Triple-negative breast cancer ( TNBC ) is a subtype of breast cancer,and it is characterized by an aggressive clinical course with a poor prognosis.Treatment for TNBC has attracted much attention in recent years,however,there is no standard treatment for TNBC in clinical setting.The pCR rates of neoadjuvant chemotherapy in TNBC ranges from 12％ to 48％ in the current published data,and it is higher than that in other types of breast cancer,however,the fluctuating range of the TNBC ’s pCR is large in literature.Although the administration of adjuvant chemotherapy for early TNBC is controversial,the regimen without anthracyclines is reported to be suitable for early TNBC patients.Standard cytotoxic agents including taxanes and anthracyclines are still the main choices for TNBC salvage treatment,and the combination with gemcitabine or capecitabine may improve the overall survival.Poly (ADP-ribose) polymerase (PARP) is a new molecular target for TNBC in ongoing studies.Further research on the target-inhibitors such as BSI-201and Olaparib will provide more effective choices to clinical treatment.

[Objective]To investigate the clinical results and significance and mechanism of early dislocation rate after total hip arthroplasty following the posterolateral approach with posterior capsulotendinous repaired.[Method]The incidence of early dislocation and the prosthetic range of rotation motion among 212 consecutive primary THAs with the posterior capsule and the short external rotators which had been sutured to the medius tendon was reviewed retrospectively,and was compared to the results of 486 consecutive unilateral primary THAs with a complete posterior capsulectomy.[Result]During a 6 months to 5 years(3.7 years in average)following-up,there were significant differences between two groups.In patients with posterior capsulotendinous repair,two of 224 primary total hip arthroplasty(0.9%) had an early posterior dislocation.In patients with a complete posterior capsulectomy,27 of 486 primary total hip arthroplasty(5.6%) had an early dislocation.The difference was statistically significant(x~2=8.51,P

Prolactin-inducible protein(PIP)is regarded as a kind of specific tumor marker,and detecting the expression of PIP from breast cancer tissues,metastatic lymph nodes and peripheral blood can find breast cancer micro-metastasis effectively.Some technologies such as immunohistochemistry,reverse transcription polymerase chain reaction and so on can detect PIP sensitivily.Recently,these technologies have been used in some clinical and basic studies.Detection rate of breast cancer micro-metastasis is improved effectively when PIP is combined with other tumor makers,especially mammag-lobin and cytokeratin 19.

Objective To investigate effects of radiofrequency vaporization in arthroscopic treatment of knee disorders.Methods Arthroscopic surgery was conducted in 126 consecutive cases of various knee disorders by using the Arthrocare 2000 System,includng 72 cases of meniscus injury,20 cases of synovitis,15 cases of osteoarthritis,8 cases of patella subluxation,6 cases of synovial syndrome,and 5 cases of anterior cruciate ligament rupture.Results All the patients were followed postoperatively.Knee function evaluation with the Neer Scores showed excellent outcomes in 82 cases,good in 35 cases,and fair in 9 cases,the rate of excellent-or-good outcomes being 92.9%.Conclusions Application of radiofrequency vaporization under arthroscopy for knee joint disorders has advantages of simple and accurate performance,effective hemostasis,minimal invasion,and quick recovery of joint functions.

Objective To analyze the clinical application of detecting anti Mullerian hormone (AMH) in the patients with polycystic ovary syndrome (PCOS).Methods A total of 480 cases with PCOS and 500 cases of healthy females were selected in the study.The levels of AMH in the patients with PCOS were compared between the two groups.Results The levels of AMH,luteinizing hormone (LH),testosterone (T) in patients with PCOS were significantly higher than those in the healthy control group (P＜0.05).Differences of the levels of estradiol (E2) and follicle-stimulating hormone (FSH) in patients with PCOS and thosein healthy control group had no statistical significance(P＞0.05).As the age increased,the level of AM H obviously declined.Conclusion The level of AMH in patients with PCOS is obviously higher than that in the healthy ones,it declines with the age increasing.

Diabetes is a kind of metabolic syndrome, always hap-pening in the case of insufficient secretion of insulin or insulin resistance, inducing the increase of blood glucose. While, dia-betic osteoporosis is a kind of chronic diabetic complications, which happens when insulin secretion is absolutely or relatively insufficient and then the insufficiency induces imbalance of hor-mone, calcium phosphorus metabolic disorders, thus leading to the decline of bone mineral density and change of bone micro-structure. The overnutrition, less exercise and the environment change lead to the increased incidence of diabetes. For all the diabetes, type 2 diabetes accounts for about 90%. Patients with type 2 diabetes show the increasing risk of fracture. This review summarizes the recent advances in the influence of type 2 diabe-tes on bones.

Purpose: To investigate the clinical effect of Yin-reinforcing and Blood-nourishing Drink plus electroacupuncture in the treatment of senile skin pruritus. Methods: Eighty patients were randomly divided into two groups. The treatment group was treated with Yin-reinforcing and Blood-nourishing Drink plus electroacupuncture and the control group, with Western medicine. Results: The total effective rate was 100.0% in the treatment group and 67.5% in the control group (P＜0.01). Conclusion: This therapy has a better effect on senile skin pruritus.

Objective To investigate the therapeutic effect and the immune function impacts of high intensity focused ultrasound(HIFU) for the treatment of hepatocellular carcinoma.Methods Twenty-five cases with primary liver cancer and 6 cases with metastatic liver cancer were treated with HIFU.The clinical symptoms,AFP levels,color Doppler ultrasound detection,CT and changes of T-cell subsets ( CD3 +,CD4 +,CD8 +,CD4 +/CD8 + ) were recorded before and after treatment.Results After HIFU treatment,among 31 patients,the clinical remission rate was 89.7％ ( 25/29 ).AFP was decreased in 83.3％ ( 15/18 ) of the patients.Color Doppler showed reduced tumor sizes and reduced or disappeared blood supply in most of the patients.CT scan displayed that the density of the target region was decreased and necrosis of the lesions occurred.The percentage of CD3 + and CD4 + cells and the ratio of CD4 +/CD8 + were significantly increased after a period of treatment( P ＜0.05 ).The proportion of CD8 + cells was decreased significantly after a period of treatment ( P ＜ 0.05 ).Conclusion HIFU is an effective technique for the treatment of hepatocellular carcinoma.Meanwhile,it can improve human immune function to certain extend.

To investigate the effect of stepwise paced breathing (PB) on pulse transit time (PTT), we collected physiological signals of electrocardiogram (ECG), respiration and arterial pulse wave during a procedure of stepwise PB, which consists of 6 different breathing rates changing in a protocol of 14.0-12.5-11.0-9.5-8.0-7.0 breath per minute (BPM), with each breathing rate lasting 3 minutes. Twenty two healthy adults involved in this experiment and the change of PTT was analyzed during the stepwise PB procedure. In our study, the PTT was measured by calculating the time interval from the R-spike of the ECG to the peaks of the second derivative of the arterial pulse wave. Ensemble empirical mode decomposition (EEMD) was applied to PTT to decompose the signal into different intrinsic mode function, and respiratory oscillation and trend component (baseline) in PTT were further extracted. It was found that the respiratory oscillations in the PTT increased with decreasing of the PB rate, and many of the subjects (14 out of 22) showed the phenomena of PTT baseline increasing during the stepwise PB procedure. The results indicated that the stepwise PB procedure induced a high level of cardiovascular oscillation and produced an accumulative effect of PTT baseline increase. As PTT is capable of predicting changes in BP over a short period of time, increase of PTT baseline indicates the decrease of blood pressure. The experiments showed that the stepwise PB procedure could reduce blood pressure for most subjects. For future work, it is necessary to develop certain indices differentiating the effectiveness of the stepwise PB procedure on the PTT baseline change, and to test the effectiveness of this stepwise PB procedure on blood pressure reduction for patients with essential hypertension.

Objective To evaluate radical gastrectomy combined with cholecystectomy for gastric cancer patients with concomitant gallbladder disease.Methods Clinical data of 702 gastric cancer patients undergoing radical gastrectomy (614 patients) only or combined with cholecystectomy during radical gastrectomy from October 2009 to September 2014 in our department was retrospectively analyzed.Results The operating time of patients with simultaneous cholecystectomy was(348 ± 111)min.the operating time of patients with radical gastrectomy only was (274 ± 89) min (t =3.812,P ＜ 0.05).Perioperative and postoperative complications,hospitalization expenses and 5-year survival rates were not statistically significant (P ＞ 0.05).Conclusions Radical gastrectomy with cholecystectomy for gastric cancer with gallbladder disease patients is safe and feasible.
