Purpose:To investigate the toxicities and res po nse of liver cancers to 3-dimensional conformal radiation therapy (3-DCRT) com bined with Trancatheter arterial chemoembolization (TACE) Methods :The histopathological confirmed liver cancer patients were treated by 3-DCRT co mbined with TACE, all patients received TACE before 3-DCRT The total irradiati on dose is 50 Gy to 58 Gy in daily fractions of 2 Gy Each planning target volu me (PTV) received a minimum of 90% of the prescribed dose Each mean liver dose not reached 30 Gy, V 30 Gy (the percentage of normal liver volume with ra diation dose≥30 Gy) less than 33% Results:Thirty patients were included in this study including 2 1 hepatocellular carcinoma Survivals at 1 year after 3-DCRT were 76%, with a median survival time of 8 months 2 patients developed Grade 1 acute liver toxi city and one patient experienced Grade 2 gastrointestinal complications No pat ient developed Grade 2 or greater liver toxicity Conclusions:The use of 3-DCRT technique for liver cancer is a safe and efficient method O ur experience indicated the total irradiation dose above 58 Gy is feasible in da ily fractions of 2 Gy, if the mean liver dose did not reach 30 Gy and V 30 G y

Objective To evaluate the application of operations of preserving spleen in the treatment of splenic injury. Methods Serial operations of preserving spleen were performed in 78 cases in the past 10 years. 18 treated with simple splenorrhaphy,15 with ligation of splenic artery and splenorrhaphy, 13 with partial splenectomy,and 32 treated with autoplasty of spleen. Results All the cases were cured with satisfactory followed-up results.Conclusion The operation of preserving spleen for splenic injury is safe and feasible.

Objective To improve the resistance exercise apparatus of climbing ladder with load on rats, and to observe the technological, athletic training and biological response to the improved experimental apparatus.Methods Thirty-six healthy SPF male Sprague-Dawley were randomly divided into control and exercise groups.The climbing ladder and weight bearing device were separately improved to develop a new resistance exercise apparatus of climbing ladder with load on rats.The exercise group took an 8-week incremental exercise program using this improved device.To observe the operation effect of this improved exercise apparatus.To observe the dynamic changes of average maximum load on SD rats from the exercise group during 8 weeks.To compare the effect of climbing ladder exercise with load on body weights of SD rats between the two groups.Results The improved climbing ladder was marked by the prioritization technique of architecture stabilization, weight bearing and easy cleaning.The improved bearing device has the advantages of being sturdy and durable, and simplemethod to calculating load.Moreover, the new experimental apparatus can be operated by one person during the exercise program.The average maximum load was successfully increased to 756 g after an 8-week exercise using the improved apparatus.Compared with the control group, the body weights of SD rats from the exercise group after training were significantly reduced (P<0.05).Conclusions The improved resistance exercise apparatus of climbing ladder with load can provide a more endurable and repeatable process in the study of rat strength training and it is recommended to use this new device in future experimental research.

Objective To observe the effect of silencing P2X7 receptor (P2X7R) by RNA interference on microglial releasing interleukin 1β (IL-1β),tumor necrosis factor-α (TNF-α) and nitric oxide (NO).Methods The small interfering RNA (siRNA) targeting P2X7R gene was observed.The primary microglial cells activated by amyloid-β (Aβ-42) were infected with the lipofectaminesiRNA.The groups were designated as Aβ-1-42,Aβ1-42/siNC,Aβ1-42/siP2X7R and blank control groups according to the different stimulus.After RNA interference for 48 hours,the microglial cells were collected.The survival rate of microglia was detected by CCK-8.The levels of P2X7R mRNA and protein were detected by Real-time PCR and Western blotting respectively.The microglial morphology and the P2X7R immune response were observed by immunocytochemistry staining.Then levels of IL-1β,TNF-α and NO in the supernatantwere measured.Results The Aβ1-42 and siRNA had no effects on the survival rate of microglia.After RNA interference silencing P2X7R,the expression levels of P2X7R mRNA and protein in Aβ1-42/siP2X7R group were decreased significantly,and in this group,the microglial activity and P2X7R immune response were all reduced.In Aβ1-42,Aβ1-42/siNC,Aβ1-42 /siP2X7R and blank control groups,the supernatant levels of IL-1β were (52.54±3.21) μg L,(54.94±2.54) μg/L,(28.70±3.58) μg/L,(24.55±4.34) μg/L,respectively and the supernatant levels of TNF-α were (64.40±4.80) μg/L,(60.94±1.63) μg/L,(25.69±3.13) μg/l,(19.21±1.97)μg/L,respectively.As compared with Aβ1-42 and Aβ-42/siNC groups,the levels of IL 1β and TNF-α in Aβ1-42 /siP2X7R group was decreased significantly (P＜ 0.05),and IL-1β level had no significant difference between Aβ1-42/siP2X7R and blank control groups.The same result was observed in the level of NOin the supernatant.The levels of NO were (1.33±0.19) μmol/L,(4.49±0.28) μmol/L,(4.78±0.33) μmol/L and (1.07±0.36) μmol/L in Aβ1-42/siP2X7R,Aβ1-42,Aβ1-42 /siNC and blank control groups respectively.Conclusions The silencing expression of P2X7 R by RNA interference effectively decreases the levels of IL-1β,TNF-α and NO released by microglia.P2X7 R can be used as an effective therapeutic target for RNA interference treatment of Alzheimer's disease.

OBJECTIVE To understand the status quo for hospital infection hospital infection to provide a scientific basis. METHODS A cross-sectional survey was taken combined with the hospital bedside investigation and records investigation. RESULTS In 1033 cases,the prevalence rate was 3.87%,and the infected sites were the respiratory site,superficial incision,skin and soft tissue. Utilization rate of anti-bacterial drugs was 60.31%,the pathagen detection rate before treatment was low only 7.73%. CONCLUSIONS Prevalence rate survey method is simple and reliable,it may be the basic reflect of hospital infection. Further strengthening the management of invasive operations,regulateing the rational use of antibiotics status quo,improving the detection rate of pathogens and reduceing preventive medication and antibiotic usage are all evitable.

AIM: G protein-coupled inwardly rectifier potassium (GIRK) channel are distributed widely in mammalian brain. In CNS, GIRK 1/2 seems to be the predominant heterotetramers which play a pivot role in the regulation of the excitability of neurons and may contribute to the resting potential by leading to a hyperpolarization of membrane potential and reduction of the action potential frequency. In the context, the Weaver mouse is the first neurological abnormality directly linked to a genetic point mutation in the GIRK2 protein which includes spontaneous seizure. GIRK2 knock out mice showed normal development but more susceptible than normal mice to seizure induced by GABA antagonist. Here, we report that the mRNA and protein expression of GIRK subunit 2 is altered in kainic acid(KA)-induced epileptic rat hippocampus. METHODS: Rats were injected with kainate 14 mg/kg intraperitoneally to establish an acute and chronic temporal lobe epilepsy model. At chronic spontaneous seizure stage, by using of in situ hybridization, immunocytochemistry and Western blotting, the GIRK 1,2 mRNA and protein were analyzed quantitatively in the dentate gyreus, CA1, CA3 regions of hippocampus. RESULTS: GIRK1,2 mRNA and proteins were expressed abundantly in all regions of hippocampus. KA induced seizures and caused a significant increase in GIRK2 mRNA abundance and immunoreacitivity; only GIRK1 mRNA was increased significantly, but no difference was found by Western blotting protocol. CONCLUSION: GIRK1,2 mRNA and protein expression in the hippocampus of epileptic rat brain is up-regulated, which may be an adaptive response to over-excitability of neuron networks and prevent the over-excitability spread in hippocampus (DG-CA3-CA1). Objective To investigate the efficacy of continuous veno-venuous hemodialysis/filtration(CVVHD/F) for the treatment of multiple organ dysfunction syndrome(MODS)caused by severe infection and to explore the mechanism in children.Method Nineteen cases of pediatric septic shock with MODS were treated with CVVHD/F in Children's Hospital Affiliated to Shanghai Jiaotong University from December 2002 to November 2007.The clinical data were studied including mortality rate,serum electrolytes,arterial partial pressure of oxygen (PO2),artery partial pressure of carbon dioxide(PCO2),FiO2/PO2,urine output,blood pressure,doses of vasoactive agents,Cr,BUN,etc.Results Cannulation and CVVHD/F were well performed in a total of 19 cases,with median age 33.4±36.5 months(from 3 months to 8 years) ,with their gender ratio of male(13 cases)to female (6 cases) to be 68.4% and 31.6%.The mean pediatric crifcal illness score(PCIS) was 69.1±10.4 and Median Pediatric Risk of Mortality score(PRMS Ⅲ)12.66±7.85,respectively.The duration of CWHD/F was 92 hours(ranged from 16 hours to480 hours).FiO2/PO2,PCO2,and PO2 were iraproved significantly after 12 to 24 hours CVVHD/F in patients with acute respiratory distress syndrome(ARDS) or lung edema (P<0.05).The concentrations of serum kalium,natrinm and HCO3- level resumed to well-balanced in 24 hours (P<0.05).The serum Cr and BUN were decreased to normal range(P<0.05).The mortality rate was 63.2%.Conclusions CVVHD/F was effective for treatment of septic shock with MODS in pediatric by improving oxygenation,maintaining normal serum electrolytes,conecting metabolic acidosis,increasing the tissue perfusion and eliminating the serum Cr and BUN.

Objective To compare the different features of contrast enhanced ultrasound(CEUS) and CT angiography (CTA) in abdominal aortic remodeling after thoracic endovascular aortic repair (TEVAR)of DeBakey type Ⅲ aortic dissection(AD),and to investigate the application value of CEUS in the follow-up of DeBakey type Ⅲ AD after TEVAR.Methods CEUS was performed in 28 patients with DeBakey type Ⅲ AD after TEVAR,in which 8 cases type Ⅲ a (simple type) were not involved of abdominal aorta,20 cases Ⅲ b(complex type) were involved in the abdominal aorta,all patients were performed CTA examination of aorta.The functional and morphological changes of abdominal aortic remodeling were observed by two methods in DeBakey type Ⅲ AD after TEVAR.Results There was no new rupture of the abdominal aorta in 8 cases type Ⅲ a AD,but 20 cases of type Ⅲ b AD showed the situation of ruptures,thrombosis,trues and falses lumen after 3 months of follow-up.CEUS was less than CTA in detecting the number of ruptures (P＜0.05),CEUS and CTA results were similar in the measurement of true and false lumen diameter (P＞0.05),but CEUS clearly showed the hemodynamic characteristics of abdominal aortic intimal tear,the size and position of rupture,moving blood flow of rupture,and true or false lumen,which provided more accurate data for the long term abdominal aortic remodeling of DeBakey type Ⅲ AD after TEVAR.Conclusions CEUS and aortic CTA can play a complementary role in DeBakey type Ⅲ AD after TEVAR of long-term follow-up,it is worth promoting.
