Objectives Discuss free fibula flap apply in mandibular defect reconstruction after oval cavity tumors resection.Methods Retrospectively review 15 cases clinical materials who undertake free fibula flap reconstruct mandibular defection.Madibular meloblastoma 5 cases,madibular fibrosarcoma 2 cases,submadibular gland adenoidcyst carcinoma 3 cases,submadibular gland mucoepidermoid carcinoma 2cases,oral floor high differentiated squamous carcinoma 3 cases.Simple fibular flap 10 cases,comples fibular osteocutaneous flap 5 cases.Results Fourteen cases fibular flap survival and grow well,1 case putrescence due to crisis of circulation postoperation.Conclusion Free fibular flap reconstruction madibular defect can recover patients visage and oral function,improve their survival quality.
3.Effects of glucagon-like peptide 2 on the absorption-related genes expressionsof residual small bowel in a rat model of short bowel syndrome

Objective To study the effect of glucagon-like peptide 2 (GLP-2) on the absorption- related genes [ sodium dependent glucose transporter 1 (SGLT1) and dipeptide transporter 1 (PEPT1)] mRNA expressions of residual small bowel in a rat model of short bowel syndrome. Methods Rats undertaken 75% small bowel resection were randomly assigned into 3 groups: short bowel control (SBC) group, growth hormone (GH) group and GLP-2 group. Another weight-matched normal rats with normal diet was used as normal (NC) group. All rats were given normal chow on the 1st postoperative day (POD). On the 6th POD, partial segment of terminal ileum was harvested to evaluate the mRNA expressions of SGLT1 and PEPT1 by semiquantified reverse transcription polymerase chain reaction. Results The mRNA expressions of both SGLT1 and PEPT1 in SBC group were significantly higher than that of NC group(P0.05). Conclusions A short term GLP-2 adiministration in early postoperative days may has no significant effect on mRNA expressions of SGLT1 and PEPT1 in residual ileum in rats with short bowel syndrome.
4.Effects of GLP-Ⅱ pretreatment on mouse intestinal mucosal immunity after ischemia/reperfusion insult

Objective: To explore the protections of GLP-Ⅱ pretreatment on mouse intestinal mucosal immunity after ischemia/reperfusion insult. Methods: Thirty ICR mice were randomly divided into three groups: namely normal control (N), control (C) and GLP-Ⅱ pretreatment (P). Group C and group P were inflicted with ligation of superior mesenteric artery for twenty minutes. The morphology of distal ileum mucosa, the rate of intestinal bacteria translocation, the level of plasma endotoxin and intestinal IgA were determined. 5.Self-emulsifying preparation of evening primrose oil

Objective:To search for the optimum formula for the self emulsification of evening primrose oil. Methods: Through solubility experiment, orthogonal screen and drawing phase diagram, taking the degree of emulsifying, the volume of the rest oil and emulsion particle size as parameters,the oil, non ionic surfactant and flux for the optimum formulation were screened. Results: In the formulation of evening primrose oil, taking Tween 85 as the non ionic surfactant and propylene glycol as the co surfactant can get the best effect of emulsifying. Conclusion: The optimum formula for self emulsification of evening primrose oil is evening primrose oil, Tween 85 and propylene glycol,with their proportion being 5∶7∶3.
6.Metabolic effects of administration of branched chain amino acids on postoperative patients

Objective: To investigate the metabolic effects of two different formula of amino acids on postoperative patients . Methods: According to prospective ,randomized , single blind and controlled rule, 120 patients after abdominal operation were randomized to receive parenteral nutrition with either amino acid enriched BCAA or balanced amino acid (18 F).From the second day after operation, total parenteral nutrition was infused to the patients with equal calorie and equal nitrogen by central or peripheral vein for 6 days. Meanwhile, every day nitrogen balance was monitored by collecting urine in 24 hours .The indices were investigated such as amino acids patterns, and serum levels of total protein, albumin, prealbumin, thansferrin and fibronectin. Results: Nitrogen balance of patients in study group on day5 was higher than those of control group. The decreased serum levels of albumin in study group were lower than the control group. Concentrations of valine increased more significantly in study group than that of the control group. Conclusions: The formula of amino acid enriched BCAA may be more suitable and effective for the postoperative traumatic patients on protein metabolism.
7.Risk factors of anastomotic leakage after laparoscopic anterior resection of rectal cancer: a Meta analysis

Objective To investigate the risk factors of anastomotic leakage after laparoscopic anterior resection of rectal cancer.Methods Literatures on the risk factors of laparoscopic anterior resection of rectal cancer were retrieved from August 2003 to August 2013,and then a Meta analysis was carried out based on the data.Data were expressed by odds ratio (OR) and 95% confidence interval (95% CI).The heterogeneity of the data was analyzed using the chi-square test or I2 test.Data were pooled using the fixed or random model.Results Eight literatures including 3 289 patients with rectal cancer were retrieved.The incidence of anastomotic leakage was 6.050% (199/3 289).The incidence of anastomotic leakage after laparoscopic anterior resection of rectal cancer in males was significantly higher than females (OR =2.17,95% CI:1.54-3.06,P <0.05).Neo-adjuvant chemotherapy might increase the risk of postoperative anastomotic leakage (OR =1.53,95% CI:1.00-2.32,P < 0.05).Interoperative blood transfusion might increase the risk of postoperative anastomotic leakage (OR =4.80,95%CI:2.98-7.73,P <0.05).Patients with low rectal cancer had greater risk of anastomotic leakage than those with high rectal cancer (OR =1.60,95% CI:1.14-2.23,P < 0.05).Number of linear stapler firings greater than 3 increased the risk of postoperative anastomotic leakage (OR =0.46,95% CI:0.27-0.78,P < 0.05).The ASA classification of anethesia risk,depth of tumor infiltration,lymph node metastasis,preventive colostomy were not correlated with the incidence of postoperative anastomotic leakage (OR =0.66,0.91,1.25,0.78,95%CI:0.36-1.20,0.55-1.51,0.75-2.09,0.50-1.23,P>0.05).Conclusion Male,neo-adjuvant chemotherapy,interoperative blood transfusion,low rectal cancer,number of linear stapler firings greater than 3 are the main risk factors for anastomotic leakage after laparoscopic anterior resection of rectal cancer.

9.Effects of glucagon-like peptide-Ⅱ on residual small bowel absorption of glucose in a rat model of short bowel syndrome

Objective: To study the effect of GLP-Ⅱ on residual bowel absorption of glucose in a rat model of short bowel syndrome. Methods: Rats undertaken 75% small bowel resection were randomly assigned to SB group and SB/GLP-Ⅱ group. Another group of weight-matched normal rats(Normal) received normal diet ad libitum was used as control group for SB group. All rats were allowed normal chow on the 1st postoperative day(POD). On the POD6,the intestinal glucose absorption data per unit length as well as per unit weight of ileum were measured by in vivo circulatory perfusion experiment. Results: Length of perfused ileum in 3 groups of rats were not significantly different. Wet weight to length ratio of perfused ileum in SB group was significantly higher than that of normal group,but significantly lower than that of SB/GLP-Ⅱ group. 45min glucose absorption of per cm ileum in SB/GLP-Ⅱ group was significantly higher than that of SB group(P0.05). Conclusions: In a rat model of SBS, GLP-Ⅱ can enhance the intestinal glucose absorption measured as per unit length of resudial ileum and GLP-Ⅱ manifests a trend to increase the glucose absorption measured as per unit wet weight of resudial ileum.
10.Study of application of the frontal flap for reconstructing defect in nose

Objective: To evaluate the application of frontal flap in reconstructing the defect of nose, the clinical data of 12 cases were reported. Methods: Twelve patients with nasal skin cancers undertaking radical dissection of tumor accompanied frontal flap reconstructing one stage were reviewed retrospectively.Results: The overall following\|up rate was 100%, 10 of these patients curled slightly after reconstructing the nose, but influence of the nasal outline was little. Two cases were recurrent locally, one occurred 24 months after operation, the other occurred 38 months. Both of them received radiotherapy. Malformation arised after radiotherapy. The total successful rate of reconstruct nasal outlook was 83.33%, the 3\|year locally recurrent rate was 8.33%,5\|year survival rate ws 100%, 5\|year survival rate without tumor was 83.33%.Conclusion:The blood supply of frontal flap is abundance;healing up soon after reconstruction; the manipulation of surgery is simple and safe. It is worth applying this operation generally in clinical practice.
