Apoptosis, an intrinsic cell death model which is regulated by organisms, is essential for the maintenance of tissue homeostasis in multicellular organisms. Recently, researchers on cell apoptosis have paid more attention on mitochondrion than on nucleus. Different death stimulus induces opening of PT pore, degradation of mitochondrial transmembrane potential, activation of caspase and inducing cell apoptosis. Bcl-2 and Bcl-XL inhibit cell apoptosis via mitochondrion while Bax, Bak and Bad induce apoptosis by means of regulation of mitochondrion.

3.The artery effects of enhanced external counterpulsation for the progressive stroke patients

Objective To discuss the artery effects of enhanced external counterpulsation for the progressive stroke patients.Methods Ninety-two progressive stroke patients who were treated in the Fourth People's Hospital of Shenzhen from August 2010 to February 2015 were selected and equally divided into the treatment group and the control group based on the random number table.The control group were received conventional therapy,and the treatment group were added treated with enhanced extemal counterpulsation therapy.Results All patients were successfully completed treatment,the C-reactive protein (CRP) and Endothelin-1 (ET-1) were presented decreased significantly after treatment (P< 0.05),and the C RP and ET-1 values in the treatment group were (2.09±2.11) mg/L,(13.98±6.30) mg/L,significantly lower than those of the control group((8.12±2.46) mg/L,(46.09± 11.02) mg/L;t =15.309,14.985;P<0.05).After treatment,the FMD and NMD values in the treatment group were significantly increased from (5.26±1.34)% to (11.93 ±2.13) %,and from (13.20±5.33)% to (22.98±8.34)%,in the control groupand were from (5.27±1.33)% to (8.10±1.43) %,and from (13.67±4.10)% to (16.09±5.12)% (t=16.934,21.787,8.443,12.345;P <0.05),and the value of FMD and NMD in treatment group were significantly higher than in the control group(t =7.982,9.113;P < 0.05).The cardio ankle vascular index (CAVI) and brachial ankle pulse wave velocity (baPWV) values in the treatment group were significantly reduced after treatment,the difference was significant (P<0.05),and compared with the control group the difference was also statistically significant (t =4.281,8.456;P< 0.05).Conclusion Enhanced external counterpulsation for the progressive stroke patients can effectively inhibit the expression of endothelin and inflammatory factors,improve endothelium-dependent relaxation of blood vessels,improve arterial elasticity,and thus play more effective treatment.

Objective: The influence on the blood glucose,ALB,LYM,K+,Na+,Cl-,TG,TC and so on after the use of different nutrition formula was observed in patients with cerebrovascular disease and type 2 diabetes.Methods: 40 patients who suffered from cerebrovascular diseases and type 2 diabetes were divided into two groups: one group using the ordinary formula and the others using special-purpose formula of diabetes.The levels of blood glucose,ALB,LYM,K+,Na+,Cl-,TG,TC were monitored.Results: The patient's blood glucose after use of ordinary formula rised obviously and insulin or the oral drug for decreasing blood glucose was used.The blood glucose in special-purpose diabetes formula group didn' t increase obviously,and there was not the need of using insulin(P
5.Prognostic value of lung ground glass opacities volume ratio in patients with acute paraquat poisoning

Objective:By comparing the volume% (V% GGOs) of ground glass opacities (GGOs) in high resolution CT (HRCT) of patients with acute paraquat (PQ) poisoning at different time points, its value in the early prognosis of patients with PQ poisoning was analyzed. Methods:The data of patients with PQ poisoning admitted to Department of Respiratory and Critical Care Medicine of Chinese Armed Police Forces from June 2017 to December 2018 were prospectively analyzed. According to the follow-up results after poisoning at 90 days, the patients were divided into the survival group and death group. Three-dimensional reconstruction technology was used to calculate the change of V% GGOs on the 3rd, 5th, and 7th day after poisoning. Chi-square test and One-Way ANOVA of variance were used to compare sex, age, and time of poisoning between the two groups. The Student's t test was used to compare V% GGOs between the two groups at different time points. The receiver operating characteristic curve (ROC) was used to determine the guiding significance of the indicator on the prognosis of patients with PQ poisoning at different time points. Results:A total of 89 patients with PQ poisoning were included in the study, 49 in the survival group and 40 in the death group. There were no statistical differences between the two groups of patients in sex, age, poisoning time, oxygenation index, mean arterial pressure, total bilirubin, blood urea nitrogen, alanine aminotransferase, and aspartate aminotransferase at admission (all P>0.05). The blood PQ concentration (mg/L) in the death group was significantly higher than that in the survival group (6.35 ±0.51 vs 3.49 ±0.21, P= 0.013). On the 3rd, 5th and 7th day after admission, the V% GGOs was significantly higher than that in the survival group (3rd day: 0.062±0.020 vs 0.049±0.007, P= 0.013; 5th day: 0.292±0.130 vs 0.123±0.044, P<0.01; 7th day: 0.334±0.116 vs 0.138±0.034, P=0.019). The area under the ROC curve showed that the prognosis AUC of the 7th day V% GGOs after poisoning was 0.967, the sensitivity was 100% and the specificity was 83.33% when the threshold was 0.16, but the time point was late. On the 5th day after poisoning, the V% GGOs judged the prognosis AUC was 0.842, the sensitivity was 82.35% and the specificity was 89.47% when the threshold was 0.14. On the 3rd day after poisoning, the V% GGOs judged the prognosis AUC was 0.708, the sensitivity was 55.00% and the specificity was 78.95% when the threshold was 0.05. At this time, the sensitivity and specificity were lower than those on the 5th and 7th day. Conclusions:The proportion of ground glass opacity volume in patients with PQ poisoning can be used to evaluate their prognosis, and the best time point is the 5th day after poisoning.
6.The therapeutic effect influencing factors of intra-aortic balloon pump support during cardiac surgery perioperative period

Objective To explore the therapeutic effect influencing factors of intra-aortic balloon pump(IABP) support during cardiac surgery perioperative period and countermeasures.Methods Clinical data of 42 patients using IABP in the cardiac surgery was analyzed retrospectively.The patients were classified into 2 groups:living group (32 patients) and died group (10 patients).The time of IABP implantation,preoperative cardiac function,the duration of cardiopulmonary bypass(CPB) and aorta block,mean arterial pressure (MAP) and inotropic score (IS) before using IABP and postoperative complications were recorded and compared.Results In died group,2 patients were used IABP preoperatively,1 patient was used intraoperatively,7 patients were used postoperatively,8 patients were in NYHA cardiac function class Ⅲ or Ⅳ preoperative,the duration of CPB and aorta block were (144.43 ± 49.03),(97.29 ± 39.99)min respectively,MAP and IS before using IABP were (57.34 ±7.25) mm Hg (1 mm Hg =0.133 kPa) and (28.22 ±17.72) scores,IABP time was (86.00 ±52.31) min.Compared with living group,all above comparisons showed significant difference [(100.43 ± 35.03) min,(60.45 ± 20.55) min,(69.34 ± 8.05)mm Hg,(10.82 ± 15.75) scores,(49.00 ± 25.23) min] (P < 0.05).Postoperative complications in died group included 7 patients of ventilator dependency,6 patients of acute renal failure,5 patients of refractory metabolic acidosis,2 patients of malignant arrhythmia,1 patient of acute myocardial infarction,significantly higher than those in living group (0,4,2,0,0) (P < 0.05).Conclusions Erroneously choose the timing of using IABP,poor preoperative cardiac function,prolonging CPB and aorta block time,MAP less than 60 mm Hg and high dose positive inotropic agent before using IABP,and postoperative complications are main influencing factors for clinical outcomes of IABP in cardiac surgery.Reasonably choosing adaptive indication and timing of IABP,preventing and treating postoperative complications effectively can improve effects of IABP in cardiac surgery.
7.Research on the expression of Caveolin-1 with EGFR mutations in non-small cell lung cancer tissues and its clinical significance

Objective This study aims to examine expression of Caveolin -1 in non smoking cell lung cancer(NSCLC)and to discusses the relationship between expression of Caveolin -1 and the Epidermal growth factor receptor mutations .Methods Immunohistochemical staining was used to determine the Caveolin -1 ex-pression and ARMS-qPCR was used to measure EGFR mutations in 40 cases of lung cancer tissue .The clinical pathological characteristics and correlations in patients were analyzed .Results The expression of Caveolin -1 in human lung cancer was significantly lower than that in normal lung tissue ,and negatively correlated with EGFR mutations ,which was statistically significant .Conclusion Caveolin -1 expression is negatively correlated with EGFR mutations in non-small cell lung cancer and related to the histologic type .Caveolin-1 may be a molecu-lar target for diagnosis and judgment of NSCLC .
8.Value of heart-type fatty acid-binding protein for early diagnosis of acute myocardial infarction

Objective To investigate the value of heart-type fatty acid-binding protein in the diagnosis of early myocardial infarc-tion.Methods In 186 cases of suspected acute myocardial infarction due to chest pain,chest tightness for 3 h,plasma CK-MB, troponin-I(CTn-I)and heart-type fatty acid-binding protein(H-FABP)were detected.The sensitivity and specificity in diagnosing early myocardial infarction were compared among 3 kinds of indexes.Results Compared with the non-infarction group and the con-trol group,plasma CK-MB,CTn-I and H-FABP in the acute myocardial infarction group were significantly increased (P <0.05 );compared with CK-MB and CTn-I,the sensitivity of H-FABP to the diagnosis of acute myocardial infarction within 3 h was higher, but its specificity was lower than that of CTn-I and higher than that of CK-MB.Conclusion For the patients with acute myocardial infarction within 3 h after onset,detecting H-FABP can increase the diagnostic rate of early myocardial infarction to a certain extent.
9.Clinical characteristics of acute ischemic stroke patients complicated with obstructive sleep apnea hypopnea syndrome

Objective:To analyze the clinical characteristics of patients suffering from acute ischemic stroke (AIS) complicated with obstructive sleep apnea-hypopnea syndrome (OSAHS).Methods:Data of patients with AIS who visited the Second Affiliated Hospital of Soochow University from January 2015 to June 2020 and underwent polysomnography monitoring (PSG) in the sleep center were collected retrospectively. Patients were divided into OSAHS group and AIS only group. Demographic information of patients, general clinical data, hematological indicators of glucose and lipid metabolism and inflammatory markers, PSG parameters and neurological function scores were collected, including the National Institutes of Health Stroke Scale (NIHSS) on admission and the modified Rankin Scale (mRS) on discharge. We compared the differences between the two groups. In addition, OSAHS group were divided into good prognosis and poor prognosis subgroups according to mRS score. The differences between the two subgroups were compared.Results:A total of 112 AIS patients combined with OSAHS and 89 AIS only patients were included. The proportion of non-rapid eye movement stages 1+2 [(N1+N2) %], arousal index, the oxygen desaturation index (ODI), percentage of total sleep time with oxygen saturation<90% (TS90) in the OSAHS group were higher than those in the AIS only group, while N3%, lowest nocturnal oxygen saturation (LSaO 2) were lower (all P<0.05). There was no statistical difference in the distribution of cerebral apoplexy lesions (cortex, subcortical, brainstem, cerebellum) between the two groups, but the proportion of patients with multifocal cerebral apoplexy in the OSAHS group was higher ( P=0.032). There was no statistical difference in NIHSS score on admission between the two groups, but the neutrophil/lymphocyte ratio (NLR) score ( P=0.004) and mRS score on discharge ( P=0.010) of the OSAHS group were significantly higher than those in the AIS only group. There were 74 patients in the good prognosis group and 38 in the poor prognosis group. The analysis showed that the NIHSS and NLR scores of the poor prognosis group were higher than the good prognosis group, admission NIHSS score was a risk factor for poor prognosis, all P<0.01. Conclusions:AIS patients complicated with OSAHS are characterized by disordered sleep structure, more severe nocturnal hypoxia, higher risk of developing multiple lesions, poor neurological function recovery at discharge, and high inflammatory index of NLR. Among them, patients with poor prognosis have poorer sleep efficiency, and high admission NIHSS score is a risk factor for poor prognosis.
10.The effects of different types of bone cement intervertebral leakage on stress distribution in endplates of adjacent vertebrae: A finite element study

Objective Finite element method was used to clarify the biomechanics effect of cement intervertebral leakage during vertebral augmentation.Present a novel classification of bone cement intervertebral leakage.Analyze the effect of stress changing of bone cement intervertebral leakage on adjacent endplate by finite element method.Methods Based on Churojana's classification method,we redefined diverse kinds of intervertebral leakage:as the Type Ⅰ (intervertebral-extradiscal leakage),Type Ⅱ (intradiscal leakage) and Type Ⅲ (combined leakage).Type Ⅱ was also been divided into Ⅱa (anterior),Ⅱb (central),Ⅱc(posterior),Ⅱd (lateral) and Ⅱe (cross-region) due to the location of the leaked bone cement.All the Type Ⅱ cases were divided into 1 or 2 two subtypes according to whether the cement had reached the adjacent vertebral endplate.We established 3D reconstruction of volunteer thoracolumbar spine using Mimics 17.0 software,and using Geomagic 2015 to generate L1 vertebral compression fracture model.In the Ansys 17.0 software,we simulated the L1 bone cement leakage into the T12/L1 intervertebral space model.After validating the validity of the model,calculate the solution of the intact model,non-leakage model and various leakage models,the stress distribution of the caudal endplate of T12 was analyzed in neutral,flexion,extension,lateral bending and torsion.Results The maximum stress of inferior endplate of T12 vertebra of intact model is 11.476 MPa,19.517 MPa,16.879 MPa,42.346 MPa,43.033 MPa,6.568 MPa,6.568 MPa in neutral,flexion,extension,left bending,right bending,left rotation,right rotation respectively.For the non-leakage model,the maximal stress of adjacent vertebral endplate was 12.967 MPa (112.99%),23.134 MPa (118.53%) and 20.403 MPa (120.88%) in neutral,flexion and extension compared to the intact model.No significant increasing can be found in other conditions.Compared to the non-leakage model,the stress of adjacent vertebral endplate is similar when type Ⅰ leakage occurs.In type Ⅱ leakage,the Ⅱa1 was 28.506 MPa (123.40%) in the flexion;the Ⅱa2 was 84.791 MPa (366.52%) in the flexion;the Ⅱb2 was 14.138 MPa (122.82%) in the neutral and 27.313 MPa (118.06%) in the flexion;the Ⅱc1 was 19.695 MPa (128.50%) in the extension;the Ⅱc2 was 67.740 MPa (441.97%) in the extension,and the Ⅱd2(right) was 123.940 MPa (285.83%) in the right bending.In the left/right rotation motions,the stress values are small,ranging from 5.095-15.585 MPa.Conclusion After vertebral augmentation,the stress of adjacent vertebral endplate increased slightly.Type Ⅰ leakage did not further increase the stress of adjacent vertebral endplates.Type Ⅱ leakage increases the stress of adjacent endplates in the direction of leaked cement.Subtype 2 of Type Ⅱ offer more stress than subtype 1.When the peripheral type of leakage (Ⅱa,Ⅱc and Ⅱd) occurred,if the spine flexes in the direction of leakage,then the stress increase of adjacent endplates will increase further.
