min vs. (26.30 ± 3.73) times/min, t=-4.434] in the treatment group were significantly lower than those in the control group ( P<0.05). After treatment, the scores of TCMSSS, APACHE-Ⅱ, CPIS and PSI in the treatment group were significantly lower than those in the control group ( t=41.310, 11.035, 8.399, 5.752, P<0.01). The treatment group in the course of antibiotics, mechanical ventilation time, hospital stay were significantly shorter than the control group ( P<0.01). Conclusion:Qingre-Tongfu enema combined with noninvasive positive pressure ventilation can improve the clinical symptoms of elderly patients with severe pneumonia, reduce inflammatory reaction, shorten the time of mechanical ventilation and antibiotic treatment, and improve the clinical efficacy.
2.Clinical Observation on 29 Cases of HIV Infection and AIDS Treated with Zhong Yan No. 2

All patients were treated with Zhong Yan No. 2 Prescription, 3 months constituting one course. Indexes of immunologic functions before and after treatment were compared and HIV -Rt - PCR detection was made in 16 cases. Results indicated that in absolute number of CD4, 8 cases increased, 14 cases decreased and 7 cases did not change; in CD4/CD8 ratio, 2 cases increased, one case decreased and 16 cases did not change; Two cases had increase of both CD4 and CD4/CD8 ratio: 4 cases had a decrease of new pterin, 11 cases increased and one case no change; 6 cases had a decrease of HIV - Rt - PCR, 8 cases increased and 2 cases no change. The total effective rate was 48.28% based on comprehensive assessment of virus, immunity, body weight and symptoms.

4.Survey and Analysis of Label Items in 32 Biological Products Injection for Treatment Instructions

OBJECTIVE:To provide reference for the further standardization of biological products injection for treatment in-structions. METHODS:32 biological products injection for treatment instructions collected from Ma’anshan Center for Disease Control and Prevention and Ma’anshan Central Hospital during Jan. to Jun. in 2015. The information of marked item was summa-rized and analyzed. RESULTS & CONCLUSIONS:Among 32 biological products injection for treatment instructions, the mark rates of drug names,main ingredients,character,indications,specification,usage and dosage,package,term of validity,opera-tive norm,license number,manufacturing enterprise and other items all reached 100%. Although the mark rates of ADR,contrain-dication,precaution,drug use of pregnant women and nursing mothers,drug interactions,pharmacokinetics,drug use of the elder-ly,storage and other items were relatively high,but some items lacked the specific description of the content. The mark rates of drug use of children,drug overdose,toxicology,warnings,clinical trial were 56.25%,62.50%,59.38%,37.50% and 18.75%. Some biological products injection for treatment instructions are not revised timely and not complete in content and non-standard in writing,which can not meet the needs that clinical pharmacists and patients get enough drug safety information from instructions. Manufacturing enterprise is suggested to label the content of package inserts completely,verify and supplement related content, standardize and improve the instructions.
5.Effect of small molecule hydrogels on proliferation, apoptosis and myocardial differentiation of bone marrow mesenchymal stem cells

BACKGROUND:A short-peptide small molecule hydrogel (SMH) developed in the previous study has more obvious advantages than other hydrogels to improve local microenvironment, carry bioactive substances and interfere with stem cell signal transduction pathways. OBJECTIVE:To explore the effect of SMHs on bone marrow mesenchymal stem cells (BMSCs) proliferation, apoptosis and differentiation into myocardial cells. METHODS: (1) Passage 9 rat BMSCs in vitro were divided into control group and experimental group, followed by routine culture and culture in SMHs, respectively. At 7 days of culture, cell proliferation and apoptosis were detected. Cells in the two groups were exposed to anaerobic environment for 12 hours, and expression levels of Bcl-2, Bax and Caspase-3 in BMSCs were detected. (2) Passage 9 BMSCs were divided into four groups and then cultured in 5-azacytidine, SMHs, SMHs+5-azacytidine, and L-DMEM (normal control), respectively. After 4 weeks of induction, expression of CTnT, desmin and Cx-43 proteins was detected and expression levels of early cardiac transcription factors, NKX2.5 and GATA-4, were also measured. RESULTS AND CONCLUSION: (1) Compared with the control group, better proliferation and lower apoptosis of BMSCs were found in the experimental group. Under anaerobic conditions, the number of survival cells was reduced in both groups, but less apoptosis or necrosis was found in the experimental group than the control group (P < 0.05). Moreover, the level of Bcl-2 was higher in the experimental group than the control group (P < 0.01), while the levels of Bax and Caspases-3 protiens were lower in the experimental group than the control group (P < 0.01). (2) NKx2.5 and GATA-4 mRNA expression was found in both 5-azacytidine and SMHs+5-azacytidine groups, and moreover, the mRNA levels of early cardiac transcription factors were significantly higher in the SMHs+5-azacytidine group than in the 5-azacytidine group (P < 0.05). In the normal control group, cTnT expressed negatively, and desmin and Cx-43 expressed weakly. The expression of cTnT, desmin and Cx-43 proteins was higher in the SMHs+5-azacytidine group than in the 5-azacytidine and SMHs groups, while there was no significant difference between the latter two groups. To conclude, SMHs as a culture medium is conducive to the proliferation of BMSCs, reduces cell apoptosis, and promotes myocardial differentiation of BMSCs.
6.Effect of health management in risk behavior control of chronic diseases

Three hundred and ninety-five administrative staffs were randomly selected and provided health management to evaluate its effects in risk factors control of chronic diseases. At 1 year, the rate of people health knowledge and attitude were increased from 14.2% to 39.2%, 7.1% to 25.1%, P≤0.01 ; risk behavior exposure, including incorrect dietary intake, sedentary habit, alcohol consumption or smoking, reduced to some extent (P<0.05).
7.Analysis in anxiety state of parents of children patients with hand foot mouth disease and nursing strategy

Objective To study the psychological health state of parents of children patients with hand foot mouth disease and provide scientific basement for clinical nursing care. Methods Zung self- rating anxi-ety scale (SAS) and self-designed investigation scale were used to survey 98 parents of children patients with hand foot mouth disease,and selective nursing intervention was adopted. Results The anxiety scores of the parents were significantly higher than those of the normal mode, and their psychological symptom was influ-eneed by various factors. After the nursing intervention, the anxiety degree was significant different from that of before intervention. Conclusions Psychological health levels of the parents of children with hand foot mouth disease were markedly lower than those of healthy individuals. Psychological nursing care should not only in-clude children but also their parents in order to alleviate their bad mental state.
8.Effect of Tonifying Kidney using Chinese Medicine(Guzhen Recipe) to Retard Aging of the Hippocampus and Hypothalamus-Pituitary-Adrenal-Thymic(HPAT) Axis in Senile Rat.

According to the TCM theory, human growth, development and aging are closely related to kidney function and the main cause of human senility is deficiency of “kidney essence”. Recent studies showed that the “kidney” in TOM include the regulatory function of the neuroendocrine and immune systems.
In this paper, the effect of Guzhen Recipe (_??__??__??_), an aging retarding prescription composed of kidney tonifying herbs such as Radix Polygoni Multiflori (_??__??_), Herba Cistanchis (_??__??_) Rubus Chiagii (_??__??__??_) etc. on the hippocampus and HPAT axis in senile rats were observed. The results showed : (1) With age the number of glucocorticoid receptors (GCR) in the hippocampus, the concentrations of NE, DA, 5-HT in the hypothalamus were markedly decreased, while the plasma corticosterone concentration in senile rats was obviously increased. (2) The thymocyte GCR nuclear transition rate was increased significantly ; (3) The contents of cytoplasmic protein, nuclear RNA and DNA, the thymus weight/body weight ratio, the serum thymic factors as well asthe rate of lymphocyte transformation, the activity of IL-2 and NK cells in senile rats were obviously decreased compared to those of young control rats. Guzhen recipe however, was capable of improving these changes in the neuroendocrine-thymic net work in senile rats to varying degrees, suggesting that the kidney with Chinese Medicine might retard the functional degeneration of thymic endocrine partly by improving the feedback regulation of the hippocampal GCR and increasing the concentrations of NE and 5-HT of hypothalamus, thus decreasing the plasma corticosterone ; Simultaneously weakening the nuclear transition rate of thymocyte GCR, which in turn improving the immune function of the senile animal.
We concluded that Guzhen Recipe may retard the functional degeneration of both the hypo-thalamuspituitary-gornadal-thymic and hypothalamus-pituitary-thyroidal-thymic axis. All the above results revealed that retarding the aging effect using TCM in volves multiple-leves, multiple-systems and links. Using holistic regulation, imbalance links in aging process are modulated and the equilibrium of Yin and Yang are maintained. This is an important characteristic of the TCM to retard aging.

9.Study of signaling pathway of LDL and oxLDL inducing kidney tubular epithelial cells transdifferentiation

Objective To explore whether LDL and oxLDL may induce kidney tubular epithelial-mesenchymal transition (EMT) and its mechanism. Methods The second generation human kidney tubular epithelial cells (TECs) were cultured for 24 hours in different conditions as (1) serum free as control, (2) treated with LDL (50 ?g/ml) , (3) treated with oxLDL(50 ?g/ml), (4) treated with LDL(50 ?g/ml) plus PD98059(5 ?mlo/L) , (5) treated with oxLDL(50 ?g/ml) plus PD98059 (5 ?mol/L). The expression of cytokeratin, E-cadherin, ?-SMA and vimentin was assessed by immunofluorescence and Western-blot. Western-blot was also performed to test the expression of collagen I and phospho-ERKl/2MAPK and phospho-GSK-3?. Results oxLDL was more potently in inducing tubular EMT than LDL at 24 hours as demonstrated by de novo a-SMA expression, increased expression of vimentin, partial loss of cytokeratin and reduction of E-cadherin expression by TECs. The expression of collagen I and phospho-ERKl/2MAPK and phospho-GSK-3? was increased in TECs stimulated by LDL or oxLDL. MAPK inhibitor (PD98059) inhibited the phosphorylation of GSK-3P and almost completely blocked oxLDL-induced tubular EMT. However, PD98059 alone was able to inhibit LDL-induced tubular EMT partially. Conclusions oxLDL is more potently in inducing tubular EMT than LDL. The ERKl/2MAPK-GSK-3? signaling pathway mediates the LDL or oxLDL-induced tubular EMT.
10.Analysis on drug-resistance of 381 strains of enterococci isolated from Hubei in 2001

Objectives To study the characteristics of clinical distribution of enterococci, to test its sensitivity to antibiotics recommended by NCCLS, and to promote rational use of antibiotics. Methods A total of 381 strains of enterococci isolated from Hubei during January to December 2001 were tested for their sensitivity to antibiotics and production of beta-lactamases with paper disk diffusion method. Results Among 381 strains of enterococci, there were 78.0% of E. faecalis, 19.4% E. faecium, 1.6% E. durans, 0.53% E. avium, 0.3% E. hirae and 0.3% E. solitarium , and beta-lactamases-producing enterorocci accounted for 3.2% of the total. And, 46.5% of the strains were isolated from uroreproductive tracts samples, ranking the first of the total, possibly because of some samples from a hospital specialized in urology, 35.4 % from respiratory tract, ranking the next, and 11.0% form skin pustule and infected wound, ranking the third. Proportions of the strains resistant to varied antibiotics were 5.8%, 7.9%, 11.0%, 24.7%, 26.8%, 38.6%, 54.6%, 54.6%, 52.5% and 79.3% to vancomycin, teicoplanin, nitrofurantoin, ampicillin, penicillin, ciprofloxacin, chloromycetin, high concentration gentamycin, high concentration streptomycin, and tetracycline, respectively. Drug-resistant rate of E. faecium was higher than that of E. faecalis. Conclusions Enterococci, most of them identified as E. faecalis and E. faecium, were isolated from samples of uroreproductive and respiratory system, and the most susceptible to vancomycin. E. faecium was more resistant to antibiotics than E. faecalis.
