Objective To explore the clinical characteristics and pregnancy outcomes of twin pregnancies complicated with severe preeclampsia. Methods The pregnant outcomes of 131 twin pregnancies (twin group) and 572 singleton pregnancies (singleton group),all complicated with severe preeclampsia, were analyzed retrospectively. All patients were treated in the Department of Obstetrics and Gynecology,West China Second University Hospital,Sichuan University from June 2007 to June 2011.The patients' age, onset of disease, gestational weeks at delivery, mean duration of expectant treatment, blood pressure, laboratory parameters and incidence of pregnancy complications, including placental abruption, heart failure, pulmonary edema, postpartum hemorrhage, uteroplacental apoplexy, eclampsia,HELLP syndrome (hemolysis, elevated liver enzymes and low platelets syndrome),hypoproteinemia, retinopathy, intracranial hemorrhage and renal insufficiency, were compared between the two groups.Perinatal outcomes such as premature delivery, perinatal mortality, neonatal intensive care unit (NICU) hospitalization, fetal distress,hypoxic-ischemic encephalopathy (HIE),asphyxia neonatorum, neonatal pneumonia, hyperbilirubinemia and neonatal hypoglycemia of two groups were also compared.Chi-square test,Fisher's exact test or t-test were used for statistical analysis. Results The severe preeclampsia incidence of twin pregnancies (5.03%,131/2604) was higher than that (1.94%,572/29 452) of singleton pregnancy (x2=106.40,P<0.001).The onset time [(33.6±1.8) weeks] and gestations at delivery [(34.6±2.9) weeks] in twin group were earlier than those in singleton group [(34.4± 2.0) weeks,t=2.364,P<0.05; (35.6±3.2) weeks,t=3.902,P<0.05].The duration of expectant treatment of twin group [(6.4±0.3) d] were shorter than that of singleton group [(7.4± 0.5) d,t=5.314,P<0.01].The incidence of placental abruption [9.9% (13/131) vs 4.2% (24/572),x2=7.013,P<0.01],heart failure [11.5% (15/131) vs 3.2% (18/572),x2=16.430,P<0.01],pulmonary edema [4.6% (6/131) vs 0.9% (5/572),x2=9.505,P<0.01],postpartum hemorrhage [16.0% (21/131) vs 7.0% (40/572),x2 =10.990,P < 0.01] and uteroplacental apoplexy [5.3% (7/131) vs 0.5% (3/572),x2 =17.650,P<0.01] of twin group were higher than those of singleton group, respectively. The incidence of premature delivery [77.1% (202/262) vs 29.9% (171/572),x2=162.000,P<0.05],NICU hospitalization [76.2% (205/262) vs 58.4%(332/572),x2 =31.980,P<0.05] and HIE [8.4% (22/262) vs 4.7% (27/572),x2 =4.392,P<0.05] of twin group were higher than those of singleton group, respectively. Conclusions Twin pregnancy women are more likely to be complicated with severe preeclampsia and more vulnerable to suffer from severe complications, resulting in poor perinatal outcomes.More attentions should be paid on the management of twin pregnancy.Once hypertensive disorders complicating pregnancy is diagnosed, active management should be provided and timing of termination should be considered.

Tutorial system for undergraduate students is an education administer pattern which can afford the individuation guidance to their study, life and ideal and cultivate their innovative ability, construct individual course system and information composition so as to decrease the blindness in their course selection and study.Actualizing tutorial system for undergraduate students in medical junior college not only has the advantage of general senior education and but also accords with the feature and development advantage of medical junior college.

Objective: To study the apoptosis mechanism of microwave ablation (MA) in tongue cancer Cal-27 cell transplanted tumor in nude mice. Methods: 20 nude mice with Cal-27 cell subcutaneously transplanted tumor were randomly divided into blank control group, 50 ℃ 1 min MA group, 50 ℃ 2 min MA group and 55 ℃ 2 min MA group (n = 5) respecitvely, and were treated with MA with indiated perameters respectively. The apoptosis of tumor cells was observed by HE staining. The expression of Caspase-3 and PCNA protein was detected by immunohistochemical SP method. Results: HE staining showed that microwave ablation induced apoptosis of the tumor cells. Immunohistochemical results showed that the expression of Caspase-3 protein was significantly up-regulated in the microwave ablation groups compared with the control group (P < 0. 05), and the expression of PCNA protein was significantly decreased (P < 0. 05) . In microwave ablation 55 ℃ 2 min group, the expression of Caspase-3 protein was increased more (P <0. 01), and the expression of PCNA protein was decreased more (P < 0. 01) . Conclusion: Microwave ablation may inhibite the proliferation of tongue cancer cells by inducing apoptosis of the cells. The mechanism may be related to the up-regulation of Caspase-3 signaling pathway and down-regulation of PCNA signaling pathway.

Objective:To evaluate the relationship between the euchromatic histone-lysine N-methyltransferase (G9a) and sodium-dependent activation of potassium channel (Slack) in the dorsal root ganglia (DRG) of rats with neuropathic pain (NP).Methods:Forty-eight clean-grade healthy male Sprague-Dawley rats, aged 1 month, weighing 100-120 g, were divided into 4 groups ( n=12 each) by a random number table method: sham operation group (S group), vector plus sham operation group (VS group), vector plus NP group (VN group), and G9a CRISPR/Cas9 knockout plus NP group (GN group). Sham operation was performed at the age of 2 months in group S. In group VS, AAV5 1 μl was microinjected into L 4 and L 5 DRG at the age of 1 month, and sham operation was performed at the age of 2 months. In VN group and GN group, AAV5 and G9a CRISPR/Cas9 knockout plasmid 1 μl were microinjected into L 4 and L 5 DRG at the age of 1 month, and NP model was established by spinal nerve ligation (SNL) at the age of 2 months.Six rats in each group were selected to measure the mechanical paw withdrawal threshold (MWT) and thermal paw withdrawal latency (TWL) before microinjection (T 0), before SNL (T 1), and at 3, 5 and 7 days after SNL (T 2-4). The animals were sacrificed after the last behavioral testing, the DRGs of lumbar segment (L 4, 5) were removed for determination of the expression of G9a, dimethylation of histone H3 at lysine 9(H3K9me2) and Slack (by Western blot). At 7 days after establishing the model, 6 rats from each group were selected to culture the primary DRG neurons. The frequency and amplitude of Slack current in DRG neurons and miniature excitatory post-synaptic currents (mEPSCs) in the spinal dorsal horn were measured by whole-cell patch-clamp technique. Results:Compared with group S, the TWL was significantly shortened and the MWT was decreased at T 2-4, the expression of G9a and H3K9me2 in the spinal dorsal horn was up-regulated, the expression of Slack was down-regulated, the amplitude and frequency of Slack currents in DRG neurons were decreased, and the frequency of mEPSCs was increased in group VN ( P<0.05), and no significant change was found in the parameters mentioned above in group VS ( P>0.05). Compared with group VN, the TWL was significantly prolonged and the MWT was increased at T 2-4, the expression of G9a and H3K9me2 in the spinal dorsal horn was down-regulated, the expression of Slack was up-regulated, the amplitude and frequency of Slack currents in DRG neurons were increased, and the frequency of mEPSCs was decreased in group GN ( P<0.05). Conclusion:The mechanism of NP is related to up-regulating the expression of G9a in DRG, thus inhibiting the expression and opening of Slack channels in rats.
