1.Clinical analysis and endoscopic findings of early esophageal cancer

Objective To research the characteristics of early esophageal cancer's endoscopy and its clini-cal diagnosis. Methods Retrospective analyze the clinical symptoms and endoscopy performance of the 24 cases of early esophageal cancer which were confirmed by opemtion. Results Unaided eye observing of 24 cases of early e-acphageal cancer:diameter of pathological changes within 1.0 cm,there are four cases of the surface rough,turbidity,vascular fuzzy texture, congestive mucosa, small pieces of rotten to the core, around the realm of clear, easy bleeding by touch of three cases. Mucosa focal irregular plaquelike erosion or superficial helcosis of five cases Mucosa mild uplift,smooth surface or erosion, with slightly different colors of 10 cases ( including one case of pelypoid change) Mucosa fo-cal wormhole-like change, a slight hollow, moss-covered, rubeosis or bleeding lesions after water rinse, irregular and clear borders of two cases, pathology report: one case of small cell carcinoma,one case of adenocarcinoma (cardia),High-level intraepithelial neoplasia transformation nine cases, one case of poorly differentiated adenocarcinoma, 12 ca-ses of squamous cell carcinoma. Conculsion Endoscopic early esophageal cancer for the mainfeatures of the mucous membrane color changes, mucosal thickening, transparency and mucosal vascular network st ructure changes, and mu-cosal rough, nodules, plaques, erosion and uplift superficial, shallow depression and other patterns chang, GeneraUe-sions were confined or isolated like chang, irregular, and the mucous membrane around the borderclear. Clinicalofte misdiagnosed, missed diagnosis, early diagnosis to have significant value.

3.Portal hemodynamics in patients with different syndromes of cirrhosis

OBJECTIVE: To explore the relationship between the portal hemodynamics or the indices of liver fibrosis and the liver function score in patients with different traditional Chinese medical syndromes of liver cirrhosis. METHODS: One hundred and forty-seven cases of liver cirrhosis with different syndromes were included in the study. Diameters and blood flow velocities of the portal vein and splenic vein were tested by color Doppler sonarography. The indices of liver function (TBIL, ALT, AST, Alb, Glb, PTA) and the indices of hepatic fibrosis (HA, PC-III, LN, VI-C) were tested. RESULTS: The diameters of portal vein in liver cirrhosis patients with different syndromes (internal accumulation of of damp-heat, stagnation of liver-qi, superabundance of dampness due to spleen-asthenia, asthenia of liver and kidney yin, asthenia of spleen and kidney yang, blood stasis) were not significantly different. The blood flow velocities and flow volumes of portal vein in patients with the blood stasis syndrome and the internal accumulation of damp-heat syndrome were significantly different (P < 0.05). The diameter of splenic vein in patients with the internal accumulation of damp-heat syndrome was significantly different to that of the asthenia of liver and kidney yin syndrome, or the blood stasis syndrome, or the asthenia of spleen and kidney yang syndrome, respectively (P < 0.05 or P < 0.01). The blood flow velocity of splenic vein in patients with the blood stasis syndrome or the asthenia of spleen and kidney yang syndrome was significantly different to that of the internal accumulation of damp-heat syndrome, or the stagnation of liver-qi syndrome, or the asthenia of liver and kidney yin syndrome, respectively (P < 0.01). The blood flow volume of splenic vein in patients with the internal accumulation of damp-heat syndrome was significantly different to that of the asthenia of liver and kidney yin syndrome, or the asthenia of spleen and kidney yang syndrome, or the blood stasis syndrome, respectively (P < 0.01). The liver cirrhosis indices HA and PC-III in patients with the asthenia of liver and kidney yin syndrome were significantly different to those of the internal accumulation of damp-heat syndrome (P < 0.05). In Child-Pugh classification, the incidence of Child-Pugh C was the highest in the asthenia of spleen and kidney yang syndrome, while the incidence of Child-Pugh A was the highest in the stagnation of liver qi syndrome. CONCLUSION: The changes of portal vein dynamics in liver cirrhosis patients with the asthenia of liver and kidney yin syndrome, or the asthenia of spleen and kidney yang syndrome, or the blood stasis syndrome are significantly different to those of the internal accumulation of damp-heat syndrome and the stagnation of liver-qi syndrome.
4.Expression of pituitary tumor transforming gene and p53 protein in large intestinal adenoma and large intestinal carcinoma tissues

ObjectiveTo investigate the expression and relationship of pituitary tumor transforming gene (PTTG) and p53 protein in large intestinal adenoma,large intestinal carcinoma and normal large intestinal mucosa tissues.MethodsImmunohistochemistry was employed to determine the expression of PTTG and p53 protein in 50 cases with large intestinal adenoma tissues,42 cases with large intestinal carcinoma tissues and normal large intestinal mucosa tissues.The relationship of the expression of PTTG and p53 protein with the clinicopathological characteristics was analyzed.ResultsThere was no positive expression of p53 protein in normal large intestinal mucosa tissues,while the positive rate of PTTG expression was 7.14%(3/42).The positive rates of PTTG and p53 protein expression were 82.00%(41/50) and 90.00%(45/50) in large intestinal adenoma tissues,88.10% (37/42) and 95.24% (40/42) in large intestinal carcinoma tissues.The positive rates of PTTG and p53 protein over expression were 45.24%(19/42) and 69.05%(29/42) in large intestinal carcinoma tissues.The positive rates of PTTG and p53 protein expression in large intestinal carcinoma tissues were higher than those in large intestinal adenoma tissues and normal large intestinal mucosa tissues,the positive rates of PTTG and p53 protein expression in large intestinal adenoma tissues were higher than those in normal large intestinal mucosa tissues,and there were significant differences(P < 0.05).The expression of PTTG was not correlated with p53 protein in large intestinal carcinoma tissues(P> 0.05 ),while the positive relationship was found between the expression of PTTG and p53 protein in large intestinal adenoma tissues (P < 0.05 ).The over expression of PTTG was significantly correlated with lymph node metastasis (P < 0.01 ),but the over expression of p53 protein was not correlated with lymph node metastasis(P > 0.05) in large intestinal carcinoma tissues.Conclusions The expression of PTTG is significantly correlated with p53 protein in large intestinal adenoma tissues,and their co-expression may be used as markers for carcinogenesis of large intestinal adenoma tissues.The over expression of PTTG and p53protein is found in large intestinal carcinoma,and the over expression of PTTG is correlated with lymph node metastasis.The over expression of PTTG may be used as a marker for lymph node metastasis of large intestinal carcinoma.
5.Effects of Different Methods of Preparation on the Contents of Chemical Components in Ligustri Lucidi Fructus

Objective To investigate the effects of different methods of preparation on the contents of nuzhenide and oleanolic acid in Ligustri Lucidi Fructus. Methods The same batch of Ligustri Lucidi Fructus was under preparation and process. RP-HPLC was used to determine the contents of nuzhenide and oleanolic acid in crude samples, wine-prepared samples, vinegar-prepared samples, and salt-prepared samples. Results The contents of nuzhenide were in the order as follow:salt-prepared samples (1.997 5%)>crude samples (1.600 7%)>wine-prepared samples (1.144 6%)>vinegar-prepared samples (0.894 6%). The contents of oleanolic acid were in the order as follow:wine-prepared samples (1.120 2%)>salt-prepared samples (0.924 6%)>vinegar-prepared samples (0.913 2%)>crude samples (0.899 8%). Conclusion Different methods of preparation have certain effects on the contents of nuzhenide and oleanolic acid in Ligustri Lucidi Fructus.
6.To Improve Security Quality of Medical Treatment by Enhancing Management of Antibiotic Drug Usage

OBJECTIVE To improve efficiency and quality of antibiotic drug usage in order to assure patients′ security of antibiotic drug usage. METHODS Inducted by strengthening security quality of medical treatment,we are bring antibiotic drug usage into medical quality management by using information technique,training medical workers,putting antibiotic drug into different classifications and surveillancing the usage of antibiotic drug.Moreover,we develop significant basic study for clinic to improve the level of rational use of drug. RESULTS Frequency and number of days for drug usage were reduced by putting measures into effect and inspecting numbers and sorts of drug usage.At the same time,we instituted individualized drug using scheme to critical patients,thereby many critical patients were retrieved. CONCLUSIONS Rational use of drug is a system project which must redeploy all enthusiasm and take comprehensive measures to fight for a safe,effective,economical goal in our work.
7.Pathogens Analysis of Traumatic Infection among Earthquake Victims

OBJECTIVE To investigate the status of traumatic infection among earthquake in order to provide reasonable evidence for anti-infective therapy. METHODS Samples were collected from 14 patients with compound fracture or osteofascial compartment syndrome for bacterial,fungal and anaerobic culturing,then according to pathogens to choose antimicrobial susceptibility test (AST). RESULTS Among 14 victims,10 had germ growth in wound secretions,the infection rate was 71.43%. Among 17 samples the bacteria,fungi and anaerobic were isolated in 12,4 and (samples,respectively),the isolated rates were 70.59%,23.53% and 5.89%. The main pathogens for the first culture were Enterobacter cloacae,coagulase negative Staphylococcus (CNS) and Acinetobacter. The Clostridium tetani and C. perfringens were all negative. We also isolated 5 strains of fungi from them 1 strain of Candida. CONCLUSIONS Most of the victims from disaster area have got bacterial infection. Most of them are combined infection,some of the infections are nosocomial infections. The key points of the treatment are to find pathogens immediately and choose a reasonable antibacterial agents according to the AST.
8.Analysis of risk factors in different gender patients with atherosclerotic renal artery stenosis

Objective To analyze the relationship between traditional & emerging risk factors and the pathogenesis of atherosclerotic renal artery stenosis (ARAS) in different gender patients.Methods Selective renal artery angiography was performed immediately after routine coronary angiography in 2060 patients with suspected coronary artery disease.Traditional & emerging risk factors were recorded and compared in different gender patients with ARAS.Results Among the patients with ARAS, smoking (59.18% vs.12.50%, x2 =17.47,P<0.01) and diabetes(28.57% vs 46.87%, x2 =5.64, P<0.05) compared with both genders were significantly differences.There were significant differences in high density lipoprotein cholesterol, ApoA1, ApoB/ApoA 1, HbA1C and Hcy (t =2.62,2.07,-2.83,-2.37,3.74;P< 0.05) in the ARAS patients compared with both genders.Conclusion The main risk factors in different gender patients with ARAS were different.It is important to adopt appropriate control strategies.
9.Effect of olmesartan medoxomil on the expression of osteoprotegerin in rats with atherosclerosis

Objective To investigate the effect of olmesartan medoxomil on osteoprotegerin and serum inflammatory factor expression in atherosclerosis model rat.Methods From 2015 April in Jiangsu Institute of schistosomiasis control,24 Sprague-Dawley rats were randomly divided into the following three groups:the control group was fed with standard food,the model group was fed with high fat diet based on standard food(normal food +2% cholesterol +5% goat fat +0.2% acid +7×105 IU/(kg·d) Vitamin D3),and the olmesartan group was given 3 mg/(kg·d) olmesartan gavage daily on the basis of high fat diet.On the fourth week of experiment the level of serum high sensitivity C reactive protein and serum lipids include total cholesterol,low density lipoprotein,high density lipoprotein and triglyceride were detected.The structure and changes of aortic pathology was observed.The expression of osteoprotegerinin in aortic was detected by immunohistochemistry staining and Western blot.Results Within the model group,the level of serum lipid and high-sensitivity C reactive protein increased significantly,endometrial thickness,intima-to-media thickness ratio and osteoprotegerin expression in aorta was significantly higher than that of normal group,the difference was statistically significant(P<0.05).The serum high density lipoprotein increased significantly,the level of other serum lipids and high sensitive C reaction protein decreased significantly,endometrial thickness,intima-to-media thickness ratio and osteoprotegerin expression in aorta of the olmesartan treated rats were significantly lower than those of model animals,the difference was statistically significant(P<0.05).Conclusion Olmesartan may suppress the development of atherosclerosis in model rats by decreasing the expression of osteoprotegerin and the level of serum inflammatory cytokines.

In this article, based on the concept of TCM wholism and meridian theory, the mechanism of acupoint pasting therapy was expounded from the aspects of medicine effects on local stimulation of the body, meridian conduction, medicine transdermal absorption. Combining with the research hot spots of modern medicine transdermal delivery system, metabolomics, and efficacy material, this article proposed a new study mode of systems biology of TCM.
