2.Clinical observation of diabetic foot ulcer treated with Shuxuetong plus external application of deproteinised ;calf blood serum injection

Objective To evaluate the clinical effect of Shuxuetong plus external application of deproteinised calf blood serum injection on diabetic ulcer foot.Methods 60 patients with diabetic foot ulcer were enrolled:30 cases (treatment group)were randomly assigned to receive deproteinised calf blood serum injection(external use)plus Shuxuetong(iv gtt),and the other 30 cases (control group)received routine western medicine therapy.One course of treatment lasted one month.In addition,both two groups received insulin to control blood sugar and antibiotics to control infection,with foot ulcer treated with debridement,drainage,and removal of necrotic tissue etc.Results Com-pared with the control group,the effective rate was higher in the treatment group(86.7% vs.60.0%,χ2 =5.455,P =0.020).The occurrence time of granulation tissue[(6.60 ±1.14)d vs.(10.80 ±2.17)d,t =-3.834,P =0.008] and wound healing time[(23.62 ±6.14)d vs.(35.65 ±4.45)d,t =-4.405,P =0.002]were shorter in the treat-ment group.The wound area in the two groups reduced after treatment,and the wound area in the treatment group reduced significantly[(3.48 ±1.16)cm2 vs.(8.60 ±2.51)cm2 ,t =-4.143,P =0.007].Conclusion As com-pared with the routine therapy with western medicine,the combination therapy of deproteinised calf blood serum injec-tion plus Shuxuetong has better efficacy for diabetic ulcer foot.
3.OBSERVATION ON MORPHOLOGICAL TYPES OF THE ARTERIOVENOUS ANASTOMOSES AND THE BEHAVIOUR OF CUTANEOUS ARTERIOVENOUS ANASTOMOSES AFTER THERMAL STIMULATION OF THE INTRAVERTEBRAL CANAL

Using inverted microscopy and circuit TV, the morphological type, diameter of arteriovenous anastomoses (AVAs) in 11 rabbit's ear chambers were observed. According the joining forms with arterioles and venules in 68 AVAs, there were 4 types: type Ⅰ 58.8%, type Ⅱ 19.4%, type Ⅲ 11.1%, type ⅣV 10.7%. The inner diameters of the AVAs when dialated, measured 36.6?1.67?m at their arterial ends, 16+0.6?m at their narrow points, 66?4?m at their venous ends.This study showed that dilation of AVAs in the rabbit's ear is a graded response which correlates with the rise in intravertebral canal temperature. Some AVAs have multiple venous segments which vary in diameters. It was observed that the behaviour were different after intravertebral canal warming.
4.Content Determination of Metacresol by GC

Objective:To establish a GC method for the determination of metacresol. Methods:The separation was performed on an Agilent DB-225 MS column (30 m × 0. 25 mm,0. 25 μm). The initial column temperature was set at 90℃,maintained 10 min, raised to120℃ with a rate of 2℃·min-1 ,and then raised to 150℃ with a rate of 10℃·min-1 and maintained for 5 min. The inlet temperature was 200℃. The detector was a flame ionization detector (FID) and the temperature was 250℃. The flow rate was 1. 8 ml ·min-1 split ratio was 1∶30 and the carrier gas was nitrogen. Results:The linear range was 0. 6-1. 8 mg·ml-1 . The average recovery was 96. 0% (RSD=2. 6%,n=9). The content of 3 batches of the samples was 99. 3%, 98. 7% and 98. 4%, respectively. Conclu-sion:The established quantitative analysis method is applicable in the quality control of metacresol.
5.Histochemical study on the change in lymphatic motion and nitric oxide synthase in the early phase of acute rat endotoxemia

AIM:To study the changes in lymphatic motion and histochemical stain of nitric oxide synthase(NOS) in the early phase of acute rat endotoxemia and explore their relationship. METHODS: After lipopolysaccharide (LPS) was injected into femoral vein, rats' mesenteric lymphatic were inspected within two hours. Paraffin section was stained by HE, frozen section by NADPH diaphorase histochemistry. RESULTS: In LPS group, the mesenteric lymphatic diameter was greater than that in the control and contractile frequency was decreased, the villi central lacteal and submucosal microlymphatics of small intestine was extremely dilated under microscope. In LPS and control group, NOS was positive in lymphatic endothelia and the nerve fibers in the vessels' wall. CONCLUSION: In the early stage of acute endotoxemia, the lymphatic vessles dilated and contractile activity diminished, NOS may play an important role in these changes.
7.Impact of colorectal cancer laparoscopic and open surgery on immune function

Objective:To analyze the impact of colorectal cancer laparoscopic and open surgery on immune function.Methods:85 patients from January 2010 to December 2013 due to surgical treatment of colorectal cancer hospitalized in our department were randomly divided into experimental and control groups.The experimental group underwent laparoscopic surgery, the control group underwent laparotomy.The peripheral blood lymphocyte subsets, immunoglobulins, CRP and IL-6 levels, NO and ROS levels were compared in two groups of patients before and after one day,seven days of surgery.Results: The postoperative CD3,CD4,CD4/CD8 level after one day and seven days of surgery was significantly lower than before surgery, CD8 was significantly higher than before surgery,P<0.05.The postoperative CD3, CD4, CD4/CD8 level after one day and seven days in the experimental group were significantly higher,CD8 was significantly lower than the control group,P<0.05.The postoperative IL-6,CRP levels after one day and seven days of surgery were significantly higher than before surgery,IgG,IgM and IgA were significantly lower than before surgery,P<0.05.The postoperative IL-6,CRP levels after one day and seven days of surgery in the experimental group were significantly lower than the control group,IgG,IgM and IgA were significantly higher,P<0.05.The NO and ROS levels after one day and seven days of surgery in the control group were significantly higher than the preoperative and postoperative period the experimental group,P<0.05.The NO and ROS levels after one day and seven days of surgery in the experimental group were no significant difference,P>0.05.Conclusion:Compared to open surgery,laparoscopic treatment of colorectal cancer has a protective and smaller effect for the body′s immune func-tion.
8.Clinical analysis of suspected acute aortic dissection with ST-segment elevation detected by inferior leads

Objective To analyze the clinical characteristics of suspected acute aortic dissection with ST-segment elevation detected by inferior leads in order to avoid the misdiagnosis of acute aortic dissection facilitating an appropriate treatment strategy carried out in time.Methods A total of 14 patients with suspected acute aortic dissection with ST-segment elevation detected by inferior leads were enrolled.Their clinical presentation,ECG features,imaging findings,laboratory testing,coronary angiography results, treatment and outcome were retrospectively analyzed.Results Clinical characteristics of suspected acute aortic dissection with ST-segment elevation detected by inferior leads suggested that hypertension as a single risk factor accounted for 79%.The patients with normal blood pressure or high blood pressure in emergency visits accounted for 86%.The amplitude of ST elevation in lead Ⅲ was greater than that in lead Ⅱ,and lead Ⅲ accompanied with ST elevation in lead V1 or V4R accounted for 86%.Significantly elevated D-dimer >2 000 ng/mL was found in those patients.Coronary angiography showed that the opening of coronary artery not seen,normal coronary arteries or a simple right coronary artery proximal lesion.Transesophageal echocardiography and computed tomography angiography were used to identify the diagnosis with 100%accuracy.The mortality rate of this group was 50%.Conclusions Patients with acute aortic dissection evidenced by ST-segment elevation detected by inferior leads are in critical setting of high mortality. Emergency surgical treatment can significantly improve the survival rate of patients.
9.Three Cases of Drug-drug Interaction Intervened by Therapeutic Drug Monitoring for Voriconazole

Objective To discuss the role of therapeutic drug monitoring (TDM) in pharmaceutical care by successful intervention of severe drug-drug interaction in 3 patients with hematological disease treated with voriconazole and rifampin.Methods Three patients with hematological disease were monitored for the plasma concentration of voriconazole before,during,and after the concomitant use of rifampin.The severity of this drug interaction was revealed,risks for developing invasive fungal infection and tuberculosis dissemination after chemotherapy were evaluated based on the TDM results,and alternative regimens were recommended.Results Voriconazole plasma concentration was normal at baseline but significantly depressed after combination with rifampin in all 3 cases.Concomnitant use of rifampin leads to a rapid decline in plasma concentration of voriconazole in 2-3 days,and withdraw of this enzyme induction effect takes 8-10 days after discontinuation of rifampin.Conclusion TDM is a helpful tool for providing pharmaceutical care,it helps to objectively visualize the degree of clinically important drug-drug interactions.Clinical evidence together with TDM results suggests high risk for developing invasive fungal infection and tuberculosis dissemination in hematology patients while using this combination therapy.Discontinuation of rifampin was suggested and accepted.For these patients,combination of voriconazole and rifampin should be avoided.
10.Cyberbullying and its effect on health risk behaviors among secondary vocational school students

Objective:
To investigate the current situation of cyberbullying and health risk behaviors among secondary vocational school students, to explore the effects of cyberbullying on health risk behaviors, so as to provide a reference for formulating intervention strategies to promote adolescent health related behaviors.
Methods:
From March to April 2023, a field survey was conducted on 1 184 students from one secondary vocational school each in Jilin City and Yanji City by random cluster sampling method. The data was analyzed using Chi square tests and generalized linear models.
Results:
Among secondary vocational school students, 839 individuals (70.9%) experienced the cyberbullying, and 1 036 individuals (87.5%) had one and more health risk behavior group. There was a positive correlation between cyberbullying and health risk behaviors ( r=0.60, P <0.01). The generalized linear model analysis showed that female and no Internet addiction were negatively correlated with the health risk behaviors of secondary vocational students ( β =-2.75, -8.90), and the single parent families and peer pressure was positively correlated with the health behaviors of secondary vocational students ( β =2.56, 4.82)( P <0.05). Secondary vocational students experienced cyberbullying who from single parent families, peer pressure and social oriented internet usage were more likely to engage in health risk behaviors ( β= 3.41, 4.88, 2.20, P <0.05).
Conclusions
The incidence of cyberbullying and health risk behaviors among secondary vocational school students is relatively high, and cyberbullying can influence the occurrence of health risk behaviors. It is important to focus on and implement guidance for internet use and health behavior education for secondary vocational students from multiple perspectives, including the family, school, and society. Corresponding intervention strategies should be taken to promote the healthy development of adolescents.
