1.The diagnosis value of rheumatoid factor and anti-cyclic citrullinated peptide antibodies in patients with rheumatoid arthritis

Objective To evaluate the value of joint inspection of rheumatoid factor ( RF) and anti-cyclic citrullinated peptide antibodies(anti-CCP antibody) in rheumatoid arthritis( RA). Methods 125 patients with rheumatic disease were selected,including RA 78 cases,non-RA 47 cases. The quantitative ELISA to detect anti-CCP antibody, latex agglutination to test RF,and the data was analyzed. Results Anti-CCP antibody positive rate was 66. 7% ,RF was 74.4% Joint detection was 62.8% , single positive rate of joint detection was not statistically significant, but the specificity of joint detection was 98.6%,positive likelihood ratio was 29.2, significantly improved compared to individual testing;Sensitivity was 49.6%,negative predictive value was 54% ,significantly lower compared to test a-lone. Conclusion Combined detection was able to improve the specificity,positive likelihood ratio and reduce sensitivity , negative predictive value. It was more useful.
2.Probe into establishment of voluntary reporting system of nursing errors

Errors and flaws of clinical nursing practice can be fully exposed to voluntary reporting system of nursing errors, and then dramatically improving patient safety. This paper aims at exploring the theoretical basis, the characteristic of the system and essentiality for establishing voluntary reporting system of nursing errors, introducing the methods practicing both in abroad and in domestic, explaining its precondition and put forward a suggestion that the voluntary reporting system would be established in our own country.

5.Investigation in evaluation of various capacity requirement of probationer nursing students by nurses in operation room

Objective To investigate in evaluation of various capacity requirement of probationer nurs-ing students by nurses in operation room in order to improve clinical teaching work. Methods A self-de-signed questionnaire was applied, 191 qualified teaching nurses from nine grade three A general hospitals in Beijing were invited to fdl the questionaires. The results underwent subsequent analysis. Results The teach-ers had high expectations for various capacity of the probationer nursing students. Conclusions Clinical teachers should establish feasible teaching scheme according to the existing teaching syllabus in order to in-crease teaching quality, make students gain new insights through reviewing old materials, so that they can mas-ter various technique in operation room in short time.

7.The relationship among structural empowerment, nurses' job stress and burnout

Objective To investigate the status of structural empowerment,job stress and burnout,and to explore the relationship aunong them.Methods The questionnaires of CWEQ-Ⅱ,job stressors and MBI were used to investigate 350 nurses working at tertiary-level hospitals.Results The average score of CWEQ-Ⅱ was (2.23±0.59),the score of EE of MBI was (29.75±13.94),PA was (27.40±11.21),both of them showed a high level of exhaustion,DP was (8.07±5.82),which showed a middle level of exhaustion.The findings showed that workload and time pressure were the most frequently encountered job stressor among staff nurses,the score was (3.23±0.95).There was a significant correlation among structural empowerment,job stressors and the level of burnout.Hierarchical regression analysis and structural equation modeling showed that structural empowerment had significant influence on every factor of job stressors and burnout,job stressors had significant influence on the every factors of burnout.Conciusins It is suggested to pay attention to the main job stressors,and take proper interventions to enhance nurses' structural empowerment to prevent burnout,and thereby to raise the quality of care.
8.Ultrasound evaluation of left atrial function in patients with old myocardial infarction by strain rate imaging

Objective To evaluate the changes of the left atrial function in patients with old myocardial infarction(OMI) by strain rate imaging(SRI). Methods The velocity of the left atrial lateral wall and atrial septum was measured by SRI in 30 patients with OMI(OMI group) and 25 normal subjects (control group). Left atrium passive eject volume index(LAPEVl) and left atium active eject volume index (LAAEVI) were measured using Simpson method. Results ①Compared with the control group, the velocity of the left atrium was reduced obviously in ventricular systole and early ventricular diastole in OMI group(P<0.05) ,but was increased in late ventricular diastole(P<0.05). ②Compared with the controls, LAAEVI was increased significantly in OMI group, but LAPEVI was decreased signicantly(P <0.001). Conclusions SRI could be used to evaluate the left atrial function accurately in patiens with OMI.
9.Effect of mitofusin 2 gene with protein kinase A phosphorylation site deletion on the proliferation of vascular smooth muscle cells

BACKGROUND: The mitofusin 2 (Mfn2) may affects vascular smooth muscle cell Ras protein and suppress cellular proliferation through inhibition of extracellular signal-regulated protein kinase signaling pathway, which plays an important role in pathogenesis of vascular disorders such as hypertension, atherosclerosis and post-angioplasty restenosis. Mfn-2 gene amino acid sequence of the first 442 serine serves as protein kinase A (PKA) phosphorylation site, which is closely related to its phosphorylation status and may be involved in its functional regulation.OBJECTIVE: To investigate the effect of Mfn2 gene with PKA phosphorylation site deletion[Mfn2-PKA (△)] on inhibiting the proliferation of vascular smooth muscle cells and related signaling pathway.METHODS: Vascular smooth muscle cells of rats infected by recombinational adenovirus carrying green fluorescent protein,Mfn2 gane and Mfn2-PKA (△), were subcultured for 3-10 passages and randomly divided into 4 groups: ① Control group without intervention. ② Control group infected with adenovirus carrying green fluorescent protein. ③ Experiment group infected with adenovirus carrying Mfn-2 gene.④ Experiment group infected with adenovirus carrying Mfn2-PKA (△). Laser confocal microscopy was used to observe the locations of Mfn2 gene with and without PKA in cells. The expressions of extracallular signal-regulated protein kinase, Mfn2 gone and Mfn2-PKA (△) were determined by Western blot analysis. The growth curve of the vascular smooth muscle cells was explored by MTT.RESULTS AND CONCLUSION: The Mfn-2 and Mfn2-PKA (△) both expressed protein-specific bands in vascular smooth muscle cells. Two kinds of gone expression products were mainly located at the out membrane of mitochondria. Compared with the control group and adenovirus carrying green fluorescent protein group, the absorbance values at 3, 4, 5, 6 days were significantly reduced in adenovirus carrying Mfn2 group (P < 0.01), and no obvious changes were observed in adenovirus carrying Mfn2-PKA (△) group. Compared with the control group and adenovirus carrying green fluorescent protein group, the extracellular signal-regulated protein kinase expression was significantly reduced in adenovirus carrying Mfn2 group (P < 0.01), and no obvious changes were observed in adenovirus carrying Mfn2-PKA (△) group. Mfn2-PKA (△) located at the out membrane of mitochondria, has no effect on suppressing the proliferation of vascular smooth muscle cells, and no inhibition effect on extracellular signal-regulated protein kinase signaling pathway.
10.tMfn 2 gene induces apoptosis of vascular smooth muscle cells

BACKGROUND: Mitofusin2 (Mfn2) profoundly inhibits proliferation and induces apoptosis of vascular smooth muscle cells (VSMCs) through phosphatidylinositol 3 kinase/protein kinase B. Notably, tMfn2, with the transmembrane domain deleted, has a 41%-reduced molecular weight, which possibly exhibits a stronger effect on inducing apoptosis.OBJECTIVE: To investigate the effect of rat tMfn2 gene, with the transmembrane domain deleted, on promoting the apoptosis of VSMCs, and to determine related signal pathway.DESIGN, SETTING AND TIME: A controlled observational study at a gene level was performed in the central laboratory, Department of Cardiology, Tongji Hospital, Tongji Medical College, Huazhong University of Science and Technology between January and October in 2008.MATERIALS: Rat VSMCs and the recombinant adenovirus containing LacZ, Mfn2 or tMfn2 was offered by Professor Chen as a gift.METHODS: The rat VSMCs cultured at 3-10 passages were divided into 4 groups. ①Blank control group: No interventions. ② VSMCs were infected by adenovirus-mediated LacZ, Mfn2, and tMfn2, respectively.MAIN OUTCOME MEASURES: ①Expressions of Mfn2 and tMfn2 following the VSMCs were infected with recombinant adenovirus for 24 hours. ②The apoptosis of VSMCs was determined with flow cytometry and enzyme linked immunosorbent assay at 24, 48 and 72 hours following infection. ③Western blot was used to analyze the expression of phosphorylated AKT following the VSMCs were infected with recombinant adenovirus for 24 hours.RESULTS: ①Both Mfn2 and tMfn2 were expressed in the VSMCs. ②The tMfn2 was superior to Mfn2 in promoting the apoptosis of VSMCs in a time-dependent manner (P<0.01). ③The protein expression of phosphorylated AKT remarkably decreased two groups, especially significant in tMfn2-infected group (P<0,01).CONCLUSION: The tMfn2 can induce the apoptosis of VSMCs more effectively via the inhibition of phosphorylated AKT signaling pathway.
