1.Improvement and discussion on outpatient service process in informational technology
Journal of Medical Postgraduates 2003;0(12):-
Objective:In the view of research on outpatient service process of a compositive hospital, the advanced method and its effects to improve the outpatient service process by informational system were discussed. Methods:The outpatient informational system is closely related to the process rebuilding. Results:It is necessary to reunite the existing process by informational technology in order to dicease the units, shunt the peak, get rid of the stricture and improve the efficency. And more a new outpatient service process being convenient, quick, good, sufficient and with low-consume should be established. Conclusion:We must hang on the guildline of "based on humanity",and break off the traditional mode, achieve the advanced mode, improve the scientific management, ensure the service quality which based on informational technology.

Objective: This study aimed to analyze the high medical expenses of hospitalized medical insurance patients for the first ten systemic diseases. Methods: We investigated the medical expenses for the first ten systemic diseases among 10 900 hospitalized medical insurance patients of a hospital, calculated the average expenses and total payment for the treatment of each of the systemic diseases that involved 5% of the total patients, computed their percentage in the total expenses, and compared them with those of all the other hospitalized cases. Results: The medical cost of the 5% of the first 10 hospitalized systemic diseases accounted for 21%-29.9% of the total expenses of all the hospitalized patients,4.3-6.0 times that of the average expenses, and 30.8%-54.9% paid by the patients themselves. In addition, the hospital stay of these patients was 1.5-4.7 times that of the average. Conclusion: The high medical expenses involving 5% of the hospitalized patients with the first 10 systemic diseases amounted to a quarter of those paid by all the hospitalized patients.Effective measures have to be taken to keep high hospitalization expenses at a reasonable level.

Objective To construct a prokaryotic expression system for human creatine kinase(CK) MM isozyme, purify the recombinant protein of CK expressed in Eschericheia coli(E.coli) and examine the stability of the recombinant protein for its application in CK measurement system as the quality control. Methods Total RNA of extracted from fetal cardiac muscle was reversetranscripted and cDNA encoding human CK was amplified which was inserted into pET-15b plasmid vector. The recombinant plasmid was transfered into E.coli BL21(DE3) and induced by isopropyl-?-D-thiogalactopyranoside(IPTG).Recombinant CK-MM was separated from bacterial proteins by affinity chromatography on a Ni2+-Sepharose column. The activity of the recombinant enzyme was observed in different matrixes. Results The enzymatic activity of the crude extracts of CK-MM was up to 280,000U/L.After one step affinity chromatography, the fusion protein showed a single band in SDS-PAGE gel.The purified protein was stable and the enzymatic activity remained unchanged for a month in the matrix containing bovine serum albumin,EDTA and 1,4-Dithiothreitol(DTT).Conclusion The recombinant CK-MM showed key properties of the native creatine kinase isozyme and may be used as control and calibrator for determination of serum CK-MM.

Objective To provide guidance for patients and enhance their trust in the hospital by achieving openness and transparency in medical fees. Methods The modular structure of the three-tiered process was employed and various information machines, inquiry devices and doctors' workstations were connected to the Hospital Information System to form an integrated inquiry system of hospital-wide coverage. Results The Hospital Information System, with its versatile functions, easy operation, strong compatibility and good reliability, could provide patients with accurate information and more convenience. Conclusion The Hospital Information System can meet different needs of all patients.

Objective:To investigate the psychological state of the patients from Wenchuan Earthquake treated in some local and military hospitals. Methods:We consulted the general data and pathogenetic condition of 116 patients from the earthquake-stricken area of Sichuan, studied their mental health using the Hamilton Anxiety Scale(HAMA) and investigated their psychological changes during the rescue and treatment with self-designed questionnaires. Results:The earthquake patients manifested obvious anxiety in the initial stage and their mental health was gradually improved towards the end of the rescue process. Conclusion:Wenchuan Earthquake caused great psychological trauma in the earthquake patients, for whom timely psychological intervention is of utmost importance.

Objective To explore the effect of phenol oxidase antigen on liver pathologic change in mice infected with Schistosoma japonicum. Methods 42d-aged adult worms were incubated in RPMI 1640 containing 0.05% sodium phenobarbital for 8 h. The worms were washed three times with PBS (pH 6.8) and homogenized with a Teflon pestle. The homogenate was then centrifuged at 3000 g for 20 min at 4 ℃. Supernatant fractions containing phenol oxidase (PO) were analyzed by means of polyacrylamide gel electrophoresis (PAGE). The rude antigen of PO was obtained by cutting the corresponding gel of PO activities. Three groups were set up to observe whether PO could induce protective immunity: experiment group, adjuvant control group and water control group. On day 42 post infection with (40?1) cercariae of Schistosoma japonicum, the mice were sacrificed to observe liver pathologic changes. Results The liver surface of PO immunized group was rather smooth and the liver color was slightly gray. A few pale nods were seen indistinctly but not clearly. Necrosis and inflammatory cell infiltration were not clear. There were many immature eggs without granuloma reaction. The mean diameter and area of the granuloma in the experiment group were less than those in the control group. There were significant differences among the 3 groups (P
