1.Clinical effect of new type of short section pedicle of vertebral arch nail system on patients with thoracolumbar fracture

Objective To explore the clinical effect of one new type of short section pedicle of vertebral arch nail system(internal fixation system by nail-stick) on patients with thoracolumbar fracture.Methods 68 patients with thoracolumbar fracture were selected.Internal fixation system by nail-stick was used on the patients with thoracolumbar fracture,the clinical effect was observed.Results After following up for six to eighteen months,the vertebral height was measured by X-ray.The anterior edge height of the vertebral was 47% before operation and 94% after operation,the posterior edge height of the vertebral was 76% before operation and 97% after operation,there was significant difference between preoperation and post operation(P<0.01).All the cases with AF intemal fixation system had better clinical effect,18 cases had satisfactory effect.Except that the neurological deficits of 4 cases had not been improved,those of the other cases all had been impreved at some degree.Conclusion Thoracolumbar fracture should be treated early,AF imtermal fixation system has significant effect on the thoracolumbar fracture,the skilled performance is important during the operation,and contraindications of internal fixation system by nail-stick should be noticed.
2.Neuroprotective effect of glucagon-like peptide-1 receptor activation in cerebral ischemia

Glucagon-like peptide 1(GLP-1) is an important member of incretin.Takingitoralymay stimulate the terminal ileum and colon L cel s to secrete GLP-1. After GLP-1 biding specific receptor GLP-1 receptor ( GLP-1R), it exerts the roles of promoting glucose-dependent insulin secretion, inhibiting glucagon secretion, and decreasing plasma glucagon level. The molecular mass of GLP-1 is relatively smal er and can directly cross the blood-brain barrier, and both central and peripheral nervous systems have the GLP-1R expression. GLP-1 significantly improves neurological deficits and reduces infarct volume. It may exert neuroprotective effect through the mechanisms of inhibiting the inflammatory response, oxidative stress, and cel apoptosis. This article review s the discovery of GLP-1, its biological characteristics and neuroprotective effect in cerebral ischemia.
4.Multiple keratoacanthoma centrifugum marginatum: first case report in China

A case of multiple keratoacanthoma centrifugum marginatum is first reported in China. A 37-year-old woman was admitted to the hospital for papules and plaques on her face, which had been increasing in number for 4 months. Cutaneous examination revealed dozenes of well-marginated, pale-red or skin-colored crateriform papules of variant size, and plaques in a geographic pattern on her face. The papules presented with a central umbilication filled with grey-brown corneous material. The plaques were surrounded by dyke-like borders, covered with thick, crusted brown corneous material, and partly depressed in the center. Histopathology showed hyperkeratosis, parakeratosis, irregular strip-like extension of epidermis into dermis, keratinous cysts and squamous eddies. The tumor cells had eosinophilic and glassy cytoplasm characteristic of keratoacanthoma.Given both the clinical and histologic evidence, a diagnosis of multiple keratoacanthoma centrifugum marginatum was made. After more than 3 months of treatment with oral acitretin and topical tretinoin, the lesions faded,leaving rugosity scars. No relapse was noted during 3-year follow-up.
5.Intracranial Aneurysm of Embolization Using Electrolytic Detachable Coil(A Report of 11 Cases)

Objective To investigate the new technique in curing intracranial aneurysms with GDC via intravascular catheter.Methods Thrombosis with GDC were undertaken in 11 cases of intracranial aneurysm,proved with DSA with well depiction of the ancurysms and thcir carrying arteries.Results All except one were thrombosed with only one piece of GDC each,the other one used 3 pieces.All the carrying arteries kept patent after the procedure.Conclusion Thrombosis of intracranial aneurysm with GDC via catheter is proved to be a safe,easy,time-saving and effective method in curing the disease.
6.Study of the correlation between TGF-?1,CD31 expression and PSA,TNM staging in prostate cancer

ObjectiveTo study the expression of transforming growth factor beta-1 (TGF-?1),platelet endothelial cell adhesion molecule-1 (PECAM-1,CD31) in prostate cancer tissues and their correlation with prostate specific antigen (PSA) values and TNM staging.MethodsUsing immunohistochemical method 36 prostate cancer specimens were tested for TGF-?1 expression, and using CD31 for marking vascular endothelial cells,the tumor microvascular density (MVD) was counted. Twelve normal tissue specimens were taken from the non-tumorous tissues adjacent to the prostate cancer as controls. The correlation of TGF-?1 expression and MVD with PSA values,TNM staging,pathologic grading and bone metastasis was analyzed in combination with the clinical data.ResultsThe positive expression rate of TGF-?1 in prostate cancer was 88.89%(32/36),while it was 16.67%(2/12)in controls, showing statistically significant difference between them ( P 20 ng/ml, MVD was (81.5?12.2) mm 2 ( P 20 ng/ml,23 cases had the TGF-?1 expression rate of 100% ( P
7.Formulation,Technology and Quality Evaluation of Dexamethasone Solid Lipid Nanoparticles Lyophilized Injection

OBJECTIVE: To study the formulation and technology of dexamethasone solid lipid nanoparticles(DXM-SLN) lyophilized injection and evalnate it's quality. METHODS: The formulation of the supporting agent was optimized and the preparation technology of lyophilized injection was established to prepare the lyophilised injection. The content of DXM in the sample was determined and its quality was investigated as well. RESULTS: 30% mannitol was supporting agent, the particle size of DXM-SLN was 340 nm, with zeta potential at -25.2 mV, entrapment ratio at 92.10% and drug-loading at 7.10%. It was stable within 3 months' at 2~8 ℃storing. CONCLUSION: The preparation technique of this preparation is simple and feasible and its quality is stable and controllable.
8.One neonate with bullae of lung whose mother diagnosed with Goodpasture's syndrome during pregnancy:case report and review of the literatures

Objective To summarize the clinical features and possible impacts of Goodpasture's syndrome in pregnancy on the pulmonary and kidney of the newborn and the mothers. Methods One patient diagnosed Goodpasture's syndrome in pregnancy hospitalized in Peking Union Medical College Hospital on August 23 in 2011 delivered a neonate with bullae of lung. And literatures including 8 cases of pregnancy complicated by Goodpasture's syndrome worldwide through Medline were reviewed. Results (1) Case report:one 31-year-old women presented with acute renal failure at 30+6 weeks of gestation and delivered a male infant with birth weight 1 900 g by caesarean section at 31+1 weeks of gestation. Diagnosis was confirmed as Goodpasture's syndrome with anti-glomerular basement membrane(GBM) antibodies in serum and renal biopsy after delivery. Then she was treated with methylprednisolone, cyclophosphamide, plasmapheresis and dialysis. The neonate showed the lung bullae in the right middle lobe and bilateral intraventricular hemorrhage but renal function was transient normal with anti-GBM as 113.1 EU/ml. The baby was treated by glucocorticoid for two months and clinical symptoms were improved. Anti-GBM antibodies and chest CT showed normal. After been followed up for two years, the baby was normal. (2) Literatures review:the main manifestations of Goodpasture's syndrome in pregnancy were malignant hypertension and renal failure but respiratory symptoms were not obvious. Treated with plasmapheresis, hematodialysis and glucocorticoid maybe have good effects. Most cases had premature delivery. Neonatal anti-GBM antibodies coming from mothers could result to cerebral, renal and pulmonary injury which could be treated by glucocorticoid. Conclusions The Clinical features of pregnancy complicating the Goodpasture's syndrome are malignant hypertension and renal failure. Diagnosis was depended on positive anti-GBM antibodies and renal pathological changes and treatment were depended on plasmapheresis, hematodialysis and glucocorticoid. Neonatal cerebral, renal and pulmonary injury resulting from anti-GBM antibodies coming from mothers should be followed up, and glucocorticoid should be taken if necessary.
9.Analysis of 368 Cases of ADR Reports In Our Hospital

OBJECTIVE:To explore regularity and characteristics of ADR in our hospital,and to provide reference for ratio-nal drug use in the clinic. METHODS:A total of 368 ADR collected from Zhengzhou People's Hospital during 2013-2014 were an-alyzed statistically in respects of patient's gender,age,drug types,route of administration,dosage form,organs or systems in-volved in ADR,clinical manifestations,ADR outcome,etc. RESULTS:Of 368 ADR reports,female was more than male (52.99% vs. 47.01%);the number of patients aged more than 50 years was the most(36.14%);most of ADR cases were induced by antibiotics and TCM preparations,accounting for 40.22%,21.47%,respectively;among route of administration,most of ADR cases were induced by intravenous administration (85.60%);main clinical manifestations were lesion of skin and its appendants (39.95%),followed by digestive system (21.47%). After treatment,all patients were cured or improved,no death case was found. CONCLUSIONS:Clinic should strengthen ADR monitoring and analysis and evaluarion,strengthen the rational use of anti-biotics and TCM injections and adopt measures to reduce drug risk.
10.Serum concentrations of sCD40L in patients with acute ischemic cerebrovascular diseases

One hundred patients with acute ischemic stroke were enrolled in the study as the trial group, and 20 healthy individuals as control group. Intima-media thickness and plaque of the carotis were detected by carotid ultrasonography, cerebral infarction was detected by CT/MRI, and serum concentrations of sCD40L were measured by enzyme-linked immunosorbent assay. Neurologie impairment score was evaluated in all patients. The results showed that in patients with acute ischemic stroke the serum concentrations of sCD40L in plaques group were significantly higher than those in no plaque group. The levels of serum sCD40L of infarction group (diameter>1.5 cm) were higher than those of lacunar infarction group ( diameter<1.5 cm ) and temporary ischemic attack ( TIA ) group. The levels of serum sD40L in trial group were all higher than those in control group. In the trial group, serum concentrations of sCD40L were correlated with neurologic impairment score. The results indicate that CD40/CD40L signaling pathway may be involved in the carotid atherosclerosis formation and the rupture of plaques, and the increase of serum CD40L levels might be a risk factor for acute ischemic cerebrovascular diseases.
