1.Analysis of blood Hcy,hsCRP,vWF and NSE levels in patients with acute cerebral infarction

Objective:To explore the relationship among serum levels of homocysteine (Hcy) ,high sensitive C reac‐tive protein (hsCRP) ,von Willebrand factor (vWF) ,neuron-specific enolase (NSE) and acute cerebral infarction (ACI) induced by cerebral thrombosis .Methods :A total of 90 patients with ACI caused by cerebral thrombosis trea‐ted in our hospital were selected as ACI group ,another 90 healthy subjects undergoing physical examination simulta‐neously were regarded as healthy control group .Serum levels of Hcy ,hsCRP ,vWF and NSE were measured and compared between two groups .Results:Compared with healthy control group ,there were significant rise in serum levels of Hcy [ (10.25 ± 3.07)μmol/L vs .(20.48 ± 5.14)μmol/L] ,hsCRP [ (2.75 ± 0.67) mg/L vs .(10.66 ± 1.45) mg/L] ,vWF [(108.52 ± 38.28) ng/ml vs .(250.52 ± 21.99) ng/ml] ,NSE [(10.45 ± 1.27) ng/L vs .(17.89 ± 2.13) ng/L] in ACI group , P<0.05 all .Conclusion:Combined detections of serum homocysteine ,high sensitive C reactive protein ,von Willebrand factor and neuron -specific enolase can provide evidence for early diagnosis and treatment of acute cerebral infarction caused by cerebral thrombosis .

3.Effects of early administration of high-dose rosuvastatin on coronary microvascular function and short-term outcome in patients treated with primary percutaneous coronary intervention for acute myocardial infarction

Objective To investigate effects of early administration of high-dose rosuvastatin(40 mg) on coronary microvascular function and short-term outcome in patients treated with primary percutaneous coronary intervention (PCI) for acute myocardial infarction(AMI). Methods Ninety-four consequent AMI patients treated with primary PCI were divided into rosuvastatin group (50 patients) and control group (44 group). The infarct-related artery flow of epicardium was classified in compliance with the TIMI criteria. Myocardial and microvascular perfusion was assessed using the TMPG. The incidence of the MACE and the cytotoxicity and hepatotoxicity of rosuvastatin was respectively recorded in 30 d follow-up period. Results Either patients in the rosuvastatin group or in the control group showed better TMPG immediately after PCI (P<0.05), compared with that before treatment. However, the post-PCI TMPG of the rosuvastatin group was obviously much better than that of control group (P<0.05). Compared with that in control group, the 30-day composite MACE rate was lower in rosuvastatin group and in the TMPG 3 patients of rosuvastatin group:12.0%(6/50) vs. 34.1%(15/44), P<0.05;11.1%(3/27) vs. 42.9%(6/14). There was no cytotoxicity and hepatotoxicity in two groups. Conclusions Early administration of high-dose rosuvastatin (40 mg) can improve coronary microvascular function and short-term outcome in patients treated with primary PCI for AMI, and it is efficient and safety.
4.Design of service platform for mobile medical collaboration

Objective: The mobile medical collaborative service platform can realize mobile nursing informatization, which will bring new changes to the nursing work, and work flow is optimized. The construction of mobile nursing system is the trend of development in informatization, in order to better service to patients, realize full nursing work informatization. Methods: Based on wireless dynamic environment, and three subsystems including nursing collaboration portal, intelligent mobile terminal application system, the message engine service system, it adopts hierarchical architecture design, based on SOA structure mode, whose modules are loosely coupled, and platform core includes: the message engine subsystem, the medical task analysis engine subsystem, WEB middleware subsystem and the client subsystem,in order to realize the service management of mobile medical collaboration. Results: According to the actual situation of nursing work in hospitals, construction of mobile nursing system by the stage of application, to meet the core work of nursing, and then realize the basic nursing work, the construction of digital implementation of nursing information, to realize the informatization construction of digital care. Conclusion: Application of the system to achieve the whole informatization of the nursing management, through access to health services, cloud services center, to share resources, to achieve full coverage of all aspects of smart care management for people's health to provide effective protection.
5.Effect of a new-type enriched material on enriching bone marrow stem cells by selective cell retention

BACKGROUND:Enriching material for bone marrow stem cells is a current research hotspot. Preparation and screening of a new type of bone marrow stem cells-enriched material is crucial for the development of tissue engineering and clinical applications. OBJECTIVE:To observe the surface character and effect of a new type of bone marrow stem cells-enriched material on enriching bone marrow stem cells of decalcified bone matrix decorated with poly-L-lysine by selective cellretention technology. METHODS:Decalcified bone matrix was decorated with poly-L-lysine as the scaffold material. Bone marrow mesenchymal stem cells were col ected by using selective cellretention technology. Control group was decalcified bone matrix and experimental groups were Decalcified bone matrix decorated with poly-L-lysine at different concentrations (0.01%, 0.05%, 0.1%, 0.5%, 1%) with freeze-drying method. Material components were analyzed by Raman spectrum and infrared spectrum. Pore size, porosity, surface and cross section were observed under three-dimension video microscope and scanning electron microscope. The number of bone marrow nucleated cells, fibroblast colony-forming units and platelets were counted pre-and post-enrichment. RESULTS AND CONCLUSION:Decalcified bone matrices were covered with wel-distributed and dense poly-L-lysine. The poly-L-lysine/decalcified bone matrix had a high porosity with much more internal y connected pores. After enrichment with enriched material prepared by 0.1%poly-L-lysine, the number of bone marrow nucleated cells, fibroblast colony-forming units and platelets were increased (3.18±0.31), (5.25±1.40) and (3.88±0.68) times, respectively;the binding efficiency were (53±12)%, (73±13)%and (34±10)%, respectively;the selective rate of fibroblast colony-forming units was 1.41±0.34. The new constructed poly-L-lysine/decalcified bone matrix has good three-dimension space with high selective retention. It is an effective enriching material for bone marrow mesenchymal stem cells.
6.Analyzing Risk Factors that were Associated with Loss of Correction Curvature after Short-segment Restoration and Fixation in Cases Who had Single-segment Thoracolumbar Fracture

Objective:To analyze risk factors that were associated with loss of correction curvature after short-segment restoration and fixation in cases who had single-segment thoracolumbar fracture.Methods:87 Cases who had experienced single-segment thoracolumbar fracture and had underwent short-segment restoration and fixation in our department from Jan 2008 to Jan 2011,and had complete follow-up imaging were included.Cobb angles were measured on lateral thoracolumbar X-ray preoperatively,postoperatively and before removal of internal fixation.And these included the angle formed by vertebras that located above and below injured vertebrae (α angle),superior endplate of injured vertebrae and its superior vertebrae (β angle),inferior endplate of injured vertebrae and its inferior vertebrae (γ angle),inferior and superior endplate of injured vertebrae (δ angle).T-test was used to analyze these angles and their changes.And correlation analysis was used to analyze relationships between α angle change and other risk factors.Results:When compared with preoperative angles,the mean α angle,β angle,γ angle and δ angle were all significantly increased (p<0.05) after the operation.The mean α angle and δ angle before the removal of internal fixation were both significantly smaller than those after the operation (p<0.05),and the mean change ofα angle was-2.85 degrees.After the correlation analysis,we found significant correlations between the change ofα angle and postoperative correction curvature(-0.342,p=0.026),injured region in endplate(0.374,p=0.015),and change of the δ angle(0.231,p=0.041).Conclusion:There was significant loss in the correction curvature before the removal of internal fixation.And the loss was significantly associated with postoperative correction curvature,injured region in endplate,and change of the δ angle.
7.HCV NS5A and NS5B Enhance Expression of Human Ceramide Glucosyltransferase Gene

Host genes involved in lipid metabolism are differentially affected during the early stages of hepatitis C virus (HCV) infection.Here we demonstrate that artificial up-regulation of fatty acid biosynthesis has a positive effect on the replication of the HCV full-length replicon when cells were treated with nystatin.Conversely,the HCV RNA replication was decreased when fatty acid biosynthesis was inhibited with 25-hydroxycholesterol and PDMP(D-threo-1-phenyl-2-decanoylamino-3- morpholino-1-propanol).In agreement with these results,the expression level of GlcT-1(ceramide glucosyltransferase),a host glucosyltransferase in the first step of GSL (glycosphingolipid) biosynthesis,was found to be closely associated with the expression and replication of HCV RNA.On the other hand,the viral RNA can also activate GlcT-1 in the early stage of viral RNA transfection in vitro.To identify viral factors that are responsible for GlcT-1 activation,we constructed ten stable Vero cell lines that express individual HCV proteins.Based on the analyses of these cell lines and transient transfection assay of the GlcT-1 promoter regions,we conclude that HCV proteins,especially NS5A and NS5B,have positive effects on the expression of GlcT-1.It is possible that NS5A and NS5B stimulate transcription factor(s) to activate the expression of GlcT-1 by increasing its transcription level.
8.The formulating and clinical significance of partial traumatic hemipelvectomy score

Objective To conclude partial traumatic hemipelvectomy score and assess its value in partial traumatic hemipelvectomy treatment.Methods Data of the managements of 14 partial traumatic hemipelvectomy patients between January 2003 and December 2015 were retrospectively analyzed.10 of these patients were males and 4 females,with an average age of 31 (range,21-55).11 patients were brought directly to the hospital emergency department,and 3 patients were transferred from other hospitals.According to Tile classification system,all pelvic fractures were type C fracture:nine with type C 1,three with type C2 and two with type C3.Partial traumatic hemipelvectomy score was concluded according to soft tissue injury severity,the distance between hemi-pelvic and body axis,injury degree of iliac vessel and nerve.All these factors were further classified into four grades (range,1-4) on the basis of injury severity.Based on clinic outcomes and our experiences,hemipelvectomy should be performed at the early stage when patients had scores more than 12;if the score was between 8 and 12,reassessment should be done according to practical situation and hemipelvectomy was recommended;if the score was between 4 and 8,limb salvage was strongly recommended;if the score was below 4,limb salvage should be done.Results All 14 patients were assessed by partial traumatic hemipelvectomy.The average score was 11 (range,9-14).12 of them were scored at the range of 9-12.2 of them had scores more than 12.3 patients died during the initial resuscitation stage.7 patients underwent completion of the hindquarter amputation after control of hemorrhage and all the patients were successfully survived.Limb preservation was attempted in 4 patients:3 of them died eventually because of infection.1 patient underwent hip disarticulation in emergency operation,but this patient eventually required hindquarter amputation for severe infection.Conclusion Partial traumatic hemipelvectomy score could be a rapid and accurate tool in initial assessment of partial traumatic hemipelvectomy.After the control of hemorrhage,early hemipelvectomy could lower the death rate of this kind of patients.
9.Validity of different formulas to predict lengths of the right internal jugular vein catheterization by anterior ap-proach

Objective To explore the most effective formula to predict the catheterization length of the right internal jugular vein by an-terior approach. Methods Sixty-seven cases performed with right internal jugular vein catheterization from January 2013 to June 2013 were enrolled in this study and 4 formulas were selected to predict the catheterization lengths. Comparing their predicted lengths with the actual lengths defined as the lengths of the internal catheters which terminals were inserted to a accurate position,and analyse their predictive validi-ty. Results The predictive error percentages of the 4 formulas were all less than 15%. Comparing the predicted lengths and the actual lengths, there was no difference between the predicted length of the 1st formula and the actual one(P>0. 05),and the predicted lengths of the other three formulas were significantly less than the actual ones(P<0. 05). Comparing the mean absolute predictive errors of the 4 formu-las in the 3 height groups of 150~159 cm,160~169 cm and 170~179 cm,respectively,those of the 4th formula were all significantly higher than those of the 1st formula in all the 3 groups(P<0. 05);and no differences between those of the 2nd or the 3rd formula and those of the 1st formula were observed(P>0. 05). Conclusion The predictive error of all the 4 formulas is less than 15%, and the 1st formula is simple,practical and associated with a much smaller error,more suitable to estimate the length of the right internal jugular vein catheterization by anterior approach.
10.Effect of ulinastatin on Th1/Th2 cell balance in a rat model of stress-induced gastric ulcer

Objective To evaluate the effect of ulinastatin on Th1/Th2 cell balance in a rat model of stress-induced gastric ulcer (GU).Methods Twenty-four male SPF Sprague-Dawley rats,aged 6-8 weeks,weighing 180-220 g,were randomly divided into 3 groups (n =8 each) using a random number table:control group (group C),stress-induced GU group (group GU) and ulinastatin group (group U).The rats were subjected to water-immersion and restraint stress for 3.5 h to establish GU model in S and U groups.At 10 min before water-immersion,normal saline 0.4 ml and ulinastatin 100 000 U/kg were injected via the tail vein in GU and U groups,respectively.After GU model was established,the abdomen was opened after anesthesia.The whole stomach was exposed,and the gastric mucosal damage was evaluated under microscope,ulcer index (UI) was recorded,and the pathological changes of the gastric mucous membrane were observed.Spleen samples were obtained to detect T-bet,Gata-3,interferon-γ (IFN-γ),and interleukin-4 (IL-4) mRNA expression by RT-PCR.The ratio of T-bet mRNA expression/Gata-3 mRNA expression (T-bet/Gata-3) was calculated.Results Compared with group C,T-bet mRNA expression was significantly down-regulated,Gata-3 mRNA expression was up-regulated,T-bet/Gata-3 ratio was decreased,UI was increased,and no significant changes were found in IFN-γ and IL-4 mRNA expression in GU group,and UI was increased,and no significant changes were found in the other parameters in group U.Compared with group GU,the ratio of T-bet/Gata-3 was significantly increased,and UI was decreased,and no significant changes were found in T-bet,Gata-3,IFN-γ,IL-4 mRNA expression in group U.The gastric mucosal damage was significantly mitigated in GU group as compared with group U.Conclusion The mechanism by which ulinastatin mitigates stress-induced GU may be related to correction of Th1/Th2 cell imbalance in rats.
