3.Effect of biomineralization on collagen-calcium phosphate composition and ultrastructure in artificial bone synthesis

BACKGROUND: Study on bone tissue-engineered material is one of the most successful fields in tissue engineering, but the mechanism on synthesis of artificial bone has not been known in many aspects.OBJECTIVE: To explore the mechanism of collagen and calcium phosphate (CP) in artificial bone synthesis.DESIGN: Single sample experiment was designed.SETTING: Material Research Room of Honghe University.MATERIALS: The experiment was performed in Material Research Room of Honghe University from July to August 2003. The materials included collagen (10 g/L acetic acid solution), calcium chloride, sodium dihydrogen phosphate (SDP), sodium hydroxide (NaOH), Tris, hydrochloric acid and deionized water (DI water).METHODS: Liquid nitrogen freezing and freeze-drying were used to prepare collagen-CP complexes A and B and the samples at different times during mineralization. UV spectrophotometer was used to determine the biomineralized dynamic curve of collagen-CP. Based on law of curve, the different times of sample collection were determined in preparation of electronic microscopic samples. According to electronic microscopic pictures and spectral data, mechanism analysis was carried on.MAIN OUTCOME MEASURES: Morphology of collagen-CP complex and law of its structure with time changeRESULTS: ①Under agitation, collagen-CP complex A was sheaf-like or needle-like in structure manufactured with retarded neutralization. ②Under static state, with biomineralization, collagen-CP complex B was in layered structure at initial phase of mineralization, which was similar to the self-assembled structure of pure collagen and the molarratio of C, O, P and Ca was 7.26: 20: 0: 2. At the end of mineralization, the structure was strip-like in high density with a certain grains and very fine rills and the molar ratio of C, O, P and Ca was 11.02: 22.5:1.06: 2.CONCLUSION: At the early phase of biomineralization, collagen iscoordinated initially with calcium ion, calcium-carrier layered collagen template is formed with the self-assembling of collagen, and then phosphates is combined with calcium ion to manufacture calcium phosphate in the formed template. By controlling agitation and acting time, collagen complex material of reticular and spinal structure is obtained.
4.Risk factor analysis of severe acute pancreatitis complicated by pancreatic encephalopathy

OBJECTIVE: To study the risk factors for severe acute pancreatitis (SAP) complicated by pancreatic encephalopathy (PE). METHODS: Clinical data from 255 patients with SAP from January 2005 to December 2006 were reviewed. Thirty-one SAP patients had PE, and 224 SAP patients did not. Clinical characteristics of SAP patients in both PE group and non-PE group were analyzed. RESULTS: Ranson scale and the incidence rates of acute respiratory distress syndrome (ARDS), renal failure, hypoproteinemia, hypocalcemia and hyperglycosemia in PE group were higher than those in non-PE group (P<0.05). There were no significant differences in acute physiology and chronic health evaluation II and CT severity index scales, the activities of amylase and lipase, the incidence rate of liver function failure, the infection rate and the operability between the PE group and the non-PE group (P>0.05). Multivariate logistic regression analysis showed that ARDS and hyperglycosemia were high risk factors. Cure rate in PE group was higher than that in non-PE group. CONCLUSION: Nosogenesis of PE is the result of multiple factors. ARDS and hyperglycosemia may be the high risk factors for PE.
5.Changes in expression of Ac-H3 and SIRT1 in dorsal root ganglions in a rat model of negative phenotype neuropathic pain

Objective To evaluate the changes in the expression of acetylated histone H3 (Ac-H3) and deacetylase silent information regulator 1 (SIRT1) in dorsal root ganglions in a rat model of negative phenotype neuropathic pain.Methods Forty-eight male Sprague-Dawley rats,weighing 250-300 g,were randomly divided into 2 groups (n =24 each):sham operation group (group S) and C-fiber dysfunction group (group CFD).The rats were anesthetized with 10% chloral hydrate 3 ml/kg.C-fiber dysfunction was induced by exposing sciatic nerve to 8% capsaicin for 30 min in group CFD.The thermal withdrawal latency (TWL) and mechanical withdrawal threshold (MWT) were measured before and on 1,3,7 and 14 days after CFD.Six rats were then sacrificed at each time and the lumbar segments (L5) of the dorsal root ganglions were removed for detection of SIRT1 mRNA expression (by RT-PCR) and Ac-H3 and SIRT1 protein expression (by Western blot).Results Compared with group S,TWL was significantly increased at 1,3,7 and 14 days after CFD,SIRT1 mRNA and protein expression was up-regulated and Ac-H3 expression was down-regulated at 3,7 and 14 days after CFD (P < 0.05),while no significant change was found in MWT at each time point in group CFD (P > 0.05).Conclusion The mechanism of negative phenotype neuropathic pain is related to up-regulation of deacetylase SIRT1 expression and decreased acetylation of histone H3 in rat dorsal root ganglions.
6.Application of problem based learning(PBL) in clicical practice of cardial pacemaker

To evaluate the effect of problem based learning(PBL)in clinical education of pacemaker implanting,30 postgraduates of clinical medicine were arranged into the PBL education group and 60 into the traditional education group.The results showed that the scores of theory and clinical skill of the PBL group were significantly better than those of the traditional education group. It also showed that PBL teaching method is more helpful and more effective than traditional education in clinical education of pacemaker installing.
7.Design and development of auxiliary measurement device for knee anterior cruciate ligament injury

Objective To design an auxiliary measurement device for anterior cruciate ligament injury in order to improve the accuracy of specialist examination and postoperative functional assessment.Methods The device consisted of a main frame and support pads,which was designed based on the principles of anatomopathology and mechanical mechanics.The frame was set above and paralleled to the tibia,and the measurement scale was put at the vertical direction of the frame.The support pads were fixed to the tibia and patella respectively to execute auxiliary measurement by providing opposite acting force.Results The success rate of preliminary diagnosis by the device was higher than that by traditional method.Conclusion The device gains advantages in convenience,practicality,low cost and etc,and is worthy promoting in the orthopedics department.

9.Ozone injection for the treatment of lumbar disc herniation: a therapeutic analysis of 104 cases

Objective To observe the therapeutic efficacy of ozone injection in treating lumbar disc herniation. Methods One hundred and four patients with CT or MRI proved lumbar disc herniation, including 144 diseased lumbar discs, were enrolled in this study. The main complains were severe pain or numbness in the low back and lower limbs. Under the X-ray guidance, a 21 G needle was punctured into the disc, followed by an injection of 4-40 ml ozone (50 ug/ml) and 40 mg prednisolone acetate into intradiscal and paravertebral space. Results All patients were followed up for 3 to 84 months with an average time of 38 months. The last follow-up check was carried out in March of 2009. The total effective rate was 77.1%, with no occurrence of any serious complications. Conclusion The percutaneous injection of medical ozone into disc and paravertebral space is an effective and safe method for the treatment of lumbar disc herniation.
10.A comparative study of intensity-modulated radiotherapy with concurrent PF or TP chemotherapy in patients with locoregionally advanced nasopharyngeal carcinoma

Objective To investigate the survival data and acute toxicities in patients with locoregionally advanced nasopharyngeal carcinoma who receive intensity-modulated radiotherapy (IMRT)with concurrent chemotherapy using nedaplatin plus 5-fluorouracil (PF) or taxol plus nedaplatin (TP).Methods A retrospective analysis was performed on the clinical data of 152 patients with stage Ⅲ or Ⅳa nasopharyngeal carcinoma who were admitted to our hospital in 2009-2010.Of the 152 patients,80 received IMRT with concurrent PF chemotherapy,and 72 received IMRT with concurrent TP chemotherapy;there were at least 2 cycles of concurrent chemotherapy in both groups.The Kaplan-Meier method was used to calculate the survival rates,and the log-rank test was used to analyze the survival difference ; the chisquare test was used to compare the acute toxicities in the two groups.Results The follow-up rate was 100%.The 2-year relapse-free survival rate,distant metastasis-free survival rate,progression-free survival rate,and disease-specific death rate for the IMRT/PF group were 95%,82%,81%,and 13%,respectively,versus 97%,83%,79%,and 12% for the IMRT/TP group (x2 =0.03,0.02,0.62,and 0.22,P=0.861,0.881,0.431,and 0.638).The incidence rates of leukopenia (grade ≥3),neutropenia (grade ≥ 3),thrombocytopenia (grade ≥ 3),ALT elevation (grade ≥ 2),and oral mucositis (grade ≥3) for the IMRT/PF group were 33%,23%,14%,8%,and 12%,respectively,versus 60%,47%,28%,18%,and 25% for the IMRT/TP group (x2 =11.33,10.29,4.59,3.94,and 3.94,P =0.001,0.001,0.032,0.047,and 0.047).Conclusions Compared with IMRT with concurrent PF chemotherapy,IMRT with concurrent TP chemotherapy does not lead to significantly better survival and results in more acute toxicities in the patients with locoregionally advanced nasopharyngeal carcinoma.
