1.Distribution and Drug Resistance of High-level Aminoglycoside Resistance in Enterococcus spp in Hospital Infection

OBJECTIVE:To investigate the distribution and drug resistance of High-level Aminoglycoside Resistance Enterococcus spp (HLAR) in hospital infection. METHODS:The clinical specimens collected from 2004 to 2007 in our hospital were isolated by routine method. Identification of the bacteria and Antimicrobial susceptibility test were performed using automatic microbiology analysis system. RESULTS:Of the total 292 strains of Enterococcus which induced HLAR in hospital infection,Enterococcus faecalis (E.faecalis) represented 49.3% (144/292) and Enterococcus faecium (E.faecium) 41.4% (121/292). 27.4% (80/292) were from intensive care unit,17.5% (51/292) the department of nephrology and 8.9% (26/292) the neurosurgery department. The urinary system infection represented 29.8% (87/292) in hospital infection. The antimicrobial resistance of E.faecium was significantly higher than that of E.faecalis against quinolones and ?-lactamase antibiotics,but the antimicrobial resistance of E. faecalis was higher than that of E,faecium(P
2.Clinical Pharmacists’ Records of Ward Round

OBJECTIVE: To establish a new pattern of ward round record for clinical pharmacist and rational use of drug. METHODS: The new pattern of ward round record was established according to domestic and foreign history pattern, The Principle and Proposed Pattern of Clinical Medication History in China, disease course record. This new pattern focused on patients’ disease and medication changes. A case of cardiovascular department was taken as example. RESULTS: Established ward around record contained two parts, i.e. first page and record chart. CONCLUSION: Established pattern is concise and practical and covers essential medical information. It makes communication more easily between pharmacist and doctors and meets the needs of clinical pharmacy practice.
3.Epidemiological Study on Disabilities of Leprosy in Newly Detected Patients in China, 1989- 1998

Objective To approach the status of leprosy disabilities in newly detected cases in recent 10 years in China and provide the scientific basis for formulating the preventive strategies. Methods Based upon the individual records from the National Leprosy Recording and Reporting System in National Center for STD and Leprosy Control, 22 437 leprosy cases detected during 1989- 1998 in China were analyzed in terms of leprosy disability. Results The proportion of disabilities in newly detected leprosy cases in 1989 in whole country was 46.49% and decreased to 32.31% in 1998, and the proportions of cases with grade II disabilities were 25.55% (1989) and 20.23% (1998). There were 18 provinces where disability rate was more than 40% . Out of patients with disabilities, those with grades I and II disabilities and with deformities (loss of eyebrow, facial paralysis or saddle nose) accounted for 37.86% , 60.64% and 1.5% , respectively. There were 25 provinces where grade II disabilities accounted for more than 50% of all patients with disabilities. The disability rates in patients aged under 15 years, 15- 65 years and over 65 years were 24% , 39.85% and 53.33% . The patients with a delay in detection of 2 years had a disability rate of 29.85% , and those with a delay of more than 2 years and 5 years had the rates of 48.82% and 61.17% , respectively. The disability rate was 52.9% in patients with leprosy reactions and 46.1% in patients with a damage of more than 3 nerves. The grade II disability rate in paucibacillary patients(28.99% ) was significantly higher than that in multibacillary ones (22.04% ). Conclusion Disability rate of leprosy in newly detected cases is still high although it has decreased in the recent 10 years. The rate is associated with delay in detection, leprosy reaction and leprosy type. It suggests that early detection of leprosy patients, regular treatment with multidrug therapy, and management of leprosy reactions will be the effective measures to prevent disabilities of leprosy.
4.Chemical constituents of the root of Myrica nana

AIM:To study the chemical constituents of the root of Myrica nana cheval.METHODS:The constituents were separated and purified by different methods of chromatography,and their structures were elucidated by UV,IR,GM and NMR.RESULTS:Seven compounds were isolated from the root of Myrica nana cheval.Their structures were identified as ?-sitosterol(1),myricanol(2),myricanone(3),ursolic acid(4),quercetin(5),myricetin(6)and myricitrin(7).CONCLUSION:Except for 5,6 and 7,the other compounds were obtained from this plant for the first time.
5.Clinical Study on Selective Posterior Rhizotomy and Post-operative Rehabilitation Training in Treating Cerebral Palsy

Objective To explore the efficacy of selective posterior rhizotomy (SPR)and post-operative rehabilitation training in treating cerebral palsy. Methods The 100 patients with cerebral palsy were randomly divided into two groups. In group A, the patients were treated by SPR, and in group B, the patients received rehabilitation training after SPR for 12 months. One year later the curative efficacy of all patients was evaluated and compared between the two groups.Results The effective rate was 52% and 76% in groups A and B respectively. Conclusion It is very important that the patients receive systematic rehabilitation training after SPR so as to gain a most satisfied functional recovery.
6.Studies on mental health of servicemen who experienced a catastrophic event

Objective To investigate the influence of a catastrophic event to the mental health of servicemen,and to observe the effect of mental intervention and drug treatment for them after experiencing a catastrophic flood attack.Methods 57 servicemen survived a catastrophic flood attack were regarded as the disaster group,and 70 others who were not involved in the flood disaster were regarded as the control group.The service men of the two groups were subjected to investigation with Chinese Military Mental Health Scale(CMMHS),and corresponding mental intervention or medication was given to those who had experienced mental disorders.Results The disaster resulted in different degrees of somatic trauma and various mental disorders including instability of emotion,mental depression,grievance,nervous tension,sensation of fear,sleep disorder and apprehensions.The influential factor scores of CMMHS,including compulsive neurosis,anxiety,depression,fear,psychosis,somatization,interpersonal sensitiveness,and total score were much higher in disaster group than that in control group,and that of norm of Chinese servicemen,while the neurasthenia factor score in disaster group showed no significant difference with that in control group and that of normal Chinese servicemen.The main influential factors to changes in mental health of the servicemen were witness of someone washed away by the flood,and participation in identifying corpses.Conclusion The mental health of servicemen who experienced a catastrophic flood disaster was harmed in various degrees,and medication or mental intervention appeared to be of value.
7.Expression and verification of VP1 protein of human parvovirus B19-XA strain in E. coli

Objective: To construct a recombinant expression vector containing the VP1 whole gene of human parvovirus(HPV) B19-XA strain by DNA recombinant technology and to induce the expression of the VP1 fusion protein in E.coli.Methods: The gene of interest was amplified by PCR from the viral DNA genome extracted from the patient infected with B19 virus,and inserted into the pET28(a) expression vector.The positive recombinants pET28(a)-VP1 were transformed into E.coli BL21(DE3),and then the VP1 proteins were expressed after induced by IPTG and analyzed by SDS-PAGE and immunoblotting.In addition,rabbits were immunized with the recombinant VP1 fusion proteins as antibodies,and their valence was detected by ELISA.Results: The positive bacteria strains containing the recombinant expressive vector pET28(a)-VP1 were constructed successfully,and IPTG induced a high expression of the VP1 fusion proteins.The valence of multiclone antibodies of anti-VP1 proteins was 1∶12 800.Conclusion: The VP1 fusion proteins of the HPV B19-XA strain can be expressed efficiently in E.coli,and these proteins possess satisfactory immunogenicity and play an important role in the preparation of diagnostic reagents and the development of vaccines.
8.Study on Water Extraction Process of Shenshu Yangwei Capsule

OBJECTIVE:To optimize the water extraction process for Shenshu yangwei capsule.METHODS:The decoction conditions were optimized by orthogonal design with table L9(34)taking the yield of Hesperidin as index.RESULTS:The optimal watering extraction conditions of Shenshu yangwei capsule were as follows:adding 8 times amount of water,then extracting for 3 times with 1 h for each time.CONCLUSION:The water extraction technology of Hesperidin is simple,with complete extraction and high yield of Hesperidin,thus applicable for industrialized large-scale production.
9.Changes of plasma ET-1,CGRP,TXA_2 and their receptor expression in lung tissue of children with congenital heart disease-associated pulmonary artery hypertension

Objective To study the changes of plasma ET-1,CGRP,TXA2 and their receptor expression in lung tissue of children with congenital heart disease-assicated pulmonary artery hypertension.Methods Plasma was collected from 60 children patients.ET-1,CGRP and TXB2 levels were measured by EIA,RIA and ELISA,respectively.Expression of their receptors in the lung tissue was detected by RT-PCR.Results The plasma ET-1 and TXB2 levels were significantly higher in CHD patients than in control children.The plasma CGRP level was significantly lower in CHD patients than in control children.Fourteen days after treatment with PGE1,the plasma ET-1 and TXB2 levels were significantly decreased with no significant changes in CGRP.The expression level of ET-1 and TXA2 receptor mRNA was significantly higher in patients with pulmonary artery hypertension than in those without pulmonary artery hypertension.However,the expression of CGRP receptor mRNA was lower in patients with pulmonary artery hypertension than in those without pulmonary artery hypertension.Conclusion Plasma ET-1,CGRP and TXA2 levels are closely related with congenital heart disease-associated pulmonary artery hypertension,and can be used as an index for its prognosis.
10.Detection and their clinical significance of cytokines levels in serum and synovial fluid from patients with rheumatoid arthritis

Aim To study cytokine(CK) changes of IL-6,IL-8,sIL-2R and TNF-a levels in sera from patients with juvenile rheumatoid arthritis(JRA) and rheumatoid arthritis(RA) and its correlation with the conventional inflammation indexes,erythrocyte sedimentation rate(ESR) and C-reactive protein(CRP). Methods Levels of IL-6,IL-8,sIL-2R and TNF-a in serum from 30 JRA patients and 34 RA patients,in the synovial fluid (SF)from 4 JRA patients,7 RA patients,6 osteoarthritis(OA) patients and 9 meniscitis(MT) patients were detected by sandwich ELISA. Results As compared with control group,① levels of IL-6 and SIL-2R in sera from 30 JRA patients and 34 RA patients and levels of IL-8 in sera from 30 JRA patients possessed marked difference P∨ 0.01 and P∨ 0.05,respectively.② There were significant difference among serum IL-8 and sIL-2R levels from systemic JRA patients, pauciaticular JRA patients,and serum IL-6 levels from polyarticular JRA patients(P<0.01). ③ There were remarkable difference among SF sIL-2R levels from 4 JRA patients and 7 RA patients and in SF IL-6 levels from 7 with RA patients.④ There were obvious correlativity between serum IL-6,sIL-2R levels and changes of serum ESR and CRP from the JRA patients(r=0.532 and r=0.621,respectively). Conclusion ① IL-6 and sIL-2R exhibit a close correlativity with the activity of JRA and RA,which is the main index of activity of JRA and RA diseases.② sIL-2R might not only participate in systemic pathologic process in JRA and RA diseases,but also serves as a major CK in local articular injuries.IL-6 participates in local articular injuries too,and seems to play a more important role in local articular injuries of RA.③ IL-8 may participate in systemic pathologic process of JRA,but play a minor part in local articular injuries.
