1.THE INFLUENCE OF JINLANBAO ON IMMUNOLOGICAL FUNCTION IN MICE

JINLANBAO (JLB), a preparation extracted from the Pecten Argopecten irradiaus and some Chinese medicinal herbs,has been reported to have several immuno-mod-ulatory effects in mice In this paper,the influence of JLB on the immuivlogcai functions in mice is presented. The results showed that JLB could enhance the phagocytosis of intraperi-toneal macrophages (Mo) inhibited by methyl-prednisone,increase the IgM level in CTX-de-pressed cells in a dose-dependent manner in the medium-dose and high-dose group. JLB has been found to have a two-way regulatory effect on ConA-induced T-lyrophocytes prolifera tion, tea promoting effect in low dose group,and an inhibiting effect in high dose group. As an immunopotentiator, the potential values and prospects for the developing of JLB should be paid attention to in pharmaceutical sciences.
2.Histochemical study on the change in lymphatic motion and nitric oxide synthase in the early phase of acute rat endotoxemia

AIM:To study the changes in lymphatic motion and histochemical stain of nitric oxide synthase(NOS) in the early phase of acute rat endotoxemia and explore their relationship. METHODS: After lipopolysaccharide (LPS) was injected into femoral vein, rats' mesenteric lymphatic were inspected within two hours. Paraffin section was stained by HE, frozen section by NADPH diaphorase histochemistry. RESULTS: In LPS group, the mesenteric lymphatic diameter was greater than that in the control and contractile frequency was decreased, the villi central lacteal and submucosal microlymphatics of small intestine was extremely dilated under microscope. In LPS and control group, NOS was positive in lymphatic endothelia and the nerve fibers in the vessels' wall. CONCLUSION: In the early stage of acute endotoxemia, the lymphatic vessles dilated and contractile activity diminished, NOS may play an important role in these changes.
3.Detection of chromosome aberration rate and micronucleus rate in patients with esophageal carcinoma before and after radiotherapy and clinical significances

Objective To study the change of chromosome aberration rate and micronucleus rate in patients with esophageal carcinoma before and after radiotherapy.Methods Twenty one patients with esophageal carcinoma who were fit for conditions were divided into two groups randomly.GroupⅠ(n=10),patients received conventional radiotherapy(radiation field was regular rectangular shape);groupⅡ(n=11),patients received conformal radiotherapy(radiation field was same to the shape of target volume),1.8-2.0 Gy,one time a day,5 times a week,the total dose was 60-70 Gy.The peripheral blood was obtained before and after radiotherapy,chromosome aberration rate and micronucleus rate were detected.Results The chromosome aberration rate and micronucleus rate were increased after conventional radiotherapy than before (P0.05).Conclusion Compared with the conventional radiotherapy,the conformal radiotherapy can decrease cytogenetics damage,the detection of chromosome aberration rate and micronucleus rate can be regarded as a simple method to evaluate cytogenetics damage in esophageal carcinoma patients induced by radiotherapy.
4.Expression of Annexin Ⅴ in hepatitis B infected placentas detected by double-labeled immunofluorenscence assay

BACKGROUND: Recently, one focus of research has been Annexin Ⅴ (AnV) existing on hepatic cells membranes as a fundamental receptor related to hepatitis B virus (HBV) infection. Also its expression in placental tissues has been a matter of debate. The study of the relationships between placental cells infected with HBV and their AnV expression will be of great value in future prevention strategies and treatments.OBJECTIVE: To investigate the presence of AnV in HBV infected human's placental cells and its potential role in HBV intrauterine transmission.DESIGN: Randomized controlled experiment.SETTING: Taishan Medical College.MATERIALS: Placental tissue was collected from HBsAg positive full term pregnant women (30 cases) admitted to Jinan Institute for Maternal and Child Health, Taian Central Hospital and Taian Institute for Maternal and Child Health from January 2003 to December 2004. Maternal serum was also obtained. Informed consents for participating in this study were obtained from all the involved pregnant women and this experiment was approved by the Hospital Ethics Committee. Rabbit-anti-human AnV purified affinity antibody (first antibody), rat-anti-human HBs mAb (first antibody),and biotinylated goat-anti-mouse IgG (secondary antibody) were supplied by Wuhan Boster Bioengineering Company.METHODS: Using SABC immunohistochemical staining reagent, 18 HBsAg positive placentas were obtained from 30HBsAg infected patients in full term pregnancy. These were considered as the positive group and the other 12 were used as negative controls. The staining process included dewaxing, dehydration of embedded slides and microwave antigen restoration. In the wet box, rabbit-anti-human AnV purified antibody (first antibody, 1:60, monoclonal antibody)was added on the slides and kept at 4 ℃ overnight. Rat-anti-human antibody HBs mAb(secondary antibody, 1:50) was added and kept at 4 ℃ ovemight, after this procedure, biotinylated goat-anti-mouse IgG(1:100), the first fluorescent antibody such as FITC-goat anti-rabbit IgG (1:50) and the second fluorescent antibody (Avidin-Cy3) were used,respectively. The slides were sealed with buffered glycerol and examined under a confocal laser scanning microscope.The images on the slides were analyzed with IPP 4.5 image programs.MAIN OUTCOME MEASURES: Detecting the simultaneous existence and distribution of HBsAg/AnV in placental cells with HBV infection.RESULTS: Ten cases from the positive group were simultaneously detected for HBsAg/AnV by double-labeled immunofluorenscence assay and confocal laser scanning microscope. AnV expression was detected in the trophoblastic, interstitial cells and vascular endothelial cells of villi interstitial blood vessels, and the coexistence of HBsAg/AnV was found even in one cell.CONCLUSION: HBsAg combined with the receptor AnV in the same placental cells is a common finding in HBV infected full term pregnant women. This finding is very suggestive of a mechanism where AnV could promote hepatitis B virus to enter the placental cells and cause intrauterine infection.
5.The influence of hemoperfusion on restless legs syndrome and sleep quality in elderly maintenance hemodialysis patients

ObjectiveTo explore the prevalence and influencing factors of restless legs syndrome (RLS) in elderly maintenance hemodialysis (MHD) patients, to analyze the relationship between RLS and sleep quality, and to observe the efficacy and safety of hemoperfusion in improving RLS and sleep quality in elderly MHD patients with RLS. MethodsA total of 118 cases with maintenance bemodialysis were involved in this study. The RLS was diagnosed according to the criteria of the International Restless Legs Syndrome Study Group (IRLSSG). The subjects' quality of sleep was assessed by the Pittsburgh Sleep Quality Index (PSQI). The elderly MHD patients with RLS received hemoperfusion once a week for 3 months. The clinical and biochemical indexes were collected including age, gender, duration of hemodialysis, pre-dialysis blood concentrations of creatinine, hemoglobin, parathyroid hormone, β2-microglobulin, etc. T test, X2 test and binary logistic regression analysis were performed. ResultsRLS prevalence was found in 26.27% (11 men, 20 women, the ratio was 0. 55 : 1) of 118 elderly MHD patients. The duration of hemodialysis,sex ratio (female/male), the level of serum phosphorus, β2-microglobulin and parathyroid hormone in elderly MHD patients with RLS were significantly higher than those in elderly MHD patients withoutRLS (t=2. 332,X2=15. 343,t=4. 784,13. 124,10. 900;all P<0. 05). And there were no significant differences in age, dry weight, material of dialyzer membrane, and levels of serum creatinine, urea nitrogen, hemoglobin, hematocrit, calcium, ferritin and transferrin saturation between patients with RLS and without RLS(t=0. 341,0. 008, 1. 055,0. 051,0. 899,0. 912,0. 601,0. 192,0. 200,1. 094;all P>0.05). Logistic regression analysis revealed that serum parathyroid hormone and β2-microglobulinwere the important risk factors for RLS in elderly MHD patients. PSQI was higher in RLS group than in non-RLS group[(10. 7±2.7) vs. (4.9±2.5), t=10. 948,P=0. 000] . The manifestations of poor sleep were poor indexes of subjective sleep quality, sleep duration, habitual sleep efficiency and daytime dysfunction in MHD patients with RLS. And the RLS severity was correlated with PSQI (r=0. 839,P<0. 05). After the treatment with hemoperfusion, the level of serum phosphorus, serum parathyroid hormone, serum β2-microglobulin, RLS severity score and PSQI were significantly decreased in RLS patients compared with pre-hemoperfusion. The vital sign of RLS patients were stable during hemoperfusion, and there were no adverse events after the treatment with hemoperfusion. ConclusionsThe prevalence of RLS is high, and poor sleep quality is found in elderly MHD patients with RLS. Serum parathyroid hormone and β2-microglobulin are the important risk factors for RLS in elderly MHD patients. Hemoperfusion can effectively improve the RLS and sleep quality and is well tolerated.
6.Expression and promoter methylation of Kras gene in thymic lymphomas induced by ionizing radiation

Objective To study the changes of mRNA and protein expressions of Kras gene in thymic lymphomas induced by ionizing radiation,and to detect the methylation of CpG islands in promoter region of Kras gene,then to investigate the mechanisms for the occurrence of radiation carcinogenesis.Methods The thymic lymphoma models of BALB/c mice were made by X-ray irradiation,then the total RNA was extracted,cDNA was synthesized and the total protein was extracted from both thymic lymphoma tissue and normal thymus tissue;the mRNA and protein expressions of Kras gene in thymic lymphoma tissue and normal thymus tissue were detected by RT-PCR and Western blotting method, and the methylation of CpG islands in promoter region of Kras gene was detected by bisulfite sequencing PCR. Results The mRNA expression level of Kras gene in thymic lymphoma tissue was significantly higher than that in normal thymus tissue(P<0.01).The protein expression level in thymic lymphoma tissue was about 1.41 times higher than that in normal thymus tissue;4 CpG sites were methylated detected by bisulfite sequencing PCR in normal thymus tissue, however, 1 CpG site was methylated in thymic lymphoma tissue,the CpG islands in promoter region of Kras gene were demethylation state in thymic lymphoma. Conclusion Ionizing radiation can cause the changes of mRNA and protein expression levels of Kras gene in thymic lymphoma tissue by demethylation state of Kras gene,eventually lead to the occurrence of tumor;it might be one of the mechanisms for the occurrence of radiation carcinogenesis.
7.Cannabinoid receptor 1 regulates the spatial learning and memory function and the expression of NR1 subunit of N-methyl-D-aspartic acid receptor in medial prefrontal cortex of neuropathic pain model rat

Objective To investigate the effect of cannabinoid receptor 1 ( CBR1 ) on spatial learning and memory function of neuropathic pain ( NP ) model rats and the expression of N-methyl-D-aspartic acid receptor 1(NR1) subunit in medial prefrontal cortex (mPFC).Methods Thirty-six healthy male Wistar rats were randomly divided into 4 groups, with 9 rats in each group: the sham operated group (SO group), the neuropathic pain model group (NP group), the NP model group with an mPFC injection of saline ( NS group ) , and the NP model group with an mPFC injection of the CBR 1 antagonist AM251 ( AM251 group).The NP model was prepared using the operation of chronic constriction injury ( CCI) of the right sciatic nerve.The mechanical withdrawal threshold ( MWT ) and the thermal withdrawal latency (TWL) of the rats in each group were detected at 3, 7, 14, 21 and 28 days after the operation.At 29 days after the operation , 18 rats of NP model were randomly selected and given an mPFC injection of saline or AM251 using a three-dimensional brain puncture.At days 30-37 after operation , the eight-arm maze test was performed to detect the spatial learning and memory function of the rats , and the rats were sacrificed immediately after this test.The expression levels of CBR1, NR1 and phosphorylated-N-methyl-D-aspartic acid receptor 1 ( p-NR1 ) ( Ser896 ) in the mPFC brain region were detected by Western blotting , RT-PCR and immunofluorescence.Results Compared with the SO group , the pain thresholds and the spatial learning and memory function of the rats in the NP group were significantly lower ( both P <0.05 ).Compared with the NS group , the rats in the AM251 group showed improvement about spatial learning and memory function ( P<0.05).Compared with the SO group ( the mRNA and protein level of CBR 1:0.23 ± 0.06,0.42 ±0.03), the mRNA(0.43 ±0.12) and protein (0.53 ±0.05) level of CBR1 in NP group increased (both P<0.05).Compared with the NS group (the mRNA and protein level of CBR1:0.42 ± 0.11,0.52 ±0.10), the mRNA (0.53 ±0.05) and protein (0.98 ±0.17) level of CBR1 in AM251 group increased (both P<0.05).Compared with the SO group (the mRNA and protein level of NR1 and the protein level of p-NR1:1.50 ±0.15,0.65 ±0.05,0.79 ±0.15), the mRNA (0.94 ±0.07) and protein (0.24 ±0.05) level of NR1 in NP group decreased (both P<0.05), the protein level of p-NR1 (0.33 ± 0.04) decreased (P<0.05).Compared with the NS group (the mRNA and protein level of NR1 and the protein level of p-NR1:1.09 ±0.14,0.26 ±0.06,0.31 ±0.08), the mRNA(1.58 ±0.10) and protein (1.42 ±0.10) level of NR1 in AM251 group increased (both P<0.05), the protein (0.95 ±0.15) level of p-NR1 increased ( P<0.05).Conclusion CBR1 can decrease the expression level of NR 1 and p-NR1 in the mPFC brain region of NP model rats and induce the spatial learning and memory impairment.
8.A prospective comparison cohort study between baseline serum uric acid level and coronary artery disease in first-degree relatives and non-first-degree relatives of type 2 diabetes males

Objective To investigate the relationship between baseline serum uric acid and the severity of coronary artery disease ( CAD ) in the first-degree relatives or non-first-degree relatives of men with type 2 diabetes. Methods Three hundred and eighty-one men with negative coronary angiography for the first time were divided into diabetes and non-diabetes groups and followed-up for 5 years. The primary outcome was acute coronary syndrome suspected during subsequent 5 years, and the coronary angiography was conducted simultaneously. The severity of CAD was assessed by the coronary stenosis index ( CSI) and the number of coronary lesion vessels. Results In normal blood glucose group, serum uric acid was higher in the first-degree relatives of diabetics compared with non-first-degree relatives(P<0. 01), along with higher morbidity of CAD, CSI, and coronary lesion vessels (all P<0.01). Correlation analysis showed that CSI(r=0. 250, P=0. 041) and coronary lesion vessels(r=0. 252, P=0. 040) in non-diabetics group were associated with baseline levels of serum uric acid. Conclusion The elevation of serum uric acid was closely related to subsequent CAD, especially in first-degree relatives of male with type 2 diabetes, which could be used as an early indicator for CAD prediction.

Based on the International Federation of Nursing Anesthetists (IFNA) education and training base, our hospital has built a training base for anesthesia specialized nurses in Nanjing from the following aspects: the application for training base of anesthesia specialized nurses, the qualification and examination of students' and teachers' qualification, the settings of training curriculum, the examination contents and methods, and the evaluation of post-training effect. This article summarizes the construction experience of this base, therefore, providing support and standard for the training of anesthesia specialized nurses.
