1.Clinical Observation of 50 Cases of Intracranial Infection after Neurosurgery

Objective To explore the clinical therapeutic effects of postoperative intracranial infection in neurosurgery.Methods Between January 2014 and January 2014,50 patients with postoperative intracranial infection in the department of neurosurgery in our hospital were selected in this study.Among the 50 patients,25 patients were treated by intravenous antibiotic therapy,15 patients were treated with intrathecal and intravenous antibiotics treatment,10 cases were treated by intrathecal injection,intravenous antibiotics and lumbar pool catheter drainage way for treatment,and then we observed the clinical therapeutic effect of three groups of patients.Results In 50 patients,37 patients were cured,3 cases had marked effect,8 patients had effect,2 patients had no effect.Conclusion For patients with neurosurgical operations,to maximally reduce the risk of intracranial infection,it is necessary to take antibiotics according to the actual situation of patients,and make different treatment schemes according to the difference of infection.

3. Study on adsorption of ten injectable drugs by eight kinds of disposable filter membranes for infusion apparatus

OBJECTIVE: To investigate the absorbability of 8 kinds of disposable filter membranes for infusion apparatus to 10 injectable drugs, thus to provide information for clinical medication. METHODS: Each drug was separately dissolved in 250 mL 5% dextrose solution. Samples of drug solutions were collected before and after being filtered by the membranes. These samples were analyzed by HPLC-MS/MS methods. RESULTS: The 8 kinds of membranes had different adsorbabilities to the 10 injectable drugs. CN-CA membrane (CN-CA) and CN-CA Enhance membrane (CN-CA-E) adsorbed more drugs than other membranes (P<0.05). The average adsorption of PES membrane, NL-B membrane and ION membrane to the 10 drugs were lower than 1%. PP membrane and NL membrane to the 10 drugs were lower than 1.5%. CONCLUSION: PES membrane, ION membrane and NL-B membrane have lower adsorbabilityies than other membranes, thus may have little influence on medical treatment. 5.Experimental study on phosphatidylinositol 3 kinase signal transduction pathway in Barrett esophagus genesis

Objective To explore the relationship between phosphatidylinositol 3 kinase (PI3K)/protein kinase B (Akt)/mammals target protein rapamycin (mTOR) signal transduction pathway and Barrett esophagus genesis.Methods A total of 140 rats were divided into sham operated group (n=10),iron group (n =10),esophageal duodenal side anastomosis group (n =30),esophageal duodenal side anastomosis plus iron group (n-30),esophageal gastric duodenal side anastomosis group (n=30) and esophageal gastric duodenal anastomosis plus iron group (n=30).In the end,10 normal esophagus tissue specimens,62 reflux esophagitis tissue specimens and 34 Barrett's esophagus tissue specimens were obtained.The expression levels of epidermal growth factor receptor (EGFR),Akt,phospho Akt (p-Akt),phospho-mTOR (p-mTOR) protein were detected by immunohistochemistry.Single factor analysis of variance,SNK between two groups and nonparametric correlation analysis were performed for statistical analysis.Results The protein expression levels of EGFR,Akt,p-Akt,p-mTOR in Barret(s esophagus tissues were higher than those in reflux esophagitis tissues and normal esophagus tissues (EGFR 0.1799±0.0367 vs 0.0438±0.0025 and 0.0277±0.0069,q=6.79,4.13; Akt 0.1874±0.0250 vs 0.0986±0.0093 and 0.0383±0.0048,q=6.51,3.56; p-Akt 0.1418±0.0130 vs 0.0592±0.0027 and 0.0281 ±0.0017,q=7.68,3.99; p-mTOR 0.1591±0.0275 vs 0.0674 ±0.0059 and 0.0112±0.0017,q=5.62,4.11; all P<0.05).The protein expression levels of EGFR,Akt,p-Akt,and p-mTORin reflux esophagitis tissues were higher than those in normal esophagus tissues(q=4.67,4.29,4.27,4.03; all P<0.05).Conclusion PI3K/Akt/mTOR signal transduction pathway were activated in reflux esophagitis and Barrett's esophagus,which provided theoretical basis for clinical multi-target treatment for diseases.
6.Comparison of endoscopic variceal ligation and pericardial devascularization in treatment of portal hypertension

Objective To compare the clinical efficacy of endoscopic variceal ligation versus pericardial devascularization in the treatment of portal hypertension.Methods The clinical data of 101 cirrhotic patients with gastroesophageal varices and variceal hemorrhage from January 2010 to January 2012 were analyzed.Fifty-three patients received endoscopic variceal ligation,and forty-eight patients received pericardial devascularization.Postoperative changes in liver function and hypersplenism were compared between the two groups.The rate of rehaemorrhagia and incidence of postoperative complications after surgery were compared as well.Continuous data were expressed as mean ±SD,and categori-cal data were expressed as number of cases or percentage.Comparison of continuous data between the two groups was made by independent-samples t test,and comparison of categorical data was made by chi-square test.Results After surgery,the variceal ligation group showed no significant changes in liver reserve function,while the albumin level was significantly decreased in the pericardial devascularization group (t=2.512,P<0.05).There were no significant changes in the counts of white blood cells and platelets in the endoscopic variceal ligation group after surgery (P>0.05),but significant increases in the counts of white blood cells and platelets were detected in the pericardial devascular-ization group (P<0.05).The rates of postoperative haemorrhage in the two groups were:3 months,7.5%vs 6.2%(χ2 =0.066,P>0.05), 6 months,7.5%vs 8.3%(χ2 =0.021,P>0.05),and 1 year,9.4%vs 8.3%(χ2 =0.038,P>0.05).The incidence rates of postopera-tive complications in the two groups were 24.5%and 50%,respectively (χ2 =7.040,P<0.05).Conclusion Compared with pericardial devascularization,endoscopic variceal ligation causes fewer microlesions,preserves liver function,and leads to a lower incidence of postopera-tive complications.However,if hypersplenism is observed in the cirrhotic patients with gastroesophageal varices and variceal hemorrhage,peri-cardial devascularization can be used to control the hypersplenism and prevent esophageal hemorrhage.
7.Protective effect of total flavones of buckwheat flowers and leaves on myocardial fibrosis of type 2 diabetic rats and its mechanism

Objective: To study the protective effect of the total flavones of buckwheat flowers and leaves (TFBFL)on the myocardial injury of the type 2 diabetic rats,and to clarify its mechanism.Methods:The diabetic rat models were established by intraperitoneally injecting streptozotocin (STZ)and taking high-fat diet.The model rats were divided into diabetic model (DM)group and TFBFL treatment (TFBFL)group,while normal control group was set up, 10 rats per group.The rats in TFBFL group were administered with 200 mL· kg-1 · d-1 TFBFL,while the rats in other two groups were given with normal water (10 mL·kg-1 ·d-1 )instead.The rats in various groups was administered once a day for 8 weeks.The body weight (BW),heart weight index (HWI), level of fasting blood glucose (FBG)and cardiac function indexes (HR,LVSP,LVEDP and ± dp/dtmax )of the rats in various groups were measured.The myocardial tissue morphology of the rats in various groups was evaluated by electron microscope and the collagen levels in myocardium tissue of the rats in various groups were observed by Masson staining.The expression levels of TGF-β1 protein in myocardium tissue of the rats in various groups were detected by Western blotting method.Results:Compared with normal control group,the HWI,FBG and LVEDP levels of the rats in DM group were increased (P <0.05 or P <0.01),and the BW, HR,LVSP and ±dp/dtmax level in DM group were decreased (P <0.05 or P <0.01).Compared with DM group,the FBG and LVEDP level in TFBFL group were decreased (P < 0.01 ),and the HR,LVSP and ± dp/dtmax level in TFBFL group were increased (P < 0.05 or P < 0.01 ); but the BW and HWI level in TFBFL group had no significant difference compared with DM group (P >0.05).The Masson staining results showed the morphology of myocardium tissue of the rats in TFBFL group was improved and the collagen level was decreased. The Western blotting results suggested that the TGF-β1 protein expression level in myocardium tissue of the rats in DM group was significantly increased compared with normal control group (P < 0.05),and the TGF-β1 protein expression level of the rats in TFBFL group was decreased compared with DM group (P <0.05).Conclusion:TFBFL exerts a certain protective effect on the myocardial fibrosis of diabetic rats through inhibiting the TGF-β1 expression in myocardium tissue.
8.Effect of medical skin collagen regeneration functional dressing on facial dermatitis

Objective To observe and evaluate the healing effect of medical skin collagen regeneration func-tional dressing on facial dermatitis.Methods 40 facial dermatitis patients who met the inclusion criteria were ran-domly divided into the observation group(group A)and control group(group B)averagely.Patients of group A were administrated with ketotifen tablets orally and with medical skin collagen regeneration functional dressing externally,while patients of group B were only orally administrated with ketotifen tablets.The total observation period last 15 days with a follow -up once after 2 days for both of the groups.Results The cure rates of group A after 12 day and 15 day were 63.2% and 65%,higher than the cure rates of group B(5.0% and 5.0%).The 12 day and 15 day cure rates of group A were significantly higher than those of group B(χ2 =14.83,15.82,all P <0.01).Conclusion Medical skin collagen regeneration dressing in combination with fexofenadine tablets provide a safe and effective healing func-tion to facial dermatitis.
9.Autologous bone marrow stem cell transplantation for thromboangiitis obliterans:5-year follow-up

BACKGROUND:The assessment for long-term efficacy of chronic ischemic disease is more important than the short-term efficacy assessment, which associates with patient’s long-term quality of life and long-term survival rate. OBJECTIVE:To observe the 5-year folow-up outcomes of autologous bone marrow stem cel transplantation for the treatment of thromboangitis obliterans. METHODS:This study enroled 43 patients of thromboangitis obliterans who underwent autologous bone marrow stem cel transplantation from August 2007 to January 2010 in the Department of Thyroid Vascular Surgery, the First Affiliated Hospital of Xinjiang Medical University. At 1, 2, 3, 4 and 5 years after transplantation, pain, cold sensation, and intermittent claudication distance were folowed up by telephone; changes in limb ulcers were observed. At 1 year after transplantation, venous oxygen partial pressure and oxygen saturation of limbs were reviewed. RESULTS AND CONCLUSION:A total of 38 thromboangitis obliterans patients with complete folow-up data were included in the final analysis. Compared to the preoperation, pain, cold sensation, and intermittent claudication significantly improved. The difference was statisticaly significant (Z values:-4.277,-5.086,-3.574, P < 0.001). Compared with 1-5 years after operation, pain and cold sensation had no statisticaly difference (P >0.05). Intermittent claudication distance had increased. Differences in terms of intermittent claudication distance was statisticaly significant (Z=43.898,P < 0.001). Significant differences in venous oxygen partial pressure and oxygen saturation were detected between preoperation and 1-year posttransplantation (tvalues: 36.790, 43.964,P values: 0.040, 0.037). Above results suggest that autologous bone marrow stem cel transplantation for thromboangitis obliterans obtained stable long-term outcomes.
10.Analysis of incidental prostatic carcinoma in 42 cases after transurethral plasmakinetic enucleation of prostate

Objective To investigate the correlation of clinical stage and treatment plan with prognosis in incidental prostatic carcinoma.Methods From 2004 to 2010,a total of 1076 patients diagnosed as BPH underwent transurethral plasma kinetic enucleation of prostate (TUPKEP) in our hospital,and their clinical data were respectively analyzed.Among them,42 cases were found to have incidental prostatic carcinoma.The correlation of clinical stage and treatment plan with prognosis in incidental prostatic carcinoma were observed.Results Among 1076 BPH patients undergoing TUPKEP operation,42 cases (3.9%) aged 56-88 years were found to have incidental prostatic carcinoma,which all displayed as prostatic adenocarcinoma including 18 cases at T1 a stage and 24 cases at T1b stage.10 cases received endocrinal therapy,14 cases received bilateral orchiectomy,14 cases underwent radical prostectomy and 4 cases were treated with watchful waiting.Patients were followed up for 24 to 96 months.The survival rates were 100.0% and the rates of progression were 0.0%,20.0%,0.0% in T1 a patients received watchful waiting,androgen-deprivation therapy and radical prostectomy,respectively.Total survival rate was 100.0% and total rate of progression was 11.1% in T1a patients.The survival rates were 71.4%,100.0% and the rates of progression were 42.9%,0.0% in T1 b patients received androgen-deprivation therapy and radical prostectomy,respectively.Total survival rate was 83.3 % and total rate of progression was 25.0 % in T1 b patients.Compared with T1 b patients,the total survival rate was higher (x2=18.19,P<0.01) and the rate of progression was lower in T1 a patients (x2 =6.52,P<0.05).Conclusions The survival rate in T1 a patients accepted androgen-deprivation therapy is similar to that in T1 a patients with watchful waiting.Compared with T1a patients,the survival rate is lower but the rate of progression is higher in T1 b patients received active androgen-deprivation therapy.The survival rate is higher in T1 b patients treated with radical prostectomy than in those received other treatments.Watchful waiting is acceptable for T1 a patients.T1 b patients should be treated with radical prostectomy for a better effect.
