1.Clinical curative effect of budesonide inhalation in the treatment of children with Pneumonia

Objective To investigate the clinical effect of budesonide inhalation in the treatment of children with pneumonia.Methods 100 children with pneumonia were randomly divided into the two groups.The control group received conventional oral medication,the observation group was given Pulmicort Turbuhaler (budesonide)inhalation therapy.The clinical efficacy and respiratory function changes were observed after treatment for 1 week.Results In the observation group,cough,shortness of breath,body temperature stable,pulmonary rales disappearance,X-ray absorption of inflammation and the average healing time were (1.28 ± 1.04) d,(2.48 ± 0.39) d,(2.04 ± 0.22) d,(2.22 ± 0.43) d,(3.39 ± 0.35) d,(3.65 ± 0.39) d,which were significantly better than those in the control group [(2.62 ± 1.63)d,(4.85 ± 1.38)d,(4.85 ± 1.31)d,(4.63 ±0.84)d,(5.53 ±0.82)d,(5.58 ±1.20) d(t =2.62,1.86,3.72,1.46,1.68,2.71,all P <0.05].In the observation group,the FEV1,PEF,FEF50%,FVC before treatment were (70.24 ± 10.46) L,(70.05 ± 12.86)L/s,(66.74 ± 11.77) %,(67.16 ± 11.98) %,after treatment were (90.14 ± 12.63) L,(92.64 ± 10.75) L/s,(84.56 ± 15.12) %,(81.65 ± 13.96) %,after treatment the indicators were significantly improved (t =2.34,4.83,1.89,3.27,all P < 0.05).In the control group,before treatment,the FEV1,PEF,FEF50%,FVC were (70.56 ±9.95) L,(70.17 ± 13.58) L/s,(66.45 ± 11.58)%,(67.44 ± 12.37) %,after treatment were (78.76 ± 11.44) L,(79.46 ± 14.36) L/s,(70.24 ± 12.39) %,(72.03 ± 12.47) %,after treatment all indicators were significantly improved (t =3.15,2.67,4.16,2.34,all P < 0.05).The improvement of lung function in the observation group was significantly better than the control group (t =2.64,2.85,1.58,2.72,P < 0.05).The effective rate of the observation group was 92%,which was significantly higher than 72% of the control group (x2 =6.7 5 5,P < 0.0 5).Conclusion Pulmicort Turbuhaler inhalation in the treatment of children with pneumonia can significantly relieve airway hyperresponsiveness,improve treatment effect,improve respiratory function.
2.Cancer cachexia diagnosis and therapy

Cancer cachexia(CC)caused by cancer is a syndrome with complicated symptoms.Traditional therapies usually give interventions for more nutrition intake at CC late stage and take weight as the outcome marker.Lots of clinical trials demonstrate that the effect is not satisfactory.Recent studies show that the level of NF-KB decreased,termination of the breakdown of myofibrillar proteins,the serum proteins and lean body mass increased and function improvement after targeting proteasome at early stage of CC combined with traditional therapy.
3.Psychological issues after organ transplantation

Conspecific organ transplantation provides the survival for the diseased patients at the later stage. The transplantation-induced emotional disorder, rejection, social and psychological burdens all affect the donor, recipients and their relatives, together with the involved medical staff regarding the physical and mental health. The differences of gender, age, cultural background, economic status, social and family roles, and religion beliefs may result in the varying psychological reactions. Therefore, it is recommended to conduct the psychological intervention for the population at each stage of the organ transplantation, as the transplantation technique increases.
4.Electrochemiluminescence Performance of Poly(aniline_luminol) Composite Nanowires Synthesized by Chemical Oxidation

Poly ( aniline_luminol ) composite nanowires were synthesized by chemical oxidation using ammonium peroxydisulfate. In contrast to the maximum fluorescence emitting wavelength of luminol at 425 nm, the maximum fluorescence emitting wavelength of the polymeric luminol in the composite nanowires was red shifted to 465 nm obviously. The poly ( aniline_luminol) composite nanowires were modified on graphite electrode surface by drop coating, forming a stable poly ( aniline_luminol ) composite nanowires film. The composite nanowires film modified electrode presented favorable electrochemiluminescence ( ECL ) performances, and the ECL response could be enhanced by hydrogen peroxide. Under the optimized experimental conditions, the modified electrode provided a linear range of 5. 0×10-9-1. 0×10-5 mol/L for the detection of hydrogen peroxide with a detection limit of 2 ×10-9 mol/L.
5.Assessment of CIK cells in adoptive cellular therapy

Cytokine-induced killer (CIK) cells therapy plays an important role in cancer adjuvant treatment.Researches show that the number of peripheral blood immunocytes will change if patients with cancer accept the treatment of CIK cells.The changes of T cell subsets,regulatory T cells are relatively obvious,which may be one of the standards that can evaluate the curative effect of CIK cells.
6.Endoscopic release of distal holdfast fibers of the flexor retinaculum for carpal tunnel syndrome

Objective To investigate the necessity of releasing the distal holdfast fibers of the flexor retinaculum(DHFFR) during endoscopic carpal tunnel release(ECTR).Methods The Experiment Group included 16 cases.The operation was conducted under brachial plexus anesthesia without the use of tourniquet.A 1 cm skin incision was made.The USE system(Universal Subcutaneous Endoscope System) was employed.Both flexor retinaculum(FR) and distal holdfast fibers of the flexor retinaculum were cut off.Postoperative outcomes were compared with another 16 cases of flexor retinaculum release only(Control Group).Results Follow-up evaluation was carried out at 6 postoperative months.According to the Kelly's criteria,there were 13 cases of excellent results and 3 cases of good results in the Experiment Group,and 8 cases of excellent,5 cases of good,and 3 cases of fair results in the Control Group.Significant difference was obser red in flameda Ⅱ or Ⅲ grade patients between the two groups in carative effects(?~2=6.278,P=0.043).No serious complications or postoperative recurrence occurred.Conclusions Flexor retinaculum is not the only structure existing in the carpal canal to be released.More attention should be paid to complete decompression of both flexor retinaculum and distal holdfast fibers of the flexor retinaculum,especially in those who have serious symptoms.
7.Endoscopic tendon sheath release for stenosing tenovaginitis: A preliminary report

Objective To evaluate the endoscopic tendon sheath release for stenosing tenovaginitis (trigger finger). Methods Five patients with stenosing tenovaginitis (2 in thumbs, 1 in middle finger, and 2 in ring fingers) underwent operations by Smith & Nephew Endoscopic Trigger Finger Release system. After two 3.0mm transverse incisions were made, the window cannula assembly was inserted subcutaneously along the sheath from the proximal portal and advanced until it passed through the distal portal. Then a 2.7 mm endoscope was passed into from the proximal portal and a retrograde knife was introduced into the operative site from the distal portal. Finally the entire length of sheath was sectioned under direct endoscopic vision. Results All operations were successfully completed. Finger's flexion and extension function recovered immediately after the operations. All the patients restarted their employment one week postoperatively. There were no complications such as distinct pain or delayed wound healing in these patients. Conclusions This method has the advantages of minimal invasion, safety, effectiveness and quick recovery, especially suited to diabetic patients or multiple trigger fingers.

9.Analysis of risk factors for venous thrombosis associated with the placement of peripherally inserted central catheters in patients with malignant tumors during the course of chemotherapy

Objective To investigate the induced-factors levels of venous thrombosis associated with the placement of peripherally inserted central catheters (PICC) in patients with malignant tumor,and establish intervention model accordingly. Methods 40 patients complicated with venous thrombosis after PICC were set as the thrombosis group, 40 patients without venous thrombosis were selected as the no-thrombosis group, 30 healthy cases were selected as the control group. All cases' serum samples were ob-tained to measure the levels of molecular biomarkers of the coagulation system, platelet parameters and molecular biomarkers of the fibrinolysis system. The results of the three groups were assessed. Results Patients in the thrombosis group were reported significantly higher levels of vWF, GMP140,TAT than those in both two other groups, but lower levels of ATⅢ. Higher levels of MPV and PCT were found in the throm-bosis group than those in both two other groups.Tthe data also showed higher PLG and PAI levels and lower levels of PLM in the thrombosis group. Conclusions From this study it shows that patients with malig-nant tumors during the placement of PICC have evident hypercoagnlahility, high- activated platelet and se-vere pre- thrombosis state caused by PICC. In treatment of malignant tumor patients with PICC, the level of coag-ulation system molecular markers, platelet parameters,fibrinolysis molecular markers shoud be used as a clinical routine and as the index of early intervention and evaluation of the PICC.
10.The variance of serum IL-18 level and lymphocyte subpopulations in the patients with obese diabetic

Objective To observe on the variance of serum IL-18 level and lymphocyte subpopulations in the patients with obese diabetic(T2DM).Methods The diabetic obese group contained 31 cases,the non-diabetic obese group contained 33 cases,healthy normal body weight group contained 23 cases,the blood glucose(FPG),insulin (FPI),blood total cholesterol(TC),triglyceride(TG),IL-18 and the cell population of lymphocyte subpopulations CD3+and CD4+、CD8+ in peripheral blood were determined,the insulin sensitivity index were calcnlated.Results Compared with the healthy normal body weight group,the blood fat,FPG,FPI,IL-18 of T2DM patients significantly increased,there were significant statistical difference(P < 0.05 or P < 0.01).The ISI significantly decreased(P < 0.01).Compared with the non-diabetic obese group,the blood fat and IL-18 of T2DM patients significantly increased (P<0.05).Compared with the diabetic obese group,the subpopulations cell population of CD3+、CD4+、CD8+ in the non-diabetic obese group significantly decreased,the ratio of CD4+/CD8+ significantly increased,there were significant statistical difference(P<0.05 or P<0.01)in the two groups comparison.Compared with the non-diabetic obese CD4+/CD8+ significantly increased(P<0.05).Conclusion T2DM patients should prevent the hyperinsulinemia,control blood sugar,reduce insulin resistance,sustain normal lipid metabolism,boost immune function.
