2.Review of seven cases of neurobrucellosis: clinical manifestations and imaging characteristics

Objective To explore the clinical features,laboratory tests,imaging characteristics of neurobrucellosis,and to improve the understanding of the disease.Methods A retrospective analysis was performed in seven patients with neurobrucellosis in our hospital from January 2002 to May 2013.Results The proportion of men and women was 6∶ 1,and the average age was 46 years.One case was attacked with the symptoms of cerebrovascular disease,another one with the symptoms of myelitis,and the other five with the symptoms of meningoencephalitis.Six had positive results of serum agglutination test,and the other one had a positive result of blood culture instead.Imaging findings lacked of specificity.There were two patients whose lesions were in the hemisphere cortex,subcortical,basal ganglia and other parts,four patients whose lesions were in the frontal lobe,one in the chest pulp,and one in both sides of cerebellum and pons.After systematical treatment,one case died,the other 6 cases recovered,and no one relapsed or got functional disability during the one-year follow-up.Conclusions Neurobrucellosis is a rare type of brucellosis,and its clinical manifestations lack of specificity,and imaging and serological detections are important for diagnosis.It is beneficial for patients if diagnosed early and treated systematically,so good understanding of this disease,early diagnosis and treatment are of great value.
3.Investigation of sexual behavior of floating population in STD clinics

Objective To understand the situation of sexual behavior and STD in floating population attending STD clinics Methods Floating population in STD clinics were investigated by questionnaires.Results Sexual behavior rate was 60.0% in unmarried floating population.Sexual behavior rate was 47.6% before marriage at the first sexual time.46.8% of the patients had more than 2 sex partners,and the rate of condom use among them was under 50%.STD prevalence rate was 37.1%,Re-infection rate was 12.2% accounting for 32.8% in all investigated STD patients.Conclusions It is importent to standardize service in STD clinics,provide health information,raise prevention knowledge,change their sexual attitude,advocate safety sexual behavior in various ways in high risk groups.
4.Establishment of a Myelin Basic Protein Fragment-Induced Wistar Rat Model of Autoimmune Encephalomyelitis

Objective To establish an experimental autoimmune encephalomyelitis (EAE) model in Wistar rats with myelin basic protein fragment (MBP69-85) and observe its pathological changes. Methods MBP69-85 dissolved in normal saline was mixed with complete Freund's adjuvant (CFA) (including 6 mg bacille Calmette Guerin) to prepare the encephalitogenic emulsion. Ten out of 70 Wistar rats were chosen as a control group, the others were divided equally into group A,B,C according to the difference of the encephalitogenic emulsion. Rats in group A were immunized with 50 μg MBP69-85 +CFA (including 6 mg BCG). Rats in group B were immunized with 25 μg MBP69-85+CFA (including 6 mg BCG). Rats in group C were immunized with 25 μg MBP69-85+CFA (including 12 mg BCG). The pathological changes of brain and spinal cord tissues were examined by light microscopy after HE staining and immunohistochemistry of MBP and NF. Results Some of the Wistar rats immunized with 50 μg MBP69-85 showed disorder at 12~16 days after immunization. The clinical symptoms included tail acratia or paralysis of tail and limbs, head tilt, etc. and the mean score was 2.38±1.89. There were infiltration of inflammatory cells inside nervous tissue and perivascular cuffings in HE stained sections. The immunohistochemistry of MBP and NF showed demyelination in the white matter and axon injury. Conclusion To some extent, the establishment of EAE depends on the dose of the immunizing antigen. The BCG in CFA was not the major cause of morbility of the rats. The EAE model induced with MBP69-85 in Wistar rats, showing typical clinical symptoms and pathological changes of multiple sclerosis, is a reliable animal model for the study of pathogenesis and treatment of multiple sclerosis.
5.The clinical analysis of nervous system damage in 4 cases with chronic manganese poisoning

Objective To summarize the clinical and laboratory features of chronic manganese poisoning. Methods A retrospective analysis was conducted on the clinical data of 4 cases with chronic manganese poisoning, including gener?al information, medical history, clinical manifestations, laboratory examination such as electrophysiological and imaging. Results Patients with chronic manganese poisoning mainly presented with mild mental disorder and autonomic nerve dis?order during early stage and then gradually developed extrapyramidal symptoms and signs. The laboratory examination of chronic manganese poisoning lacked of specificity. EMG showed neurogenic damage in 3 cases and normality in 1 case. EEG showed slightly increased full guide slow wave in 1 case and normality in 3 cases. cranial MRI revealed the damag?es in bilateral symmetry of the basal ganglia nuclei in 4 cases of Chronic manganese poisoning. There was no significant correlation between the changes of urinary manganese level before or after treatment and the clinical manifestations. Conclusions Although there is lack of specific clinical manifestations of chronic manganese poisoning, the possibility of this disease should be considered when patients with mild mental disorders or autonomic nerve disorder with or without extrapyramidal symptoms. The main treatment of chronic manganese poisoning includes excretion of manganese, symp?tomatic and supportive treatment. Patients usually have the sequelae of tremor, muscle tension, and other symptoms.
6.The changes of ghrelin, growth hormone, growth hormone releasing hormone and their clinical significances in patients with chronic obstructive pulmonary disease

Objective To investigate the changes of plasma ghrelin,growth hormone (GH) and growth hormone releasing hormone (GHRH) and gastric ghrelin in patients with chronic obstructive pulmonary disease( COPD ) and to explore their clinical significances.Methods Plasma ghrelin,GH,GHRH,TNFα,IL-6 and C reactive protein (CRP) were measured in 40 COPD patients and 20 controls with chronic bronchitis. Correlated factors of plasma ghrelin,TNFα,IL-6,CRP were analyzed. Body composition was assessed with bioelectrical impedance analysis.The expression of gastric ghrelin in patients with COPD was detected.Results Plasma ghrelin was higher in the underweight patients than in the normal weight patients and in the controls [ ( 1.78 ± 0.46 ) ng/L,( 1.39 ± 0.46 ) ng/L,( 1.36 ± 0.39 ) ng/L,respectively].Plasma GH was lower in the underweight patients than in the normal weight patients and in the controls [(4.12 ±0.83) μg,/L,(5.17 ±0.72) μg/L,(6.49 ± 1.13) μg/L,respectively].Plasma GHRH was lower in the underweight patients than in the normal weight patients and in the controls [ (20.43 ± 4.41 ) ng/L,(23.47 ± 3.97) ng/L,( 27.48 ± 10.06) ng/L,respectively ].Plasma ghrelin was higher in the underweight patients than in the controls ( P < 0.01 ).Plasma ghrelin was higher in the underweight patients than in the normal weight patients with COPD.Plasma ghrelin (log transformed) was negatively correlated with BMI and percentage of body fat in the COPD patients.Plasma GHRH was positively correlated with ghrelin in the underweight patients ( r =0.515,P < 0.05 ),while no correlation was found between plasma G H and ghrelin in the underweight patients (r =0.415,P > 0.05 ).Plasma ghrelin was positively correlated with TNFα and IL-6 in the underweight patients.The gastric expression of ghrelin showed no evident difference between the patients with COPD and the controls.Conclusions The plasma GH in COPD patients may not be correlated with ghrelin.The plasma ghrelin level may be a useful indicator for malnutrition in COPD patients.Plasma ghrelin might be involved in the pathogenesis of CODP by affecting the body energy metabolism.
7.Study on human UDP-glucuronosyltransferase (UGT) isoforms involved in in vitro metabolism of trans-resveratrol.

OBJECTIVETo study major human UGT isoforms involved in trans-resveratrol (TR) phase II metabolism.
METHODtrans-resveratrol and 12 major human UGT isoforms were incubated in vitro and then glucuronic acid metabolites were determined by HPLC-MS, in order to preliminarily analyze the structure and observe the effect of different UGT isoforms on the generation rate of glucuronic acid metabolites.
RESULTIn in vitro metabolic system, two metabolites-4'-O-monoglucuronide resveratrol (M-1) and 3-0-monoglucuronide resveratrol (M-2)-were generated from trans-resveratrol after being catalyzed by UGT. During the cause, generation of M-1 and M-2 were catalyzed by UGT1A1, UGT1A3, 1A8, 1A9 andlA10, whereas only UGT1A6 and 1A7 contributed to the forma-tion of M-2. Both the formation rate of M-1 and M-2 catalyzed by UGT1A1, 1A10 and the formation of M-2 catalyzed by UGT1A8 slowed down with the increasing concentration of substrates, causing the phenomenon of "substrate inhibition".
CONCLUSIONUGT1A1, 1A8, 1A9, 1A10 get involved in the formation of M-1, and of them UGTIA9 is the most important contributor. UGT1A3 also makes small contribution to the formation of M-1 and M-2, while other UGT isoforms show hardly any reaction with the trans-resveratrol phase II metabolites.

8.Percutaneous occlusion of left atrial appendage in experimental canine models:the establishment of the delivery pathway

Objective To evaluate the feasibility and safety of a delivery pathway for the performance of percutaneous left atrial appendage (LAA) occlusion in experimental canine models. Methods Transseptal puncture was performed via femoral vein approach under fluoroscopic and angiographic guidance in 12 experimental dogs. A pigtail catheter was advanced into the left atrium (LA), which was followed by LA angiography. The diameters of the neck of LAA were measured on LAA angiogram obtained in appropriate projection. After the delivery sheath was advanced along the wire into LA, a pigtail catheter was inserted into the ostium of the LAA and the sheath was then advanced over the pigtail into the LAA. LAA angiography was then performed through the delivery sheath to confirm the position of the delivery sheath. One hour after the procedure both electrocardiography (ECG) and transthoracic echocardiography (TTE) were carried out in five dogs to check the results, immediately after which the five dogs were sacrificed to macroscopically observe the damages of the puncture site of inter-atrial septum as well as inside the LA and LAA. One hour and 2 weeks after the procedure TTE was conducted in the remaining 7 dogs and these dogs were followed up for one month. Results One dog died of pericardial tamponade during the operation. In 8 dogs the LAA was clearly displayed in the projection position of right anterior oblique (RAO) 30°/cranial (CRA) 20°,while in 3 dogs the LAA was well visualized in the projection position of RAO 30° , and in one dog in the projection position of RAO 30°/caudal (CAU) 20°. The diameter of LAA neck was (13.6 ± 5.2) mm. The delivery sheath was safely advanced into the LAA along the pigtail catheter in all dogs, and no air embolism, thrombus or pericardial tamponade occurred. Hematoma at puncture point of groin occurred in 2 dogs, which was absorbed through pressure dressing. Macroscopic examination of the heart performed immediately after the operation showed that no bloody pericardial effusion was found, and mild hematoma at posterior wall of LA was seen in one dog and mild damage of the upper-margin intima of LAA was noted in 2 dogs. The mean fluoroscopy time was (10.1 ± 2.5) minutes and the mean operation time was (58 ± 12) minutes. TEE showed no pericardial effusion 2 weeks after the procedure. During the follow-up period of one month no sudden death, stroke or infection occurred. Conclusion This method of placing the delivery sheath into the LAA is clinically safe and effective, and it can reliably establish a pathway to advance the LAA occluder into LAA.

Ischemic stroke is one of the most important causes of death and disability in humans,but the effective methods for treating brain injury after stroke are quite limited.Sphingosine 1-phosphate (S1P) is a pleiotropic lipid.There is certain interdependence between its metabolism and regulation and the molecular mechanisms involved in important biological events following cerebral ischemia.Membrane lipid therapy with S1P as the core may be an effective neuroprotective strategy of ischemic stroke.
10.Establishment of an intervention model for adolescent obesity based on component isochronous substitution method

Objective:
To explore the relationship between isochronous substitution and BMI, waist circumference (WC), and body fat rate (FAT) among physical activity (PA), sedentary (SB), and sleep (SLP), so as to provide effective measures for obesity control in adolescents.
Methods:
A total of 193 adolescents aged 12-15 (90 males and 103 females) was randomly selected, and their height, weight, and BMI were measured using routine testing methods from May to August 2022. The PA, SB and SLP of the participants were measured using a 3D accelerometer (ActiGraph GT3X+).
Results:
The arithmetic mean value overestimated SLP (40.8%) and SB (39.6%) to some extent, and underestimated LPA (16.1%) and MVPA (3.5%) to some extent. Based on the ISM at 15 min, MVPA was substituted for other activity, BMI Z decreased by 0.17-0.22 units, WC Z decreased by 0.16-0.20 units, and FAT Z decreased by 0.17-0.22 units. The substitution between MVPA and for other activity exhibited significant asymmetry. The effects of MVPA substitutions for SB was the largest, followed by the effects of MVPA substitutions for SLP, and the effects of MVPA substitutions for LPA was the lowest. As MVPA substitutions for other behaviors, it reached its maximum (0.06-0.08 units ) when the MVPA time increased by 5 minutes.
Conclusions
MVPA plays an irreplaceable role in the control of adolescent obesity . While reducing SB time, MVPA duration should be increased to ensure that the daily MVPA duration is not less than 55 minutes in order to effectively control obesity.
