1.Application of modular femoral prosthesis in total hip revision surgery for Paprosky type Ⅲ bone defect of proximal femur

[Objective]To report the mid-term results and main points of modular femoral prosthesis in total hip revision surgery for Paprosky type Ⅲ bone defect of the proximal femur.[Method]Nine cases of Paprosky type Ⅲ bone defect of the proximal femur were treated with modular femoral prosthesis MP and ZMR from May 2003 to October 2006.There were 5 males and 4 females with the age range of 39-65 years(mean age 57 years).All the patients were followed up for 8-34 months(mean 19 months).Causes for revision included loosening and osteolysis of primary cemented prosthesis in 7,and coral face prosthesis sterile loosening in 2.The shortest time from the last operation to the revision was 4 years,and the longest was 12 years.There were 5 of type ⅢA and 4 of type ⅢB according to the Paprosky bone defect classification.[Result]The Harris score was 35 points(28-54) before operation and 85(70-90) after operation.Seven patients walked without the help of crutches,one had a mild pain in the thigh and a leg crispation of 2.5 centimeter,and the other having a medium pain in the thigh limping with a crutch.No significant loosening,infection or osteolysis of distal femur was found.One prosthesis sinking 12 mm in 1 case.[Conclusion]Total hip revision surgery for Paprosky type Ⅲ femoral bone defect using modular femoral prosthesis has optimal mid-term results.It offers stabilization both in the initial and late stages.Due to the separation of prosthetic stem from the proximal prosthesis,the implantation is easy to fix.Modular femoral prosthesis,which is benificial to control offset,anteversion angle,and limb length makes the total hip revision surgery simpler and safer.
2.Clinical investigation of drug-eluting stent in treatment of elderly female patients with multi-vessel coronary artery disease

Objective To retrospectively analyze the effectiveness and safety of drug-eluting stents (DES) on prevention of restenosis in old female patients (≥65 years) with multi-vessel coronary artery disease. Methods 387 old female patients with multi-vessel coronary artery disease underwent percutaneous coronary intervention (PCI) were divided into the DES group (n=139) and the metallic stent (BMS) group (n=248). Acute and long-term outcomes were compared between the two groups. Results The ratio of patients with diabetic disease was higher in DES group than that in BMS group (P0.05). Objective To evaluate long-term clinical effectiveness in old female patients with multi-vessel coronary artery disease (≥65 years) treated with percutaneous coronary intervention (PCI). Methods 601 female patients with multi-vessel coronary artery disease underwent PCI were divided into the elder group (≥65 years, n=354) and the younger group (0.05). Conclusion PCI can be performed in old female patients with multi-vessel coronary artery disease with high success rate and few complications. Long-term clinical outcomes of PCI in old females with multi-vessel disease are similar to younger females.
4.Protective effects of glutamine for the morphological and functional damage of gastrointestinal mucosa induced by introperitoneally 5-FU in rats

Background and purpose:Glutamine is the major fuel for the enterocyte and major energy origin of intestinal mucosal cell which prevent gastrointestinal mucosal cell from atrophying.Although there were a lot of studies of how to protect the gastrointestinal mucosal in various stress,such as trauma,burn and complex operation,but the impact of how the glutamine affected the gastrointestinal mucosa in the period of chemotheraphy was rarely reported.Our aim was to investigate the protective effects of glutamine for the morphological and functional damage of gastrointestinal mucosa induced by introperitoneally fluorouracil(5-FU) in rats in order to find the ways to prevent the intestinal mucosa from damage in the period of chemotherapy.Methods:Forty Wistar rats were randomly divided into four groups:A:Blank control group,B:5-FU group,C:5-FU + Glycin group,D:5-FU + Glutamine group.The rats in group C were given Glycin,group D glutamine(0.35 g/kg each).Group B,group C and group D received 5-FU(100 mg/kg) injection intraperitoneally on the fourth day respectively to make the model of gastrointestinal barrier injury.The content of endotoxin in peripheral blood was measured in all groups after the rats were killed on the seventh day.The mesenteric lymph nodes(MLN) and liver were harvested for comparison of bacterial translocation incidence among the four groups.Ileum tissue was taken for pathologic examination including comparison of mucosal thickness,villus height and width and morphology change of ileum of each group.Results:The test showed that the weight and the height,the width,the thickness of the villus intestinal mucosa of group D were more than those of groupB and group C.The content of endotoxin in peripheral blood and the positive cases of bacterial translocation in MLN and liver of group D were less than group B and group C.The weight and the height,the width and the thickness of the villus intestinal mucosa of group A were more than those of group B,group C,and group D.The content of endotoxin in peripheral blood and the positive cases of bacterial translocation of MLN and liver of group A were less than group B,group C and group D.Conclusion:There were damage to the rat's gastrointestinal mucosa during the giving of 5-FU intraperitoneally.Glutamine could protect rat's gastrointestinal mucosa from damage during the giving of 5-FU intraperitoneally.Glycin could not protect rat's gastrointestinal mucosa against from damage during the giving of 5-FU intraperitoneally.
5.One-stop surgery of cryoballoon ablation combined with left atrial appendage closure for atrial fibrillation:a single-center experience

Objective To report single-center experience on CBA combined with LAAC in treat-ment of AF.Methods A retrospective study was conducted on 27 AF patients undergoing one-stop surgery of CBA combined with LAAC in Department of Cardiovascular Diseases of the People's Hospital of Liaoning Provincie from August 2020 to November 2022.The efficacy and safety of the surgery were analyzed.Results There were 22 patients(81.5%)with LAmbre and 5 patients with Watchman(18.5%,including one case of 24 mm Watchman FLX).All the patients achieved complete pulmonary vein isolation,and conversion to sinus rhythm during operation.During a mean follow-up of 30.0±9.2 months,24 patients(88.9%)maintained sinus rhythm at 12-month follow-up,and 22 patients(81.5%)maintained sinus rhythm at 24-month follow-up.TEE at 3 months after operation displayed that all the devices were in good positions and no PDL or DRT was observed.In the 11 patients undergoing cardiac enhanced CT in 12-36 months after surgery,PDL was detected in one patient(9.1%),and uncomplete endothelialisation of the device was observed in another one(9.1%)using the Watchman device.TEE at 26 months revealed one patient(3.7%)of DRT.Conclusion One-stop surgery of CBA combined with LAAC is a feasible treatment option for patients with NVAF and at high risk of stroke.
