OBJECTIVE:To investigate the pharmacokinetic profiles of baicalin(a constituent in Sanhuang decoction) in rats.METHODS:Blood samples were collected as schedule from rats after intragastrically administration of Sanhuang decoction, then the content of baicalin was determined by HPLC and the chief pharmacokinetic parameters were computed.RE SU1TS:The chief pharmacokinetic parameters of baicalin in rats were as follows:t_(max1)=(15?4.23) min;t_(max2)=(6.8?0.54) h;C_(max1) =(4.49?1.56)?g?mL~(-1);C_(max2)=(3.25?1.23)?g?mL~(-1);AUC =(35.52?12.35)?g?h?mL ~(-1);t_(12)=(5.12?0.23)h;CL=(3.86?0.91) 1?h~(-1).CONCLUSION:The method is simple in sample treatment,rapid and accurate,specific and sensitive,and it can be used as a means for the monitoring of the serum concentration of baicalin containing Chinese patent medicines and so as to provide reference for clinical rational drug use.

Objective Hotelling T2 Test was used to analyze the effect of sibutramine and to provide some information for new drug's exploration. Method Fifty-five patients with body mass index (BMI) over 25kg/m2 were selected according to generally accepted including and excluding criteria. The subjects were divided into 2 groups randomly. Control group was given placebo, and the experimental group were given sibutramine. Biological and biochemical indexes were examined pre-,during and post- clinical trial. 4.Ropivacaine used for brachial plexus block on children

Objective To study comparatively the clinical anaesthetic potency and adverse effects to children of 0.375% ropivacaine used for brachial plexus block in upper-limb paediatric surgery.Methods After undergoing basal anesthesia with ketamine,36 children scheduled for hand and forearm surgery were randomly allocated to receive brachial plexus block with 0.375% ropivacaine(group A) and to receive intravenous anesthesia with ketamine(group B).The effect of sensor block and motor block,the changes of vital sings,the occurrence of side effect(nausea and vomiting) and palinesthesia of children were observed.Results The patients in group A kept a steady vital signs,their blood pressure,cardiac and respiration rate were similar to the backgroud level before anesthesia,whereas in group B the blood pressure,cardiac and respiration rate of patients were increased and kept a rather higer level,fluctuated with the depth of anesthesia.Conclusion Ropivacaine used for brachial plexus block in upper-limb paediatric surgery is a simple,feasible,and safe method with anesthetized perfectly and less complication.

Objective To explore the dynamic changes and clinical significance of relative pituitary hormones in children after craniocerebral injury.Methods The quantitative analysis and dynamic observation were performed in 125 children after craniocerebral injury and 20 voluntary healthy children of relative pituitary hormones including serum prolactin(PRL),cortisol(PTC),three free triiodothyronine (FT3),free thyroxine (FT4),thyroid stimulating hormone (TSH),growth hormone (GH) by applying electrochemical luminescence method.Tbe hormone variational characteristics were analyzed according to posttraumatic time,Glasgow Coma Scale(GCS) scores on admission and Glasgow Outcome Scale (GOS) scores on discharge,and the relationship between hormone variational characteristics of 58 cases was followed up over 2 years and the activities of daily living (ADL) were also investigated.Results The serum PRL was significantly increased on the first,third and fifth day compared with the healthy control group (P =0.000 0,0.000 0,0.006 7),respectively.There was significant difference between mild,moderate and severe groups within 30 days after suffering from craniocerebral injury (P < 0.05).PTC was heavily increased within 3 days,and significant difference existed among mild and moderate groups mild and severe groups (all P < 0.05) ; TSH,FT3,FT4 decreased slightly after injury and gradually rose in later;GH change wasn't significant;and the larger variation of relative pituitary hormones was responsible for lower GCS scores;FT3,FT4,TSH,and GH decreased in different degrees,which were found in parts of children with craniocerebral injury,and the significant difference of serum PRL existed between GOS scores 4-5 and GOS scores 1-3 groups (P =0.000 1).Conclusions The changes of relative pituitary hormones were associated with the posttraumatic time and the severity of craniocerebral injury.The PRL in serum can aid in prediction of outcome for the children with craniocerebral injury.
7.Changes of relative pituitary hormones and their significance in adults with traumatic brain injury

Objective To investigate changes of relative pituitary hormones in adults with traumatic brain injury (TBI) and the related clinical significance.Methods Quantitative analysis and dynamic observation of relative pituitary hormones were performed in 158 TBI patients by electrochemical luminescence method.Measured indices included plasma total cortisol (PTC),free triiodothyronine (lT3),free thyroxine (FT4),thyrotropin (TSH),growth hormone,follicle stimulating hormone (FSH),luteinizing hormone (LH),estradiol,testosterone,and prolactin.Results Prolactin and PTC increased in the acute phase,but gradually reduced three days after trauma.TSH,FT3,and FT4 slightly decreased after trauma,followed by a gradual return.While there were no significant changes in FSH,LH,estradiol,testosterone,and growth hormone after trauma.Changes in relative pituitary hormones were more profound in patients with a lower GCS.Some patients presented different degree of reduced hormones in recovery period and needed hormone replacement therapy.Among the patients with poor activity of daily living (ADL),lvothyroxine replacement therapy was applied in 2 patients (2/6),prednisone acetate in 1 (1/6),and eleven acid testosterone in 2 (2/6).Among patients with mild ADL,levothyroxine replacement therapy was applied in 2 patients (11%),prednisone acetate in 1 (6%),and eleven acid testosterone in 3 (17%).Among patients with good ADL,levothyroxine replacement therapy was applied in 2 patients (4%) and eleven acid testosterone in 3 (6%).Persistent prolactin elevation was found in patients with poor outcome.Conclusions Changes of relative pituitary hormones in adult patients with TBI are associated with severity and duration of trauma.Abnormal prolactin level can affect outcome of the patients.
8.Relation between vagus nerves and remodeling of gap junction in superior vena cava myosleeve in dog with atrial fibrillation

Objective To investigate the relationship between cardiac vagus nerves and changes of connexins(Cx)and intracellular gap junction(GJ)distribution pattern in superior vena cava(SVC)myosleeve in dog with atrial fibrillation(AF).Methods Twenty four hybrid dogs were divided into sham operation group(Sham group,n=8),SVC-AO fat pad removed group(RM group,n=8)and SVC-AO fat pad reserved group(RS group,n=8).In RM group and RS group,right atrial pacing was performed at a frequency of 500~650/min for 6 weeks to establish AF model.AF was induced by programmed stimulation or burst stimulation of atrial pacing.The expression and distribution of Cx40 and Cx43 in the SVC myosleeve tissue in three groups were analyzed by immunofluorescence staining.Transmission electron microscopy was used to observe the uhrastructural organization of gap junction(GJ).Results The rate of inducing sustained AF(> 15 min)in RS group was significantly higher than that in RM group (P < 0.01).The expression of Cx40 and Cx43 in the SVC myosleeve in sham group and RS group were significantly higher than that in RM group(P < 0.05).Furthermore,the expression of Cx40 and Cx43 in RS group were obviously higher than that in sham group(P <0.05).The ratio of end-to-end to side-to-side in RS group was lower than that in Sham group and RM group.Comparing with RM group,the channel of GJ became shorter and wider in RS group(P <0.05).Sarcomere was dissolved and mitochondrion showed vacuole degeneration in RS group.Conclusion The remodeling of Cx40 and Gx43 in SVC myosleeve tissue may be mediated by vagus nerves.It is conducive to the maintenance and stability of AF.However,this effect can be weakened by removing SVC-AO fat pad of canine.
9.Application of modified Delphi method in the construction of curriculum of emergency professional postgraduates

Objective:At present, the postgraduate education of emergency medicine in China mainly relies on the internal medicine system and lacks specialty pertinence. The purpose of this study is to establish a compulsory curriculum system for professional postgraduates of emergency medicine.Methods:Modified Delphi method was used to subscribe questionnaires to experts, and the contents were mainly about medical education management and research of emergency in the affiliated teaching hospitals of Peking University. Microsoft Excel 2016 was used for double entry of questionnaire contents, and SPSS 22.0 software was used for statistics.Results:The panel was consisted of 14 experts in total and two rounds of Delphi questionnaires were completed. The response rate of the two rounds were both 100%. The authority coefficient of experts was > 0.9 and the average score of each evaluation index was 4.07-5.00 points. The coefficient of variation of each index was 0-0.22 and the Kendall coefficient of concordance was 0.07-0.10.Conclusion:As a result, a comprehensive emergency professional postgraduate curriculum system has been established.
10.Genetic and clinicopathologic study on a family with central nervous system hemangioblastoma in Luodian County, Guizhou Province

Objective To explore the pathogenic gene for the family with central nervous system hemangioblastoma (CNS-HB) in Luodian County,Guizhou Province and the pathologic features of familial and sporadic CNS-HB.Methods The peripheral blood from 42 members of the family with CNS-HB was obtained for DNA extraction.The exons 1,2 and 3 of VHL gene were amplified and sequenced by PCR.Light microscopy and immunohistochemical staining were applied for pathological observation of the tumor tissues harvested intraoperatively from 6 cases of the familial CNS-HB and from another 9 cases of sporadic CNS-HB as controls.A total of 9 specimens of familial CNS-HB (6 cystic and 3 solid) and 9 specimens of sporadic CNS-HB (8 cystic and one solid) were harvested.Results VHL gene mutations were not detected among the 42 members of familial CNS-HB.HE staining showed that the tumors were made up of capillary network and large vacuolated interstitial cells with abundant cytoplasm which were pale eosinophilic,polygon-shaped,foamy or ground-glass opaque,and scattered between the capillary network.No nucleus fission or necrosis was observed.Immunohistochemical staining showed the positive expression of Inhibin-α was 11/18 and that of Ki-67 was 14/18 in the interstitial cells and the positive expression of CD31 in the endothelial cells was 17/18.There were no significant differences between familial and sporadic CNS-HB in the positive expression of Inhibin-α,Ki-67 or CD31 (P>0.05).Conclusions The pathogenesis of familial CNS-HB may be associated with not only VHL gene mutation but also other factors.Familial and sporadic CNS-HB may be similar in pathological features.
