1.Cost-minimization Analysis of Four Kinds of Premixed Insulin in the Therapy for Newly Diagnosed Type 2 Diabetes

Objective:To evaluate the short-term economic effects of four kinds of premixed insulin in newly diagnosed type 2 dia-betes mellitus. Methods:A total of 120 newly diagnosed patients with type 2 diabetes mellitus were divided into four groups according to the kind of premixed insulin, group A was treated with insulin aspart 30 injection, group B was treated with insulin lispro 25 injec-tion, group C was treated with isophane protamine biosynthetic human insulin injection and group D was treated with protamine zinc re-combinant human insulin injection. The course of treatment was three months. The therapy efficacy was assessed by the remission rate in three months. The short-term economic effect was evaluated by the cost-minimization analysis method. Results:The remission rate of group A, B, C and D respectively was 48. 39%, 48. 28%, 51. 61% and 51. 72% without significant difference (P>0. 05). The average cost per person of the four groups was 1195. 52, 1202. 41, 1220. 69 and 1258. 84 yuan, and the average medicine cost per person was 750. 52, 689. 41, 754. 69 and 764. 34 yuan, respectively. There was no significant difference in cost among the four groups (P >0. 05). Conclusion:All the four kinds of premixed insulin can be used for the starting treatment with the similar total cost, and in relative terms, aspart 30 injection and insulin lispro 25 injection are better for the initial treatment of diabetes.
2.The lower urinary tract obstruction of urodynamic study on young and middle-aged patients with chronic prostatitis

Objective To study the urodynamic change of young and middle-aged patients with chronic prostatitis,and provide reference for clinical diagnosis and treatment.Methods Seventy young and middle-aged patients with chronic prostatitis were selected,the bladder capacity,bladder stability,bladder compliance,detrusor pressure and bladder pressure volume curve etc were measured.Results Unstable of detrusor contraction of 20 cases (28.6%,20/70),bladder sensitivity of 35 cases (50.0%,35/70),detrusor underactivity of 46 cases (65.7%,46/70).In 70 young and middle-aged patients with chronic prostatitis,the maximum bladder capacity was 100-497 (380 ± 80) ml,the detrusor pressure at maximum flow rate was 11.2-112.2 (38.4 ± 3.9) cm H2O (1 cm H2O =0.098 kPa),the drop of maximum flow rate was (10.53 ± 1.92) ml/s.Conclusions Through the urodynamic examination,the lower urinary tract obstruction syndrome can be found in young and middle-aged patients with chronic prostatitis,especially who have no significantly improvement after drug therapy.Now,the urodynamic examination for the lower urinary tract obstruction is a credible way to diagnosis,which plays an important role in diagnosis of chronic prostatitis and is useful for clinical practice.
3.The clinical study about levetiracetam on cognitive function and emotional influence in the patients with partial epilepsy

Objective To investigate the clinical influence about levetiracetam on cognitive function and emotional influence in the patients with partial epilepsy.Methods A total of 62 patients with partial epilepsy were divided into carbamazepine group (30 cases) and levetiracetam group (32 cases) by random digits table method,carbamazepine group was treated by carbamazepine while levetiracetam group was treated by levetiracetam.The patients were assessed before treatment and 4,8,12,16 weeks after treatment by the Montreal cognitive assessment scale (MoCA),self rating anxiety scale (SAS) and self rating depression scale (SDS).Results There was no significant difference in MoCA score between two groups before treatment and 4 weeks after treatment (P > 0.05).MoCA score at 8,12,16 weeks after treatment in levetiracetam group was better than that in carbamazepine group [(22.6 ± 2.1) scores vs.(20.8 ± 2.6) scores,(23.5±2.7) scoresvs.(21.3± 2.8) scores,(24.6±4.7) scoresvs.(21.2±3.0) scores],the difference was statistically significant (P < 0.05).There was no significant difference between SAS score,SDS score between two groups before and after treatment (P > 0.05).Conclusion In the process of treating the patients with partial epilepsy by drug,levetiracetam is superior to carbamazepine on the improvement of cognitive function,but the mood improvement is not obvious.
4.Soil-transmitted nematode infection of children and its influencing factors in poverty-stricken areas in two provinces of southwest China

Objective To understand the infection status and the main risk factors of soil-transmitted nematodes in children in the poverty-stricken areas of Guizhou Province and Sichuan Province,so as to provide the evidences for making suitable control strategies in these areas. Methods A total of 95 villages and 6 primary schools in 6 poor counties in the two provinces were select-ed as investigation sites according to the stratified random sampling method. Eleven preschool children aged 3-5 years in each sam-ple village and 11 school children aged 8-10 years in each sample school were chose as investigation objectives,their feces were collected and examined by Kato-Katz technique. In addition,the village doctor and cadres in each village were investigated by a standardized questionnaire to understand the deworming condition of children and the social economy and sanitary status in 2009. Then the correlation between average soil-transmitted nematode infection rate and its influencing factors was analyzed by the Tobit model. Results A total of 1 707 children from 95 villages in the two provinces were examined. In Guizhou Province,the total in-fection rate of soil-transmitted nematodes in children was 46.1%,and the infection rates of Ascaris lumbricoides,Hookworm,and Trichuris trichiura were 31.1%,4.1% and 10.8% respectively,and the corresponding rates in Sichuan Provinces were 9.8%, 3.6%,3.5%,2.7%,respectively. In the dimension of school,a total of 890 school-aged kids from 46 schools in the two provinces were examined,the infection rates of soil-transmitted nematodes,Ascaris lumbricoides,Hookworm,and Trichuris trichiura in Guizhou Province were 53.8%,32.7%,6.6% and 14.4%,respectively,and the corresponding rates in Sichuan Province were 7.3%,2.2%,2.9% and 2.2%,respectively. The results from Tobit analysis indicated that the proportion of children accepted deworming treatment and the paddy field in the cultivated area were statistically correlated to the infections of soil-transmitted nematodes and Ascaris lumbricoides(all P < 0.05). Conclusions The soil-transmitted nematode infection rates of children are still at a high level in poor areas of southwest China. In order to decrease the infection rates,besides the long-term deworming, water supply and sanitary improvement,and the health education should be strengthened,and the treatment of soil-transmitted nematodes should be covered by the New Rural Cooperative Medical System.
5.A clinical research of short-cycle repeat transurethral resection combined pirarubicin treatment of T2 invasive bladder cancer

Objective To investigate the short-cycle repeated transurethral resection ( Re-TURBT) combined pirarubicin treatment of T2 muscle invasive bladder tumor efficacy and safety.Methods From October 2010 to December 2013, 56 clinically diagnosed cases of bladder cancer were divided into two groups;the experimental group was taken a short-cycle repeat transurethral resection surgery, and the con-trol group taken along open surgery to remove the tumor and its surrounding 2cm bladder tissue, postopera-tive patients were given pirarubicin infusion therapy.Results Each patient completed the trial.For the ex-perimental group, blood loss and operative time was significantly lower than the control group ( P <0.05). For experimental group after 3 months, 6 months, and 12 months, maximum bladder capacity ( MBC) and patient perception of bladder condition ( PPBC) were significantly higher than control group ( P <0.05 ) . For experimental group after 3 months and 6 months, maximum urinary flow rate ( Qmax) was significantly higher than control group ( P <0.05) , but no significant difference between two groups at 12 months.The incidence of adverse events, vascular endothelial growth factor ( VEGF) , and a total recurrence rate showed no significant difference ( P >0.05).For experimental group, bladder perforation in 1 case, 1 case was forced to open surgery, the incidence rate was about 6.7%.Conclusions The short-cycle repeated trans-urethral resection combined pirarubicin treatment of T2 invasive bladder tumor was simple, effective and se-curity.
6.Relationship between orthostatic hypotension and antihypertensive therapy in the elderly hypertensive population

Objective To investigate the relationship between orthostatic hypotension (OH) and antihypertensive therapy in the elderly hypertensive population.Methods A total of 1174 elderly retired people (849 cases with hypertension and 325 cases without hypertension) in Guangzhou Military Region underwent health physical examination.All people were surveyed by questionnaires.The orthostatic blood pressure and heart rate were measured in supine position after resting for more than 5 minutes and then at 0 and 2 minutes after standing.Comparative analysis was made on the prevalence of OH and the relationship between OH and antihypertensive therapy.Results A significant difference in the prevalence of OH was observed between hypertensive patients and nonhypertensive patients [29.4% (250/849) vs.15.7% (51/325),x2 =23.32,P<0.01].There was no significant difference in the prevalence of orthostatic hypotension in patients with versus without antihypertensive therapy [30.5% (220/721) vs.23.4% (30/128),x2 =2.62,P>0.05].Conclusions Orthostatic hypotension is common observed in elderly hypertensive patients.Antihypertensive therapy may not increase the prevalence of OH.
7.Analysis of epidermal growth factor receptor expression and gene expression status in tissue microarray of cervical squamous cell carcinoma

Objective To elucidate the protein expression and gene expression status and the relationship between epidermal growth factor receptor (EGFR) protein expression and EGFR gene status.Methods Tissue microarray containing 72 cervical squamous cell carcinoma tissues was constructed,and EGFR protein expression and gene status were evaluated by immunohistochemical and fluorescence in situ hybridization (FISH) techniques.Results Protein expression of EGFR:69 of 72 cervical squamous cell carcinomas were observed.The results demonstrated it was significant association with invasion depth,lymphnode metastasis and lymph-vessel invasion (x2 =4.998,P < 0.05 ; x2 =4.299,P < 0.05 ; x2 =4.686,P < 0.05) in cervical squamous cell carcinomas.For FISH assessing EGFR gene,64 of 72 carcinomas were observed; 7 of 64 cases showed EGFR gene amplification,and 25 disomy,23 trisomy and 9 polysomy were detected.There were high levels of protein expression in all the EGFR gene amplification cases,and there were significant association between EGFR protein expression and the gene copy number (x2 =13.564,P < 0.05).Conclusions EGFR may participate in the occurrence,progression and metastasis of cervical squamous cell carcinoma.Overexpression of EGFR protein may result from gene amplification and gene copy number increases,which showed that EGFR gene expression status may be a more effective biological indicator of cervical squamous cell carcinoma targeted therapy.
8.The expression and clinical pathological significance of PDCD in laryngocarcinoma.

OBJECTIVE:
To investigate the expression and clinical pathological significance of PDCD4 in laryngocarcinoma tissue and its potential significance to clinic.
METHOD:
Western-blotting and immunohistochemistry ana lyse to measure the protein expression of PDCD4 in 54 cases of laryngocarcinoma tissues (studying group) and their paraneoplastic normal tissues (control group). The correlations of PDCD4 with clinical pathological parameters were analyzed.
RESULT:
PDCD4 protein was positively expressed in paraneoplastic normal tissue while which was lost or decreased in laryngocarcinoma tissue by Western blot and immunohistochemistry. Immunohistochemistry assay showed the location of PDCD4 protein in cells was different between the studying group and the control group. The expression level of PDCD4 was related to the pathological grades of the laryngocarcinoma. It's higher in the well-differentiated tumor group than that in the poorly differentiated ones. But the expressions of PDCD4 were no differences among other clinical parameters including sex, age, clinical classification, clinical stage and the cervical lymphonodus who had been metastases or not.
CONCLUSION
PDCD4 gene is anti-oncogene. It may play an important role in the pathogenesis and development of laryngeal carcinoma and it may be a new target of therapy for laryngo carcinoma.

9.Pathogenic bacteria distribution and drug resistance of postoperative infection in elderly patients with gastric cancer complicated with diabetes mellitus

Objective To investigate the distribution and drug resistance of pathogens in elderly patients with gastric cancer complicated with diabetes mellitus.Methods 367 elderly patients with gastric cancer and diabetes mellitus were selected from the hospital from March 2010 to March 2017.The samples were isolated and cultured.The Gram positive bacteria were tested by GN201,the Gram negative bacteria susceptibility test was carried out by GP,and the drug susceptibility test was carried out by disk diffusion method.Results There were 68 cases of postoperative infection in gastric cancer patients with diabetes mellitus,and 79 strains were isolated and cultured.Among the gram negative bacteria,Escherichia coli,Klebsiella pneumoniae and Pseudomonas aeruginosa were the main pathogens.Among the Gram positive bacteria,Staphylococcus aureus and Staphylococcus epidermidis were the main pathogens.The main Gram negative bacteria,Escherichia coli to ceftazidime with levofloxacin resistance rates were 93.33% and 80.00%;Klebsiella pneumoniae had a higher drug resistance rate for cefoperazone and ciprofloxacin,76.92% and 69.23% respectively;a relatively high rate of Pseudomonas aeruginosa resistant to levofloxacin was 80.00%.Among the gram positive bacteria,the re-sistance rates of Staphylococcus aureus to penicillin and erythromycin were relatively high,respectively 91. 67% and 83.33%.Staphylococcus epidermidis had relatively high resistance rates to penicillin and erythromy-cin,100.00% and 77.78% respectively.Conclusion Gastric cancer complicated by diabetes mellitus elderly patients with postoperative infection of pathogenic bacteria distribution is given priority to with Gram-negative bacteria was the most common in,Pseudomonas aeruginosa,the main pathogenic bacteria of Escherichia coli to ceftazidime with levofloxacin resistant rate of Klebsiella pneumoniae was higher on cefoperazone and ciproflox-acin resistant Pseudomonas aeruginosa,the resistance rate of Pseudomonas aeruginosa to levofloxacin is high, and it is important to apply antibiotics rationally according to drug sensitivity test.
10.Progress in individualized use of linezolid in adult critically ill patients

Linezolid is a n anti-infective drug commonly used in clinic. Considering the large difference of individual condition , severe basic disease ,poor organ function and large variety and quantity of drugs ,standard dose of linezolid may not be suitable for all critically ill patients. This paper reviews the relevant researches on the application of linezolid in adult critically ill patients in recent years ,analyzes the pharmacokinetic characteristics of critically ill patients ,and summarizes the influence of common physiological and pathological changes in critically ill patients on drugs. When using linezolid ,the clinical comprehensive evaluation of this special group should be strengthened. In addition to appropriately reducing the drug dosage of patients with liver/ kidney function injury ,it is also necessary to consider appropriately increasing the drug dosage in other cases. After medication ,in order to avoid excessive or insufficient drug exposure ,clinical medication monitoring should be strengthened ,especially the important mean as therapeutic drug monitoring should be used well.
