Deep vein thrombosis is the common complication of trafifc injury , it is one of the dififculties of forensic identiifcation to identify disability grade and the relationship between injury and disease. When post-thrombotic syndrome happens, circulatory system and leg will be dysfunction, caused by the destruction of valve and venous wall and by the blockage of thrombus. The quality of life and working ability will be seriously affected. The impact of different types of deep vein thrombosis on human body was analysised after looking up relevant literatures, then it was discussed that the identiifcation of following questions including: the relationship between injury and disease, how to identify disability grade and it’s best time.

We expound the origin and development of preventive treatment's concept in the Huangdi's Internal Classic and the Treatise on Cold-induced and Miscellaneous Diseases. We also discussed how Zhang Zhonjing transmitted his knowledge about preventive treatment from the Huangdi's Internal Classic. We explained in detail the importance and the clinical applications of treating before occurrence of disease, treating early after occurrence, preventing the propagation, avoiding critical evolution of disease and preventing the relapse when the patient is recovered.

Objective To evaluate the feasibility and safety of trans-umbilical NOTES to perform abdominal adhesion lysis using a flexible endoscope. Methods A 42-year-old female was admitted to our hospital with a complain of abdominal pain due to abdominal adhesion after hysterectomy.A gastroscope (GIF-2TQ260M,Olympus)was inserted into the peritoneal cavity through an incision(1.5 cm)of the umbilical opening.Pneumoperitoneum was established by the endoscopic air supply system using CO2 instead of air.Abdominal exploration showed that omentum and bowel were stuck to the scar of the low central peritoneum incision.Separation of the abdominal wall adhesions was performed with an IT knife and a hook knife respectively. After most of the adhesions were separated, bleeding was found from the isolated adhesion wound of the omentum and bowel.Hemorrhaged blood vessel was difficult to be found with the flexible gastroscope. Then a 5 mm Trocar was placed at the left lower abdomen and laparoscopic assistance was adopted to search the bleeding point. However,no bleeding site was found and the hemorrhage was stopped automatically.Residual adhesion lysis was conducted with laparoscopic assist. After the procedure, the umbilical incision was closed with absorbable suture, but lower left abdominal incision was left without suturing. Results The operation time was 40 minutes. The patient suffered from minus pain from the umbilical incision without need of treatment. She had normal diet and got out of bed after reconsious. She was discharged 2 days after the procedure without any complaints. Conclusion Trans-umbilical NOTES using a flexible endoscope for peritoneal adhesion lysis is safe and feasible, but the effectiveness remains to be further confirmed.

Objective To investigate the effect of continuous health education on the control of glucose and the changes of behavior. Methods One hundred and twenty-six newly diagnosed type 2 diabetic patients were randomly divided into control group and observation group, each group included 63 patients. The control group just received health education in hospitalization period, and the observation group received continuous health education both in and after hospitalization. The control of glucose and the changes of behavior were evaluated 3 months and 1 year later. Results After 3 months and 1 year treatment, the FPG,2hPG and HbA1c were significantly decreased compared to pre-treatment both in control group and observation group(P<0. 05). The FPG,2hPG and HbA1c were significantly decreased in the ob-servation group compared to the control groups(P<0. 05). The behaviors contained that mastery of diabetic knowledge, control of food in-take,persist in physical exercise, rational administration, self-monitoring of blood glucose and regular reexamination were significantly im-proved both in control group and observation group compared to pre-treatment (P<0. 05),and the behaviors were significantly improved in the observation group compared to the control group (P<0. 05). Conclusion Compared to control group, observation group had more ad-vantages in the control of glucose and the changes of behavior in newly diagnosed type 2 diabetic patients.

Chinese Pediatric Emergency Medicine 2012;19(4):399-401
Objective Investigations regarding the status of pediatric emergency and relevant facilities distribution in our province represent a crucial prerequisite for further setting up emergency network and improving our first aid capabilities. Methods We had conducted relevant surveys on the medical institutions within our province, including lever 2 and lever 3 hospitals as well as maternal and child care service centers. Results The feedback from 151 institutions indicated that they had 4485 pediatric beds and treated 5 308 severe patients in 2010.Among all,38 hospitals had ventilators for children and 4 hospitals could perform pediatric blood purification treatment. However,no one had mastered the children ECMO technique. Conclusion There is an inadequacy of some emergency equipments and the development in emergency and first aid is uneven. There is a strong request to launch and develop the blood purification and bronchfiberscope techniques,in combination with improvements in the first aid capabilities and emergency network.

Objective To study the factors related with balance ability of the elder. Methods 37 persons above 60 years were assessed with the balanced function reflectoscope reflector, grouped according to their exercise practice. Results The length of locus and length/envelope area as eyes closed increased significantly compared with that as eyes opened in the less exercise group(P<0.05),while they were increased significantly compared with those in the more exercise group either when eyes closed or opened(P<0.05).Conclusion The exercise practice can improve the balances function of the old persons.

Objective SD rats were immuned with the transgenic tomatoes which carried fused gene of a region of PAc Strep-tococcus mutants and cholera toxin B subunit. The immunogenicity was tested to explore secure and economic edible vaccines a-gainst dental caries. Methods A total of 18 eighteen-day-old female SD rats were subdivided randomly into three groups:the exper-imental group which were fed with transgenic tomato juice containing chimaera protein PAcP/CTB;the positive control group which were treated with deactivated S.mutans;the negative control group which were not treated with transgenic tomato juice.Rats were immuned once per week for four weeks.Blood and saliva were collected at one day before the first immunity and one week after each immunization.IgG of blood serum and SIgA of saliva were detected using the enzyme-linked immunosorbent assay (indirect ELISA) testing. On day 70,rats were terminated. The maxillary and mandibular bones were subsequently taken out to count dental caries′scores. Results Post immunization, the experimental group and the positive control group had statistical significant levels of speci-ficity IgG in serum and SIgA in saliva compared to the negative control group (P <0.05).There was a significance difference be-tween the experimental group and the negative control group except in Dx levels of caries loss (P <0.05).Conclusion The targeted protein expressed on the transgenic tomatoes is immunogenic, which can effectively induce mucous membrane immune response and the systematical immunoreaction to suppress the occurrence of the dental caries.
