1.Clinical Evaluation of Pulmonary Nodules and Calification in Nodules with Dual-energy Subtraction Digital Radiography

Objective To evaluate the clinical value of dual-energy subtraction digital radiography in diagnosing the pulmonary nodulesand calification in nodules.Methods Using American GE Revolutln DR-XR systems,All 80 patients were examined by dual-energy.Tissus images、bone images and the original digital radiographic images were obtained.Dual-energy digital subtraction images were compared with the original digital radiographic images.Results Of 186 pulmonary nodules proved by CT,126(94 uncalified and 32 calified)were detected by tissues images and bone images and 109(83 uncalified and 26 calified)were detected by the original digital radiographic images.Conclusion Dual-energy subtraction imaging is superior to the original digital radiographic images in detection of pulmonary nodules and calification in a nodule.Dual-energy subtraction images can act as a complement for the original digital radiographic images.
2.Study on Distribution in Mouse of Tetrandrine Polylactic Acid Microsphere in Vivo

OBJECTIVE:To study the lung targeted characteristic of tetrandrine polylactic acid microsphere(TPM).MET_HODS:TPM and tetrandrine injection were injected intravenously into mouse,an RP-HPLC was established to measure the content of TPM in biological samples,and the concentrations of tetrandrine in different tissues of mice were determined and compared.RESULTS:The determinable concentration of tetrandrine in plasma was in the linear range of 0.519~17.000?g/ml(r=0.9 996),the recovery was 97.32%,the mean value of RSD was 4.46%.After the use of TPM,the concentrations of tetrandrine in mouse tissues were significantly higher than tetrandrine injection,and the highest concentration was detected in lungs of mouse.CONCLUSIONS:TPM is distinguishingly lung targeted.
3.Study on Pharmacokinetics of Lung Targeted Tetrandrine Polylactic Acid Microspheres in Rabbits

OBJECTIVE:To provide scientific evidence for the microsphere's m etabolism in vivo of lung targeted tetran?drine polylactic acid(TET-PLA)microspheres.METHODS:Blood was sampled from carotid artery after microspheres were injected into rabbit through auricular vein,the concentration of TET in plasma was determined by RP-HPLC method;The pharmacokinetic parameters were obtained by using3p87program.RESULTS:Concentration-time curves of TET micro?spheres were fitted to a2-compartment model with t 1/2? of(286.49?237.55)min and AUC of(810.33?287.49)(?g?min)/ml.CONCLUSION:TET-PLA microspheres show sustained release effects in rabbits.
4.Observation and nursing of intravitreous injection of Avastin combined with laser photocoagulation for macular edema induced by vein occlusion

Objective To investigate the treatment effect and nursing of intravitreous injection of Avastin combined with laser photocoagulation in macular edema caused by vein occlusion.Methods 38 patients(38 macular edema eyes)who had macular edema caused by retinal vein occlusion and accepted intravitreous injection of Avastin combined with laser photocoagulation were selected.Relevant nursing was given before and after the clinical treatment.The visual acuity and retinal thickness were observed and recorded.Results Visual acuity was improved in 36 eyes(94.7%),no change occurred in 2 eyes(5.3%).The thickness of macular was statistically significant compared with before treatment and 2,4,6 weeks after treatment.Conclusions Intravitreous injection of avastin combined with laser photocoagulation is effective on macular edema caused by vein occlusion.Successful treatment is guaranteed by psychological nursing,treatment guidance and health guidance.
5.Surgical treatment of traumatic mitral regurgitation

Objective To study the clinical features of traumatic mitral regurgitation and its surgical treatment methods and outcomes. Methods Clinical data of 16 patients with traumatic mitral regurgitation surgically treated from December 2000 to November 2008 were retrospectively analyzed. The cardiac thoracic ratio was 0.55 ±0.07 and the left ventricular ejection fraction (LVEF) was (51.2% ±23.2)%. According to the New York Heart Association (NYHA) functional classification, 10 patients were at grade Ⅰ - Ⅱ before operation. The surgical procedures consisted of mitral valvuloplasty in 14 patients and mitral replacement in two, when correction of other cardiac injuries was made at the same time.Of all, 14 patients obtained follow-up for (35.2 ± 25.7 ) months. Results The interval between trauma and appearance of mitral regurgitation was (23.3 ±50.90) months. Of 13 patients with echocardiography during follow-up, there were four patients with normal tricuspid hemodynamics, seven with trace tricuspid regurgitation and two with mild regurgitation. The value of LVEF was (66.8 ± 9.0)% at followup, significantly higher than preoperation (P < 0. 05 ). There were 13 patients at grade Ⅰ - Ⅱ of NYHA,with a significant increase compared with preoperation ( P < 0. 01 ). Conclusions Mitral regurgitation may emerge either immediately or several years after trauma. Traumatic mitral regurgitation patients can get satisfactory outcomes when comprehensive mitral valvuloplasty or mitral replacement is performed promptly.
6.Clinical and genetic characteristics of hereditary spastic paraplegia

Objective To investigate the clinical and genetic characteristics of hereditary spastic paraplegia(HSP).Methods The clinical material of 113 patients in 39 families with HSP was analyzed retrospectively.Results The ratio of male to female was 1:1.17.The age at HSP onset was from 2 to 58 years old, the mean age was 21.4 years old, and 81.7% of the patients had HSP before 30. 89.4% of the patients had positive family history and they showed mostly autosomal dominant inheritance. The rate of consanguinity was 28.2%. 24 cases had pure while 89 cases had complicated spastic paraplegia. In the HSP group, we could found the weakness of legs in 65.5% patients, spasticity and hyperreflexia of lower limbs in 96.5%, extensor plantar responses in 68.1%, ataxia in 46.9%, muscular atrophy in 32.7% and dementia in 18.6%.Conclusion In the HSP group, the year of onset was mostly before 30. The female HSP cases were more than the male's, and the complicated cases were more often than the pure. Autosomal dominant was the mostly frequent inheritance, and there were more chances of HSP in the consanguineous families.
7.The clinical value of procalcitonin detection in diagnosis and treatment of ventilator associated pneumonia

Objective To assess the value of procalcitonin(PCT) in diagnosis and treatment of ventilator associated pneumonia(VAP). Methods 60 patients on ventilators were divided into VAP group and non-VAP group depending on whether the patients developed VAP in 7 days or not. The VAP group again was divided into PCT group and the control group. Data of PCT, C-reactive protein (CRP) and WBC were assessed at baseline, in 6 hours when VAP was suspected, and every two days after mechanical ventilation. The control group received antibiotics according to usual practice and stopped when CPIS≤6 scores. Antibiotic treatment of the PCT group was based on serum PCT concentrations. Results The CRP and WBC showed no obvious difference between the VAP and non-VAP group before mechanical ventilation ( all P > 0. 05 ), after ventilation both mean values increased, with the P 0. 046 and 0. 822,respectively;Taking CRP≥28mg/L and WBC≥ 10×109/L as the cutoff value,the diagnostic sensitivity of CRP and WBC for VAP were 73.3% and 66.7% respectively,their specificity was 50% and 43.3 % ,respectively.Taking PCT ≥0.40μg/L as the cutoff value, the PCT positive percentage did not show difference between VAP group and non-VAP group before mechanical ventilation( P > 0. 05 ). However, it was much higher in the VAP group than that of non-VAP group after mechanical ventilation( P < 0. 01 ). The PCT level of 0.40 ug/l yielded a 93.3% sensitivity and a 73.3 % specificity for VAP in the study cohort ( AUROCC = 0. 823; 95 % CI,0.71-0. 94; P < 0. 01 ). The antibiotic duration of the PCT group was( 12. 6 ± 5.6) days compared with( 15. 1 ± 9.1 ) days for the control group (P < 0. 05). Patients assigned to the PCT group had 2.5 days shorter mean duration of antibiotic therapy for the first episode of infection than the control group ( P < 0. 05 ). Conclusion PCT had high sensitivity in the diagnosis of VAP. CPIS ≥6 combined with serum levels of PCT ≥0. 40μg/L markedly improved the specificity( 100% ). PCT guidance substantially reduced antibiotic use in VAP, so that timely surveillance of serum PCT was necessary for patients on ventilator.

9.Effect of modified Wuhuang oil on fibroblast growth and proliferation

BACKGROUND: Wuhuang oil has a bacteriostatic action to treat burn wound and promote traumatic healing, but the action on inhibition of scars formation remains poorly understood.OBJECTIVE: To investigate the effects of modified Wuhuang oil at different concentrations and administration times on the growth and proliferation of human fibroblasts in vitro.DESIGN, TIME AND SETTING: Comparison observation regarding cytology in vitro was performed at the Burns Institute in the First Affiliated Hospital of Nanchang University between April 2006 and January 2007.MATERIALS: Prepuce specimens were harvested from patients who underwent circumcision in Department of urinary surgery, at First Affiliated Hospital of Nanchang University and Jiangxi Provincial Children Hospital. All patients aged 2-12 years old, and informed consents were obtained from their relatives. Wuhuang oil and modified Wuhuang oil (water-solubility) were offered by Department of Pharmaceutical Preparation in the First Affiliated Hospital of Nanchang University, China. METHODS: Human fibroblasts cultured in vitro were divided into 2 groups at random, experiment and control. Experiment group was treated with 300 g/L Wuhuang oil, while control group with 300 g/L modified Wuhuang oil. Serum-free culture fluid was used to prepare 6 concentrations of oil solution: 0 (blank control), 100, 150, 200, 250, 300 g/L.MAIN OUTCOME MEASURES: MTT assay was used to determine the growth and proliferation of human fibroblasts at 2, 3, 4, 5,6 days; inhibition rate of cell growth was observed at 2, 4, 6, 8, 10 days.RESULTS: Modified Wuhuang oil (0-300 g/L) concentration positively correlated with inhibition of human fibroblast proliferation;the inhibition was not related to culture time. Modified Wuhuang oil (300 g/L) had the greatest inhibition rate of human fibroblasts at 8-10 days, there were significant differences between experiment group and control group (P < 0.01).CONCLUSION: Modified Wuhuang oil has an effective inhibition on the proliferation of human fibroblasts in vitro, and shows a dose-dependent tendency. Compared with Wuhuang oil, 300 g/L modified Wuhuang oil is superior to suppress the growth of human fibroblasts.

The modern large-scale pyrosequencing technology is a revolution of DNA sequencing.One of the key points in this technology is to get an ATP sulfurylase immobilized on the surface of magnetic beads and with a high activity.Biotinylated ATP sulfurylase can be immobilized on magnetic beads coated with streptavidin through the specific conjunction between biotin and streptavidin, but using chemical modification method to biotinylate ATPS will affect the activity of the enzyme.ATP sulfurylase fused with the carboxyl terminal 87 residues of Escherichia coli biotin carboxyl carrier protein(BCCP87) was expressed in E.coli using fusion expression strategy.Results from Western blot analysis and SDS-PAGE analysis showed that the fusion protein could be biotinylated in vivo, and the molecular mass of the fusion protein was about 64 ku.The biotinylated ATP sulfurylase could be immobilized on the surface of magnetic beads coated with strepavidin, and the immobilized ATPS could be used for quantification of PPi and pyrosequencing.An effective enzyme for the large-scale chip-based pyrosequencing system was supplied.
