1.Mobilization of bone marrow-derived mesenchymal stem cells in the treatment of ischemic cardiomyopathy

BACKGROUND: Mobilization of autologous bone marrow-derived mesenchymal stem cells (MSCs) for repair of myocardialnecrosis is a safe and effective method for treatment of ischemic cardiomyopathy. OBJECTIVE: This review serves to summarize the research progress in mobilization of bone marrow-derived MSCs for treatment of ischemic cardiomyopathy. RETRIEVAL STRATEGY: Using the terms "bone marrow stem cells, myocardial infarction", manuscripts that were published from December 1996 to December 2006 were retrieved from the PubMed database. Manuscripts were primarily screened. Inclusion criteria: studies on bone marrow-derived MSCs used for treatment of ischemic cardiomyopathy, and studies that were recently published or in high-impact journals. Exclusion criteria: repetitive studies. LITERATURE EVALUATION: The manuscripts primarily focused on the randomized controlled experiments of bone marrow-derived MSCs for treatment of ischemic cardiomyopathy. Among the 28 included manuscripts, 28 were review studies, and the remaining were clinical or basic experimental studies. DATA SYNTHESIS: Bone marrow-derived MSCs possess self-renewal, proliferative, and multi-directional differentiation potential. They can differentiate into cardiac-like cells under the regulation of appropriate cytokines. Previous studies have demonstrated that the stem cells in the bone marrow blood circulation can reach infarcted myocardium and are involved in the tissue regeneration. A great progress has been made in the study on bone marrow-derived MSCs used for treatment of ischemic cardiomyopathy. However, there exist some problems, such as, how to preserve the seeded cells, the mechanism underlying differentiation and proliferation in the in vitro culture of cells remains unclear, and the biomechanical performance of cardiac-like cells are not satisfactory. CONCLUSION: Mobilization of bone marrow-derived MSCs is easy, fast, less-injury, and without immunologic rejections in the autotransplantation for treatment of isehemic cardiomyopathy. However, there are still some unsolved problems.
2.Therapeutic Observation of Tiao Du Tong Ren Needling for Post-stroke Depression

Objective To observe the clinical efficacy ofTiao Du Tong Ren (regulating the Governor Vessel and Unblocking the Conception Vessel) needling in treating post-stroke depression.Method Sixty patients with post-stroke depression were randomized into a treatment group and a control group, 30 cases in each group. The treatment group was intervened byTiao Du Tong Ren needling, while the control group was by ordinary acupuncture. The Hamilton Depression Scale (HAMD) was observed before and after intervention, and the clinical efficacies were compared between the two groups.Result The total effective rate was 93.3% in the treatment group versus 80.0% in the control group, and the difference was statistically significant (P<0.05). The HAMD scores were significantly changed in both groups after intervention (P<0.05). The change of HAMD score after intervention in the treatment group was markedly different from that in the control group (P<0.05).ConclusionTiao Du Tong Ren needling method is an effective approach in treating post-stroke depression.

4.13C-methacetin breath test for evaluating of children's liver disorder

Objective To explore the clinical value of 13C-methacetin breath test for the assessment of liver disorder and to analyze its predictive value to the severity of liver function injury in children.Methods Eighteen healthy children served as healthy control group,and 40 patients with different etiology and severity served as experimental group,and then the latter were divided into 2 subgroups,28 patients in Child-Pugh classification A,and 12 cases in below B(11 cases in B and 1 case in C).An oral dose of 2 mg/kg tracer 13C-methacetin was administered to each subject for the 13 C-methacetin breath test.At the same time,serum liver function markers including serum transaminase,bilirubin,albumin and prothrombin time were measured.The acquired data were analyzed by SPSS 17.0 software.Results (1) Metabolisation velocity (MV) max30 and cumulated dose (CUM) 120 in experimental group (46.64 ± 27.93,59.29 ± 30.73) were much lower than those of the healthy control group(73.56 ± 26.03,102.97 ± 41.80) (t =2.450,3.165,all P <0.05);(2) MVmax30 and CUM120 were closely correlated with the liver function markers of albumin,total bilirubin,direct bilirubin,prothrombin time (P < 0.05);(3) MVmax30 and CUM120 could predict liver diseases in children,especially the CUM120.With CUM120 =85.80 as a cut-off value to predict liver diseases,the Youden index was 0.578 at its maximum,and the sensitivity and specificity were 77.8% and 80.0%;(4) Compared with the Child-Pugh classification A,the CUM120 in Child-Pugh classification B and lower B was significantly lower(P < 0.001);(5) CUM120 could predict the severity of liver diseases.With CUM120 =56.15 as a cut off value to predict the severity of liver diseases,the Youden index was 0.857 at its maximum,and the sensitivity and specificity were 85.7% and 100.0%.Conclusion 13C-methacetin breath test index of CUM120 could predict liver diseases in children and the severity of liver function.
5.Median effective plasma concentration of remifentanil inhibiting cardiovascular response to tracheal intubation and skin incision when combined with propofol in patients of Uighur nationality

Objective To determine the median effective plasma concentration of remifentanil (EC50) inhibiting cardiovascular response to tracheal intubation and skin incision when combined with propofol in patients of Uighur nationality.Methods Thirty ASA Ⅰ or Ⅱ Uighurs aged 20-60 yr with body mass index of 18-30 kg/m2 undergoing elective laparoscopic cholecystectomy were enrolled in this study.Anesthesia was induced and maintained with propofol infusion.BIS value was maintained at 40-50.TCI of remifentanil was then started.The initial target plasma concentration (Cp) was 6.0 ng/ml.EC50 of remifentanil was determined by modified Dixon' s upand-down sequential experiment.Each time Cp increased/decreased by 0.5 ng/ml.Tracheal intubation was facili.tated with cisatracurium 0.2 mg/kg at 2 min after a new Cp was set when target effect-site concentration (Ce) was balanced with Cp.Positive cardiovascular response was defined as increase in SBP by 15% and/or HR > 90 bpm lasting>15 s.The EC50 (95% confidence interval) of remifentanil blunting responses to tracheal intubation and skin incision was calculated.Results EC50 (95 % confidence interval) of remifentanil inhibiting cardiovascular response to tracheal intubation and skin incision when combined with propofol was 3.4 (2.3-4.5) ng/ml and 3.8 (2.8-4.9) ng/ml respectively.Conclusion When depth of anesthesia was maintained at BIS value 40-50 with propofol infusion,the EC50 of remifentanil inhibiting cardiovascular response to tracheal intubation and skin incision is 3.4 and 3.8 ng/ml respectively.
6.Quantong Recipe integrated with ureteroscopic laser lithotripsy for ureteral calculi.

Background: Endoscopic surgery of Western medicine has developed rapidly in treating ureteral calculi while traditional Chinese medicine (TCM) therapy also has its advantage, thus the integrated traditional Chinese and Western medicine therapy may have an even better efficacy in dealing with this problem. Objective: To observe the clinical effects of sequential therapy of Quantong Recipe, a compound traditional Chinese herbal medicine, combined with ureteroscopic laser lithotripsy on ureteral calculi. Design, setting, participants and interventions: A total of 151 patients with ureteral calculi (stone diameter of 3 to 21 mm) from Department of Urology, Yueyang Hospital of Integrated Traditional Chinese and Western Medicine, Shanghai University of Traditional Chinese Medicine were included, of which 106 patients with stone diameter of 3 to 10 mm were treated with Quantong Recipe for the first two weeks. Twenty patients with unexpelled stones and 45 patients with stone diameter of more than 10 mm were treated with ureteroscopic laser lithotripsy. The 65 patients except one open operation were divided into regular treatment group (32 cases) and regular treatment combined with Quantong Recipe group (32 cases) after lithotripsy. The efficacy was assessed after one week. Main outcome measures: Cure rate in the patients with stone diameter of less than 10 mm after two-week Quantong Recipe treatment was calculated, and clearance rate in the patients with residual stone for ureteroscopic laser lithotripsy after one-week Quantong Recipe treatment was also observed. Results: After taking Quantong Recipe for two weeks, the stones in 86 patients were completely discharged, and the stones in 6 patients were not completely discharged, but the unexpelled stones had moved down more than one segment of ureteral stenosis; the total response rate was 86.79% (92/106). The 64 patients with stones unexpelled or with stone diameter of more than 10 mm were treated with ureteroscopic laser lithotripsy, and the clearance rate of residual stone was 93.8% in regular treatment combined with Quantong Recipe group, and was 71.9% in regular treatment group, and there was a significant difference between the two groups (P<0.05). Conclusion: Sequential therapy of Quantong Recipe integrated with ureteroscopic laser lithotripsy demonstrates a significant advantage of integrated traditional Chinese and Western medicine therapy in treating ureteral calculi.
7.The role of esophageal multichannel intraluminal impedance monitoring for 24 hours in diagnosis of aerophagia

Objective To investigate the clinical characteristics of aerophagia in children,and to evaluate the effect of 24 hours esophageal multichannel intraluminal impedance monitoring in diagnosis of aerophagia.Methods The clinical data of 30 children with aerophagia hospitalized in Department of Digestive Disease,Nanjing Children's Hospital Affiliated to Nanjing Medical University from July 2005 to June 2013 were reviewed.The 24 hours esophageal multichannel intraluminal impedance monitoring was performed in 8 cases of aerophagia.The treatment of these patients normally included psychological counseling,dietary adjustment,Iactulose,Domperidone and vitamins,while Simethicone was given to the patients if normal treatment was invalid.Results Of the 30 patients,aged 2 to 13 years,averaged (8.6 ± 4.5) years,the chief complaints were abdominal distention in 30 cases (100.00%),visible or audible air swallowing in 28 cases (93.33%),repetitive belching in 27 cases (90.00%),reduced appetite in 25 cases (83.33%),constipation in 22 cases (73.33%),psychological stresses in 15 cases (50.00%),recurrent abdominal pain syndrome in 10 cases(33.33%),chronic diarrhea in 3 cases(10.00%) and acute abdominal pain in 3 cases(10.00%).Among the 30 patients,24 hours esophageal multichannel intraluminal impedance monitoring was performed in 8 cases,averaging 149.63 (47-553)times of air swallowing were recorded.In the 30 patients,24 cases (80.00%) were relieved after being treated with psychological counseling,dietary adjustment,lactulose,domperidone and vitamins,and only 6 cases(20.00%) were invalid for normal treatment but well responeded to Simethicone.Conclusions Aerophagia is a kind of functional gastrointestinal disorders in children.Abdominal distention,air swallowing,repetitive belching,reduced appetite,constipation and pain are common symptoms.It can be cured through psychological counseling management,diet modification,laxatives,propulsives and vitamins.24 hours esophageal multichannel intraluminal impedance monitoring is a useful technology in the diagnosis of aerophagia children.
8.Effects of dexmedetomidine on activity of nuclear factor kappa B in neutrophil granulocytes during one-lung ventilation in patients undergoing pulmonary lobectomy

Objective To evaluate the effects of dexmedetomidine on the activity of nuclear factor kappa B (NF-κB) in neutrophil granulocytes during one-lung ventilation in the patients undergoing pulmonary lobectomy.Methods Thirty-eight ASA physical status Ⅰ or Ⅱ patients,aged 40-64 yr,weighing 50-75 kg,scheduled for elective pulmonary lobectomy,were randomized into 2 groups (n =19 each) using a random number table:control group (group C) and dexmedetomidine group (group D).In group D,dexmedetomidine 1.0 μg/kg was infused intravenously over 15 min before induction of anesthesia,followed by infusion at a rate of 0.5 μg· kg-1 · h-1 until 30 min before the end of operation.The equal volume of normal saline was given in group C.At 5 min after intubation,0,30 and 60 min of one-lung ventilation,0 and 30 min of two-lung ventilation,and 30 min after operation,blood samples were taken from the radial artery for blood gas analysis and for measurement of plasma concentrations of tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6).The oxygenation index and respiratory index were calculated.The nuclear protein of neutrophil granulocytes was extracted for measurement of NF-κB DNA-binding activity.Results Compared with group C,the respiratory index,plasma concentrations of TNF-α and IL-6 and NF-κB DNA-binding activity were significantly decreased,and no significant difference was found in the oxygenation index in group D.Conclusion Dexmedetomidine can inhibit the activation of NF-κB in neutrophil granulocytes and is helpful in reducing the systemic inflammatory responses during one-lung ventilation in the patients undergoing pulmonary lobectomy.

Objective To comfirm the anti-fibrotic effect of spironolactone and investigate the influence of spironolactone on concentration of angiotensinⅡ(AngⅡ) in rat hepatic fibrosis. Methods Ninety male Sprague-Dawly rats were randomly divided into three groups: control group (8 rats, subcutaneous injection of pure peanut oil, normal diet), model group (42 rats, subcutaneous injection of 40% CCl 4, 0.3 mL/100 g, twice a week, high cholesterol and low protein diet, 5% alcohol as the drink), and spironolactone group (40 rats, same dosage of CCl 4, high cholesterol and low protein diet, gastropufusion of spironolactone at the dose of 100 mg?kg -1?d -1). The animals were sacrificed at 2, 4, 6 and 8 weeks of administration. AngⅡ was assayed by radioimmunoassay. The liver tissues were stained, coded and examined. 10.King cobra egg yolk antibody from the egg yolks of immunized hens

Aim To develop King cobra antivenom from the egg yolks of immunized hens,and study dynamic expression of IgY in egg yolks.Methods chickens(white Leghorn) were immunized with detoxicated King cobra venom by formaldehyde ,Egg yolk antibody (IgY) was isotated by thiophilic interaction chromatography and identified by SDS-PAGE. Activity of IgY was evaluated by enzyme-linked immunoserbent assay(ELISA) and Double immuodiffusion. Protein was measured using folin-lowry method. Results Specific antivenom could be detected in the yolk laid by the hens 9 d after immunization, At the 60th day after primary immunization ,ELISA titers reached 1∶ 100 000,and 97.5 mg IgY?ml -1 yolk was obtained from thiophilic interaction chromatography. IgY was highly specific, No cross reactivity was observed among IgY and agkistrodon acutus venom and vipera venom ;Little cross reactivity was shown with cobra genus venoms.Conclusion King cobra IgY was obtained and purified from the egg yolks of immunized hens,The dynamic expression of IgY was manitered during the course of immunization,Further investigation is needed.
