To explore the efficacy and safety of human recombinant endostatin (rh-endostatin) plus pemetrexed and cisplatin in the treatment of advanced non-small cell lung cancer (NSCLC).A total of 79 patients with advanced NSCLC were randomly divided into control group (n =39,pemetrexed plus cisplatin) and study group (n =40,rh-endostatin plus pemetrexed and cisplatin).The short-term relative risks of the test group (19/40,47.5％) was significantly higher than that in the control group (10/39,25.6％).And the clinical benefit rate the test group (33/40,82.5％) was higher than that of the control group (23/39,59.0％,P ＜ 0.05).The predominant adverse effects were bone marrow suppression and digestive reactions.And the incidence rate was not significantly different between two groups.

Objective: To explore the roles of CD4+CD25+and CD8+CD28- T regulatory cells in spontaneous tolerance of inbred rats orthotopic liver Transplantation.Methods: The orthotopic liver transplantation models of spontaneous tolerance were established between BN (donor) and LEW (recipient) inbred rats whose survival time was more than 120 days, The expression of CD8+CD28- T and CD4+CD25+T cells in PBMC and spleen were determined by flow cytometry. CD8+CD28- T and CD4+CD25+T cells were sorted by immunomagnetic beads. The expressions of CD25, CD28, CD152 and CD45RO on CD8+CD28- T and CD4+CD25+T regulatory cells were also determined by flow cytometry. Results: Compared with control groups, the proportion of CD8+CD28-T regulatory cells in spontaneous models was significantly increased in peripheral blood mononuclear cells (PBMC) and spleen [PBMC:(30.2?5.8)% vs(16.4?6.1)%;Spleen(23.7?7.2)% vs(13.4?5.5)% P

OBJECTIVE:To provide theoretical support for the scientific and reasonable policy-making of 23-valent pneumococ-cal polysaccharide vaccine(PPV-23)in elderly. METHODS:Retrieved from CJFD,Wanfang Database,Elsevier and PubMed,lit-erature about the pharmacoeconomics evaluation of PPV-23 in elder were selected and statistically analyzed in respects of cost stud-ies,effect indexes and research perspectives. RESULTS:Totally 13 literatures were included,involving 900 472 patients,who were older than 60 years old. Study locations were mainly Colombia,the United States,Italy,Belgium and China. Study results showed,each additional quality-adjusted life-year(QALY)cost was between $ 9 239-$ 33 000 in respect of cost;cost-effectiveness ratio was between $ 9 239-$ 45 161/QALY in respect of effect indexes. Most researches showed PPV-23 in elderly older than 65 years old had certain cast-effectiveness. Only a research in Netherlands held the idea that PPV-23 in elderly was not considered cost-effectiveness.CONCLUSIONS:Putting PPV-23 inelderly has certain cost-effecctiveness and most countries has put it into nation-al immunization program. The present researches can't ensure the economy of putting ppv-23 in elderly in china,so it needs more researches with high quality to ensure it.

Objective To compare the clinical outcome of unipedicular versus bipedicular vertebroplasty treating for osteoporotic vertebral compression fracture.Methods Sixty-four patients(68 vertebra)were divided into two groups by treated methods:unipedicular kyphoplasty group(33 cases)and bipedicular vertebroplasty group(31 cases).The Cobb angle and vasual analogue pain scale(VAS)were measured preoperatively and postoperatively.The operation time was recorded.Compared the Cobb angle,VAS and the operation time between two groups.Results Of unipedicular kyphoplasty group preoperative,24 hours and 3 months after operation,VAS were(8.42 ± 1.33),(2.21 ± 1.67),(2.09 ± 1.58)scores,the Cobb angle were(31.24 ±9.12)°,(14.21 ±9.21)°,(14.43 ±9.36)° ;while those of bipedicular vertebroplasty group were(8.36 ± 1.52),(2.13 ± 1.80),(2.00 ± 1.71)scores and(30.84 ±8.77)°,(13.94 ± 8.87)°,(14.07 ± 9.87)°.VAS and the Cobb angle of both groups at 24 hours and 3 months after operation were lower than those preoperative(P＜ 0.01).VAS and the Cobb angle of both groups were similar at the same time preoperatively and postoperatively(P ＞ 0.05).The operation time of unipedicular kyphoplasty group and bipedicular vertebroplasty was(45.00 ± 8.76),(72.00 ± 9.32)min,respectively,there was statistically significant difference between two groups(P ＜ 0.01).Conclusions Compared with the bipedicular vertebroplasty,the advantages of unipedicular kyphoplasty are as follows:less trauma,less operation time and less X-rays rediation accepted of the patient and the operator.And it has the similar clinical outcome with the bipedicular vertebroplasty.

Objective To acquire antigen specific CD8+CD28-Tr by donor specific transfusion (DST) in vivo, and evaluate their suppressive effect on the acute rejection responses in rat liver transplantation through adoptive transfer experiment. Methods DST was used to induce CD8 + CD28 Tr to LEW antigens in naive Brown Norway(BN) rat. Then the induced CD8+CD28- Tr were adoptively transferred into the recipients of LEW→BN liver transplantation, which were acute rejection models. The survival time and histological changes were observed. Statistic analysis was performed with Log-Rank test by SPSS11.0 software, to compare the survival rate. Results Adoptive transfer of the DST-induced CD8+CD28-Tr attenuated the acute rejection responses in acute rejection models of rat liver transplantation. Conclusion DST can induce large numbers of CD8+CD28-Tr in naive BN rats in vivo. The adoptive transfer of the induced CD8+CD28Tr suppressed the acute rejection responses in rat liver transplantation. DST may become one of the methods that induce antigen specific CD8+CD28- Tr in vivo.

Objective To evaluate the suppressive effect of CDS+ CD28-regulatory T cells(Treg)in vivo from spontaneous tolerance models on the acute rejection responses in rat liver transplantation.Methods Spontaneous tolerance models of inbred rat liver transplantation were established.CDS+ CD28-Treg isolated from recipients of spontaneous tolerance models were adoptively transferred into the recipients with acute rejection responses one day before the operation.The survival time and histological changes were observed in the adoptive-transferred models.Results CDS+ CD28-Treg from spontaneous tolerance models(LEW→DA)prolonged the survival time of the recipients in acute rejection models(LEW→BN)(from 14.0±2.2 days to 24.0 ± 3.0 days,P=0.0049),and the severity of rejection was alleviated in liver pathology.Conclusion CDS+ CD28-Treg from spontaneous tolerance models can suppress the acute rejection responses in rat liver transplantation,with antigen-specific properties.

Objective To discuss the clinical value of subintimal angioplasty (SIA) in treating long segment occlusion of superficial femoral artery (SFA) in the elderly,and to observe the follow-up results.Methods Subintimal angioplasty was performed in 66 elderly patients with stage Ⅱ-Ⅳ long segment occlusion of SFA.The improvement of ankle-brachial index (ABI) and the long-term patency rates were observed and analyzed before and after treatment.Results Of 66 elderly patients,the surgery was successfully performed in 64 cases (97 ％).Skin temperature was improved,the claudication disappeared or a claudication distance was prolonged,the rest pain disappeared or relieved,foot infection was under control and turned into the dry gangrene in these patients.SIA was given up in 1 case because the wire failed to return to the true lumen and the distal outflow segment was occluded.One patient underwent bypass grafting instead of SIA because the wire failed to get into the subintimal.The ABI got obvious improvement before and after treatment (0.59±0.16 vs.0.84± 0.21,P＜0.01).The patency rates in 62 patients undergoing SIA at 6,12,24,36 months were 85.5％ (53/62),69.4％ (43/62),54.8％ (34/62) and 48.4％ (30/62) after surgery,and the 3-years limb salvage rate in 62 cases was 96.6％ (60/62).Conclusions Subintimal angioplasty is an effective method for treating long segment occlusion of superficial femoral in elderly patients,and it has the satisfactory long-term follow-up results.

Objective To discuss the clinical value of CT combined electronic level guided percutaneous transthoracic needle biopsy in diagnosis of small pulmonary nodules.Methods 44 cases of lung mini-nodule underwent CT combined electronic level guided percutaneous transthoracic needle biopsy.After operation the tissue was sent for pathologic examination.The nodules were divided into two groups,including Group A (diameter 0.5-1.0 cm,N=10) and Group B (diameter 1.1-2.0 cm,N=34),according to the different sizes of diameter.The excision of postoperative pathological and diagnosis result of clinical diagnosis and treatment of more than 12 months follow up were taken as the final result.The sensitivity,specificity,veracity and complications were compared and analyzed using paired x2 test,taking P＜0.05 as significant difference.Results (1)The arrival rate of target puncture were 100 ％ in 44 cases,with the sensitivity rate of 90.9％,specificity rate of 100 ％,veracity rate of 88.6 ％,positive predictive value of 97.5 ％,and negative predictive value of 100 ％.The sensitivity,specificity and veracity of Group A were 77.8％,100％ and 70.0％ respectively.And the sensitivity,specificity and veracity of Group B were 94.1％,100％ and 94.1％ respectively.The sensitivity,veracity of Group A was higher than Group B (P＜0.05),while there was no difference in specificity.(2)Complication:the incidence rate of pneumothorax,pulmonary bleeding symptoms and hemothorax were 9.1％,11.4％,2.3％,respectively.The incidence rate of total complication was 22.7％,and all be cured after the treatment.The lesions with emphysema around is a risk factor for pneumothorax and pulmonary bleeding symptoms (x2 =11.2,10.2,P＜0.05).However,the position,size and deepness was not correlated with the complications mentioned above (P ＞0.05).Conclusion CT combined electronic level guided percutaneous transthoracic needle biopsy is a safe,effective and accurate diagnostic method for lung mini-nodule lesion.

Objective The feasibility and the curative effect of bipolar hemiarthroplasty for hemiplegic side femoral neck fractures in elderly patients were evaluated.Methods The patients suffering from hemiplegic side femoral neck fractures and undergoing bipolar hemiarthroplasty in Beijing Tongren Hospital were followed-up during 2012-2015.The postoperative Harris Score and the complications such as infection,deep vein thrombosis and dislocation were collected.Results Totally 25 patients(25 hips)were followed-up.The average age was 70.1 years,and mean follow-up period was 24.3 months (10-42 months).The mean Barthel index score was higher (77.4 ± 14.7,ranging from 40-95 points) after operation than before operation[25.00 ± 5.95 (15-35 points)] with a statistically significant difference(t=-21.374,P＜＜0.05).The postoperative Harris Score was 83.5± 8.9(60-95 points).According to Harris score,the excellent and good rate was 80.0％.The patient's satisfaction rate was 88.0％.Conclusions Choosing proper prosthesis and fixing measure,and keeping the function and tension of short extorsion muscles can achieve the satisfactory results for hemiplegic side femoral neck fractures in elderly patients.
