Objective To establish programming of home care nursing service by investigation and analysis of the current status of the elderly requirements towards community nursing in TaoYuanJu community.Methods Establish the evaluation sheet about nursing demands of in-home elderly to investigate 240 elderly people's comprehensive functional status and home care nursing service requirements by professional staff entering house.Results 67.08% of the old residents had home care nursing service demands.Conclusion Developing familial nursing care, can effectively improve the self- health care consciousness and capacity of aged people, having important significance for ensuring healthiness life of the elderly.

Objective To understand the progress of paclitaxel carriers, to provide theoretical support for research of paclitaxel carriers and clinical use of paclitaxel.Methods To study literatures on paclitaxel carriers recently and understand the research direction of paclitaxel carriers.Subsequently, the advantages and disadvantages of various paclitaxel carriers were analyzed and summarized.Results Paclitaxel as an effective anti-tumor chemotherapy drugs, had poorly water-soluble, serious toxicity and side effects, which limited the wide application of paclitaxel to some degree.Development of liposomes, micelles, microspheres, nanoparticles and other new dosage forms had broken these restrictions, which had vital significance.Conclusion Paclitaxel carriers can improve the solubility of the drug, reduce side effects, and develop toward multi-functional and finely targeted.

Objective To evaluate the usefulness of the transect imaging and 3D-reconstruction with spiral CT in acetabulum fracture.Methods 23 cases of acetabulum fracture proved by clinical plain radiograph,transect imaging and 3D-reconsection were reviewed retrospectively.3D-reconsection were obtained on the basis of spiral transect imaging with use of 2~3 mm collimation.3 pitch and 1.2~2 mm reconstruction interval.Results Fractures were demonstrated definitely with transect imaging.3D-reconstruction in all patients the transect imaging displayed the fractures and the displacement of the burden opex of acetablulum,the disruption within joint,the direction of fracture.Displacement of detached bone fragments and capitis femoris were display stereoscopically by 3D-reconstruction.Conclusion The transect imaging and the 3D-reconstruction of spiral CT technique with spiral CT could afford the more valuable messages.Surgeon will benefit greathy from their usefulness in the choice of the surgical intervention and operative planning.

Objective To investigate the relation between plasma macrophage migration inhibitory factor(MIF) concentration and the activity of activator protein-1(AP-1) in peripheral blood and coronary heart disease(CHD),and their relation with coronary arteriosclerosis plaques in patients with CHD.Methods One hundred and forty two patients were divided into coronary heart disease(CHD) group(n=107) and control group(n=35) according to coronary angiographic(CAG) result.The CHD group was further divided into stable angina pectoris(SAP) group(n=33) and acute coronary syndrome(ACS) group(n=75) according to clinical manifestations.In addition,according to the type of coronary lesions,the CHD group was further divided into the type A lesion(n=30) group,type B group(n=48) and type C group(n=29).According to the degree of coronary stenosis,the CHD group was also divided into mild stenosis group(n=21),moderate stenosis group(n=31) and severe stenosis group(n=55).The amount of Phospho-c-Jun in lysate and plasma MIF concentration were measured by enzyme-linked immunosorbent assay(ELISA).The results of Phospho-c-Jun was demonstrated with absorbance.Which reflect the amount of AP-1.Results The absorbance of Phospho-c-Jun in the CHD group was higher than that in the control(1.43?0.33 vs 0.71?0.13,P

Objective To explore the differences of serum free fatty acids (FFA)between the hyperlipidemia group and the healthy people with different ages and genders.Methods 912 healthy individuals of physical examination (healthy control group) and 1 061 patients with hyperlipidemia were selected and detected the serum FFA,total cholesterol,triglyceride,high density lipo-protein-cholesterol,low density lipoprotein-cholesterol,fasting plasma glucose and body mass index.The covariance analysis was used to investigate the differences in the level of serum FFA among different genders and age groups.The relationship between ser-um FFA and other serum lipids index in the patients with hyperlipidemia was analyzed by the multiple correlation analysis.Results The serum FFA level in the middle-aged females,middle-aged males and young males with hyperlipidemia was higher than that in the corresponding control groups;the serum FFA level in the young females with hyperlipidemia was lower than that in the other groups with hyperlipidemia;in the population groups with normal blood lipid,the serum FFA level in the old-age group was signifi-cantly higher than that in the middle-age and young group.Conclusion There is a difference in the serum FFA level in the hyperlip-idemia group with different ages and genders.

Objective To establish an appropriate cut-off value of high sensitivity cardiac troponin T (hs-cTnT) and optimal combination measurement in the early diagnosis of acute myocardial infarction (AMI).Methods This research is a prospective study.342 patients admitted to emergency department with chest pain,43 patients with renal failure,40 patients with pneumonia and 18 premature with patent ductus arteriosus were involved from June 2012 to June 2013 in Peking University Third Hospital.The plasma hs-TnT,NT-proBNP,cardiac troponin Ⅰ (cTnI),CK-MB and copeptin were measured.The distribution of hs-cTnT among associated diseases was analyzed,the diagnostic performance of hs-cTnT and the role of combination hs-cTnT with NT-proBNP,CK-MB and copeptin were evaluated by receiver operating characteristic (ROC) curve.The statistical method was used to calculate the Sensitivity,specificity,negative predictive value and positive predictive value of hs-cTnT in the diagnosis of AMI.Results As compared to patients with STEMI(median 0.52 μg/L,range 0.037-7.610 μg/L),hs-cTnT was lower in the patients with Non-STEMI(median 0.127 5 μg/L,range 0.021-4.260 μg/L).However,the levels of hs-TnT in other diseases were also increased increased in varyng degrees (Chi-square =76.432,P ＜ 0.05)The areas under the curve (AUC) for hs-cTnT and cTnI in the diagnosis of AMI were 0.862 (95％ CI:0.729-0.928) and 0.748 (95％ CI0.666-0.818) respectively (Z =2.713,P ＜ 0.05).Taking 0.014μg/L and 0.035 μg/L as cut-off value of hs-TNT,the sensitivities were 100％ vs 95.1％,the specificities were 44.4％ vs 65.7％.The combination of hs-cTnT,NT-proBNP,CK-MB resulted in a increase in AUC (0.915,95％ CI:0.838-0.964) (Z =2.147,P ＜ 0.05) and the combination of hs-cTnT and copeptin resulted in a increase in AUC 0.921 (95％ CI:0.820-0.975) (Z =2.589,P ＜ 0.05).Conclusion With the cut-off value of 0.035 μg/L for diagnosis of AMI was appropriate,and the combination measurement can improve the accuracy of early diagnosis of AMI.

Objective To integrate nurses' humanistic quality into professional nursing training, in order to improve humanistic quality and nursing quality. Methods Nurses with working years within 6 years in our hospital were chosen from May, 2009 to May, 2010. And through non-professional reading, specified materials reading, multimedia lectures, particularly the refining and essence of reading reports, humanities knowledge and humanistic care consciousness was melted into personal morality, words, deeds and cultivation. Investigation about satisfaction degree was carried out among patients before and after the training. Results Satisfaction degree of patients increased after training, so did the satisfaction degree of nurses to reading reports. Conclusions To integrate nurses' humanistic quality into professional nursing training can improve humanistic quality of nurses and nursing quality. Nurses' professional image will be increasingly standard, overall quality and service quality can be improved, and thought of dedicated service for patients will be consolidated.

Objective To establish human choriocarcinoma JeG-3 cell line resistant to floxuridine (FUDR)and describe the characteristics of this FUDR-resistant subline.The thymidylate synthase (TS) expression level in FUDR-resistant subline was also discussed.Methods The FUDR-resistant sub-line JeG-3/FUDRA was established by intermitted exposure to grads increased FUDR.Reversed microscope was used to observe the morphological changes in FUDR-resistant sub-line.Population doubling time was calculated and compared based on the growth curve of these two cell lines,cell cycles and chromosomal ploidy were assayed with flow cytometry methods.The chemo-luminescence assay was used to detect the hormone secretion by two kinds of cell lines.The resistant index (RI) was measured by cell counting kit-8 (CCK-8)assay.Quantitative RT-PCR was used to detect the mRNA expression level of TS and we also detected the TS mRNA expression level in different doses exposed subline.Results The RI of JeG-3/FUDRA was 31.62.Compared with the JeG-3 cell,the FUDR-resistant cell line had gross changes in morphological,cell growth,cell cycles and chromosomal numbers.The ability of human chorionic gonadotrop(hCG) and progesterone secretion was lower in JeG-3/FUDRA subline.The trend of TS mRNA expression was:while exposed to low concentration of FUDR,the TS mRNA expression level was downregulated,then followed the increasing dose of the drug,the expression level of TS mRNA ascended gradually.When the terminal concentration was reached,the expression level of TS mRNA in JeG-3/FUDRA subline was higher than that of JeG-3 cell line (P＜0.05).Conclusions We established the FUDR-resistant subline of JeG-3 successfully.The TS mRNA expression level is stage-related to the different concentration and different phase in FUDR exposure.Our data suggested that TS mRNA expression level may not be used as a biomarker to predict the chemosensitivity in FUDR-based chemotherapy.

Objective By reviewing the relevant literature, we explored the current status and problems of the evaluation of the training effectiveness, and put forward the corresponding countermeasures, in order to provide a theoretical basis for the effect evaluation of nurse training in China. Methods By searching Citation online search and integration system (CITE), the Chinese biomedical literature service system (CBM), we included the literatures on the effect evaluation of the nurses training in the core journals from January 2006 to November 2015. Retrieval literatures were imported the bibliographic management software Note express by database. The corresponding field were analyzed in the literature′s age distribution, evaluation object, evaluation model, evaluation method, evaluation index and evaluation tool, et al. Results There were 343 literatures had retrieved, the total number of literatures presented a increasing tendency. The evaluation methods, evaluation tools and evaluation indicators presented a tendency of diversity, and the evaluation model was relatively single. Only 16 (4.7%) literatures tried to construct the training effect evaluation system of nurses, which was not based on some theory or hypothesis, which had not been effectively put into practice. Conclusion The effect evaluation system of nurse training needed to be done in depth, such as reconstruction of the new training evaluation model, the establishment of training evaluation system of different professional, different levels, different positions, to research evaluation methods, evaluation criteria and clinical practice testing, to design, training evaluation management software etc.

Objective To study the value of MRI sequence and MRA in the diagnosis of reversible cerebral vasoconstriction syn-drome (RCVS).Methods MRI and MRA of 5 patients with RCVS were reviewed retrospectively.Abnormal distribution,morpholo-gy,signal intensity of the affected arteries and complication of RCVS on MRI and MRA were analyzed.Results All patients showed multifocal segmental vasoconstriction and dilatation of cerebral arteries on MRA,which sometimes was described as “string and beads”.There were 4 patients with cerebral parenchyma lesions,in which posterior reversible encephalopathy syndrome was showed in one case,cortical subarachnoid hemorrhage in one case,and watershed infarcts in 3 cases.Five patients recovered after 3 months. Conclusion MRI and MRA can clearly show cerebrovascular morphology and observe parenchymal secondary lesions,which have an important guiding significance for clinical treatment and prognosis assessment.
