1.Experimental study of transplantion human embryonic OECs on transected spinal cord of rat

[Objective]To investigate the therapy of human enbryonic olfactory ensheathing cells(hOECs) transplantation on adult rats' transected spinal cord.[Method]Separated,cultured hOECS from abortus were obtained and percent age of P75 positive cells was calculated.Twenty-four Wistar rats were choosen and divided into experimental group and control group randomly.All rats of two goups were transected at the T_(10) segment of spinal cord.Nine to ten days later 5 ul(2.5?10~5) hOECs(labled by Hoechst 33342) were injected in the contusion area of lesion site in transplantation group.In control group same volume of DMEM-F12 were transplanted.1,2,4,6,8 and 10 weeks later all rats were estimated by system of the BBB locomotion score respectively.Then part of them were killed.The tissue section of spinal cords were separated and inspected by fluorescence microscope or the methods of immunohistochemical staining.Data were processed with statistical tools in double-blinded condition.[Result]Transplanted hOECs can survive for at least ten weeks and even migrated from the injuried area in spinal cord.From 4 weeks to 10 weeks the BBB locomotion scores of experimental group were improved significantly contrasting to that of control group(P

Objective To investigate the clinical effect of mini -plate and kuntscher pin fixation in the treatment of metacarpus and phalange fracture.Methods 113 FMB and FPB patients were divided into OSG (58 patients)and CTG(585patients)group according to random number table.Statistics of postoperative recovery and complications in the two groups were collected and analyzed after research.Results In 58 patients of OSG group,the total active bending from 2 -5 metacarpophalangeal joints to phalangeal joints of 28 cases(48.28%,χ2 =8.562,P =0.035 )were beyond 220°,19 cases (32.76%,χ2 =8.935,P =0.024)were between 180 -220°, 11 cases (51.72%,χ2 =9.365,P =0.012)were less than 180°.The total active bending fromthumb metacarpopha-langeal articulation to interphalangeal joint of 30 cases (51.72%,χ2 =9.365,P =0.012)were beyond 220°, 18 cases (31.03%,χ2 =8.747,P =0.009)were between 180 -220°,10 cases (17.24%,χ2 =8.932,P =0.041) were less than 180°(P <0.05).Conclusion AO Mini -plates surgical method has advantage of parking position accuracy,dissection stable fixation,functional recovery soon and less complication in the treatment of FMB and FPB patients,which is worthy of promotion.
3.Revisional surgery for improving outward appearance of reconstructed digit in toe-to-hand transfer

Objective To present some revision surgery for correcting short and bulky outward appearance of reconstructed finger pulp in toe-to-hand transfer. Methods Since Sep. 1998 to Dec. 2006,in a series of 33 patients, 39 fingers had been reconstructed with 2nd toe. In order to change the shape of bulbous distal toe segment into a normal tapering fingertip, revisional operations had been designed, i.e. (1)Excision of bulky skin and excessive soft tissue from one or both sides of the finger pulp. (2)Transfer and inlaid the excised lateral soft tissue flap to the central constricting part of the finger pulp. (3)Full thickness skin graft to palmar central narrow part of toe pulp. 4. Rotational transfer of local lateral "L" shape flap. Results All the patients healed by first intention with no skin necrosis occurred. The bulbous rectangular shaped toe pulp were corrected and outward appearance were much improved in most cases. Though the outward appearance in one simple skin graft case was not satisfactory in correcting flexion deformity of distal segment and increased its circumference. Conclusion In toe-to-hand transfer, the distal segment of reconstructed finger often shown to have an bulbous toe pulp appearance, which may bring psychological burden to the patient and their relatives. Simple revisional surgery recommended here may yield favorable improvement.
4.Role of the E gene in the infectivity of SINV and SINL in infected cells

Objective To elucidate the molecular basis on the differences of infectivity in infected cells between Sindbis virus(SINV:YN87448 virus)and Sindbis like virus(SINLV:XJ-160 virus).Methods Compare the E(glycoprotein)gene sequence and secondary structure of YN87448 virus and XJ-160 virus by bioinformatics analysis.Analyze the contribution of E gene to the biological differences between SINV and SINLV by constructing recombinant virus.Results By bioinformatics analysis,YN87448 virus and XJ-160 virus have the same genomic structure,which has 11 717 nt and 11 626 nt respectively.There are 82 amino acid differences between E gene of these two viruses,and showed scattered distribution.The main peak is basically the same for the hydrophobic of the E gene protein,but in some region existing small differences.The recombinant virus which exchanged the E gene of XJ-160 virus with YN87448 virus totally showed the biological character of YN87448 virus,either in the showing time of CPE,plaque forming time and plaque diameter,or in expression of functional proteins.Conclusion E gene plays a major role in the differences of infectivity in infected cells between SINV and SINLV,this result provide the molecular biological evidences for elucidating the biological differences between SINV and SINLV.
5.Application of nail skin flap of big toe connected with tibial flap of second toe in reconstruction of thumb and first web

Objective To introduce 13 clinical cases of reconstructing traumatic defect on skin of thumb and first web using nail skin flap of big toe connectedwith tibial flap of second toe.Methods From May 2003 to April 2007,a series of 13 case8 of traumatic thumb and first web defect were treated by transplantation of nail skin flap of big toe connecting with tibial skin flap of second toe.Among which two total hand degloving injury cages were included too.Two kind of way according the Gilbert type of the Ist metatarsus dorsal artery were used.Results All the cases were successful and healed by first intention.The contour of reconstructed thumbs showed to be normal or approximately normal.Follow-up examination were obtained in nine cases for 3-17 postoperative months(average 7 months).Sensations of pain,touch and temperature recovered.The two-point-discrimination sensation were found to be 7-17 mm respectively.Opposition measured were 60°-85°(average 75°).All the patients can be back to work and restored activities of daily living.Conclusion In treating soft tissue defect of thumb and first web,transplantation of the nail skin flap of big toe connecting with tibial lateral skin flap Can yield ideal skin covering and exceHent motor function.
6.Clinical application of transposition of polydactyly with neurovascular bundles in repairing of thumb form

Objective To discuss the clinical application and therapeutic effect of repairing deformed thumb in the way of transposition of abandoned polydactyly with neurovascular bundles.Methods From January 2005 to December 2011,six cases of type Ⅳ deformities of thumb duplications were treated with excision of thin and deformed thumb phalangette of trunk,the redundant hyperdactylia,which was well-formed and involved in the first web space,was then transposed with neurovascular bundles to the thumb phalangette of trunk by the first web space and the side of thumb for fixation followed by repairation of form of thumb phalangette.The surplus metacarpal bone and phalanx of duplications were removed and the first web space was reconstructed.Results All 6 cases of heterotopic thumb phalangette transplanted to the deformed thumb phalangette were completely survived.Kirschner wires were removed 4 weeks after operation.Fracture healing occured after 4 to 6 weeks after operation.Postoperative follow-up period ranged from 3 months to 1 year.Transplanted thumbs possessed well-formed appearance with previous sensation on the finger pulp as well as excellent function of grabbing and opposing.Conclusion To repair deformity of trunk thumb in the way of microsurgery by means of transposition of duplicated thumb with neurovascular bundle which should be abandoned,is a simple operation with satisfactory and reliable postoperative effect.It shall be spred and applied.

Objective To investigate the effect of hydroxycam ptothecine on PS2 and COX-2 in nude mice by human bile duct carcinoma QBC939 cells in order to analyze the correlation between PS2 and COX-2 expressions and the development of the tumor.Methods After the model of the transplanted tumor of subcutaneous tissues located the nude mice was constructed by human bile duct carcinoma QBC939 cells,30 nude mice were randomly divided into 3 groups,namely,control group,hydroxycamptothecine low-dose group and high-dose group.The growth of the transplanted tumor was observed and immunohistochemical staining combined with image analysis was used to determine the expression of PS2 and COX-2 in the tumor tissues.Results Compared with that of the control group,the content of PS2 in the tumor tissues was increased in hydroxycamptothecine low-dose group and high-dose group.The content of COX-2 was decreased and the growth of the transplanted tumor was inhibited in both hydroxycamptothecine low-dose group and high-dose group(P

AIM:To observe the effect of Jianwei Yuyang Granule(Radix Bupleuri,Radix Codonopsis,Rhizoma Atractylodis macrocephalae,Rhizoma Corydalis,etc.) on the expressions of PS_2 and platelet activating factor(PAF) of helicobacterpylori-associated gastritis and study the potential mechanisim. METHODS:80 cases were divided into two groups.Jianwei Yuyang Granule group(42 cases) was given Jianwei Yuyang Granule and the control group(38 cases) was given three kinds of western medicines combined.H.pylori was detected by urease test on biopsy specimen of gastric mucosa and Warthin-Starry method and the changes of expressions of PS_2 and platelet activating factor(PAF) were observed.The contents of aninohexose and phosphatide in gastric mucosa were determined at the same time.All patients were rechecked gastroscope in the 6th week. 9.Prevalence of nutritional risk and malnutrition among hospitalized elderly abdominal surgical patients with malignant tumors

ObjectiveTo investigate the prevalence of nutritional risk and malnutrition among hospitalized elderly abdominal surgical patients with malignant tumors.MethodsTotally 269 elderly patents ( ≥ 65 years) with malignant tumor who were hospitalized in our department of abdominal surgery from December 2009 to November 2010 were consecutively enrolled.Nutritional Risk Screening 2002 ( NRS 2002 ) was performed on the next morning after admission.Body mass index (BMI) lower than 18.5 kg/m2 was considered as malnutrition.Results The NRS 2002 was completed in all the 269 enrolled patients.The overall prevalence of malnutrition was 30.1% (81/269) ; more specifically,37.5% (21/56) among geriatric patients ( ≥80 years) and 17.6% (43/245) among the other age groups (P =0.001 ).The overall rate of nutrition risk was 38.3% (103/269) ; more specifically,57.1% (32/56) among the geriatric patients and 29.3% (72/245) among the other age groups (P <0.001 ).The rate of nutrition risk in patients with pancreas cancer was 58.3%,which was higher than other elderly patients; on the contrary,and the rate of nutrition risk in the patients with colorectal cancer was relatively lower.ConclusionThe rates of nutrition risk and malnutrition in hospitalized elderly abdominal surgical patients ( ≥ 65years) with malignant tumor are relatively higher than other age groups,especially among the geriatric patients.
10.The in-hospital and long-term follow-up of unprotected left main coronary artery stenting in patients aged 70 years and older

ObjectiveTo observe the in-hospital and long-term results in patients with drugeluting stenting age≥70 years with unprotected left main (UML) coronary artery disease.MethodsIn this retrospective study, 100 patients with UML disease were enrolled. Death, myocardial infarction, repeated revascularization and composite end points during follow-up were compared between groups aged ≥70 years and control aged <70 years.ResultsThere was no remarkable distinction between the two groups in cardiovascular risk factors, anatomic findings of coronary artery disease and stent variables. No significant differences were found between the two groups in procedure success rate[96.2% (50 cases) vs. 97.9% (47 cases) ,x2 = 1.75, P>0.05] and in-hospital mortality [3.8% (2 cases) vs. 2.1% ( 1 case), x2 = 0.27, P >0.05]. Patients were clinically followed for an average time of 22.0 months in the elderly group and 23.0 months in the control group (t= -0.78, P>0.05). There were no significant differences in death[3.9%(2 cases) vs. 2.1%(1 case) ,x2 =2.51,P>0.05], myocardial infarction[7.7 % (4 cases) vs. 4.2 % (2 cases), x2 = 0.55, P>0.05], repeated revascularization [13.5% (7 cases) vs. 12.5%(6 cases) ,x2 =0.02, P>0.05]and composite endpoints of death, myocardial infarction and repeated revascularization[30.7% (16 cases)vs. 18.8% (9 cases),x2 = 1.92, P>0.05] between the elderly group and the control group.ConclusionsThe procedure success rate and in-hospital mortality of drug-eluting stent implantation in elderly patients aged≥70 years old with unprotected left main coronary artery are comparable to group aged<70 years. The main endpoints including death, myocardial infarction and repeated revascularization are favorable at about 2 years clinical follow up. It is safe and efficacious to implant drug-eluting stent in patients aged ≥70 years old with unprotected left main coronary artery.
