1.Entry of antibiotic ointment into the anterior chamber following sutureless clear corneal incisions phacoemulsification:a case report

·AIM:To report a case of entry of antibiotic ointment into the anterior chamber following sutureless clear corneal incisions phacoemulsification.·METHODS:A 62-year-old man had uneventful phaco-emulsification with clear corneal incisions with intraocular lens(IOL) implantation. Postoperative medication included antibiotic/steroid ointment; the eye was firmly patched at the end of the procedure. On the first postoperative day, the patient was seen an oily droplet floating in the anterior chamber. Immediate surgical removal of the ointment without IOL exchange was performed.·RESULTS:During follow-up for six months after irrigation/aspiration of the anterior chamber and removal of the oily droplet, visual acuity and intraocular pressure (IOP) were unchanged and anterior segment was formed and quiet.·CONCLUSION:Ophthalmic ointment may ingress into the anterior chamber through unstable clear corneal incisions. The case highlights the importance of appropriate wound construction and integrity, as well as the risk of tight eye patching following placement of ointment.
2.Effects of procainamide on a canine model of electropharmacology as assessed by programmed electrical stimulation

A canine model of electro-physiologic-electropharmacologic testing on ischemic ventricular tachyarrhythmias were established in the open-chest dogs subjected to programmed electrical stimulation (PES) on 5~8days after acute my-ocardial infarction produced with two -stage occlusion of left anteriol decending coronary artery (LAD) followed by partly reperfusion. The electrophysiologic -electropharmacologic effects of pro-cainamide (PA ) were observed in this canine model. pA distinctly lengthened the QTc interval and the effective refractory period ( ERP) of normal and infarct myocardium in both ventricles and decreasedthe dispersion of ERP in infarct myocardium (IDR) as well as the dispersion of ERP in left ventricle (VDR). The PES - induced ventricular tachycardia (VT) or ventricular fibrillation (VF) was prevented in 5 out of 6 PA treated dogs (n = 6),Normal saline (NS) did not prevented PES- induced VT/VF. The results suggest that PA may be effective in preventing the onset of reentrant ventricular tachyarrhythmias after myocardium ischemic damage.
3.Analysis of curative effect of elder C type tibial plateau fractures patients with open reduction and internal fixation with bilateral plate

Objective To analyze the results of treatment of complicated tibial plateau fractures with open reduction and internal fixation with bilateral plates .Methods 29 elder patients with C type tibial platform fractures underwent internal fixation with bilat-eral plates and bone grafting .Fixation with bilateral plates was performed during operation with dynamic compression plate or 1/3 Tubular steel plate placed medially and the dissection plate or LISS system ones inserted laterally after the underlying separation . Meanwhile ,sufficient bone grafting during operation could effectively support reduction of articular surface .Results All patients were followed up of an average of 13 .8 ± 2 .43 months .According to Rasmussen score standard ,the excellent and good rate was 82 .7% .There was significant difference of TPA and PA between postoperation and postoperation one year later .Conclusion It is a simple and effective method to treat complicated elder tibial platform fractures with bilateral plates .It will improve the effect and re-duce complications significantly to perform sufficient bone grafting intraoperatively ,and make the drainage unobstructed and do the early functional exercises postoperatively .
5.Effects of atorvastatin on liver cystathionine-?-synthase of apoE~(-/-) mice

Objective To explore the influence of homocysteine(Hcy)on liver cystathionine-?-synthase(CBS)and methylenetetrahydrofolate reductase(MTHFR)system in apoE-/- mice,and determine the effects of atorvastatin and/or folate/vitamin B12 on liver CBS and MTHFR system.Methods Eighty male 6-week-old apoE-/- mice were randomly divided into two groups:65 mice were fed with a chow diet containing 2%(wt/vol)L-methionine(homomethionine group)and 15 mice were fed with normal saline(control group).Two months later,the 60 mice survived in homomethionine group were subdivided into four groups:group Ⅰ(untreated),Ⅱ(3 mg/kg atorvastatin),Ⅲ(3 mg/kg atorvastatin+2 mg/kg folate+30 ?g/kg vitamin B12)and Ⅳ(2 mg/kg folate+30 ?g/kg vitamin B12).After one month,Western blotting was performed to detect the liver CBS and MTHFR system protein expression in each group.Results The relative expression of liver CBS and MTHFR was significantly lower in group Ⅰ than in control group(P
6.Optimization of Fermentation Medium for Honggumycin Using Response Surface Methodology

Under the shaking-flask culture, fermentation medium of honggumycin produced by Streptomyces 702 were studied.The experiment was used response surface methodology to optimize the shaking-flask fermentation medium.Firstly, we applied full factorial design to screen important factors soybean meal and industrial peptone which affected hongmycin produced by Streptomyces 702.Furthermore, we designed experiment to obtain the steepest ascent path and optimal level by the central composite design.The optimum medium consisted of (g/L): maize starch 20, maize meal 20, glucose 20, soybean meal 23, industrial peptone 9, KNO_ 3 2.5, (NH_ 4 )_ 2 SO_ 4 2.5 KH_ 2 PO_ 4 0.3, NaCl 3, CaCO_ 3 6, bean oil 5mL/L.Under the optimal medium, the yield of honggumycin was up to 1500 g/mL, which was increased by 308% than the original medium.
7.Application and comparison of three diagnostic criteria for metabolic syndrome in Beijing population

Objective To compare application in Beijing population using the diagnostic criterias for metabolic syndrome(MS)according to the criterias of the International Diabetes Federation(IDF),Third Report of the National Cholesterol Education Program(NCEP)Expert Panel on Detection,Evaluation,and Treatment of High Blood Cholesterol in Adults[ATP Ⅲ(revised)]and Chinese Diabetes Society(CDS), respectively.Methods This study was based on an investigation on the risk factors of cardiovascular diseases involving 4 628 cases.MS was diagnosed according to these three definitions respectively and prevalence derived from these three definitions was compared.Results The prevalence of MS were 40.3 %, 44.3% and 35.7% according to the IDF,ATP Ⅲ(revised)and CDS respectively.The agreement in the diagnosis of MS using IDF and CDS,ATP Ⅲ(revised)and CDS,IDF and ATP Ⅲ(revised)were 81.9%, 83.7% and 96.0% respectively.There were 4.0% subjects among the MS patients diagnosed by ATP Ⅲ (revised)who can not be screened by IDF criteria.The prevalence of obesity was 19.6% according to ATP Ⅲ,much lower than the prevalence using CDS and IDF,whereas this rate increased to 58.6% using ATP Ⅲ(revised)criteria,showing diagnostic agreement with CDS.The coincidence rate between ATP Ⅲ (revised)and CDS was 80.6% Conclusion This investigation shows good agreement in the diagnosis of MS using IDF,CDS and ATP Ⅲ(revised)among Beijing population and these criterias can be used in China.

9.Construction of Prokarocyte Expression Recombinant of Human Thymosin α 1 and Its Expression in Escherichia coli

Objective: To acquire human thymosin α1(Tα 1) pep tides by gene engeering techniques.Methods: Tα1 gene was acquired b y molecular biological techniques and cloned into vector pGEMEX1-Tα1.An approp riate Escherichia coli (E.coli) strain was transformed by the recombinant pG EMEX1-Tα1 and its transcription and expression in E.coli we tested.Results: pUC18-Tα1,the cloning recombinant of human Tα1,was const ructed and confirmed by sequencing,and pGEMEX1-Tα1,the prokarocyte expression recombinant of human Tα1,was successfully constructed.Then it was confirmed tha t pGEMEX1-Tα1 could be transcribed in JM109 (DE3).A 36 kD fusion protein was p roduced after JM109 (DE3) transformed with pGEMEX1-Tα1 and induced with IPTG.Conclusion: The recombinant pGEMEX1-Tα1 can be successfully constr ucted and expressed in E.Coli.
10.The treatment of femoral shaft hypertrophic nonunions with exchange nailing versus augmentation plating

ObjectiveTo compare the outcomes and indications between exchange nailing (EN) and augmentation plating (AP) with a nail left in situ for femoral shaft hypertrophic nonunion after femoral nailing. MethodsFrom April 1998 to June 2009, 20 patients with femoral shaft hypertrophic nonunions after femoral nailing were treated with EN (11 patients) and AP (9 patients) respectively. There were no significant differences between the two groups with respect to the patient's age, gender, associated injuries, anatomical location and type of femoral fracture. Patients were evaluated by imaging and clinical function at 1, 2, 3, 4,6, and 12 months after surgery, and then every year postoperatively, to observe the callus and the recovery condition of the affected limb function. Fisher exact test and t'-test were performed to compare the outcome and complications respectively. ResultsAn unpaired t'-test showed no significant differences with respect to follow-up time, operating time, intraoperative blood loss, postoperative drainage, length of stay, time to radiographic union, time to clinical union, and AAOS score between the two groups. The cost of hospitalizations in the EN group was higher than in the AP group(t'=16.4, P=0.013). Four nonunions in the EN group failed to achieve union, which 3 patients were subsequently treated with AP and simultaneous autogenous bone grafting and 1 patient was treated with nail dynamization. All 9 hypertrophic nonunions in the AP group obtained osseous union. Fisher exact test showed a higher nonunion rate of EN compared with AP (χ2=6.01 ,P=0.008). ConclusionEN has been an excellent choice for aseptic isthmal femoral nonunion without a large bone defect subsequent to intramedullary fracture fixation, and AP can be an effective solution in cases of nonisthmal femoral nonunion and bone defect and failed exchange nailing.
