Objective To analyze the safety and efficacy of early diet after laparoscopic resection of colorectal cancer.Methods A prospective study of 128 patients suffered laparoscopic surgery of colorectal cancer was done in our hospital.According to whether the adoption of early diet program,patients were divided into early oral group (EOF group,n=54)and traditional oral diet group (TOF group,n=74).Age,gender,preoperative nutritional status and other basic factors and postoperative ambulation time, postoperative hospital stay time,the number of cases the use of analgesics,complications of two groups were analyzed statistically. Results The first time of postoperative ambulation time and postoperative hospital stay time of EOF group were significantly re-duced compared TOF group [(1.2±0.8)d vs .(2.5±1.3)d]and [(3.2±1.4)d vs .(5.3±1.6)d](P <0.01).Conclusion Early diet after laparoscopic resection of colorectal cancer is good for patients fast recovery,and postoperative complications of two groups are similar.

4.Vascular endothelial growth gactor and its receptor expression in mandibular fracture healing

Objective:To study the expression of vascular endothelial growth factor(VEGF) and its receptor Flt1 in the process of mandibular fracture healing.Methods:The fracture models were made in the middle of the left mandible of 25 rabbits,then the specimens from the fracture site were taken 48 hours and 1,3,5 and 8 weeks after operation respectively. The expression of VEGF and Flt1 was detected by immunohistochemistry and in situ hybridization.Results:VEGF mRNA, VEGF and Flt1 were detected in many kinds of cells in the fracture site from 48 hours to 8 weeks after fracture, and high expression of VEGF and Flt1 were detected from 1 to 3 weeks after facture.Conclusions:VEGF and Flt1 are expressed through the process of fracture healing, VEGF may play important role in the process of blood vessel rebuilding after fracture.
5.EFFECT OF LIPOPOLYSACCHARIDE ON EXPRESSION OF INTERLEUKIN-6 IN COCULTURED LUNG MICROVASCULAR ENDOTHELIAL CELLS

Rat lung microvascular endothelial cells (LMVEC) were cocultured with rat pulmonary arterial smooth muscle cells (PASMC). The cultures were divided into 5 groups: normal group (N), lipopolysaccharide (LPS) 2h (L 2), LPS 8h (L 8), LPS 16h (L 16 ), and LPS 24h (L 24 ). The activity of IL 6 in LMVEC and PASMC supernatants was determined by radio immunity method, and the expression level of IL 6 mRNA in LMVEC was detected with RT PCR. The results showed that the activity of IL 6 in LMVEC and PASMC was enhanced by LPS at L 2, reaching the higher level after 8 hour stimulation. Furthermore, the activity of IL 6 was stronger in homotypic groups than that of coculture groups. The expression of IL 6 mRNA in LMVEC was increased in L2 group, and highest in L6 group, but lowered in L12 group, though it was still stronger than that of N. It suggest that IL 6 mRNA and the activity of IL 6 in LMVEC and PASMC were enhanced in both homotypic and coculture groups by LPS. And the mRNA level and activity of IL 6 were higher in homotypic groups than that of coculture groups.
6.Efficacy comparison of interlobar node dissection by video-assisted thoracoscopic surgery and thoracotomy in treatment of non-small cell lung cancer

Objective To investigate the positive detective rate of interlobar node in the operation of non-small cell lung cancer (NSCLC), and to compare the effect of interlobar node dissection by video-assisted thoracoscopic surgery (VATS) with thoracotomy in treatment of lung cancer. Methods 108 patients with NSCLC who received the surgery by the same group of thoracic surgeons were collected from Aug 2012 to Mar 2016 in Guangdong Medical University Affiliated Kaiping Central Hospital. The positive transfer and detective rates of interlobar node in patients with different clinical and pathological stage were analyzed. Then the patients were divided into traditional open thoracotomy group and VATS group according to the operation mode, meanwhile, the number of interlobar nodes dissection in the two groups were compared. Results The positive transfer rates of the interlober nodes in patients with different clinical stage were ⅠA 5.7 % (2/35),ⅠB 5.0 % (1/20), ⅡA 14.3 % (3/21), ⅡB 13.3 % (2/15), ⅢA 29.4 % (5/17); For patients with different pathological T stage were T1A 8.3 % (2/24), T1B 9.1 % (2/22), T2A 12.9 % (4/31), T2B 14.3 % (2/14), T3 21.4 %(3/14), T4 33.3 % (1/3), and over-all transfer rate was 13.0 % (14/108). In pathology stage and pathology T stage, the number of interlober nodes had no statistical difference in traditional open thoracotomy group and VATS group (all P>0.05). Conclusions Even in the early clinical stage, the transfer of interlobar node may be positive. The transfer rate of interlobar node is correlated positively with pathology T stage. VATS can achieve the same effect in interlobar node dissection compared with the traditional approach.
8.Clinical experience in classification, diagnosis, and treatment for Wunderlich syndrome

Objective To explore the clinical features, diagnosis and treatment of Wunderlich syndrome (WS).Methods Fifteen patients with WS were included from September 2008 to February 2014, and their clinical features, diagnosis and treatment were retrospectively reviewed.The most common clinical manifestations were flank or abdominal pain (15/15), hypovolemic shock (5/15), gross hematuria (4/15) and percussion pain on kidney region ( 15/15 ) .Laboratory tests showed anemia ( 9/15 ) and coagulation abnormalities (5/15).Five of 15 cases were critical patients with moderate to severe shock ( systolic blood pressure≤90 mmHg, 1 mmHg =0.133 kPa ) accompanied with severe anemia ( Hb <60 g/L ) and coagulation abnormalities.Results In the acute stage, the diagnostic ratios of ultrasonography and contrast enhanced CT for WS were 11/15 and 15/15, and cause determination ratios were 4/15 and 10/15, respectively.The latter was significantly higher than the former ( P <0.05 ) .Contrast enhanced MRI was performed in 5 cases, and the results were identical to those of CT.According to the causes made by emergent imaging, critical patients underwent emergent operations or renal arteriography plus selective arterial embolization, and the other patients underwent conservative management, emergency operations, or renal arteriography plus selective arterial embolization, respectively.The causes of WS included angiomyolipoma (8 cases), renal cell carcinoma (3 cases), metastatic tumor of lung cancer (1 case) and renal cyst (3 cases) in this study.The mean follow-up period was 34 months.One critical patient died, and all the other patients were cured or relieved.Conclusions WS has no specific clinical features.Contrast enhanced CT or MRI is the main approach for diagnosis and cause determination, which is superior to ultrasonography.Treatments for WS vary according to severity classification and imaging diagnosis.
9.Mediastinal lymph node dissection by video -assisted thoracoscopic surgery versus thoracotomy in the treat-ment of lung cancer

Objective To compare the effect of mediastinal lymph node dissection by video -assisted thora-coscopic surgery (VATS)with thoracotomy in the treatment of lung cancer.Methods 96 patients with non -small cell lung cancer were collected.Patients undergoing VATS were matched with those undergoing thoracotomy in terms of gender,age,clinical tumor stage,tumor location and surgical procedure.Results After matching,48 patients in VATS group and 48 patients in open group were eligible for analysis.In the VATS and open groups,the mean total numbers of dissected lymph nodes were (27.2 ±7.4)and (28.8 ±10.6)(P =0.507),the numbers of N1 nodes were (9.4 ±4.0)and (8.3 ±4.6)(P =0.323).And the number of N2 nodes was similar between the VATS and open group [(18.5 ±6.9)vs (21.3 ±9.9),P =0.201].No significant differences were observed between the two groups(all P >0.05 ).But the days of the postoperation and the chest tube indwelling in thoracotomy group were (8.1 ±3.9)and (7.3 ±4.4)days,which in the VATS group were (6.8 ±3.5)and (5.6 ±3.5),the VATS group had more advantages than the thoracotomy group(P <0.05).The intraoperative blood loss more than 400mL and peri-operative blood transfusion rate of the thoracotomy group were 27.38% and 25.00%,those of the VATS group were 7.03% and 8.60% respectively,which in thoracotomy group were higher than the VATS group.Conclusion Under-go our retrospective study,after mature VATS to treat lung cancer,with regard to the number of the dissected lymph nodes,VATS lobectomy can achieve complete mediastinal lymph node dissection compared with the traditional approach.There are more advantages by VATS in the complications.
10.Determination of 4-( 4-Amino-3-fluorophenoxy )-N-methylpyridine-2-carboxamide in Regorafenib by LC-MS/MS

Objective: To establish an LC-MS/MS method for the determination of 4-( 4-amino-3-fluorophenoxy )-N-methylpyri-dine-2-carboxamide ( AFP-PMA) as a genotoxic impurity in regorafenib. Methods: The content of AFP-PMA was determined by an LC-MS/MS method. A Waters XBridge Shield RP18 column was adopted to separate the samples and the column temperature was 50℃. The mobile phase consisted of 5 mmol·L-1ammonium acetate aqueous (A)-acetonitrile (B) with gradient elution (0~9 min, 5%B→90%B) at a flow rate of 1. 0 ml·min-1. An electrospray ionization source (ESI) was used in a positive-ion and multiple reactions monitoring mode. The ion channel was m/z 262. 2→244. 1. Results:The standard curve was linear within the range of 2. 41-980. 90 ng·ml-1(r=0. 9998) and the limit of quantification was 8. 02 ng·ml-1. The limit of detection was 2. 41 ng·ml-1, which was e-quivalent to 0.000241% for the concentration of regorafenib. The average recovery was 100.95% and RSD was 2.37% (n=9). Conclusion:The method has good specificity, promising accuracy and high sensitivity, which can be used for determining the trace genotoxic impurity AFP-PMA in regorafenib.
