Objective To explore the clinical effect of ulinastatin on capillary injury improvement of renal syndrome hemorrhagic fever and renal leakage. Methods Patients were divided into 2 groups according to the random number table. The experimental group(25 cases)was given with ulinastatin and the control group(25 cases) was given 5% sugar solution as a blank control. All patients were treated with nutritional support ,rehydration , prevention of bleeding and other symptoms. In addition, according to the number of days of fever in the experimental group, the patients were divided into two groups. The experimental group A(9 cases)had fever 1 ~ 4 days;the experimental group B(16 cases)had fever 5 ~ 7 days. All patients were measured in microalbuminuria ,serum creatinine, plasma albumin and other clinical indicators after 7 day treatment. Results Compared with the control group, microalbuminuria of the experimental group was significantly decreased;Creatinine recovery rate was faster than that in the control group;Plasma albumin had significantly increased; The number of symptomatic days of concurrent perfusion of other tissues had also significantly reduced. Compared with the experimental group B , microalbuminuria of the experimental group A was significantly decreased;Creatinine recovery rate was faster than that in the experimental group B;Plasma albumin had significantly increased; The number of symptomatic days of concurrent perfusion of other tissues was also significantly reduced. Conclusion Ulinastatin could effectively treat vascular injury and syndrome due to capillary leakage caused by epidemic hemorrhagic fever virus ,and the best effect occurs in early application in the fever.

Descriptive epidemiological methods were adopted to analyze the epidemiological characteristics of scarlet fever in Hubei Province from 2009 to 2018. During this study, the epidemiological data was obtained from the Chinese Center for Disease Control and Prevention Information System. The results showed that there were 7 329 cases of scarlet fever reported in Hubei Province from 2009 to 2018 and the annual average incidence was 1.25 per 10 000. The number of cases was increased by years, and in 2018 it was the highest peak during these ten years and up to 1 758 which was account for 24% in these ten years, and the incidence was 2.98 per 10 000. The number of reported cases of male and female was 4 473 and 2 856, respectively, and both of the incidence rates were on the rise. The reported cases were distributed in 0-60 year-old group, mainly in the <15-year-old group (98.4%, 7 214/7 329). The season with the highest incidence rates were summer and winter. There were cases in all cities and prefectures, and the number of cases were the highest in Wuhan ( n=2 105), Yichang ( n=1 390), Jingzhou ( n=954) and Enshi ( n=1 090). Hence, it was very necessary to be on guard for the outbreak of scarlet fever and strengthen the surveillance in summer and winter for the people who were younger than 15 years old, especially in Wuhan, Yichang, Jingzhou and Enshi.

Objective:To evaluate the deformity changes in the treatment of congenital clubfoot in infants by ultrasound, and to explore its clinical application value. Methods:Seventeen infants with congenital clubfoot treated in the Department of Pediatric Orthopaedics of the Third Affiliated Hospital of Zhengzhou University from March 2020 to June 2021 and 27 normal infants in the same period were selected. The distance between medial malleolus and scaphoids (MM-N distance) of all feet was measured by ultrasound. The distance from the tangent line of the lateral edge of calcaneus to the midpoint of the lateral edge of the chondroid bone (C-C distance), medial soft tissue thickness and tibial calcaneal angle were measured by ultrasound. The data before, after treatment and during follow-up were statistically analyzed. Results:A total of 88 feet of 44 infants were evaluated. The mean number of cast was 4.7±1.1, the follow-up time was (96.9±59.3)days. The MM-N distance, C-C distance and posterior tibial calcaneal angle in the clubfoot group were improved after treatment and at the last follow-up, and the differences were statistically significant (all P<0.01). During the treatment, 1 case had false correction, and 2 cases recurred in the last follow-up. Conclusions:Ultrasound can clearly display the medial, dorsal, lateral and posterior articular structures of normal and clubfoot, and can observe the deformity changes of clubfoot during the correction process, which can provide guidance for the clinical treatment of clubfoot.
