1.Clinical study on treatment of irritable bowel syndrome by massage combined with microecological regulators

Objective To explore the treatment of irritable bowel syndrome, massage combined with microecological modulator therapy. Methods For 40 cases of irritable bowel syndrome patients using micro ecological regulator treatment, and classified as the control group, the other 40 patients used massage combined with microecological regulating agent treatment, and to the observation group, two groups of patients were in our hospital from March 2015 to January 2017. Results No significant difference between the two groups of patients in the treatment of the bacteria, after the number of packet treatmentintestinal aureus two groups had no significant difference, other bacteria were the observation group of patients obviously improved, improved the difference was significant(P<0.05). The total effective rate in observation group was 97.5%, only 75% of the control group, the observation group were significantly higher. Conclusion The clinical study of massage combined with microecological preparation in treatment of irritable bowel syndrome, the treatment is simple compared to micro ecological regulator, has certain advantages, can improve clinical symptoms of patients. At the same time, improve the therapeutic effect, so it is worthy of clinical use of priority.
2.Clinical Observation of Jiedu Xiaoyu Cream Combined with Glucosamine Potassium Sulfate Capsules in the Treatment of Knee Osteoarthritis

OBJECTIVE:To observe clinical efficacy and safety of Jiedu xiaoyu cream combined with Glucosamine potassium sulfate capsules in the treatment of knee osteoarthritis. METHODS:196 patients with knee osteoarthritis were randomly divided in-to observation group and control group,with 98 cases. Both group were given Glucosamine potassium sulfate capsules 0.5 mg/time, bid;observation group additionally received Jiedu xiaoyu cream for local use,once a week;control group additionally received Di-clofenac diethylamine emulsifiable paste for local use,bid. A treatment course lasted for 2 weeks,and both groups received 3 cours-es of treatment. Clinical efficacy,VAS scores,WOMAC scores and ADR of 2 groups were observed. RESULTS:The total effec-tive rate of observation group was 94.7%,which was significantly higher than that of control group(88.3%),with statistical sig-nificance(P<0.05). Compared with before treatment,VAS and WOMAC scores of 2 groups decreased obviously,with statistical significance(P<0.05);the decrease of observation group was more significant than that of control group,with statistical signifi-cance(P<0.05). There was no statistical significance in the incidence of ADR between 2 groups(P>0.05). CONCLUSIONS:Jie-du xiaoyu cream combined with Glucosamine potassium sulfate capsules has good clinical efficacy in the treatment of knee osteoar-thritis,can relieve knee pain and improve the function of activity.
3.Relationship between pulmonary arterial hypertension and fibrinolysis in elder patients with chronic obstructive pulmonary disease

A total of 40 patients with COPD (excluding those with correlated/relevant diseases)were measured for inflammation parameters of erythrocyte sedimentation rate (ESR) and c-reactive protein (CRP) after hospital admission and some coagulation/fibrinolysis parameters including D-dimer,thrombinantithrombin (TAT),prothrombin fragment 1 + 2,(tissue plasminogen activator) tPA,plasminogen activator inhibitor 1 (PAI-1),von Willebrand factor (von WF),endothelin receptor A,thromboxane B2,P-selectin and pulmonary arterial pressure (PAP) by ultrasonic cardiography after the settling of the symptons of acute period.All patients were then divided into 2 groups according to PAP [< 40 mm Hg (1 mmHg=0.133 kPa) (n=24),>40 mm Hg (n=16)].The values of CRP and ESRin the group with PAP > 40 mm Hg were significantly higher than those in another group (P =0.044 and P =0.002respectively) while tPA was lower (P =0.04).A moderate positive correlation existed between tPA and TXB2 (r =0.547).Moreover,a highly positive correlation was found between TXB2 and PAl-1 (r =0.929).The results indicated that the COPD patients with pulmonary arterial hypertension (PAH) tend to have a higher level of inflammation,and their fibrinolysis becomes impaired leading to a prothrombotic state.
4.Correlation between the C-reactive Protein gene polymorphism in periodontitis and the chronic periodontitis with type2 diabetes

Objective: To study the relationship between C-reactive protein (CRP) gene polymorphisms and the risk of chronic peri-odontitis and severe chronic periodontitis (CP) with type2 diabetes to confirm the effect of genetic factor in chronic periodontitis and chronic periodontitis with type2 diabetes. Methods; DNA was extracted by Chelex-100 from buccal swabs of patients who suffered from chronic periodontitis or chronic periodontitis with type2 diabetes and patients with healthy periodontium. PCR-RFLP was used to test the CRP genotype distribution. The correlationship between the incidence of chronic periodontitis and chronic periodontitis with type2 diabetes and CRP gene polymorphism was analyzed statistically. Results; There was no statistical difference in the distribution of CPR +1059 genotype and allele frequency between experiment group and control group (X~2 = 0. 223, P=0.994). The genotype and allele frequency distribution were in line with Hardy-Weinberg equilibrium. Conclusion; There is no correlation between CRP + 1059G/C single nucleotide polymorphisms and the susceptibility of chronic periodontitis as well as chronic periodontitis with type2 diabetes.

6.Antigen selection, optimized expression and polyclonal antibody preparation of O-GlcNAcase.

In order to probe the biological function of O-GlcNAc and the pathogenesis of associated diseases, it is essential to prepare a potent and specific O-GlcNAcase (OGA) antibody. Based on protein sequence analysis, we found N terminal 1-350 amino acids of OGA (sOGA) has high antigenicity and hydrophilicity and then constructed it into plasmid pET28a vector. First, we optimized the expression of sOGA in Escherichia coli BL21(DE3) (0.05 mmol/L IPTG, 10 hours) and purified it with the Ni-NTA affinity chromatography and size exclusion chromatography respectively. SDS-PAGE verified the molecular weight (45 kDa) and the purity (>95%) of sOGA and the purified protein was subjected to immunize New Zealand rabbits. Finally, we obtained OGA polyclonal antibody by affinity purifying the antiserum with CNBr-activated Sepharose 4B beads. Western blotting and ELISA assay showed that this antibody could recognize three OGA isoforms with high specificity and the sensitivity was 0.11 ng/mL (the titer was 1:80 000). These results indicated the prepared polyclonal antibody of OGA can be used for the biological function study of OGA.

7. Nasal high flow oxygen therapy for chronic obstructive pulmonary disease with hypercapnia

Nasal high flow oxygen therapy (HFNC), an open nasal catheter system conveying heated and humidified air or oxygen, has been used to treat hypoxemic respiratory failure. At present, some researches indicated that HFNC can be applied for chronic obstructive pulmonary disease (COPD) with hypercapnia, which attenuates the retention of carbon dioxide by positive end-expiratory pressure effect, washing out anatomical dead space and reducing work of breath. HFNC is proved to decrease respiratory frequency, increase alveolar ventilation and end-expiratory lung volume, and its therapeutic effect is similar to non-invasive ventilation. Application of HFNC managed to be optimized in a way of increasing the flow rate in a single prong and mouth-closed breathing. This article summarizes the application of HFNC in COPD with hypercapnia.
8.The value of preoperative NLR and PLR combined with the consensus in surgical decision making for branch duct intraductal papillary mucinous neoplasm of the pancreas

Objective To analyze the difference among the 3 guidehnes used to make surgical decision for branch duct intraductal papillary mucinous neoplasm (BD-IPMN),then the guidelines were combined with neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR) respectively for further analysis.Methods Clinical data of 51 appropriate BD-IPMN patients who underwent surgical resection from January 2008 to December 2015 was retrospectively analyzed.Results The significant difference was exist in the consensus for followup criterion (P < 0.05).The preoperative NLR and PLR were helpful for the differential diagnosis between malignant and benign BD-IPMN,because the receiver operating characteristic curve (ROC) of NLR and PLR for prediction were 0.686 and 0.692,and the best boundary values were 2.64,92.56 respectively.The consensus combined with PLR could improve the specificity and positive predictive value (PPV),besides,the specificity and PPV could achieve 70.3%,54.2% respectively while the sensitivity (92.9%) still remained at an ideal level after international consensus guideline combined with PLR.Conclusions NLR ≥ 2.64 and PLR ≥ 92.56 were predictive markers for the presence of BD-IPMN associated invasive tumor.The addition of PLR as a criterion to the international consensus guideline improved the accuracy of international consensus guidelines in estimating invasive BD-IPMN.
9.Expression of cytidine triphosphate synthetase 1 in pancreatic cancer cell lines and its clinical significance

Objective To investigate the cytidine triphosphate synthetase 1 (CTPS1) expression in pancreatic cancer cell lines and its impaction on prognosis.Methods The GEO profiles database was searched to collect clinical data of 39 patients of pancreatic ductal adenocarcinoma.Kaplan-Meier curve was used to analyze the relationship between the patients' prognosis and the expression of CTPS1.qPCR and Western blot were applied to detect the expression of CTPS1 in 4 pancreatic cancer cell lines.Results The mRNA expression of CTPS1 in Sw1990,BxPC-3,Panc1,and MIA-Capa2 was 1.00) ± 0.20,2.92±0.95,4.29±0.14,and 2.26±0.33 (t=-16.19,-11.45,-8.09,-13.12,P<0.05) when compared to the normal pancreatic ductal epithelial cell line HPDE of 7.70 ± 0.72.The relative protein expression of CTPS1 in pancreatic cancer cell lines Sw1990,BxPC-3,Panc1,and MIA-Capa2 was 0.40 ± 0.06,0.20±0.09,0.68±0.11,and0.48±0.06 (t=-8.97,-10.5,-4.39,-7.88,P<0.05) when compared to HPDE of 1.09 ±0.12.The overall survival (OS) in patients with higher CTPS1 expression than adjacent tissue was 28 months (95% CI:9.2-46.8 months),and that with lower CTPS1 expression was 13 months (95% CI:8.2-17.8 month),(x2 =4.02,P<0.05).Conclusions CTPS1 in the pancreatic cancer cell lines are of low expression,and the level is positively related to patients' survival time.

Coronavirus disease 2019 (COVID-19) can cause great damage to the elderly patients and lead to high mortality. The clinical presentations and auxiliary examinations of the elderly patients with COVID-19 are atypical, due to the physiological ageing deterioration and basal pathological state. The treatment strategy for the elderly patients has its own characteristics and treatment protocol should be considered accordingly. To improve the diagnosis, treatment, and prevention of COVID-19 in the elderly, the Expert Committee of Geriatric Respiratory and Critical Care Medicine, China Society of Geriatrics established the "Expert consensus for the diagnosis, treatment, and prevention of coronavirus disease 2019 in the elderly" . We focused on the clinical characteristics and key points for better treatment and prevention of COVID-19 in the elderly. (1) For diagnosis, atypical clinical presentation of COVID-19 in the elderly should be emphasized, which may be complicated by underlying disease. (2) For treatment, strategy of multiple disciplinary team (mainly the respiratory and critical care medicine) should be adopted and multiple systemic functions should be considered. (3) For prevention, health care model about integrated management of acute and chronic diseases, in and out of hospital should be applied.
