1.Introduction to the Four Principles of Biomedical Ethics in Confucius

It is not only the problem of the real life,but also the challenge of the biomedical ethics for how to get the balance and compromise between the principlism and situationism.The fairness in Confucian is constructed on the basis of the external nature,and also is a standard and content-full ethical principle,which is different from the political one in the western culture.The Confucian basis is the principlism but does not refuse the situationism.In the four principles of Confucian,both fairness and respect for autonomy belong to the principlism,while the other two of knowing where to stop and no harm or the principle of benefit of utilitarianism are the basis of the situationism.The principle of knowing where to stop is the internal core spirit of the Confucian moral virtue and ethical principles,but fairness is the first principle of its all external principles.
2.Perioperative nutritional support in patients with obstruction caused by colon cancer

Objective:To investigate the importance and appropriate application of perioperative nutritional support in patients with obstruction caused by colon cancer.Methods:19 patients received parenteral or enteral nutritional support in the period of perioperation.When nutrition was applied,the nutritional indices,liver function and renal function were observed.Results:Hb,blood K~(+) and Na~(+) were significantly increased after nutritional support.The nutritional indices did not change and were kept within normal range during nutrition.Conclusion:The nutritional support is one of the important perioperative treatments in patients with obstruction caused by colon cancer.
3.Application of mini-nutritional assessment in malnutrition risk evaluation of elderly inpatient with cardiovascular disease

Objective To investigate the application of the mini nutritional assessment (MNA)in the malnutrition risk eval-uation of elderly inpatients with cardiovascular disease(CVD).Methods The MNA method was adopted to detect the MNA scores in 316 elderly inpatients with CVD.The inpatient′s nutritional status was evaluated according to the MNA scores.Results Among 316 cases,52 cases(16.46%)were malnutrition,60 cases (18.99%)had the malnutrition risk and 204 cases (64.56%)were in a good nutritional status;the occurrence rate of malnutrition and malnutrition risk in the female elderly patients were higher than those in the male patients,the differences were statistically significant(P <0.01);the occurrence rate of malnutrition and malnutri-tion risk in the elderly inpatients(≥80 years old)were higher than those in the low age patients(60~<80 years old),but the differ-ences had no statistical significance(P >0.05);the occurrence rate of malnutrition and malnutrition risk in the rural patients were higher than those in the patients lived in city,the differences were statistically significant(P <0.05).Conclusion Elderly inpatients with CVD have the higher prevalence of the malnutrition risk,the MNA can be applied in elderly inpatients with CVD for conduc-ting the nutritional status monitoring and early screening the malnutrition risk.
4.Influence factors analysis of acute-on-chronic liver failure effects of chronic hepatitis B virus infection

Objective To investigate the influence factors of chronic hepatitis B virus(HBV) infection complicated with chronic liver failure.Methods One hundred and eighty-six chronic HBV infection patients with chronic liver failure were selected as our subjects,who were hospitalized in the Affiliated Hospital of Hebei United University from Jul.2008 to Dec.2013 and they served as case group.Meanwhile,186 patients with chronic HBV infection were selected and served as control group,who were hospitalized during the same period.A self-mad questionnaire was used to collect the information.The influence factors related to HBV infection complicated with acute on chronic liver failure were recorded.Results Multivariate conditional Logistic regression analysis showed that 8 variables were risk factors in terms of chronic HBV infection complicated with acute on chronic liver failure and they were virus overlap infection (OR =6.523,95% CI:2.034 -10.030),drug application (OR =9.012,95% CI:3.018-13.241),alcohol (OR =7.2520,95% CI:1.985 -11.247),bacterial infection(OR =4.378,95% CI:2.032-5.648),surgical operation (OR =8.514,95% CI:2.114-17.253),emotional stress and fatigue (OR =2.217,95% CI:1.729-5.648),genetic (OR =11.124,95% CI:2.168-13.429),high PCR-HBVDNA quantity (OR =1.628,95% CI:1.504-3.282).And one protective factorwas the usage of antiviral drug(OR=0.163,95%CI:0.085-0.417).Conclusion The risk factors include virus over infection,application of hepatotoxic drugs,disease before drinking,bacterial infection,surgical operation,emotional stress and fatigue,the genetic parents and high PCR-HBVDNA quantification;and antiviral drugs application is the protective factor in terms of Chronic HBV infection complicated with acute-on-chronic liver failure.
5.Application of Inorganic Layered Materials in Electrochemical Sensors

Inorganic layered materials with the open structure of low dimension and good biological compatibility and stability are considered as one of the most promising materials for immobilizing biomolecules, which can not only adjust the spacing of layers according to guest molecular size to prevent leakage of guest molecule, but also reduce the effects of surrounding elements on immobilized molecules significantly. Moreover, they can promote electron transfer between enzyme and the electrode. Therefore, the inorganic layered materials have great application potential in electrochemical sensors. This paper reviews the ways of immobilizing guest molecules of inorganic layered materials and the applications and development prospects of the different types of inorganic layered materials including cationic, anionic and nonionic inorganic layered materials in electrochemical sensor.
6.Clinical analysis of 4 cases of Fasciola gigantica infection from Binchuan County, Yunnan Province

Objective To explore the epidemiological characteristics,clinical manifestations,diagnosis and treatment of Fasciola gigantica infection,and to provide reference for future diagnosis and treatment.Methods Four cases of confirmed Fasciola gigantica infection were analyzed retrospectively for epidemiological information,clinical manifestations,laboratory results,imaging findings,diagnosis and treatment outcomes.Results Four Fasciola gigantica infection cases were farmers from Binchuan County,Yunnan Province.All presented with fever,hepatalgia,percussion pain,hepatomegaly,eosinophilia,hyperglobulinemia,neutrophilia and anemia.Computed tomography (CT) scans showed multiple low density shadows scattered in the liver parenchyma.Eggs of Fasciola gigantica were detected in feces of the patients,and results of antibody against Fasciola gigantica detected by enzyme-linked immunosorhent assay (ELISA) were positive.Triclabendazole treatment was effective.Conclusion Human Fasciola gigantica infection is rare,without specific clinical manifestations,and triclabendazole is the first choice of treatment.
7.MR guided focused ultrasound for treating uterine fibroids:a Meta analysis

Objective To investigate the recent effect and safety of magnetic resonance guided focused ultrasound (MRgFUS) for treating uterine fibroids with a Meta analysis. Methods Such databases as Cochrane Library,PubMed,EMbase, CBM,CNKI,CQVIP and Wanfang were searched from their establishment. After thestudy selected, assessment and data extraction conducted by two reviewers independently, Meta-analysis were performed by using the Comprehensive Meta-analysis V2 software. The volume of non-perfusion ratio (NPVR), fibroid shrinkage, clinical symptom severity (SSS score) and adverse reactions were analyzed.Results Nighteen retrieved articles were included, with a total of 1 549 patients. The results of Meta-analysis showed that:(1)nonperfused volume ratio (NPVR),Mean=57.1%(95%CI 45.0% —69.2%).(2)Percentage of FBs shrinkage:mean=31.4% (95% CI 29.2% —33.6%).(3)Symptom severity score (SSS):baseline mean=51.00(95%CI 41.61—60.40),3 months mean=32.50 (95%CI 25.90—39.10),6 months mean=28.62(95%CI(23.57—33.66),12 months mean=24.46(95%CI 15.44—33.49), the average of the severity score of the patient symptoms had a tendency to reduce.(4)The degree of adverse reactions were mild and could relieve itself, there was fewer serious adverse reactions. Conclusions The current evidence indicates that MRgFUS in treatment of uterine fibroidsis reliable and safe. MRgFUS can significantly reduce the volume and the symptom severity score ofthe patients.
8.Perceived stigma, mental health and unsafe sexual behaviors of people living with HIV/AIDS

Objective: To determine the relationship among perceived stigma, mental health and unsafe sexual behaviors of people living with HIV/AIDS. Methods: Cross-sectional research was used to interview people living with HIV/AIDS (PLWHA) from April 2012 to March 2013 in Changsha, China. The questionnaires included General Questionnaire, Sexual Behavior Questionnaire, Patient Health Questionnaire Depression Scale (PHQ-9), General Anxiety Disorder Scale (GAD-7) and Psychometric Assessment of the HIV Stigma Scale. hTe results were statistically analyzed with SPSS18.0. Results: hTe total score of perceived stigma and its 4 dimensions were positively correlated with anxiety and depression. The total score of perceived stigma and its dimensions were associated with disclosure, but no signiifcantly correlated with other sexual behaviors. Hierarchical regression showed perceived stigma had an effect on anxiety. Conclusion: hTe stigma perceived by PLWHA is above the average level. Perceived stigma has an effect on mental health, especially anxiety, but no effect on unsafe sexual behaviors.

The establishment of medical morality and behavior appraisal system and research on its application developed in comprehensive hospitals were clarified.Several aspects related to the medical morality and behavior appraisal,such as index,standard,approach,method,record and the result,were investigated and practiced,and the establishment of a scientific and reasonable medical morality and behavior appraisal system was investigated in the aspects of consummating the organization management of medical morality appraisal,subdividing the medical morality appraisal system,quantifying the medical morality appraisal standard,operating and performing the medical morality appraisal and establishing the electronic archives of medical morality appraisal to construct an efficient incentive and restraint mechanism,and moreover,to promote the establishment of hospital convention and the enhancement of the overall management level.
10.Effect of probucol on inflammatory response in rabbits with arteriosclerosis obliterans

Objective To explore the effect of probucol on the inflammatory response in rabbits with arteriosclerosis obliterans.Methods SPF male new zealand white rabbits were subjected to incomplete right femoral artery ligation model of arteriosclerosis obliterans.The model rabbits were divided into experimental group and control group.The rabbits in experimental group were treated by probucol.After treatment for 4 weeks,HE staining was used to evaluate the injury severity of right femoral artery in rabbits;ELISA was used to measure the levels of IL-1,IL-6,TNF-α and CRP;Realtime PCR was used to detect the expressions of IL-1,TNF-α in right femoral artery;Western blot was used to measured the nucleus accumulations of NF-κB,and the phosphorylation of IκB.Results Compared with control group,the degree of pathological injury of right femoral artery was significantly atteunated,the levels of IL-1 、IL-6 、TNF-α and CRP in plasma,the mRNA expressions of IL-1 、TNF-α and the nucleus accumulations of NF-κB,and the phosphorylation of IκB in right femoral artery decreased significantly.Conclusion Probucol attenuates the inflammatory response in rabbits with arteriosclerosis obliterans significantly.
