1.Comparison of the clinical effect of gastrectomy and simply repair in the treatment of acute gastric perforation

Objective To investigate the clinical efficacy of gastrectomy and simple repair in the treatment of acute gastric perforation.Methods 132 patients with acute gastric perforation were randomly divided into the control group and observation group by random number table,66 cases in each group.The control group received gastrectomy,the observation group was given simple repair treatment.The clinical efficacy,clinical indicators and ulcer recurrence Visick grading of patients in two groups were observed.Results The total effective rate of the observation group (87.9%) was significantly higher than that in the control group(71.3%) (x2 =5.634,P < 0.05).The operative time,bed time,gastrointestinal function recovery time and hospitalization time in the observation group were (38.76 ± 6.23) min,(4.26 ± 1.67) d,(28.68 ± 4.74) h,(11.52 ± 2.32) d,those in the control group were (72.13 ± 10.78) min,(10.01 ± 2.21) d,(45.61 ± 5.69) h,(25.65 ± 3.95) d,there were statistically significant differences between the two groups(t =21.773,25.058,18.572,16.863,all P < 0.01).The relapse rate in the observation group (4.6%) was significantly lower than that in the control group (15.2 %) (x2 =4.181,P < 0.05).Conclusion The simple repair in the treatment of acute gastric perforation has significant effect,it can effectively reduce the value of clinical parameters and the recurrence rate of patients,which has important clinical application value.
2.Clinical characteristics,treatment and prognosis of 11 cases with extraovarian peritoneal serous papillary carcinoma

Background and purpose:Extraovarian peritoneal serous papillary carcinoma(EPSPC) originats from peritoneum with rare incidence, sometimes may affect the surface of ovary and have multi-focal lesions, whose standard therapy have not been established. In this study, we preliminarily reviewed the clinical characteristics, treatment and prognosis of EPSPC in 11 cases. Methods:The clinical characteristics of 11 cases with EPSPC were first retrospectively analyzed. All the patients underwent cytoreduction surgery followed by chemotherapy. Finally, short-term and long-term effi cacy, time to progression (TTP) and overall survival were evaluated by RECIST, respectively. Results:Abdominal pain, distention and ascites were the most common presenting symptoms, but tumors could be palpable in only 18.2% of the patients. The positive rate of ascites, abdomen B ultrasound,MRI scan and ascending CA125 level in the plasma and ascites was 100%,45.5%,100%,72.7%,81.2%,respectively. The successful rate of cytoreduction surgery was 45.5% for the EPSPC. After chemotherapy, the cases of complete remission, partial remission, stable disease and progression disease were 1(11.1%,1/9),3(33.3%,3/9),2(22.2%,2/9) and 3(33.3%,4/9), respectively. TTP was 5-14 months for all the patients and the median TTP was 8.6 months. The 1,2,3-year overall survival was 72.7%,18.2%,0%, respectively and the median overall survival was 14.6 months. Conclusions: Ascite, abdomen MRI scan and CA125 level are the most meaningful factor to diagnose EPSPC. EPSPC is a carcinoma of poor prognostic with non-specific clinical characteristics, low successful rate of cytoreduction surgery and is chemotherapy-resistent.
3.Treatment of complicated intra-articular distal radius fractures with extended flexor carpi radialis approach

Objective:To discuss the effect of treatment of complicated intra-articular distal radius fractures with extended flexor carpi radialis approach.Methods: A retrospective analysis of 38 cases with fresh complicated intra-articular distal radius fractures treated by using extended flexor carpi radialis approach in our hospital from October 2012 to March 2015,with 25 males and 13 females.The average age was (52.76±8.62) years (32-64 years).The average time to surgery was (5.42±1.91) d (3-10 d),with left wrist 17 cases and right wrist 21 cases.All the patients were with C3 distal radius fractures according to Association for the Study of Internal Fixation (AO/ASIF) classification.The follow-up was conducted 1,2,3,6,and 12 months after operation,including AP and lateral X-ray,wrist extension and flexion,radial deviation and ulnar deviation,forearm pronation and supination,and grip strength.At the end of 6 and 12 months after operation,all the patients were evaluated by using the mo-dified Garland-Werley score and patient rated wrist evaluation (PRWE).Results: All the patients got good bone union,and their follow-up time was more than 12 months.The average follow-up time was (16.37±2.85) months (12-22 months).The score of modified Garland-Werley evaluation 6 months post-operation was 5.37±2.82,excellent and good rate was 84.21%,the score of modified Garland-Werley evaluation 12 months post-operation was 5.03±2.60,excellent and good rate was 86.84%.The score of PRWE 6 months post-operation was 15.82±8.38,the score of PRWE 12 months post-operation was 12.17±7.58.Conclusion: The extended flexor carpi radialis approach is effective for the treatment of complicated intra-articular distal radius fractures and can avoid the complications of volar and dorsal combination approach.
4.Observation of antihyperlipidemic effects of the extract from mussel on experimental hyperlipidemia in rats

In this study,we have observed the effects of extract from mussel ( Mytilus edulis Linnaeus ) on experimental hyperlipidemia in rats.The results showed that the extract from mussel could reduce the levels of total cholesterol and triglyceride in blood serum of the experimental hyperlipidemic rats.

7.EFFECTS OF LIPOPOLYSACCHARIDE ON LUNG MICROVASCULAR ENDOTHELIAL CELLS AND PULMONARY ARTERIAL SMOOTH MUSCLE CELLS BY COCULTURE

Rat lung microvascular endothelial cells (LMVEC) was cocultured with rat pulmonary arterial smooth muscle cells (PASMC), and the cultures were randomly divided into 4 groups: normal homotypic group (NH), normal coculture group (NC), lipopolysaccharide homotypic group (LH), LPS coculture group (LC). The cell cycles were analysed with flow cytometry. The results showed that after the treatment of 100ng/ml LPS for 16h, the number of cells in G 1 phase was decreased in both homotypic and coculture of LMVEC and PASMC, while that in S+G 2/M were increased. The number of cells in G 1 phase was larger in LC than LH, but the number of cells at S+G 2/M phase was less in LC than LH. Compared with normal group, the unmber of LMVEC at S phase was increased by homotypic and coculture after the treatment with LPS. It is concluded that LPS could effect the cell cycle and accelerate the cell division, and there is a complicated regulatory mechanism in cell proliferation between LMVEC and PASMC.
8.CHANGES IN IL-10 CONTENT IN PLASMA IN RATS WITH ACUTE LUNG INJURY INDUCED BY OLEIC ACID AND LIPOPOLYSACCHARIDE

To investigate the interleukin 10 (IL 10) content in plasma in rats with acute lung injury(ALI) induced by oleic acid (OA) and lipopolysaccharide(LPS). OA (0.2ml/kg) and LPS (2mg/kg) was given to Wister rats to produce ALI. The respirtory rate,PaO 2 , wet weight/dry weight (W/D) of the lung, and pathological changes were observed, and IL 10 was determined with enzyme linked immunosorbent assay (ELISA). The results showed that ALI was produced in rats with OA+LPS, and there was a significant increase in IL 10 content in plasma in rats, especially in OA+LPS/4h group. The above results suggested that OA+LPS might produce ALI in rats, and the development of ALI was related to an obvious increase of the IL 10 content in plasma.

To investigate the interleukin 13(IL 13) content in plasma in rats with acute lung injury (ALI) induced by lipopolysaccharide (LPS), Wistar rats were given increasing doses of LPS (2mg/kg,4mg/kg,6mg/kg,8mg/kg) to produce ALI. The respirtory rate,PaO 2 ,wet weight/dry weight (W/D) of the lung, and pathological changes in the lung were observed. ELISA was used to determine plasma IL 13. It was found that: (1)ALI could be produced in rats with LPS, but ARDS occurred only when the dose of LPS reached 6mg/kg. or larger. (2)LPS produced an elevation of the content of IL 13 in plasma in rats, peaking when the dose of LPS reaching 6mg/kg or over. These results suggested that LPS might induce ALI in rats, and ARDS could be produced when the dose of LPS reached ≥6mg/kg. (3)The high increase in plasma IL 13 content might play an important role in producing ARDS induced by LPS.
10.Effects of hypoxia on STAT3 gene expression and proliferation of rat pulmonary arterial smooth muscle cells

Objective To explore the effects of hypoxia on expression of signal transduction and activators of transcription (STAT3) gene and proliferation of rat pulmonary arterial smooth muscle cells (PASMCs). Methods RT PCR and western blot were used to assess the levels of STAT3 mRNA and tyrosine phosphorylated STAT3 protein after primarily cultured rat PASMCs were exposed to hypoxia for 2h, 6h, 12h, 24h respectively. 3 H TdR incorporation was used to observe the cell proliferation. Results The expression of STAT3 mRNA was increased after exposure of PASMCs to hypoxia for 2h, and peaked when exposed for 6h, then declined when exposed for 12h, but still higher than that in PASMCs when exposed to normal oxygentension. The level of tyrosine phosphorylated STAT3 was increased when exposed for 6h,peaked after 12h and delined after 24h in PASMCs exposed to hypoxia. With prolongation of time of hypoxia, 3 H TdR incorporation in PASMCs was significantly higher, and reached a maximum after 24h. Conclusion The results suggested that hypoxia could induce the expression of STAT3 mRNA and activation of STAT3 protein in PASMCs, implying that STAT3 gene might play an important role in the early stage of hypoxia induced PASMCs proliferation
