1.Reflections on the follow-up quality survey and evaluation of the medical graduate students

This paper analyzes the characteristics of the follow-up quality survey and evaluation of medical graduate students in China.It raises that the follow-up quality survey of graduate students in medical schools should be institutionalized,standardized and normalized; the survey and evaluation should be conducted through close cooperation with the relevant departments and training units; comparative studies on graduates of different years should be conducted; and the indicator system and survey content should value the characteristics of times,training units and subjects; Finally,specific technical issues such as when and to whom the evaluation and survey should be conducted,survey arrangements,evaluation methods,the diversity of participants and determination of results are discussed and suggestions are put forward.
2.Application of isola system in reconstruction of lumbosacral joint after resection of high-level sacral tumors

[Objective]To investigate the operation indications and techniques of the ISOLA spinal system in reconstruction of the lumber-pelvis continuity after resection of high-level sacral tumors. [Method]A retrospective study was performed in 43 patients with high-level sacral tumor (HLST) between July 2001 to January 2007. Symptoms, signs, histological findings of the tumors and their outcomes were analyzed. All patients underwent resection of HLST and spinopelvic reconstruction with ISOLA spinal system,11 unilateral and 32 bilateral.[Result]Among the 43 patients, 33(77.1%)had chordoma, giant cell tumor of bone and neurogenic tumors. Thirty-four patients alive were followed-up for an average of 35 months(12~62 months), 5 patients had a recurrence of chordoma and 29 patients with HLST were under control. The pain originated from the lumbosacral joint and the spinal nerve root was obviously relieved.The patients could stand and ambulate normally. No loose screw or broken rod were found.X-rays showed no signs of the enlarged screw canal or lowered lumbar spine.The effect was 76.5%(26/34) excellent judged by the ISOLS system.[Conclusion]Spinopelvic reconstruction by ISOLA system after resection of HLST is characterized by easy manipulation, less complications and instant stable fixation. It may be widely used in the clinical practice.
3.Construction of human bone marrow mesenchymal stem cell line genetically modified with human proenkephalin gene

Objective To construct h n bone marrow mesenchymal stem cell line genetically modified with human proenkephalin gene. Methods The packaging cell line Phoenix-293T was transfected with the recombinant pBABE-PENK vector to aquire virus. The recombinant virus was then collected and used to infect hMSCs. Stable expression of proenkephalin gene and leucine enkephalin protein and the concentration of leucine enkephalin protein were detected by RT-PCR, immunofluorescence and ELISA respectively. Results The expression of proenkephalin gene and leucine enkephalin protein were significantly up-regulated in the hMSC-PENK cells, and the concentration of leucine enkephalin protein was also increased in the culture medium. Conclusion A human mesenchymal stem cell line that expresses proenkephalin gene and secrets enkephalin was successfully established.
4.Analysis of correlated risk factors in evolving cerebral infarction

Objective To analyse correlated risk factors of evolving cerebral infarction(ECI).Methods The follow data was recorded in the acute cerebral infarction (ACI)cases when they were admitted :sex,age,systolic blood pressure(SBP),diastolic blood pressure(DBP),history of disease (diabetes mellitus,hypertension,hyperlipidaemia,cerebral infarction,hyperuricemia or smoking).The following examinations or determinations were done,including:white blood cell count (WBC),platelet count (PLT),blood gluconate(GLU),PT-INR,fibrinogen(FG),TT-INR,APTT-INR,C-reactive protein(CRP),D-dimer(D-D),products of fibrin degradation(FDP),anti-thrombin Ⅲ(ATⅢ),apolipoprotein A(apoA),apolipoprotein B(apoB),blood urea nitrogen(BUN),creatinine(CR),triglyceride (TG),high density lipoprotein (HDL),low density lipoprotein (LDL),very low density lipoprotein (VLDL),GOT,lactate dehydrogenase(LDH),creatine phosphokinase (CPK),MB isoforms of creatine phosphokinase (CPK-MB),? hydroxybutyrate dehydrogenase (?-HBDH),troponin I(TnI),cerebral CT or MRI and carotid ultrasonography.The patients were divided into two groups,ECI and completed cerebral infarction (CCI),according whether neurons function scale deteriorated.When cases of both groups exceeded 50,we took statistic test by SPSS10.0 statistic software.Results 8 of the above 46 markers had significant defference between the two groups,including CRP,WBC,apoB,GLU,LDH,CPK-MB,?-HBDH and DBP.ECI was more common in patients who had lower DBP or higher CRP,WBC,apoB,GLU,LDH,CPK-MB,?-HBDH when they were admitted.Conclusion The increase of CRP,WBC,apoB,GLU,LDH,CPK-MB,?-HBDH or decrease of DBP when patients are admitted can be predictive markers of ECI.
5.Expression of bone morphogenetic protein-4 mRNA in the cervical posterior longitudinal ligament after decompression

BACKGROUND: It has been thought that posterior cervical decompression increases the risk of cervical instability and the stress on posterior longitudinal ligament may change growth factor expression in vivo. However, the association between BMP-4 expression and heterotopic bone formation has not been confirmed. Bone morphogenetic protein 4 (BMP-4) is one of factors that can lead to heterotopic bone formation alone.OBJECTIVE: To observe the effects of posterior cervical decompression on BMP-4 mRNA expression in the posterior longitudinal ligament.DESIGN, TIME AND SETTING: A randomized controlled animal experiment was performed in Taishan Medical College between January 2005 and December 2006.MATERIALS: A total of 48 adult Sprague Dawley rats of either gender and of clean grade, weighing 270-350 g, were included in this study.METHODS: All rats were randomly and evenly divided into 3 groups: experimental, sham-operated, and blank control. In the experimental group, C36 laminectomy for decompression was performed.In the sham-operated group: only skin incision was made. Rats in the blank control group received no any treatment. Four rats were allocated for each time point (1, 2, 4, and 8 weeks post-surgery) for harvesting posterior longitudinal ligament.MAIN OUTCOME MEASURES: Detection of BMP-4 mRNA expression in the cervical posterior longitudinal ligament by semi-quantitative reverse transcription-polymerase chain reaction.RESULTS: In the experimental group, obvious BMP-4 mRNA expression was found at 1-8 weeks post-surgery. While no BMP-4 mRNA expression was found at all times in the sham-operated and blank control groups.CONCLUSION: Posterior cervical decompression can increase BMP-4 mRNA expression in the posterior longitudinal ligament.Endogenous BMP-4 may contribute to ligamentous ossification and development post-surgery.
6.Impact of deferoxamine intervention on perihematoma tissue in rats with intracerebral hemorrhage

Objective To investigate the effect of the abnormally accumulated iron ions in the perihematoma tissue lesions and the impact of deferoxamine intervention. Methods A total of 135 SD rats were randomly assigned to sham-operation (n = 15), intracerebral hemorrhage (n =60) and deferoxamine (n =60) groups. A rat model of intracerebral hemorrhage was induced by the infusion of autologous blood. The neurological deficit score, brain water content (dry/wet weight method), blood-brain barrier permeability (Evans blue extravasation method), DNA fragmentation (TUNEL staining), and the impact of deferoxamine intervention were observed at different time points in all groups.Results One to seven days after intracerebral hemorrhage, the neurological deficit score, perihematomal water content, the blood-brain barrier permeability, and the numbers of TUNEL positive cells in the intracerebral hemorrhage group were significantly higher than those in the sham-operation group (P <0.01, P <0.05, P <0.01, and P <0.05). All the observation indexes in the deferoxamine and intracerebral hemorrhage groups showed the same trend, however, the neurological score, perihematomal water content, the blood brain barrier permeability, and the numbers of TUNEL positive cells were significantly lower than those in the intracerebral hemorrhage group (P < 0.05, P < 0.05, P < 0.05, and P < 0.05). Conclusions The abnormal accumulation of iron ions involves in the pathological injury of perihematoma tissue after intracerebral hemorrhage, and deferoxamine may reduce this injury.
7.Effect of a new-type enriched material on enriching bone marrow stem cells by selective cell retention

BACKGROUND:Enriching material for bone marrow stem cells is a current research hotspot. Preparation and screening of a new type of bone marrow stem cells-enriched material is crucial for the development of tissue engineering and clinical applications. OBJECTIVE:To observe the surface character and effect of a new type of bone marrow stem cells-enriched material on enriching bone marrow stem cells of decalcified bone matrix decorated with poly-L-lysine by selective cellretention technology. METHODS:Decalcified bone matrix was decorated with poly-L-lysine as the scaffold material. Bone marrow mesenchymal stem cells were col ected by using selective cellretention technology. Control group was decalcified bone matrix and experimental groups were Decalcified bone matrix decorated with poly-L-lysine at different concentrations (0.01%, 0.05%, 0.1%, 0.5%, 1%) with freeze-drying method. Material components were analyzed by Raman spectrum and infrared spectrum. Pore size, porosity, surface and cross section were observed under three-dimension video microscope and scanning electron microscope. The number of bone marrow nucleated cells, fibroblast colony-forming units and platelets were counted pre-and post-enrichment. RESULTS AND CONCLUSION:Decalcified bone matrices were covered with wel-distributed and dense poly-L-lysine. The poly-L-lysine/decalcified bone matrix had a high porosity with much more internal y connected pores. After enrichment with enriched material prepared by 0.1%poly-L-lysine, the number of bone marrow nucleated cells, fibroblast colony-forming units and platelets were increased (3.18±0.31), (5.25±1.40) and (3.88±0.68) times, respectively;the binding efficiency were (53±12)%, (73±13)%and (34±10)%, respectively;the selective rate of fibroblast colony-forming units was 1.41±0.34. The new constructed poly-L-lysine/decalcified bone matrix has good three-dimension space with high selective retention. It is an effective enriching material for bone marrow mesenchymal stem cells.
8.Comparative study on digital orthopedic three-dimensional visualization technology combined with image-based computer navigation and simple image-based computer navigation for percutaneous screw fixation of acetabulum anterior column fractures

Objective To compare the clinical effect of digital orthopedic three-dimensional visualization technology combined with image-based computer navigation and simple image-based computer navigation for percutaneous screw fixation of acetabulum anterior column fractures.Methods A retrospectivecase-control analysis was made on 19 cases undergone percutaneous screw fixation of acetabular anterior column fractures under image-based computer navigation from January 2015 to 2016 March.There were 12 males and 7 females,aged from 21 to 66 years (mean,39.3 years).AO fracture classification was A3 type in 17 cases and B1 type in 2.Based on the application of three-dimensional digital programming,the cases were assigned to two groups:group A (n =9),virtual three-dimensional model was reconstructed and the virtual screw were inserted to uninjured side by software Mimics and group B (n =10),patients were only prepared for routine preoperative preparation.Time of anterior column screw insertion,intraoperative bleeding,intraoperative fluoroscopy frequency,fracture reduction and Majeed score were compared between the two groups.Results All cases were followed up for mean 8.4 months (range,3-12 months).There were no significant differences between groups A and group B in iutraoperative bleeding [(14.1 ± 3.0) ml,(15.1 ± 2.2) ml],good to excellent rate of reduction (89%,80%),good to excellent rate of Majeed score (89%,80%) (P > 0.05).Time of anterior column screw insertion [(22.4-± 3.4) min] and intraoperative fluoroscopy frequency [(24.9 ± 3.8)times] in group A were significantly less than those[(29.4 ± 4.5)min,(30.5 ± 5.8)times] in group B (P < 0.05).Conclusion Digital orthopedic three-dimensional visualization technology is associated with shortened time of anterior column screw insertion and reduced intraoperative fluoroscopy frequency,indicating an effective adjuvant technique for percutaneous screw fixation of acetabulum anterior column under navigation.
9.Internal fixation with pubic nails and sacroiliac screws aided by 3D navigation for pelvic fractures of Tile type B2 or B3

Objective To discuss the clinical effects of internal fixation with pubic nails and sacroiliac screws aided by 3D navigation for pelvic fractures of Tile type B2 or B3. Methods A total of 27 patients with pelvic fracture of Tile type B2 or B3 received operative treatment in our hospital from De-cember 2013 to December 2016. They were 15 men and 12 women, aged from 20 to 68 years ( average, 39. 2 years ) . According to the Tile classification, 12 cases were type B2. 1, 8 type B2. 2, and 7 type B3. Of them, 20 were unilateral pubic rami fractures and 7 bilateral pubic rami fractures; there were 3 dislocations of the sacroiliac joint, 17 fractures of the sacral Denis regionⅠ, and 7 fractures of the sacral Denis regionⅡ. All the 27 cases were treated by internal fixation with retrograde pubic nails and sacroiliac screws aided by 3D navi-gation. Quality of fracture reduction, functional recovery of the pelvis and postoperative complications were recorded to evaluate the clinical effects. Results Totally 26 out of the 27 patients were followed up for 6 to 18 months ( average, 11. 4 months ) , with one lost to the follow-up. According to the Matta scoring, the pelvic reduction was evaluated as excellent in 20, as good in 4 and as fair in 2 cases, yielding an excellent and good rate of 92. 3%. All the fractures united after 12 to 20 weeks ( average, 13. 8 weeks ) . According to the Majeed scoring at the final follow-ups, the pelvic function was evaluated as excellent in 20, as good in 3 and as fair in 3 cases, giving an excellent and good rate of 88. 5%. Screw loosening happened in one patient who had ambulated too early. No permanent pain, wound infection, neurovascular injury, heterotopic ossification or thrombosis occurred. Conclusion For the treatment of pelvic fractures of Tile type B2 or B3, internal fixation with pubic nails and sacroiliac screws aided by 3D navigation can lead to satisfactory short-term outcomes due to advantages of limited operative invasion, short operation time and limited intraoperative blood loss.
10.The clinical effect of levofloxacin used on patients with hospital acquired pneumonia (HAP)

ObjectiveTo explore the clinical effect of levofloxacin used on patients with HAP. Methods134 cases of patients with HAP as studied object were randomly divided into observation group and control group,observation group were given therapy of levofloxacin,and control group were given therapy of azithromycin. ResultsThe efficiency cases,effective cases,vain cases in observation group were 51,13 and 3,the efficiency rate and total effective rate were 75.0% and 93.3%, the data in control group were 38,17,12,51.7 % and 81.7 %. The efficiency rate ( x2 =4. 818, P < 0.05 )and total effective rate( x2 =4. 804 ,P < 0.05 ) of observation group was significantly better than that of control group. ConclusionLevofloxacin used on patients with HAP, could acquire good clinical effect,and the side-reactions were light.
