1.Investigation of the cognition and treatment rate of hormone replacement therapy in perimenopause women

Objective To investigate the cognition and treatment rate of hormone replacement therapy (HRT)in perimenopause women.Methods A survey was conducted in 262 perimenopause women first coming to outpatients care.A predesigned structured questionnaire was used to collect information:the symptoms of perimenopa-use women,the cognition and acceptance to the treatment.Results Finally,248 women with the mean age of (50.30 ±2.10)years finished the questionnaire,the natural menopause age was (48.90 ±1.90)years.48.79% of these women understood the role of HRT in clinical application,and only 12.50% knew the side effect of HRT. Among them,35.48% thought that HRT was necessary for the duration of perimenopause.40.73% of them had the attitude to treatment with HRT.After the education of perimenopausal acknowledgement,117 patients (47.18%) accepted the treatment with HRT.Conclusion It showed that the awareness and treatment rate of HRT in perimeno-pause women was low.The health education should be strengthened to improve the level of perimenopause health care.
2.A pilot study on soft tissue inflammation of percutaneous implants

Objective:To study the soft tissue inflammation of percutaneous implants using bacteria investigation method, so as to provide basic data for further clinical study of percutaneous implants. Methods: Implants were surgically placed into tibias of the goats. The inflammatory secretion from the peri-implant soft tissue was taken to perform bacteria test on 4, 6 and 9 weeks post-operation. Meanwhile, the soft tissue inflammatory reaction was also observed weekly. Results: 5 of the 16 implants were infected. The soft tissue showed tumefaction and ulceration 2 months post-surgery. As to bacteria investigation, the number of G.Streptococci increased when the inflammation was getting more severe. Conclusion: The extent of the soft tissue inflammation of percutaneous implants probably has positive correlation with the infection of G.Streptococci.
3.Change of pulse transit time during stepwise paced breathing.

To investigate the effect of stepwise paced breathing (PB) on pulse transit time (PTT), we collected physiological signals of electrocardiogram (ECG), respiration and arterial pulse wave during a procedure of stepwise PB, which consists of 6 different breathing rates changing in a protocol of 14.0-12.5-11.0-9.5-8.0-7.0 breath per minute (BPM), with each breathing rate lasting 3 minutes. Twenty two healthy adults involved in this experiment and the change of PTT was analyzed during the stepwise PB procedure. In our study, the PTT was measured by calculating the time interval from the R-spike of the ECG to the peaks of the second derivative of the arterial pulse wave. Ensemble empirical mode decomposition (EEMD) was applied to PTT to decompose the signal into different intrinsic mode function, and respiratory oscillation and trend component (baseline) in PTT were further extracted. It was found that the respiratory oscillations in the PTT increased with decreasing of the PB rate, and many of the subjects (14 out of 22) showed the phenomena of PTT baseline increasing during the stepwise PB procedure. The results indicated that the stepwise PB procedure induced a high level of cardiovascular oscillation and produced an accumulative effect of PTT baseline increase. As PTT is capable of predicting changes in BP over a short period of time, increase of PTT baseline indicates the decrease of blood pressure. The experiments showed that the stepwise PB procedure could reduce blood pressure for most subjects. For future work, it is necessary to develop certain indices differentiating the effectiveness of the stepwise PB procedure on the PTT baseline change, and to test the effectiveness of this stepwise PB procedure on blood pressure reduction for patients with essential hypertension.

4.Application of dexmedetomidine on patients with pancreatic cancer treated by high intensity focused ultrasound under general anesthesia

Objective To investigate the effect of dexmedetomidine on patients with pancreatic cancer treated by high intensity focused ultrasound (HIFU) under general anesthesia.Methods One hundred and twenty ASA Ⅱ or Ⅲ patients with pancreatic cancer undergoing HIFU treatment,were divided into control group (group C) and dexmedetomidine group (group D) by random digits table method,each group with 60 cases.Dexmedetomidine was intravenously infused at 0.2 μ g/(kg· h) until the end of operation after a loading dose of 0.7 μg/kg over 15 min before induction of anesthesia in group D,while the equal volume of normal saline was infused in group C.Anesthesia was induced with intravenous injection of fentanyl 2-3 μg/kg and propofol 1.5-2.0 mg/kg and cisatracurium 0.2 mg/kg at 5 min after administration of the loading dose.Anesthesia was maintained with sevoflurane,remifentanil,propofol,cisatracurium.The propofol,remifentanil and sevoflurane consumption,the incidence of postoperative agitation,delirium,delayed recovery were recorded.Results The intraoperative heart rate in group D was significantly lower than that in group C (P < 0.05).The propofol,remifentanil and sevoflurane consumption in group D were significantly shorter than those in group C [(341.4 ± 45.7) mg vs.(520.5 ± 50.8) mg,(1.7 ± 0.5) mg vs.(2.3 ± 0.8) mg,(11.6 ± 4.1) ml vs.(16.7 ± 3.8) ml,P < 0.05].The incidence of postoperative agitation and delirium in group D were significantly lower than those in group C [6.7%(4/60) vs.21.7%(13/60),8.3% (5/60) vs.26.7% (16/60),P < 0.05].There was no statistically significant difference in the incidence of delayed recovery between group D and group C [3.3% (2/60) vs.1.7% (1/60),P > 0.05].Conclusions Dexmedetomidine given HIFU treatment of pancreatic cancer patients can reduce the amount of general anesthetic,reduce the incidence of postoperative agitation and delirium,intraoperative heart rate.
5.Design of ECG signal acquisition system based on MSP430 microcontroller

Objective To design a high performance and low power consumption ECG signal acquisition system which can meet the demand for long time monitoring of the physiological status of patients.Methods The prototype system utilized low power ECG analog front end ADAS1000 and MSP430F5529 microcontroller to achieve configuration of AFE and back-reading of ECG data by SPI bus. Results This system implemented 24-hour dynamic ECG monitoring of patients in active state, and the data acquired were accurate and reliable.Conclusion The system realizes PCB integration, low power consumption, and can be used for battery powered portable application such as wearable devices.
6.Motion parameters analysis and pilot clinical trials of the dual mobility hemi-knee artificial prosthesis

ObjectiveAim at the problems in the treatments of the children malignant bone tumor of distal femur,we put forward the concept of the dual mobility hemi-knee prosthesis and try to perform the motion parameters analysis and the pilot clinical trials.MethodsBase on the CT data from samples of knee joint in adult,we adopted the Mimics/Geomagic/Pro-E software and computer numerical control milling machine technology to design and produce the dual mobility hemi-knee artificial prosthesis,and then motion parameters was analyzed in vitro test,and at last pilot clinical trial was performed.ResultsIn vitro experiment showed that the displacement of the internal femoral condyle flexion facet center was(2.59±0.43) mm in normal knee group,(2.22±0.52) mm in dual mobility hemi-knee prosthesis group,and (1.18±0.43) mm in total knee arthroplasty group; the displacement of the external femoral condyle flexion facet center was (11.95±6.62) mm in normal group,(11.25±6.19) mm in dual mobility hemi-knee prosthesis group,and (1.26±0.42) mm in total knee arthroplasty group; the maximum relative rotation angle was 13.17°±7.58° in normal knee group,11.69°±6.49° in dual mobility hemi-knee prosthesis group,and 5.40°±1.29° in total knee arthroplasty group.After that,we accomplished the pilot clinical trials successfully for a patient with malignant bone tumor in distal femur.The patient recovered well and the treatment outcome was satisfactory.ConclusionThe motion parameters analysis showed that the motion mode of the dual mobility hemi-knee prosthesis is similar with that of the normal knee.The dual mobility hemi-knee prosthesis provides a whole new concept and method for the treatment of malignant bone tumor of distal femur in children,and the device and concept of ligament reattachment provide a solution to the reconstruction of knee ligament.
7.The efficacy of high intensity focused ultrasound for hepatocellular carcinoma

Objective To investigate the therapeutic effect and the immune function impacts of high intensity focused ultrasound(HIFU) for the treatment of hepatocellular carcinoma.Methods Twenty-five cases with primary liver cancer and 6 cases with metastatic liver cancer were treated with HIFU.The clinical symptoms,AFP levels,color Doppler ultrasound detection,CT and changes of T-cell subsets ( CD3 +,CD4 +,CD8 +,CD4 +/CD8 + ) were recorded before and after treatment.Results After HIFU treatment,among 31 patients,the clinical remission rate was 89.7% ( 25/29 ).AFP was decreased in 83.3% ( 15/18 ) of the patients.Color Doppler showed reduced tumor sizes and reduced or disappeared blood supply in most of the patients.CT scan displayed that the density of the target region was decreased and necrosis of the lesions occurred.The percentage of CD3 + and CD4 + cells and the ratio of CD4 +/CD8 + were significantly increased after a period of treatment( P <0.05 ).The proportion of CD8 + cells was decreased significantly after a period of treatment ( P < 0.05 ).Conclusion HIFU is an effective technique for the treatment of hepatocellular carcinoma.Meanwhile,it can improve human immune function to certain extend.
8.A prospective,randomized controlled clinical study to evaluate porous β-tricalcium phosphate for lacunar bone defects repair

Objective To evaluate the clinical outcomes of a hovel porous β-tricalcium phosphate (β-TCP)and control allograft for the repair of lacunar bone defects caused by solitary bone cyst curettage.Methods From January 2003 to December 2008,the patients with solitary bone cyst were randomized into an experimental(55 cases)and a control(40 cases)group.The control group received particulate allograft bone as the graft material,and the experimental group received β-TCP.At 1 week,1,2,3,6,12,24,48months after surgery,a new radiographic scoring system was employed to calculate the biodegradation of bone graft and evaluate the influence of multiple factors.Histologic characteristic of the degradation process of β-TCP were also evaluated.Results All the cases were followed up for average 28.4 months.Radiographic semi-quantitative analysis revealed that the biodegradation effieiencies were not significantly difierent between β-TCP and allografts(P=0.424).Degradation percentage of the implanted β-TCP or allograft was higher in younger patients than those in the older ones.Degradation of β-TCP was significantly higher than that of allografts over 3 years after surgery(P=0.04).In the experimental group,β-TCP degradation was greater in the loose packing treatment than that in the dense packing treatment (P=0.03).Histological observation demonstrated that the process of new bone formation accompanied the degradation of β-TCP.Conclusion The interporous β-TCP could be an advantageous alternative to allografts for repair bone defects caused by bone cyst.The clinical application of β-TCP is safe and reliable,which shows better biodegradation and osteogenesis than allografts in long-term follow-up.
9.The combinative biological reconstruction of bony defect following limb bone tumor resections

Objective To analyze the image and histological results of the combined use of allograft/extracorporeally frozen tumor-bearing bone and vascularized fibular flap for the reconstruction of bony defects following tumor resection,guiding clinical practice.Methods From March 2007 to June 2013,we enrolled 63 patients who had combinative biological reconstruction after bone tumor resection (11 in humerus,22 in femur,21 in tibia,4 in calcaneus).There were 36 male and 27 female in this series.The average age at time of operation was 20 years,ranging from 9 to 48 years.The follow-up ranged from 16 to 102 months with average of 48 months.We investigated the X-ray and CT images for all patients and histological findings of two patients.Patients were assessed functionally with the Musculoskeletal Tumor Society 93 score.Results Three patients with local soft tissue recurrence and one patient with infection underwent amputation.The survival of construct was 93.6%.Bone union achieved in all cases with the average MSTS score of 92.8%.Bone union ranged from 11 to 28 months in allograft group and 9 to 14 months in devitalized tumor bearing bone group.Significant difference of bone union time was found between two groups (Z=-3.638,P=0.000).Viability of the fibular grafts was verified in 58 of 63 patients (92%).Three types of images were observed in complex.Osteopenia and spongy change in fibula were found in 51 patients (81%) with stable fixation of the complex.Five complexes with failed blood supply of fibula and stable fixation revealed no density change of fibula,small amount of callous formation and relative delayed union.In seven complexes (11%) with unstable complex due to patients' incompliance,fibula reacted with dense hypertrophy and microfracture.Fusion of grafts with amount of callus was obviously observed.Union at allograft-host bone junctions occurred by residual host bone-derived external callus and fibular-derived internal callus that bridged the junction and filled the gap between abutting cortices.Callus from fibular graft was mature than that from periosteum of residual host bone.Internal repair was observed at the internal surface of the allografts.Fibula showed significant spongy changes.Conclusion Recycled tumor-bearing bone in combined with fibular flap is a reliable reconstruction as an alternative to traditional Capanna technique.The survival of the fibula is a cornerstone in success of complex reconstruction.Sponginess of fibula and internal repair of allograft compromise the intensity of complex,necessitating the strong instrumentation during reconstruction.
10.Influence of looming cognitive style and fear of negative evaluation on anxiety in college students

Objective To explore the effect of looming cognitive style and negative evaluation on anxiety in college students.Methods A total of 300 college students were assessed with looming maladaptive style questionnaire(LMSQ),Beck anxiety inventory(BAI) and fear of negative evaluation scale (FNE).Results (1)There was significant gender difference in fear of negative evaluation in college students (90.16±11.69 vs 97.63± 11.69,t=-2.770,P<0.05),and there was no significant gender difference in looming cognitive style and anxiety(50.49± 11.59 vs 53.67± 11.65,t=-2.770,P>0.05;29.82±7.89 vs 28.84±7.22,t=0.813,P>0.05).(2) LCS were positively correlated with fear of negative evaluation and anxiety (P< 0.01).(3) Looming cognitive style could forecast anxiety in college students (β=0.257).Looming cognitive style and fear of negative evaluation could forecast anxiety in college students together (β1 =0.137,β 2 =0.247).(4) The meditation effects of fear of negative evaluation in total effect were 0.468.Conclusion Looming cognitive style and fear of negative evaluation are important factors influencing the anxiety of college students,fear of negative evaluation plays an important mediation role between looming cognitive style and anxiety of college students.
