1.Efficacy observation of shugan-jieyu capsule and escitalopram in treating post-stroke depression

Objective To explore the efficacy of shugan -jieyu capsule and escitalopram in treating post -stroke depression (PSD).Methods 80 cases with PSD were randomly divided into the study group(administered with shugan -jieyu capsule and escitalopram)and the control group(administered only with escitalopram),40 cases in each group.Hamilt Depression Rating Scale(HAMD),Modified Einberg -Scandinavian Stroke Scale(MESSS)and Treatment Emergent Symptoms Scale(TESS)were used to assess treatment effect before treatment and at the weekend of 2nd,4th,8th after treatment.Results The scores of HAMD and MESSS in the study group were much lower than those in the control group.The scores of HAMD at 4th weekend and 8th weekend in the study group were (16.92 ± 3.75),(8.60 ±3.25),those in the control group were (19.27 ±4.11),(11.13 ±3.72)(t2w =-2.671,t4w =-3.239,P <0.05).The scores of MESSS at 4th weekend and 8th weekend in the study group were (20.06 ±3.87), (12.47 ±3.97),which in the control group were (21.68 ±3.12),(15.09 ±3.20)(t2w =-2.061,t4w =-3.250, P <0.05),while the effective rate in the study group(80.0%)was significantly higher than that in the control group (45.0%)(χ2 =5.227,P <0.05).Conclusion The efficacy of shugan -jieyu capsule combined with escitalopram for PSD is superior to use escitalopram alone.

3.Efficacy of low -frequency repetitive transcranial magnetic stimulation and setraline in treating Parkinson's ;disease patients with depression

Objective To explore the efficacy of low -frequency repetitive transcranial magnetic stimulation (rTMS)and setraline in treating Parkinson's disease patients with depression.Methods 80 cases of Parkinson's dis-ease with depression were randomly divided into the study group(administered with rTMS and setraline)and the con-trol group(administered only with setraline)according to random number table,40 cases in each group.Hamilt De-pression Rating Scale(HAMD),Unified Parkinson's Disease Rating Scale(UPDRS)and Treatment Emergent Symp-toms Scale(TESS)were assessed before treatment and 1,2,4 weeks after treatment.Results The scores of HAMD and UPDRS in the study group were lower than those in the control group[the score of HAMD at 2nd week in the study group was (18.44 ±5.56 )points,the score of HAMD at 4th week in the study group was (10.51 ± 4.27)points,the score of HAMD at 2nd week in the control group was (21.23 ±5.18)points,the score of HAMD at 4th week in the control group was (14.05 ±4.10)points,t2w =-2.322,t4w =-3.782,all P <0.05],and the score of UPDRS at 2nd week in the study group was (30.14 ±4.92)points,the score of HAMD at 4th week in the study group was (28.39 ±3.38)points,the score of UPDRS at 2nd week in the control group was (33.74 ±4.87)points, the score of HAMD at 4th week in the control group was (31.09 ±3.94)points (t2w =-3.289,t4w =-3.290,all P <0.05),while the effective rate of the study group(77.5%)was significantly higher than that in the control group (45.0%)(χ2 =4.450,P <0.05 ).Conclusion The efficacy of rTMS combined with setraline for Parkinson 's disease with depression is superior to that of using setraline alone.

5.Antidepressant Effects of Yiyukang in Mice and Rats

Objective To study the antidepressant effect of Yiyukang and to explore its possible mechanism.Methods By using the mice forced-swimming models,mice tail suspension models and mice stereotyped behavior models,the antidepressant effect of Yiyukang and its mechanism were observed.Result In the mice forced-swimming test,the im- mobility-time was significantly reduced by high and medium dosage Yiyukang with the reduced rate being 21.65% and 24.84%(P<0.01).The high dosage can also reduce the immobility time of tail-suspension mice with the rate of 29.58%(P<0.01).Yiyukang can strengthen the mice head shaking behavior and had synergistic action with 5- HTP on reducing stereotyped behavior in mice.Conclusion Yiyukang has certain antidepressant effect,and the mecha- nism may be associated with promoting the actions of 5-hydroxytryptamine and dopamine in mice brain.
6.Determination of Bis-(5-formyl-furfuryl)-ether and 2,4-Dihydroxy-6-methoxy-3-methyl-acetophenone in the Root of Yuexiandaji(Euphorbia ebracteolata) by HPLC

An HPLC method for the quantitative analysis of bis (5-formyl-furfuryl ) -ether an d 2, 4-dihydroxy-6- methoxy-3-methyl- acetophenone in Chinese drug,Langduc (Euphorbia ebracteolata)was developed. The solvent system used was methanol-water-10% acid (45 : 55 : 2 ) (adjusted to pH 5.4 by 10% ammonia ) on ODS columm at 289nm.Tetrahydro palmatine was used as an internal standard. The operation can be completed in 30min.This method is sensitive,simple and accurate with good reproducibility.It can be applied to the quality control of the crude drug of E. ebracteolata.
7.Basic researches on the construction of tissue-engineered meniscus

BACKGROUND:The meniscus injury is one of the most common sports injuries of knee joint. The treatment after injury is always a difficult problem in the clinic. Researchers have been trying to apply the method of using tissue-engineering to solve the problem of meniscus repair after injury.
OBJECTIVE:To summarize tissue-engineered meniscus-related basic research.
METHODS:The first author retrieved China National Knowledge Infrastructure and Medline for literatures on tissue-engineered meniscus. The key words were“meniscus, tissue engineering, basic research”. Articles with unrelated objective and repeated articles were excluded. Final y, 35 articles were included.
RESULTS AND CONCLUSION:There was some controversy about the classification of the cel of the meniscus. The outer tissue of the meniscus is mainly composed of the fibrous cartilage cel s and the extracel ular matrix. The inner tissue of the meniscus is mainly composed of a smal atypical class of cartilage cel s. The biomechanical properties of the meniscus can be properly adjusted when it is subjected to external pressure. The lateral meniscus is assumed to bear al the pressure of the outer part of the knee joint during knee flexion, but the medial meniscus is assumed to bear 50%of the medial part of the knee joint. The etiology and pathophysiology of meniscus injury are not the same, which is highly dependent on the age of onset of the patient. However, in each age group, right knee meniscus injury was the majority. The way of surgical repair of the meniscus tear can be divided into the technology from inside to outside, and that from outside to inside, total internal repair under arthroscopy and open repair technology.
8.Factors Affecting the Preverbal Communication Skill in Prelingually Deaf Children with Cochlear Implants

Objective To study the factors affecting preverbal communication skill in prelingually deaf children with cochlear implants.Methods A total of 31 prelingually deaf children with cochlear implantation participated in the study.They received cochlear implantation the age of 12 months with a mean of 46.71 months.The video analysis were applied to assess the preverbal communication skill including rurn-taking,autonomy,eye contact and auditory awareness.According to the implant age,preoperative use of hearing aids,preoperative speech rehabilitation training,preoperative surgerg ear averrage residual hearing.Wilcoxon signed-rank test was used to evaluate the differences among the groups.The implant age distribution:16 cases in ≤36 months group;15 cases in >36 months group.Preoperative speech rehabilitation training (recovery time more than 3 months) : 14 cases in rehabilitation group, 17 case in no rehabilitation group.Preoperative surgerg ear averrage residual hearing:6 cases in ≤80 dB HL group;25 cases in >80 dB HL group.Preoperative use of hearing aids (use time more than 3 months) distribution: 21 cases in use group, 10 cases in no use group.Results There was statistical signifficance in preoperative use of hearing aids,preoperative speech rehabilitation training,preoperative surgerg ear averrage residual hearing(P<0.05)and there was no statistics significance group of implant age(P>0.05).Conclusion Preoperative use of hearing aids,preoperative speech rehabilitation training,preoperative surgerg ear averrage residual hearing in prelingually deaf children with cochlear implants relate with better preverbal communication still.
9.Safety and Feasibility of Femoral Vein Thrombectomy for Deep Vein Thrombosis in the Lower Limbs

Objective To investigate the safety and feasibility of femoral vein thrombectomy for patients with acute deep vein thrombosis in the lower limbs.Methods During June 2003 to August 2006,a succession of 43 patients with acute deep vein thrombosis in the lower extremities received femoral vein thrombectomy in our hospital.An inferior vena cava filter was employed before the operation.Anticoagulation and regional thrombolysis were taken postoperatively.Analyse the data of operation procedure,curative effect and complications.Results The operation time ranged from 55 to 115 minutes with a mean of 92.6 minutes.Four of the patients received blood transfusion during the operation.The mean blood loss was 384.6 ml (100 to 1000 ml).Five patients showed transient fluctuation of blood pressure during the surgery.None of the patients had pulmonary embolism during and after the operation. Five patients developed lymph leakage from the incision at the inguinal area,and was cured in 1 week.One aged patient suffered from postoperative postatic pneumonia and was cured before being discharged;no other complications were found in this series.The thrombectomy achieved good outcomes in our patients.The edema and pain in the lower limbs were relieved quickly after the operation, the difference between the circumferences of the two legs decreaed from a median of 5.5 cm (thigh) and 4.0 cm (crus) to 1.5 cm and 1.0 cm (Z=-5.345,P=0.000 and Z=-5.461,P=0.000),respectively,32 of the patients received re-examination 6 months postoperation,none of them showed recurrent thrombosis.Conclusions Femoral vein thrombectomy is feasible and safe for serious acute deep vein thrombosis in the iliac and femoral veins.Patients can recover quickly after the operation.
10.Pre-occlusion and Stump Pressure of the Internal Carotid Artery in Carotid Endarterectomy

Objective To investigate the value of the pre-occusion and stump pressure(SP)of the internal carotid artery in carotid endarterectomy(CEA).Methods During Januray 2007 to January 2009,12 patients with carotid arterial stenosis underwent CEA in our deparment.With cervical plexus block,we performed pre-occlusion test and measured the stump pressure(SP)by using vascular shunt selectively.The patients were monitored with clear consciousness during the whole procedure of CEA.The data of postoperative complications and results of pre-occlusion and SP as well as the follow-up were analyzed afterwards.Results During the operaion,three patients had transient increase of heart rate and blood pressure.The lowest SP was detected in two patients,who had moderate or severe stenosis of the contralateral carotid artery complicated with lesions in the vertebral basilar artery.Two patients showed positive results in pre-occusion test,and vascular shunt was used in four cases.After the operaion,one patients developed hyperperfusion syndrome.Nine patients showed improvement of symptoms of dizziness and decreased eyesight.This series were followed up for a mean of 11 months(ranged from 1 to 26 months).During the period,two of the patients showed re-stenosis of the carotid artery;none of them had cerebral infarction.Conclusions Cervical plexus block combined with pre-occusion test ensures the safty and effectiveness of minimally invasive carotid endarterectomy.We recommend selective vascular shunt according to the results of pre-occlusion test,SP,and the severity of the lesions in the contralateral carotid artery and vertebral basilar artery.
