1.Effects of electro-acupuncture combined with repetitive transcranial magnetic stimulation on protein kinase A-cyclic adenosine monophosphate response element binding protein signal transduction system after focal cerebral ischemia in adult rats

Objective To investigate the effects of electro-acupuncture (EA) combined with repetitive transcranial magnetic stimulation (rTMS) on protein kinase A-cyclic adenosine monophosphate response element binding protein (PKA-CREB) signal transduction system after focal cerebral ischemia in adult rats and to explore the mechanism of EA combined with rTMS in treating ischemic brain injury.Methods The animal model of transient focal ischemia was made by artificial middle cerebral artery occlusion. Seventy-five Wistar rats were randomly divided into normal group, model group, EA group, rTMS group and EA+rTMS group. The expressions of PKA and pCREB in hippocampus were detected and the neurologic impairment rating was observed at the 7th, 14th and 28th days, respectively, after infarction.Results The average gray densities of PKA and pCREB expressions in hippocampus after focal cerebral ischemia in model group were higher at the 7th d, lower at the 28th d than that in normal group (P<0.05);higher in EA group, rTMS group, EA+rTMS group than that in model group at all time points (P<0.05), higher in EA+rTMS group than that in EA group and rTMS group at 7th and 14th d(P<0.05), and there was no difference between EA group and rTMS group(P>0.05).The improvement of neural motor function was obvious in EA group, rTMS group and EA+rTMS group compared with model group (P<0.01), especially in EA+rTMS group.Conclusions EA combined with rTMS can promote the functional recovery after cerebral ischemia,enhance the expression of PKA-CREB signal transduction system in hippocampus after focal cerebral ischemia, which might be one of the important mechanisms of EA combined with rTMS in treating ischemia brain injury.
2.The correlation between lesion size and contrast-enhanced ultrasound in breast cancer

Objective To investigate whether there are differences of the characteristics of the contrast‐enhanced ultrasound among breast cancer vary in size .Methods The contrast‐enhanced ultrasound perfusion mode were retrospectively analyzed in 113 cases of breast cancer patients with 119 lesions .They were divided into two groups according to the maximum diameter of the lesion based on ultrasound measured ,one group (≤2 0. cm ) 47 cases ,the other group (> 2 0. cm ) 72 cases .The characteristics and mode of contrast‐enhanced ultrasound of the lesion were analyzed ,including whether is greater than the two‐dimensional ultrasound in enhanced range ,enhanced strength ,the distribution of the contrast agent ,contrast agent perfusion sequence in lesions ,contrast mode ,and the existence of perforator vessels and perfusion defects .Results Enhanced strength ,the existence of perforator vessels and perfusion defects associated with the size of the lesions .The breast cancer lesions more than 20 cm were apt to higher enhance strength ,the presence of perfusion defects ,perforator vessels ( P < 0 0.5) .However ,there were something that had nothing to do with the size of the lesions including whether or not contrast‐enhanced ultrasound range was greater than the two‐dimensional ultrasound range ,contrast agent distribution characteristics ,contrast agent perfusion sequence in lesions ,and differences in contrast patterns ( P >0 0.5) .Regardless of breast lesions size ,the enhanced range tended to larger than two‐dimensional ultrasound range ,and contrast‐enhanced ultrasound present inhomogeneous ,centrality and integrity filling and perfusion mode of quickly rising and falling .Conclusions Within breast tumor angiogenesis is gradual .With the increase of tumor volume ,more significant vascular heterogeneity ,breast cancer lesion are more prone to uneven high perfusion ,accompanied by perfusion defects and perforating vessels ,but the main sign of differentiating benign and malignant showed consistency between different sized lumps in breast cancer ,which included whether or not contrast mode and enhanced range greater than two‐dimensional ultrasonic range .
3.Relationship between perfusion defect and contrast-enhanced ultrasound features,clinical pathological parameters of breast cancers

Objective To study the ultrasonic imaging of breast cancer with or without filling defect with contrast-enhanced ultrasound and clinical pathologic correlation.Methods The suspicious breast lesions underwent contrast-enhanced ultrasound examination, the imaging of 1 1 9 breast cancer with pathological results were retrospectively analyzed,and the contrast enhanced ultrasound effect,distribution of contrast agent,sequence of contrast agent into the tumor,contrast enhanced ultrasound mode,perforator vessel,filling defect and flow classification were observed.Then clinical and pathological data were collected including the tumor size,clinical stage,pathological grade and the situation of micro calcification.The perfusion defect as the grouping factor,there were 37 cases of perfusion defect and 82 cases without perfusion defect.A mono-pactor and multi factor analysis was performed by the logistic regression models to the relationship between perfusion defect and contrast-enhanced ultrasonography,clinical pathological parameters.Results Both mono factor and multifactor analysis displayed that whether to display the filling defect in contrast enhanced ultrasound was associated with ultrasound contrast distribution,the sequence of contrast agent into the tumor and histological grade(χ2=19.5050,21.1210,10.3640,OR =6.977,0.261, 2.509,all P <0.05).Multivariate analysis showed that tumor size and whether to display the filling defect in contrast enhanced ultrasound were close(OR =1.692,P <0.05).Clinical stage,whether the blood flow signal penetrates was meaningful in univariate analysis but excluded in multivariate analysis.Both mono factor and multifactor analysis displayed that contrast enhanced ultrasound mode,contrast enhanced ultrasound effect,flow classification and pathological micro calcification were not associated with whether to display the filling defect.Conclusions Whether to display the filling defect and the related factors of contrast-enhanced ultrasound in diagnosis of benign and malignant were not completely consistent.Breast cancers with filling defects of contrast-enhanced ultrasound tend to present large masses,contrast agent distribution of peripheral enhancement,centripetal filling of ultrasound contrast agents and the higer histological grading.
4.Extraction of Total Alkaloids from Yaotong Granules

Objective:To study the optimum extraction conditions for Yaotong granules. Methods:An orthogonal test was adopted to optimize the percolation extraction process of total alkaloids by orthogonal experiment with the yield of dry extract and total extraction rate of tetrandrine and fangchinoline as the indices. Results:The optimal extraction conditions were as follows:the medicinal materials were soaked with crude powder in 60% ethanol for 48h,then percolated at the rate of 2 ml·min-1 ·kg-1 . Conclusion:The extraction technology for Yaotong granules is simple and rapid with stable results,which can provide theoretical basis for the production of Yaotong granules.
5.Effects of Electroacupuncture Combined with Repetitive Transcranial Magnetic Stimulation on the Expression of Nestin in Neural Stem Cell after Focal Cerebral Ischemia in Adult Rats

Objective: To investigate the influence of electroacupuncture (EA) combined with repetitive transeranial magnetic stimulation(rTMS) on the temporal profile of nestin expression after induction of focal cerebral isehemia in adult rats and to explore the mechanism of EA combined with rTMS in treating ischemic brain injury. Method: The model of transient focal ischemia was produced by occlusion of middle cerebral artery. Seventy-five Wistar rats were randomly divided into normal group, model group, EA group, rTMS group and EA +rTMS group. The neurologic impairment rating and ability of learning and memory were observed at the 7th、14th and 28th d after infarction respectively. Meanwhile, Western blotting was used to observe the number of nestin expression positive cells. Result: Nestin-positive cells were found in cortex, subgranular zone (SGZ), subventricular zone (SVZ) of the ipsilateral side at different time points after cerebral isehemia. The number of nestin-positive cells peaked at the 7th d, began to decrease at the 14th d and was significantly higher in EA+rTMS group than that in model group (P<0.05), then almost reached normal at the 28th d. The improvement of neural motor function deficits as well as the indexes of learning and memory were more obvious in EA+rTMS group compared with model group (P<0.01, P<0.05). These effects were most obvious in EA+rTMS group compared with the EA and rTMS group (P<0.05). Conclusion: EA and rTMS possess the potency of building up and can increase the number of nestin-positive cells in some brain regions after focal cerebral ischemia, which might be one of the important mechanisms of EA combined with rTMS in treating ischemia brain injury.
6.Lowing-IOP effect of endoscopic cyclophotocoagulation combined with phacoemulsification and IOL in rabbit chronic glaucomatous eye

Background Endoscopic cydophotoeoagulation (ECP) combined with phacoemulsification (Phaco) and intraocular lens (IOL) (ECP+Phaco+IOL) surgery has been widely used,but its lowing intraocular pressure (IOP) effect is unclear.Objectives This study was to investigate the lowing-IOP effect of ECP+Phaco+ IOL in chronic glaucoma.Methods This experimental process was approved by Experimental Animal Ethics Committee of Medical College of Nanchang University.Chronic glaucoma models were established bilateral eyes of 30 pigmented rabbits using laser 360°trabecular meshwork photocoagulation.Forty-eight model eyes were randomized to four groups.ECP of 180°,270° and 360° was performed in the A,B and C groups,respectively,and Phaco+IOL was simultaneously carried out;while trabeculectomy (Trab) combined with Phaco+IOL surgery (Trab+ Phaco+IOL) was performed in the D group.IOP was measured and compared by iCare rebound tonometer before surgery and 1 day,1 week,2 weeks,1 month,3 months and 6 months after surgery.Results The IOP was(13.31±1.85)mmHg before surgery and that in 1 week after surgery was (33.19±5.59)mmHg,with a significant difference between them (t=94.676,P=0.000).No significant difference was seen among the four groups before operation (P>0.05).The IOP was significantly lower in the C group compared with the A group and B group in postoperative 1 day (P =0.001,0.035),and in the first week after operation,the IOP was significantly lower in the C group than that of the A group (P=0.013).In the second week after operation,the IOP of the B group was considerably lower than that of the A group,and that of the C group was significant reduced in comparison with the B group (P=0.002,0.006).The IOP was lowest in the C group in postoperative 1 month in comparison with the A,B and C groups (P=0.001,0.009,0.026),and lower IOP was found in the C group compared with the A group in 3 months and 6 months after operation (P=0.008,0.011).In addition,the lowing-IOP range was larger in the group compared with the A group and B group in2 weeks and 1 month after operation (2 weeks:P=0.018,0.014;1 month:P=0.024,0.016).No serious complication was found in various groups.Conclusions Both ECP+Phaco+IOL and Trab+Phaco+IOL can reduce IOP effectively and safely.The lowing-IOP effect is better in larger range of cyclophotocoagulation eye during the ECP+ Phaco+IOL surgery.
7.Value of contrast-enhanced ultrasound in the evaluation of neoadjuvant chemotherapy for breast cancer

Objective To explore the application value of contrast-enhanced ultrasound(CEUS) in evaluating the effect of neoadjuvant chemotherapy(NAC) on breast cancer.Methods Image features of 80 breast cancer patients with neoadjuvant chemotherapy were observed and analyzed by CEUS.In the process,the changes of primary foci before and after neoadjuvant chemotherapy and the changes of qualitative and quantitative indexes of blood perfusion were evaluated.The correlation between qualitative change and pathological response was also studied.Results The change of focus size measured by CEUS was higher than that of conventional ultrasound before and after neoadjuvant chemotherapy(P <0.05).However,there was no significant difference between the two measurements before NAC(P > 0.05).All the qualitative observation indexes of the ultrasound contrast after NAC revealed obviously benign conversion(P <0.05).Among them,the pathological response was closely related to the changes of the enhancement degree,the distribution of the contrast agent,the order of the contrast and the contrast mode.After NAC,the perfusion parameters of the lesions showed slowly crawl and slowly retreat,low enhancement and the diminished total amount of microcirculation perfusion.Conclusions Before and after neoadjuvant chemotherapy,the changes of the lesion size deteted by CEUS were more close to the pathological evaluation than those detected by conventional ultrasound.The benign outcome of qualitative and quantitative indexes of the CEUS was more obvious than the changes of the lesion size,which had practical value for guiding the neoadjuvant chemotherapy for breast cancer,especially the CEUS indexes related to pathological assessment.

Objective To study the changes of heart rate variability (HRV) and the effects of treatment with nasal continuous positive airway pressure (nCPAP) in elderly patients with sleep apnea/ hypopnea syndrome (SAHS). Methods SAHS patients and controls were examined by polysomnography(PSG) and dynamic cardiograph(DCG). Mean RR interval(Mean RRi), standard deviation of RR interval (SDRRi), low frequency(LF), high frequency(HF) and their ratio(LF/HF) were calculated. Objective To study the effect of electroacupuncture (EA) combined with repetitive transcranial magnetic stimulation (rTMS) on endogenous neural atem cells, as well as learning and memory in rats with cerebral ischemia. Methods One hundred and twenty male Wistar rats were randomly divided into 5 groups: a normal group, a model group, an EA group, a rTMS group and an EA plus rTMS group. After establishing acute middle cer-ebral artery occlusion, the rats in the various experimental groups were treated with EA, rTMS or EA plus rTMS. The rats' ability to avoid an electric shock was observed using a jumping stand, Immunohistochemical techniques were used to measure the infarct area and the number of Brdu-positive cells. Results A small quantity of Brdu labelled cells was observed in the subgranular zone (SGZ) and the subventricular zone (SVZ) in rats in the normal group. In the EA, rTMS and EA plus rTMS groups, the numbers of Brdu labelled cells in the SVZ and SGZ was significantly higher than in the model group after both 7 and 14 days. The expression of Brdu around the SVZ and SGZ of the 3 treatment groups increased, especially in the EA plus rTMS group. At 7, 14 and 28 d there was significant improve-ment in the neural function and electric shock avoidance test scores in the EA, rTMS and EA plus rTMS groups com-pared with the model group,with the EA plus rTMS animals scoring best. Conclusion EA plus rTMS can promote the proliferation of nerve stem cells, facilitate neurological recovery and improve learning and memory in rats after cerebral ischemia.
10.Clinical observation of the herpes zoster with electric acupuncture,surrounding acupuncture and moxibustion with cotton

Objective To observe the clinical effect of electric acupuncture,surrounding acupuncture and cotton moxibustion in the treatment of herpes zoster.Methods Forty cases with herpes zoster were randomly divided into a treatment group and a control group,with 20 patients in each group.The treatment group was treated with electric acupuncture,surrounding acupuncture and cotton moxibustion,while the control group was treated with valaciclovir and vitamin B1.In the two groups severity of pain and the time of pain beginning to ease,pain lasting blisters disappearing,incrustation and decrustation during treatment were observed and recorded,referring to the People's Republic of China Traditional Chinese Medicine Industry Standard(Chinese disease diagnosis and efficacy standard)for efficacy evaluation.Results There was significant difference between the treatment group and the control group with regard to the cure rate(75% versus 35%,P<0.05);however,the total effective rates of two groups had no significant difference(P>0.05).After treatment VAS score in treatment group was significantly lower than that in control group(P<0.05);in treatment group the time of pain beginning to ease and pain lasting were significantly less than those in control group(P<0.05);the time of blister disappearing in treatment group was significantly faster than that in control group(P<0.01);the time of incrustation and decrustation in treatment group were significantly less than those in control group(P<0.05).Conclusion The advantages of electric acupuncture,surrounding acupuncture and cotton moxibustion in the treatment of herpes zoster were shown as faster pain easing,shorter pain lasting,shorter duration of blister and incrustation.This treatment for herpes zoster is safe,effective and affordable.
