1.CHANGES IN BLOOD LIPOPOLYSACCHARIDE, LIPOPOLYSACCHARIDE-BINDING PROTEIN、SOLUBLE CD14 LEVEL AND ITS CLINICAL SIGNIFICANCE IN PATIENTS WITH CHRONIC SEVERE VIRAL HEPATITIS B

To explore the changes in blood levels of lipopolysaccharide(LPS), lipopolysaccharide-binding protein(LBP),and soluble CD14 (sCD14), and their clinical significance in patients with severe chronic viral hepatitis B. blood levels of LPS were determined with chromogenic limulus amebocyte lysate assay, and LBP and sCD14 were assayed with ELISA in 24 patients of severe chronic viral hepatitis B. 10 normal subjects and 16 patients with chronic hepatitis B were also enrolled as controls. The results showed that the blood levels of LPS, LBP and sCD14 were significantly higher in patients with the early stage, midterm, late periods of severe chronic viral hepatitis B than in normal subjects and in those with chronic hepatitis B. The blood levels of LPS, LBP and sCD14 were also significantly higher in patients who died of severe chronic viral hepatitis B than in survivors of the same disease. It suggested that when patients with severe chronic viral hepatitis B were complicated by intestinal endotoxemia (IETM), the sensitivity of Kupffer cells to endotoxin was significantly increased, resulting in hepatocyte injury by TNF ?,even in the presence of very low endotoxin concentration .

Objective To observe the effects of Qifuyin on learning and memory ability and expression of somatostatin in hippocampus on Alzheimer’s disease (AD) rats. Methods SD rats were randomly divided into blank control group, sham operation control group, model group, donepezil group and Qifuyin group. The AD model was established by microinjection of A?1-42 into the bilateral hippocampus. The learning and memory ability of rats were evaluated with a series of tasks to find a hidden platform in Morris Water Maze. 3.Distribution of Traditional Chinese Medicine Syndrome Types of Functional Dyspepsia in Guangzhou and Their Correlation with Some Factors

Objective To investigate the distribution of traditional Chinese medicine (TCM) syndrome types of functional dyspepsia (FD) in Guangzhou, and to explore their correlation with some factors such as residence time in Guangzhou, gender, age, and Helicobacter pylori (Hp) infection. Methods A total of 326 cases of FD patients from Guangzhou were investigated. Database of the related clinical data was built and analyzed. Results The commonly-seen TCM syndrome types of FD patients in Guangzhou were spleen-deficiency and qi stagnation, disharmony of liver and stomach, dampness-heat of spleen and stomach, deficiency-cold in spleen and stomach, and concurrence of cold and heat, accounting for 41.71%, 26.07%, 11.96%, 11.04% and 9.20%, respectively. There were significant differences among the TCM syndromes(P < 0.05). The differences of average age of onset, residence time in Guangzhou, FD subtypes and Hp infection were significant among each syndrome type (P < 0.05). Conclusion The distribution of TCM syndrome types of FD in Guangzhou is dominated by spleen-deficiency and qi stagnation, and the syndrome types are correlated with residence time in Guangzhou, gender, average age of onset, FD subtypes, and Hp infection.
4.Improvement of antitumor effect of ionizing radiation to treat nasopharyngeal carcinoma in combination with rapamycin

Objective To explore the possibility of rapamycin to up-regulate radiosensitivity of nasopharyngeal carcinoma (NPC) and its molecular mechanism.Methods In vitro,with untreated cells as the control,NPC cells were treated with rapamycin,irradiation (IR),or both rapamycin and IR.Phosphorylation of S6 and GSK3β,expression of Cyclin D1,clonogenic survival,number of residual γH2AX foci,and cell cycle status between study groups were compared.In vivo,athymic mice bearing CNE1 tumor were similarly treated.Tumor weight,Cyclin D1 and phosphorylated S6 in the xenograft model were compared between study groups.Results The results showed that rapamycin alone decreased the phosphorylation of S6 and glycogen synthase kinase 3 β (GSK3β),and the expression of Cyclin D1 in NPC cells.Thus,rapamycin-treated NPC cells had lower cell viability,higher DNA damage and more G1 arrest than the control,which was reflected by the in vivo study that rapamycin significantly attenuated tumor growth and decreased the levels of Cyclin D1 and phosphorylated S6.Moreover,the combination of rapamycin and IR caused the highest cell death,DNA damage,G1 arrest and tumor regression compared to those treated either alone.Conclusions Rapamycin up-regulate NPC radiosensitivity by inhibiting signal transduction of Akt/mammalian target of rapamycin (mTOR)/S6 pathway and Akt/GSK3β pathway,and by downregulating Cyclin D1 expression.
5.Assessment of the performance of red blood cells folic acid analyzer

Objective Through performance analysis of domestic red blood cells folic acid instrument to verify its testing ability. Methods According to national committee for clinical laboratory standards(NCCLS)EP5-A2,the precision was obtained through two level quality product detection within 20 days then the intra-assay variation coefficient(CV),inter-assay CV and CV in labora-tory was calculated and compared with the precision requirement coming from the red blood cells folic acid biological variation.Ac-cording to EP6-A,the linear evaluation was investigated with method of polynomial regression.By measuring the red blood cells fo-lic acid from 20 persons who presented the total healthy people,the reference scope given by the manufacturer was verified.Results Within the group CV was less than 6.67%(1/4 of biological variation allow total error),between the group CV was less than 8.90%(1/3 of biological variation allow total error),the laboratory CV was less than 13.35%(1/2 of biological variation allow to-tal error).At medical decision level,the calculated bias and the confidence limit were smaller than the allow range of error.Both fac-tors of quadratic polynomial and cubic polynomial regression models were not statistically significant compared with zero,so detec-tion system was linear.The reference scope given by manufacturer was not suitable for the local people.Conclusion The precision, accuracy and linearity of domestic red blood cells folic acid analyzer were accord with requirements.The laboratory should establish reference interval suitable for the local population.
6.Relationship among smoking, levels of homocysteine, C reactive protein and coronary heart disease onset in young people

Objective:To analyze the relationship among smoking, levels of homocysteine (Hcy), cystatin C (CysC), C reactive protein (CRP) and coronary heart disease (CHD) onset in young people.Methods:A total of 152 patients, who received selective coronary angiography because of chest pain in our hospital, were enrolled, and all subjects were <45 years old.According to examination results, they were divided into CHD group (n=100) and non-CHD group (n=52).Clinical data were analyzed in both groups, and Logistic multi-factor regression analysis was used to analyze independent risk factors for CHD in young people.Results:Compared with non-CHD group, there were significant rise in percentages of men (30.8% vs.65.0%), smoking (46.1% vs.68.0%) and hypertension (34.6% vs.51.0%), levels of CysC[(0.85±0.16) mg/L vs.(1.34±0.28) mg/L], CRP [(1.26±0.85) mg/L vs.(6.93±0.85) mg/L] and Hcy[(7.16±1.16) mol/L vs.(20.85±2.16) mol/L],P<0.05 or <0.01;multi-factor Logistic regression analysis indicated that male, hypertension, smoking, Hcy, CysC and CRP were risk factors for CHD in young people (OR=1.34~3.42, P<0.05 or <0.01).Conclusion:Male, smoking, total cholesterol, homocysteine, Cys C and C reactive protein are risk factors for CHD in young people.Therefore, these risk factors should be eliminated, or its risk should be reduced.

8.Comparison of the effect of FOLFOX4 regimen and XELOX regimen in the treatment of patients with ad-vanced colorectal cancer

Objective To observe the efficacy and untoward reactions of FOLFOX4 regimen and XELOX regimen in the treatment of patients with advanced colorectal cancer.Methods Fifty four advanced colorectal cancer patients were randomly divided into two groups.Group A (FOLFOX4):28 patients were treated with oxaliplatin 85 mg/m2 on day 1,LV 200 mg/m2 as a 2h infusion followed by bolus 5 -FU 400 mg/m2 and a 24h infusion of 5 -Fu 600 mg/m2 ,repeated for consecutive days every 2 weeks.One cycle was 14 days and efficacy was evaluated after 4 cycles.Group B(XELOX):26 patients were treated with oxaliplation 130mg/m2 on day 1,capecitabine 1 000 mg/m2 orally daily for 14 days.One cycle was 21 or 28 days and efficacy was evaluated after 2 cycles.Efficacy of the two groups was evaluated at 2 cycles.Results All 54 patients were available for evaluating objective response.In A group,the overall response rate was 46.4%,and the overall response rate was 42.3% in B group.The overall response rate of A group was slightly higher than B group,but there was no statistically significant difference(χ2 =0.093,P =0.791 ).Adverse reactions:thrombocytopenia incidence rate and the peripheral nerve toxicity had no significant differences between the two groups(χ2 =0.552,0.108,P =0.550,0.787).The incidence rates of nausea, vomit,diarrhea and leucocytopenia in B group were lower than those in A group(χ2 =9.495,4.862,6.273,P =0.003,0.033,0.108).The incidence rate of hand -foot syndrome was higher in B group than in A group(χ2 =13.389,P =0.000),but the severity was less(grade Ⅰ ~Ⅱ).Conclusion The response rate in the treatment of advanced colorectal cancer was similar between XELOX and FOLFOX4 regiments,but XELOX regiment has the advantages of more convenience,better safety.Thus,XELOX regiment is worth recommending as a first -line therapy for the treatment of patients with advanced colorectal cancer.
9.Efficacy observation of pemetrexed treatment in advanced non -squamous non -small cell lung cancer who failed first line therapy

Objective To observe the clinical effects of pemetrexed alone or pemetrexed combined with platinum in the treatment of advanced non -squamous non -small cell lung cancer who failed first line therapy. Methods 34 patients with advanced NSCLC who had failed to previous chemotherapy and all of these patients had been confirmed with pathology or cytology.Pemetrexed monotherapy group included 14 cases and pemetrexed plus platinum group included 20 cases.21 days as one cycle.All patients who received 2 or more cycles could be evaluated. Results No complete response (CR)occurred.There were 3 cases of partial response (PR),15 cases of stable disease(SD)and 16 cases of progressive disease(PD)in the 34 patients.The disease control rate was 52.9%(18 /34).The median progressive free survival was 2.7 months.The major toxic reaction included leucopenia,anemia and gastrointestinal response.Conclusion Pemetrexed or pemetrexed combined with platinum can prolong the survival time of some patients of non -squamous non -small cell lung cancer who failed first line therapy.The toxic effects can be tolerated.
10.Perception of artificial airway suction from patients and nurses: a literature review

Objective To study the perception of patients and nurses for the artificial airway suction,in order to provide theoretical reference for building the artificial airway suction clinical practice guidelines.Methods CNKI,Wanfang,VIP,Pubmed,Science direct databases were searched for papers of patients and/or nurses' perception over the limited period of 2005 to 2013.The retrieved papers were analyzed.Results Nineteen eligible papers were identified.Extract relevant contents found the majority of patients retained the memory of that airway suction,mainly for pain,choking,suffocation,and eager to get the relevant knowledge and information.There were few researches on nurses' subjective feeling about airway suction.Conclusion We should pay attention to the perception of patients,while strengthening the research on nurses' perception of artificial airway suction and improve communication with patients,in order to relieve their discomfort experience,and could be helpful for the building of airway suction clinical practice guidelines.
