Objective To analyze the effect of iodine-131I in the treatment of patients with hyperthyroid heart disease(HHD).Methods The clinical materials and therapeutic effect by 131 I were reviewed in 100 patients with HDD.Results The heal rate of hyperthyroidism and HHD were 82.3％,86.5％ in treatment group,and were higher than that of the control group (69.0％ and 76.2％ ) ( x2 =3.80,3.83,P ＜ 0.05 ) ; HHD with atrial fibrillation was 65.0％,average cardioversion after 131I treatment was 74.4％ after the treatment by 131I;The LVEDD、LVESD、LVEDV and HR after 131 I treatment were lower compared with before treatment ( all P ＜ 0.05 ),SV and EF were increased ( all P ＜ 0.01 ).Conclusion 131I treatment in patients with HHD,can significantly improve the efficacy of a heart disease

Objective Tc explore the value of application of Sevoflurane anesthesia induction in anesthesia management of foreign matter removal from trachea in children.Methods Sixty five cases of children with tracheal foreign matter related in our hospital from January 2015 to December 2016 were selected as the study objects,and were divided into two groups according to random number table method.The control group of 32 cases was given 2.5 mg/kg propofol to induce anaesthesia,and the observation group of 33 cases was given 8％ Sevoflurane inhalation to induce ancsthesia.One shot success rate,deoxygenation time,operation time,recovery time and the relative parameters (systolic pressure,diastolic pressure,respiratory rate and pulse oxygen saturation) at entrance time,before and after setting the mirror were contrasted and analyzed,and the intraoperative and postoperative adverse reactions were recorded in the two groups.Results There were no significant differences in respiratory rate,blood oxygen saturation,systolic blood pressure and diastolic blood pressure between two groups at entrance time and before setting the mirror (P ＞ 0.05).The respiratory rate and blood oxygen saturation of the observation group were higher than those of control group after setting the mirror and at withdrawing mirror time,with statistically significant differences (P ＜ 0.05).No statistically significant differences in other indexes (P ＞ 0.05).Deoxygenation time,operation time and recovery time of the observation group were significantly lower than those in the control group (P ＜ 0.05).The total incidence of adverse reactions in the observation group was significantly lower than that in the control group (P ＜ 0.05).Conclusions Sevoflurane anesthesia induction can obviously shorten operation time and recovery time in foreign matter removal from trachea in children,and reduce the incidence of intraoperative and postoperative adverse reactions.It has good clinical effect.

Objective To identify the factors influencing the response to clopidogrel.Methods Seventy-two patients with acute coronary syndrome who were treated at Beijing Tongren Hospital from September,2005 to April,2006 were eligible for this study.10 ?mol/L ADP induced platelet aggregation was measured before and 7 days after clopidogrel treatment by the traditional optical aggregometry.According to the inhibition of platelet aggregation(?A),the population was divided into low response group and normal response group.Then some factors possibly influencing the response to clopidogrel were analyzed by specific statistical methods.Results Baseline characteristic showed that compard with that of normal response group,the body mass index and the percentage of obesity and STEMI were lower in low response group,while its percentage of hypertension and diabetes were higher(P

Objective To provide a scientific basis for early clinical diagnosis and treatment as well as secondary and tertiary protection through 10 years inquiry of multiple organ failure in the elderly(MOFE).Results Among 153 MOFE cases,the first inducement of 89.5%(137/153) patients was infection,among which 93.4% (128/137) was pulmonary infection.The vertical percentage of the first failure organ was as follows: pulmonary (39.0%,60/154),heart (19.5%,30/154),brain (14.9%,23/154),kidney (10.4%,16/154),alimentary canal (8.4%,13/154),liver (6.5%,10/154);while the horizontal percentage was as follows: pulmonary (42.9%,60/140),heart (40.9%,56/137),kidney (32.6%,30/92),brain (21.9%,23/105),alimentary canal ( 14.5%,9/62).The survival time of the first organ failure from shortness to length was as follows: pulmonary,brain,heart,kidney.Conclusions Most MOFE cases have pulmonary failure at first.The organs failure sequence is pulmonary,heart,brain and kidney in order.

Children without company of parents in pediatric intensive care unit,often irritable crying,injured,or scratching themselves,pulling in all kinds of wire and/or catheter on the body,and even adverse events,such as dropping of bed.To ensure the safety of the children,we invent a multifunctional strait-jacket,and it is applied in clinic in order to solve the above problems.The structure of this strait-jacket is open,easy to install wire and all kinds of operation;shoulder,cuff with button loops fix bed guardrail by a rope,to prevent the roll shift;cuff with double fold gloves can avoid scratch the skin,remove the line,having the effect of palm warm;waist adding two constraints,not improving constraints on the waist but abdomen to keep warm.By clinical application,this strait-jacket improves the comfort of the constraint,can prevent the occurrence of complications due to improper constraints,such as skin abrasions,physical damage,blood circulation obstacle,alleviate the pain for the children.

Objective To discuss the reasons for and preventive methods of acute encephalocele during traumatic cerebral operation.Methods Retrospective analysis of 38 patients with acute encephalocele was made.Results The main cause of acute encephalocele were delayed intracranial hematomas,acute diffuse brain swelling,cerebral contusion and laceration in Sylvain tissue and ischemic anoxia.Conclusion Complete removal of hematoma and delayed hematoma,sufficient decompression and decrease of ICP are effective methods for prevention and treatment of acute encephalocele.

Objective To study the feasibility of using fluorescent immunofiltration test based on quantum dots (QDs)for rapid and quantitative detection of C-reactive protein.Methods Based on homemade QDs and QDs-antibody bioconjugates,an immune de-tection method was established via the double antibodies sandwich technique on the immunochromatography card.The test results could be read under the irradiation of UV light,and quantitative results could be measured through the combination of a laser and fluorescent spectroscopy.Results Under UV light irradiation,the minimum detection concentration of CRP was 0.156 mg/L.U-sing the quantitative detection method,the fluorescent intensities on the cards could be established a linear relationship with the con-centration of CRP,and the linear equation was that log(Y )=0.563 log(X)+4.570,r2 =0.958.Conclusion The fluorescent quan-tum dot immunofiltration assay can be used for quantitative detection of CRP;The quantum dots immuno-labels have the potential to develop new type of immune-diagnostic reagents.

BACKGROUND:Mental material has shadow intervention effects on multi-slice helical CT (MDCT).Mental wall thickness and lumens inner diameter in stent structure can significantly affect longitudinal axis imaging quality of MDCT target vessel stent.However.there are few studies involved in this aspect.OBJECTIVE:To analyze the effect of various coronary stent materials and structures on patency images of target vessel by MDCT evaluation,and to provide clinical evidences for improving stent technique.DESIGN,TIME AND SETTING:The comparison observation was conducted at the Shenzhou Hospital,Shenyang Medical College.and Shengjing Hospital,China Medical University from January 2006 to December 2008.PARTICIPANTS:A total of 139 patients with coronary heart disease who were treated with stent implantation were divided into material and construct groups (227 stents were implanted).There were 92 stainless steel stents,135 nick-eltitanium alloy stents,85 thin mental wall stents (<140μm).142 thick mental wall stents (≥140μm).71 small diameter stents (<3 mm),and 156 big diameter stents (≥3mm).METHODS:During following up,patients were checked using 64-slice helical CT and routine coronary arteriongraphy to compare patency images of target vessels in the two groups.MAIN OUTCOME MEASURES:MDCT was scored by four-mark standard to evaluate sensitivity,specificity,accuracy,positive and negative predictive values of MDCT.RESULTS:A total of 227 stents were implanted into 139 patients.CT images of stainless steel stent group were poorer than nickel-titanium alloy group,and the indicators including image score,sensitivity,specificity,positive and negative predictive values of the stainless steel stent group were significantly less than nickel-titanium alloy group (P<0.05).CT images of thick-wall stent were poorer than thin-wall stent,while the indicators including image score,sensitivity,specificity,positive and negative predictive values of the thick-wall stent were significantly less than thin-wall stent (P<0.05).CT images of small-diameter stent were poorer than large-diameter stent,while the indicators including image score,sensitivity,specificity,positive and negative predictive values of the small-diameter stent were significantly less than large-diameter stent (P<0.05).CONCLUSION:Materials,wall thickness and diameter of coronary stent may influence images of target vessels via MDCT evaluation.

Objective To investigate the expression level and the significances of prognosis by miR-451a in nasopharyngeal carcinoma (NPC) in Guangxi. Methods The expressions of miR-451a in 89 cases of nasopharyngeal carcinoma were detected by real time RT-PCR. The relation among the expression level , the clinicopathologic features of NPC and its prognosis were analyzed. Results The expression of miR-451a were found in all of nasopharyngeal carcinoma. The expression level of miR-451a in nasopharyngeal carcinoma was negative correlated to overall survival and disease free survival (P = 0.01,P = 0.04). Conclusions miR-451a may play a key role in detection of nasopharyngeal carcinoma with poor prognosis.

Objective:To provide some thoughts for pharmaceutical treatment and care for the patients with pulmonary infection af-ter renal transplantation. Methods:Clinical pharmacists participated the whole treatment process of one case of pulmonary infection af-ter renal transplantation. According to the literatures combined with medical history, clinical symptoms and lab results, the drug treat-ment process of the patient was analyzed, and the key points of the optimized pharmaceutical care were summarized. Results: The pharmaceutical care included the dose adjustment of immunosuppressants at the early phase of the disease and after the improvement of clinical symptoms, attention paid to the interactions between multiple anti-infective drugs and immunosuppressive agents, dosage ad-justment based on the renal function of the patient, monitoring adverse drug reactions and drawing up personalized regimen. Conclu-sion:Through comprehensive medication monitoring, clinical pharmacists can help physicians develop timely and effective treatment programs and provide professional and effective pharmaceutical care for patients.
