1.Clinical Observation of Comprehensive Therapy for Treatment of Myopia in Teenagers and Children

Objective To observe the effect of atropine mydriasis, wearing myopia glasses for short or long distance, and massage on preventing and controlling myopia in teenagers and children. Methods A total of 198 myopia cases (involving 393 eyes) were randomized into control group and treatment group. The control group was given eyesight examination after mydriasis with atropine sulfate eye gel and wearing myopia glasses for long distance. The treatment group was given eyesight examination after mydriasis with atropine sulfate eye gel, wearing myopia glasses for short distance, and massage therapy. Eyesight examination was carried out once every 2 months and lasted for one continuous year. The changes of diopter were compared between the two groups. Results ( 1) Of 194 affected eyes from the treatment group, 96 eyes were effective, 68 were stabilized, and the effective rate was 45.5%and stabilizing rate was 35.1%. Of 199 affected eyes from the control group, 3 eyes were effective, 9 were stabilized, and the effective rate was 1.5%and stabilizing rate was 4.5%. The differences of effective and stabilizing rates were significant between the two groups ( P<0.01). ( 2) After treatment, the myopia diopter was increased in the control group ( P<0.01 compared with that before treatment) , and the myopia diopter in the treatment group maintained stable ( P>0.05). The difference of diopter was significant between the two groups (P<0.01). (3) The difference of astigmatism was insignificant in both groups before and after treatment and between the two groups after treatment (P>0.05). Conclusion Mydriasis with atropine sulfate eye gel, wearing myopia glasses for short or long distance, and focal massage therapy show certain preventive effect on myopia in teenagers and children by controlling the development of myopia diopter.

Three nuclear type segeragants were isolated from a heterokaryon of Fusarium oxysporum f.sp.vasinfectum strain Ag149.Their cultural character and pathogenicity were different.The differences of their GC content for total DNA and nuclear DNA were between 0 1% to 0 4% and 0 4% to 0 8% respectively,as measured by thermal denaturation method.The 18S rDNA partial sequences of 1477 base pairs in these segregants were identical.

AIM:To evaluate the protective effect of hypothermic ventricular fibrillation without aortic-cross clamping under cardiopulmonary bypass(CPB)on canine lung.METHODS:Fourteen dogs were randomly divided into two groups.All dogs received a standardized anesthetic technique.A conditional CPB was performed in every instance.Ventricular fibrillation was induced by systemic hypothermia to 28 ℃ and pericardial cooling saline in the experimental group.A standard CPB was performed in control group.The concentration of IL-8 in serum was measured by ELISA.The expressions of NF-?B and ICAM-1 were determined by using immunohistochemical staining.RESULTS:Serum IL-8 level in experimental group was significantly lower than that in control group(P
5.Application of informatization exam system in the objective structured clinical examination for nursing skills

Objective To explore the advancement and efficiency of informatization exam system and provide practice basis for its application in the nursing assessment through the construction and application of informatization exam operation system platform in the nursing skill assessment of the objective structured clinical examination (OSCE).Methods OSCE for nursing skills was performed on nursing undergraduates from a university school of nursing in 2015 using the informatization exam system. Voice calls was used for calling students in order. The teachers used paperless PAD for evaluation. After the examination, students were required to finish the informatization evaluation questionnaire to assess the satisfaction rate.Results A total of 91 nursing students were included in the study. The highest dimension score was exam environment (4.20±0.86), while the lowest dimension was the exam time (3.89±1.10). The entry score ranked in the top three were reasonable location (4.31±0.89), clear instructions content (4.25±1.01) and clear play sound (4.18±1.06). 72.73% of the examinees thought that the combination of high-tech in the informatization exam system were advanced, and the exam arrangement was clear and time-saving. 27.27% of the examinees thought that the system response and test arrangement required improvement to increase the flow velocity and avoid nervous and anxiety caused by long waiting.Conclusions The application of the informatization exam operation system changes the traditional model of artificial operation examination. The examinees give the affirmation to exam environment. The examination schedule needs to be improved. The paperless scoring system improves the working efficiency of the evaluation and management in teaching management.
6.Construction and Application of Clinical Experience Knowledge Graph for Renowned TCM Doctors in Treating Coronary Heart Disease

Objective To build a knowledge graph;To visualize the knowledge structure relationships and clinical thinking in the treatment of coronary heart disease by renowned TCM doctors;To provide methodological reference for the inheritance of experience of renowned TCM doctors.Methods Medical records about treatment of coronary heart disease by renowned TCM doctors were retrieved from CNKI from the establishment of the database to 30th,Nov.2022.The characteristics of TCM diagnosis and treatment and the characteristics of the theoretical system of syndrome differentiation and treatment in TCM were analyzed.Concept types and relationships between concepts were sorted out and extracted to form a pattern layer of knowledge graph;based on the characteristics of the pattern layer,Python 3.11(PyCharm 2022.3.2)was used to write rules,and knowledge extraction and data import were implemented through the Pandas library,Openpyxl library and Py2neo library,which were stored in the graph database Neo4j-Community-5.2.0 to complete the construction of the knowledge graph.Implementing query application was realized through Cypher language.Results The data of 643 medical cases of 144 renowned TCM doctors were included,which were entered into the Neo4j graph database,forming a knowledge graph consisting of 2 744 nodes and 23 795 relationships under 8 concepts and 10 relationships,to achieve visual presentation and query application of the diagnosis and treatment process of coronary heart disease by renowned TCM doctors.Conclusion The knowledge graph can intuitively display the relationship of diseases-symptoms-syndromes-treatments-prescriptions-medicine in medical records,develop a knowledge system that is easy to retrieve,and improve the accessibility of domain knowledge,which can provide methodological reference for the inheritance of experience of renowned TCM doctors.
7.Clinical application of adjustable traction skin stretchers in repair of wounds at the lower leg, foot and ankle

Objective:To evaluate the clinical effects of adjustable traction skin stretchers used in repair of wounds at the lower leg, foot and ankle.Methods:A retrospective study was performed to analyze the clinical data of 56 patients who had been treated for skin defects at the lower leg, foot and ankle from August 2016 to September 2022 at The First Affiliated Hospital of Zhengzhou University, Honghui Hospital, Affiliated to Xi'an Jiaotong University Medical College, The First Affiliated Hospital of Henan Polytechnic University, and Yunnan Zhongde Orthopedic Hospital. There were 35 males and 21 females, aged (39.9±18.7) years. There were 43 traumatic wounds, 3 burns, 6 inflammatory wounds, 3 relief incisions due to osteofascial compartment syndrome, and 1 scar. The areas of skin defect ranged from 2.5 cm × 2.0 cm to 20.0 cm × 10.0 cm. The duration of wounds was (8.6±7.8) d. All the wounds were repaired with adjustable traction skin stretchers. The row-hook type of skin stretchers was used in 28 cases, the single-rod type in 20 cases, the single-rod type combined with an external fixator in 5 cases, and a combination of the row-hook type and the single-rod type in 3 cases.The time for wound traction closure, color of wound skin margin, skin swelling around the wound, functional recovery of affected limb and complications were recorded.Results:The time from skin stretching to wound closure was (7.8±3.8) d in the 56 patients. The color of wound skin edge after stretching was normal in 16 cases, dark red in 38 cases, and dark in 2 cases; the skin swelling around the wound was degree 1 in 21 cases, degree 2 in 33 cases, and degree 3 in 2 cases. The 56 patients were followed up for (8.9±4.1) months. Primary wound closure was achieved in 48 patients, and secondary wound closure in 8 patients after repair with an autologous skin graft. Partial skin necrosis occurred due to tension blisters after skin stretching in 2 patients, one of whom was repaired with an autologous skin graft and the other of whom by dressing change. Deep bone infection recurred in 2 patients whose wounds healed after their bone defects were repaired using Ilizarov technique of bone transfer. In the 56 patients, the muscle strength of the lower extremity beyond the wound was recovered to normal, and the range of motion of the joints adjacent to the wound also recovered to normal.Conclusion:In repair of wounds at the lower leg, foot and ankle, adjustable traction skin stretchers can lead to fine clinical effects and limited complications, because the stretchers can control the tension of skin digitally and precisely.

Outpatient humanistic care refered to providing a full process of caring medical services to outpatients. In order to standardize the human caring services for outpatients in medical institutions, promote the comprehensive service level of outpatient services, and improve the patient′s medical experience, Chinese Association for Life Care issued the group standard of Management Norms for Human caring of Outpatients in April 2023. This standard clarified the relevant terms and definitions of human caring for outpatients, specified the basic requirements for human caring, the humanistic quality and care responsibilities of outpatient staff, the outpatient care environment and facilities, the outpatient care process and measures, and quality management. It designed standardized and personalized full process care service norms, providing references for medical institutions at all levels to promote the development of human caring for outpatients.
9.Clinical analysis of diversity of defect repair with supraclavicular island flap after head and neck tumor surgery.

Objective:To investigate the diversity and clinical effect of supraclavicular island flap in repairing the defect after head and neck tumor surgery. Methods:A retrospective analysis was performed on 30 patients who received the repair of head and neck defects with supraclavicular island flaps at Department of Otorhinolaryngology Head and Neck Surgery of the First Affiliated Hospital of Chongqing Medical University from January 2017 to March 2023. The sites and types of defects, intraoperative blood loss, time of flaps preparation, areas of flaps, survival of the flaps and other complications were recorded. Results:A total of 30 patients were enrolled, including 26 males and 4 females, aged 36-82 years. Among them, 22 patients with hypopharyngeal partial defect were repaired (19 patients with ipsilateral defect and 3 patients with contralateral defect). In addition, 2 patients were repaired with contralateral pectoralis major musculocutaneous flap around the hypopharynx, the neck skin defect was repaired in 2 patients, the parotid skin defect was repaired in 2 patients, the temporal bone skin defect was repaired in 1 patient, and the cervical esophageal defect was repaired in 1 patient. The average blood loss during the operation was 8 ml, and the average time was 32 min. The flap areas ranged from 5.0 cm×4.0 cm to 20.0 cm×8.0 cm. 27 of 30 flaps survived(90.0%), and pharyngeal fistula occurred in 6 patients after operation(4 flaps survived after local dressing). One patient was complicated with venous thrombosis(the flap necrosis after local dressing). Shoulder and neck functions(lift, internal rotation and abduction) were not significantly affected in 29 patients, and the function of 1 patient with shoulder infection was not affected after treatment. Conclusion:Supraclavicular island flap is a highly vascularized axial fascial flap. It is easy to make, thin, and soft in texture, and can be used to repair different sites and types of postoperative head and neck tumor defects with a low donor site complication rate. Good results in post-operative repair of head and neck tumors are worth promoting.

Ankylosing spondylitis (AS) combined with spinal fractures with thoracic and lumbar fracture as the most common type shows characteristics of unstable fracture, high incidence of nerve injury, high mortality and high disability rate. The diagnosis may be missed because it is mostly caused by low-energy injury, when spinal rigidity and osteoporosis have a great impact on the accuracy of imaging examination. At the same time, the treatment choices are controversial, with no relevant specifications. Non-operative treatments can easily lead to bone nonunion, pseudoarthrosis and delayed nerve injury, while surgeries may be failed due to internal fixation failure. At present, there are no evidence-based guidelines for the diagnosis and treatment of AS combined with thoracic and lumbar fracture. In this context, the Spinal Trauma Academic Group of Orthopedics Branch of Chinese Medical Doctor Association organized experts to formulate the Clinical guideline for the diagnosis and treatment of adult ankylosing spondylitis combined with thoracolumbar fracture ( version 2023) by following the principles of evidence-based medicine and systematically review related literatures. Ten recommendations on the diagnosis, imaging evaluation, classification and treatment of AS combined with thoracic and lumbar fracture were put forward, aiming to standardize the clinical diagnosis and treatment of such disorder.
