1.Combined laparoscopic technique for superficial bladder neoplasms involved orifice

Objective To assess clinical efficacy of the combined laparoscopic technique for superficial bladder neoplasms involved uretetic orifice. Methods A totsl of 12 patients with bladder neoplasms involved ureteric orifice were treated and reviewed.The clinieal tumor stage was Tis in 6,T1 in 6.During the procedure,the transurethral resection of bladder neoplasms was performed firstlythe extent including 0.5-1.0 cm formal bladder tissue around ureteric orifice,deep to superficial muscle of bladder,then ureteroneocystostomy was done with transperitoneal laparoscopic technique.Results The operations were successfully in all 12 cases.The average procedure time was 2.2 h.The average volume of hemorrhage was 25 ml.The double J stents were removed 3 months postoperatively.Six months after surgery intravenous pyelography(IVU)and cystography showed no ureter obstruction in all cases,Ⅰ-Ⅱ degree vesicle-ureteral reflux in 2 cases.One year after surgery custography showed Ⅰ-Ⅱdegree vesicle-ureteral reflux in 4 cases,Ⅱdegree relux in 3 cases.During 324 months' follow-up,B type ultrasound and IVU showed moderate hydronephrosis in 1 case,mild in 5. Conclusion The treatment of combining laparoscopic technique for superficial bladder neoplasms involved ureterie orifice might be an effective and safe method.
2.Clinical managements of therapeutic hypothermia in newborns with hypoxic-ischemic encephalopathy

Therapeutic hypothermia is believed to improve neurodevelopment outcome of infants with moderate-to-severe hypoxic-ischemic encephalopathy(HIE),however,the severe brain injury and neurologic sequelae still can be observed in some infants received therapeutic hypothermia.Optimal monitor and management of systematic complications presented by infants during cooling treatment are necessary for improvement of overall outcome.Therefore,it is essential to understand the functional change of each system of the whole body,to adapt adequate diagnostic methods and to train multidisciplinary staffs to monitor and manage moderate-to-severe HIE infants during therapeutic cooling.With the development of therapeutic hypothermia,it is currently considered as a standard of care for infants with moderate-to-severe HIE.It is recommended that any neonatal intensive care unit(NICU) using routinely therapeutic hypothermia to reference the national or international benchmarking protocols in order to improve the medical quality and prognosis of infants.
3.Optimization and application of protein C-terminal labeling by carboxypeptidase Y.

Proteolytic cleavage is one of the post-translational modifications and plays important roles in many biological processes, such as apoptosis and tumor cell metastasis. The identification of the cleavage events can improve our understanding of their biological functions in these processes. Although proteomic approaches using N-terminal labeling have resulted in the discovery of many proteolytic cleavages, this strategy has its own inherent drawbacks. Labeling of protein C-termini is an alternative approach. Here, we optimized the labeling procedure in the profiling protein C-termini by enzymatic labeling (ProC-TEL) and improved the labeling efficiency for the positive isolation of protein C-terminal peptides and mass spectrometric identification. We applied this approach to a complex protein mixture from Escherichia coli and identified many C-terminal peptides and internal cleaved peptides from more than 120 proteins. From the identified cleavages, we found several previously known internal proteolytic cleavage sites and many novel ones which may play roles in regulating normal biological processes. This work provides a potential new way, complementary to the N-terminomics, for the identification of proteolytic cleavages in complex biological systems.

4.Three-dimensional finite element analysis of different abutment angles and loads of the maxillary central incisor

BACKGROUND:To optimize the oral implant design in the abutment angle has an important effect on bone resorption, and meanwhile, the high bite force from patients is also crucial to rebuild bone absorption.
OBJECTIVE:To optimize the model design of the maxilary anterior teeth using Ansys Workbench 13.0 software and to investigate the stress size on the cortical and cancelous bone from different angled abutments and different loads of the central incisor.
METHODS:A three-dimensional finite element model of the V-shaped cylindrical threaded implants in the maxilary bone. Abutment angle was set as 0°, 5°, 10°, 15°, 20°, 25°, 30°, and the load stress was set as 90, 105, 120, 135, 150, 165, 180, 195, 210 N. Occlusion of the central incisor was simulated on the implants, and then, buccolingual mechanical loads were loaded on the center of prostheses to observe the effects of different abutment angles and loads on the Von Mises peak stress of the maxila and mandible.
RESULTS AND CONCLUSION:Under the influence of a single factor, when the abutment angles acted as variables, the Von Mises peak stress of the cortical and cancelous bone was respectively increased by 60.60% and 69.30% under labial or palatal loads; when the loading stress acted as variables, the Von Mises peak stress of the cortical and cancelous bone was increased by 68.74% and 69.30% under buccolingual loads. When the loading stress was less than 150 N and the abutment angle was less than 25°, the slop of tangent for the mandible Von Mises stress response curve was-1 to 0. It seems from the mechanical analysis that the stress of cancelous bone is more susceptible to abutment angle and load stress; the optimal abutment of screwed implant should be designed to an angle less than 25° and an bite force less than 150 N.
5.Clinical study of gemcitabine combined with nedaplatin concurrent chemotherapy and radiotherapy in treatment of advanced nasopharyngeal carcinoma

Objective To observe the clinical effect of gemcitabine combined with nedaplatin concurrent chemotherapy and radiotherapy in treatment of advanced nasopharyngeal carcinoma.Methods Seventy-six advanced nasopharyngeal carcinoma patients were divided into observation group(38 cases)and control group(38 cases)by radom digits table.The patients in observation group were treated with gemcitabine combined with nedaplatin concurrent chemotherapy and radiotherapy.The patients in control group were treated with fluorouracil combined with cisplatin concurrent chemotherapy and radiotherapy.The clinical effect and adverse reaction were compared between two groups.Results The total effective rate in observation group was 84.2%(32/38),which significantly higher than that in control group[60.5%(23/38)](P<0.05).There was no significant difference in 1 year overall survival rate between two groups[89.5% (34/38)vs.76.3%(29/38)](P > 0.05).The rate of gastrointestinal reactions and oral ulcers in observation group were 42.1%(16/38)and 7.9%(3/38),which significantly lower than those in control group[73.7% (28/38),31.6%(12/38)](P < 0.05).The rate of bone marrow suppression between two groups had no significant difference[31.6%(12/38)vs.23.7%(9/38)](P > 0.05).Conclusion Gemcitabine combined with nedaplatin concurrent chemotherapy and radiotherapy in treatment of advanced nasopharyngeal carcinoma can enhance the clinical effect,and decrease the adverse reaction,which can be applied in clinic.
6.Detection of three kinds of cytokines in patients with viral myocarditis.

Objective To investigate the changes of TNF-?,IL-2 and IL-1? level in serum of patients with viral myocarditis and its clinical meaning.Methods The level of serum TNF-?,IL-2 and IL-1? in 45 cases of viral myocarditis was detected by ELISA,and the data were compared with control group.Results Serum TNF-?,IL-2 and IL-1? levels in viral myocarditis patients were significantly higher than control group and related to severity of the disease.Conclusion It suggests that immune dysfunction exists in patients with viral myocarditis.Higher level of TNF-?,IL-2 and IL-1? may be associated with development of the viral myocarditis,and it can be a new indicator for viral myocarditis.
7.Expression and its significance of caveolin-1 in liver and gallbladder of gallstone mice model

Objective To investigate the role and possible mechanism of caveolin-1 (CAV1) in the forming of cholesterol gallstone in mice fed with lithogenic diet.Methods Cholesterol gallstone susceptible C57BL/6 mice were study objects.The mice of control group (n=6) and experiment group (n=6) were fed with normal diet and lithogenic diet for four weeks respectively.The condition of cholesterol gallstone forming,changes of serum lipid and bile composition were measured,and the expressions of CAV1 and scavenger receptor classB member Ⅰ (SR-BⅠ) at mRNA and protein level in the liver and gallbladder were detected by realtime-polymerase chain reaction and Western blot,respectively.The t test was performed for mean comparsion between the two groups.Results The incidence rate of gallstone in experimental group was 100% after fed with lithogenic diet for four weeks,the lipid level significantly increased,and the proportion of cholesterol in bile raised and bile salt decreased.Compared with those of control group,the expressions of CAV1 at mRNA and protein level in the liver and gallbladder tissues siginificantly decreased (in liver tissue,mRNA 0.53 ± 0.13 vs 1.00 ± 0.32,t =3.330,protein level 0.39 ± 0.07vs 0.92±0.06,t=10.280; in gallbladder tissue,mRNA 0.40±0.22 vs 1.00±0.22,t=3.823,protein level 1.04±0.07 vs 1.34 ± 0.04,t =6.367,all P<0.01).There was no significant difference in the relative expression of SR-BⅠ at mRNA and protein level in the liver and gallbladder tissues between the mice of experiment group and control group.Conclusion The changes of CAV 1 expression at mRNA and protein level in liver and gallbladder tissues may affect lipids metabolism and cholesterol transportation in liver and gallbladder tissues of experiment mice,which might play an important role in the formation of cholesterol gallstone.
8.Impact of obesity and spleen length on laparoscopic splenectomy combined with pericardial devascularization

Objective To evaluate the impact of obesity and spleen length on laparoscopic splenectomy combined with pericardial devascularization.Methods We retrospectively analyzed 121 patients with portal hypertension who underwent laparoscopic splenectomy combined with pericardial devascularization in our hospital.Using BMI,these patients were classified as the obesity and the non-obesity groups.Using length of the spleen,the patients were divided into two subgroups:spleen AC diameter > 20 cm and spleen AC diameter ≤20 cm.Results (1) For the Obesity group,the operation time,the rate of conversion to open operation and the complication rate after operation were higher than the non-obesity group [(184.0 ± 49.0) min vs (142.0 ±39.0) min,26.1% vs 8.0%,26.1% vs 6.7%,respectively,P<0.05].However,the differences were not significant for mean blood loss,intraperitoneal drainage and complication rate after operation.For patients with massive splenomegaly,the obesity group had higher rates of conversion into open operation and complication (42.9% vs 11.7%,33.3% vs 8.8%,respectively,P <0.05).For patients with non-massive splenomegaly,the differences were not significant between the obesity and non-obesity groups (P > 0.05).(2) For obesity patients,the spleen AC diameter > 20 cm group had a longer operation time and a higher rate of conversion to open operation [(224.0 ± 42.0) min vs (152.0 ± 44.0) min,42.9% vs 12.0%,respectively,P < 0.05].The length of spleen had no effect on the operation and its complication (P > 0.05).Conclusions Obesity extended the operation time and increased the rates of conversion to open operation and complication after operation.The spleen length had a major impact on the rates of conversion to open operation and complication after operation for the groups of obesity patients.
9.Management of hepatic HBV reinfection after liver transplantation

Objective To study the effect of antivirus therapy of HBV reinfection and YMDD mutation after liver transplantation. Methods Fifteen of 317 patients with HBV-related end-stage liver diseases received lamivudine ( LAM ) monothereapy, others received combination low-dose hepatitis B immune globulin( HBIG) and LAM (or adefovir dipivoxil, ADV) therapy, as prophylaxis against HBV reinfection after OLT. Hepatitis serum markers, HBsAg, HBeAg, HBcAb-IgM, and HBcAg were detected every 2 weeks by immunohistochemistry. Serum HBV DNA was examined by PCR every 2 weeks. HBsAg and HBcAg in the liver specimens were examined by immunohistochemistry. YMDD mutation was detected by PCR in those patients with recurrence of positive HBV DNA posttransplantation. Results In LAM monotherapy group, 4 developed HBV reinfection out of 15 patients with pretreatment positve HBV DNA. 10.Development and clinical application of fluorescent real-time RT-PCR to hand, foot and mouth disease

Objective To develop a rapid, accurate, specific method to detect causative agent of hand, foot and mouth disease (HFMD). Methods Specific primers and probe were designed based on highly conserved VP1 region of enterovirus 71, coxsackie virus A16 and enterovirus. The sensitivity and specificity of the real-time RT-PCR was evaluated with 35 stool samples collected from pediatric patients with suspected HFMD and 20 clinical samples from health pediatric patients. Results Out of 35 clinical samples from suspected HFMD, 35 samples were identified as positive for enterovirus, 25 clinical samples were identified as positive for enterovirus 71, 8 clinical samples were identified as positive for coxsackie virus A16, among which 3 clinical samples were identified as positive for enterovirus 71 and coxsackie virus A16. The clinical diagnostic accordance rate is 85.71%. Out of 20 clinical samples from normal pediatric patients, 5 clinical samples were identified as positive for enterovirus, 20 clinical samples were negative for enterovirns 71 and coxsackie virus AI6. Conclusion Our results indicate real-time RT-PCR offers a rapid, sensitive, specific and cheap method to detect pathogen of HFMD from clinical specimens.
